Social Determinants of Health of Multicultural Adolescents in South Korea: An Integrated Literature Review (2018~2020) (국내 다문화 청소년의 사회적 건강결정요인: 통합적 문헌고찰(2018~2020))

  • Purpose: This study is an integrated literature review to analyze health problems and social determinants of multicultural adolescents in South Korea. Methods: An integrative review was conducted according to Whittemore & Knafl's guideline. An electronic search that included publications from 2018 to 2020 in the PubMed, EMBASE, Cochrane Library, CINAHL, RISS, and KISS databases was conducted. Of a total of 67 records that were identified, 13 finally met full inclusion criteria. Text network analysis was also conducted to identify keywords network trends using NetMiner program. Results: The health problems of multicultural adolescents were classified into mental health (depression, anxiety, suicide and acculturative stress) and health risk behaviors (smoking, risky drinking, smartphone dependence and sexual behavior). As social determinants affecting the health of multicultural adolescents, the biological factors such as gender, age, and visible minority, and the psychological factors such as acculturative stress, self-esteem, family support, and ego-resiliency were identified. The sociocultural factors were identified as family economic status, residential area, parental education level, and parents' country of birth. As a result of text network analysis, a total of 41 words were identified. Conclusion: Based on these results, mental health and health risk behaviors should be considered as interventions for health promotion of multicultural adolescents. Our findings suggest that further research should be conducted to broaden the scope of health determinants to account for the effects of the physical environment and health care system.

Chronic pain control in patients with rheumatoid arthritis (만성통증 환자의 통증 조절)

  • Rheumatoid arthritis is the one of the chronic diseases, one of its major symptoms is a chronic pain. Despite developing medical treatment and surgical techniques, it is suggested that to control the pain is the goal of the treatment. But pain is an inner experience and even those closest to the patient cannot truly observe its progress or share in its suffering. The National Academy of Sciences Institute of Medicine's report on Pain and Disability concluded that there is no objective measure of pain-(exactly) no pain thermometer-nor can there ever be one, because the experience of pain is inseparable from personal perception and social influence such as culture. To explore chronic pain experience is to understand the process and property of the patient's perception of pain through the response to pain, the coping with pain, and the adaptation to pain. Therefore a qualitative study was conducted in order to gain an understanding of pain experience of patients with RA in korea. I used naturalistic inquiry as a research methodology, which had 5 axioms, the first is that realities are multiple, constructed, and holistic, the second is that knower and known are interactive, inseparable, the third is only time and context bound working hypotheses(idiographic statements) are possible, the forth is all entities are in a state of mutual simultaneous shaping, so that it is impossible to distinguish causes from effects and the last is that inquiry is value-bound. Purposive sampling was conducted as a sampling. 20 subjects who experienced pain over 10 years, lived in middle-sized city and big city in Korea, and 17 women and 3 men. The subject's age was from 32 to 62 (average 48.8), all were married, living with their spouse and children, except two-one divorced and the other widow before they became ill. I collected data using In depth structured interview. I had interviews two or three times with each subject, and the interviews were conducted at each subject's home. Each interview lasted about two hours an average. A recording was taken with the consent of the subject. I used inductive data analysis-such as unitizing and categorizing. unitizing is a process of coding, whereby raw data are systematically transformed and aggregated into units. Categorizing is a process wherby previously unitized data are organized into categories that provide descriptive or inferential information about the context or setting from which the units were derived. This process is used constant comparative method. The pain controlling process is composed of behavior of pain control. The behaviors of pain control are rearranging of ADL, hiddening role conflict, balancing treatment, and changing social relation. Rearranging of ADL includes diet management, sleep management, and the adjustment of daily life activities. The subjects try to rearrange their daily activities by modified style of motions, rearranging time span & range of activities, using auxillary facilities, and getting help in order to keep on the pace of daily life. Hiddening role conflict means to reduce conflicts between sick role and their role as a family member. In this process, the subjects use two modes, one is to control the pain complaints, and the other is to internalize the value which is to stay home is good for caring her children and being a good mother. To control pain complaints is done by 'enduring', 'understanding' the other family members, or making them undersood in order to reduce pain. Balancing treatment is composed of two aspects. One is to keep the pain within the endurable level, the other is to keep in touch with medical personnel in order to get the information of treatment and emotional support. Changing social relation is made by information seeking and sharing, formation of mutual support relation, and finally simplification of social relationships. The subjects simplify their social relationships by refraining from relations with someone who makes them physically and psychologically strained. In particular the subjects are apt to avoid contact with in-laws, and the change of relation to in-laws results in lessening the family boundary. In the course of this process, they confront the crisis of family confict result in family dissolution. This crisis is related to the threat of self-existence. Findings from this study contribute to understanding the chronic pain experience. To advance this study, we should compare this result with other cases in different cultural contexts. I think to interpret these results, korean cultural background should be considered. Especially the different family concept, more broader family members and kinship network, and the traditional medical knowledge influences patients' behavior.

