생분해성 고분자의 전기분사를 이용한 약물방출 스텐트용 금속표면 코팅 및 ALA방출 거동 (Metal Surface Coating Using Electrospray of Biodegradable Polymers and $\alpha$-Lipoic Acid Release Behavior for Drug-Eluting Stents)

  • 의료용 금속스텐트는 관상동맥계 심장질환을 앓고 있는 환자에 시술되어 상대적으로 생존율을 높여 준다. 그러나, 재협착 및 후기 혈전증으로 인하여 새로운 스텐트의 개발이 시급하게 되었다. 이러한 문제점을 해결하기 위해서 신생내막 과대증식을 막을 수 있는 것으로 알려진 alpha lipoic acid(ALA)를 생분해성 고분자인 poly(lactide-coglycolide)(PLGA), poly(L-lactide)(PLLA) 및 poly($\varepsilon$-caprolactone)(PCL)과 혼합하여 전기분사 방식으로 스테인레스 스틸 표면 위에 코팅하였다. 코팅된 고분자로부터 약물방출 거동은 고분자의 종류와 농도, 용출속도 및 용매의 종류에 따라서 조사하였다. 약물방출 속도는 유리전이온도($T_g$)가 낮은 PCL에서 가장 빨랐으며 PLGA, PLLA 순서를 보였다. 고분자 표면의 거친정도는 용출속도가 증가함에 따라서 증가하였고, 용매의 비등점의 차이에 의해서 약물방출속도가 변화됨을 알 수 있었다. 이러한 약물방출 거동을 조절함으로써 ALA가 담지된 생분해성 고분자로 코팅된 약물방출 스텐트를 실제 임상적용이 가능할 것으로 기대된다.

Comparison between Basic and Inverse Dual Drug and Peptide-coated Stents in a Porcine Restenosis Model

  • Dual drug-eluting stents (DES) is a primary treatment method for coronary arterial diseases in current interventional cardiology practice. However, their pathological results according to the sequence of coating of drugs have not been reported yet. The peptide-dopamine dissolved in acetonitrile was coated onto the Chonnam National University Hospital (CNUH) stent using an electrospinning coating machine. For secondary coating (e.g., sirolimus coating, designated as SPS), sirolimus (SRL) and poly lactic-glycolic acid (PLGA) were mixed in tetrahydrofuran (THF), and the solution was then coated on the CNUH stent that had underwent the primary peptide coating using an electrospinning and spray technique. Next, the peptide-dopamine was coated on the SRL