Related factors to dental care utilization and oral health status in immigrant workers in Korea (외국인 이주노동자의 구강건강수준과 치과 의료이용 관련요인)

  • Objectives: The purpose of the study is to investigate the related factors to dental care utilization, oral health behaviors, and oral health status in immigrant workers in Korea. Methods: The subjects were 504 foreign immigrant male workers over 20 years old who visited Daegu labor consultation center for oral health survey and oral examination. The questionnaire included 5 questions of socioeconomic characteristics, 8 questions of oral health practice behavior, 6 questions of dental clinic visit, 8 questions of social relations and Korean language proficiency. The question for health behavior was measure by body mass index(BMI). Social relations and Korean language proficiency instrument was modified by Seol from "Family welfare survey in Korean international marriage" and scored by Liker 5 scale. Results: The oral health examination of the immigrant workers was as follows: decayed teeth - 76.6%, filling teeth - 27.4%, missing teeth - 69.8%, dental caries experience above five or more - 60.2%, periodontal pocket tissues - 58.9%. Simplified Oral Hygiene Index was very poor and accounted for 49.0%. Dental care utilization experience was closely associated with social relation indexes including attendance in family events, household stuff help, financial help and counseling for hard work(p<0.01). Dental care utilization experience proportionally increased with proficiency in Korean literacy including speaking, listening, and writing abilities of Korean language(p<0.01). Conclusions: In order to improve the oral health condition of the immigrant workers, it is important to provide social network, Korean language proficiency support, and health insurance coverage through economic burden reduction by the Korean government.

Family and Community Factors Associated with Life Satisfaction of the Urban Community-dwelling Elderly across Age Groups: Focusing on the Importance of Social Capital (도시 지역 거주 노인의 연령집단별 삶의 만족감에 영향을 미치는 가족 요인과 지역사회 요인: 사회적 자본의 중요성을 중심으로)

  • Purpose: The aim of this study was to identify differences and influencing factors in the level of life satisfaction among the urban community-dwelling elderly by age group. Methods: The study was conducted utilizing the secondary data of 2017 Seoul Survey in a cross-sectional design. Of 42,688 participants in total, the data of 7,927 adults aged 65 or older were analyzed. The data were analyzed using descriptive statistics, independent t-test, chi-square test, Pearson's correlation coefficients, and multiple linear regression. Results: There were significant differences between age groups, and it was found that the old elderly groups had significantly higher life satisfaction than the oldest elderly group (t=8.37, p<.011). In common, family and community factors influencing life satisfaction in the two age groups were companion animals (old elderly: β=.03, p=.002; oldest elderly: β=.06, p=.021), social network (old elderly: β=.10, p<.001; oldest elderly: β=.08, p=.008), and social support (old elderly: β=.05, p<.001; oldest elderly: β=.08, p=.005). Conclusion: Based on these results, social welfare and nursing care services focusing on social capital and age group-specific interventions are needed to improve life satisfaction of the elderly. This study might provide the possibility and evidence for a program to improve life satisfaction for the urban community-dwelling elderly, including social capital elements.

Design of Network Architecture in Underground Structure Field Information Based on VI-GNSS (VI-GNSS 지하구조물 현장정보 네트워크 아키텍쳐 설계)

  • Recently, the integrated utilization of technology with IT is in demand for the effectiveness of field management together with the prevention and prompt action on safety accident at construction site. In addition, the establishment of construction site support system is necessary to implement the securing of worker's safety, smooth work instruction, efficiency in construction, and others. Data standardization and network architecture were designed regarding data and sound information for data transmission between systems and management. These were to construct USFSS based on integrated VI-GNSS technology in this research. In the stability test of data for each system constructed through it, around 98% stability was secured between workers and for transfer vehicle system within underground structure and field server system in regards to the data transmission stability, around 100% stability was secured between field server system and control system, respectively. Also, in the sound transmission stability test, around 99% reliability could be secured with 1km distance as its standard in case of sound transmission from underground structure construction site to field office near the field through wireless FRS system.

  • The 'Anytime, Anywhere' services have been the biggest topic in the recent digital broadcasting research area. The related applications for the Anytime and Anywhere Services are as follows: the personalized broadcasting based on the PVR (Personal Video Recorder) for the Anytime Services, and the DMB (Digital Multimedia Broadcasting), the mobile devices supporting the wireless network and the Second TV and so on for the anywhere services. The personalized broadcasting has been researched on the one of the killer applications in the various broadcasting environment - the ground wave broadcasting, the satellite broadcasting, IPTV and CATV broadcasting suchlike. The applied contents of the personalized broadcasting have advanced from the main contents for the broadcasting to the interstitial contents like advertisements and coupons, the personalized interstitial broadcasting needs to develop the technology about the generation, storing and management of the interstitial metadata. but, the personalized broadcasting has the limitation that the TV environment is somewhat difficult to support the personalized broadcasting individually, because all family usually share the main TV at home. The Second TV is possible to solve the environmental limitation. This paper designs and develops the static set top box at home, the Second TV and their multimedia streaming techniques for the personalized broadcasting of the contents including the interstitial contents.

A Machine Learning Based Method for the Prediction of G Protein-Coupled Receptor-Binding PDZ Domain Proteins

  • G protein-coupled receptors (GPCRs) are part of multi-protein networks called 'receptosomes'. These GPCR interacting proteins (GIPs) in the receptosomes control the targeting, trafficking and signaling of GPCRs. PDZ domain proteins constitute the largest protein family among the GIPs, and the predominant function of the PDZ domain proteins is to assemble signaling pathway components into close proximity by recognition of the last four C-terminal amino acids of GPCRs. We present here a machine learning based approach for the identification of GPCR-binding PDZ domain proteins. In order to characterize the network of interactions between amino acid residues that contribute to the stability of the PDZ domain-ligand complex and to encode the complex into a feature vector, amino acid contact matrices and physicochemical distance matrix were constructed and adopted. This novel machine learning based method displayed high performance for the identification of PDZ domain-ligand interactions and allowed the identification of novel GPCR-PDZ domain protein interactions.

Differential Functions of Ras for Malignant Phenotypic Conversion

  • Among the effector molecules connected with the group of cell surface receptors, Ras proteins have essential roles in transducing extracellular signals to diverse intracellular events, by controlling the activities of multiple signaling pathways. For over 20 years since the discovery of Ras proteins, an enormous amount of knowledge has been accumulated as to how the proteins function in overlapping or distinct fashions. The signaling networks they regulate are very complex due to their multiple functions and cross-talks. Much attention has been paid to the pathological role of Ras in tumorigenesis. In particular, human tumors very frequently express Ras proteins constitutively activated by point mutations. Up to date, three members of the Ras family have been identified, namely H-Ras, K-Ras (A and B), and N-Ras. Although these Ras isoforms function in similar ways, many evidences also support the distinct molecular function of each Ras protein. This review summarizes differential functions of Ras and highlights the current view of the distinct signaling network regulated by each Ras for its contribution to the malignant phenotypic conversion of breast epithelial cells. Four issues are addressed in this review: (1) Ras proteins, (2) membrane localization of Ras, (3) effector molecules downstream of Ras, (4) Ras signaling in invasion. In spite of the accumulation of information on the differential functions of Ras, much more remains to be elucidated to understand the Ras-mediated molecular events of malignant phenotypic conversion of cells in a greater detail.

A Study on the Implementation of a Portable Healthcare System using Zigbee (Zigbee를 이용한 휴대형 헬스케어 시스템 구현에 관한 연구)

  • Recently there are some trends to construct smart home system infrastructure depending upon the development of Information and Communication Technology. Also requirements of the Ubiquitous Healthcare Systems at home which can monitor the status of health continuously are increased rapidly comparing with hospitals. Healthcare service can be divided into two categories. The first one is Alarm Service that can be used for the emergency status and the other one is Remote Support Service which can monitor the patient including home environments and give those diagnosis information to medical office or to his family. Generally wired networks and fixed healthcare measuring system have some limits to transmit reliable realtime based information for both categories described above comparing with portable monitoring system. Getting over the inefficiency we will design and implement portable healthcare system under the wireless Zigbee network environments.

Demographic Characteristics and Health Problems of Low Income Children in Underserved Area (취약지역 빈곤아동의 인구.보건학적 특성 분석)

  • Objectives: The purpose of this study was to identify the major health problems of poor children, and to provide basic information for developing health care program for low income children in underserved area. Methods: Health data were collected through medical examination(KAHP Social Welfare Service) and analysed for 3,081 poor children in 106 local children's centers nationwide. Results: 1. The mean height and weight of poor children were lower than those of nonpoor children. The differences were increased by age. 2. The rate of relative low weight was higher in poor children than in nonpoor children. On the contrary, The obesity rate was higher in nonpoor children than in poor children. 3. Poor children were more likely to have vision problem, anemia, high blood pressure, and oral health problems than nonpoor children. 4. The Health problems of children were the most serious in single father family. Conclusion: In order to improve children's health status, health promotion program for poor children should be developed and implemented. Health promotion program should include activities including regular health examination, home visiting, nutrition support, managed health care, health counseling and education. And the community support network was suggested for the efficacy of the program, including home, school and community.
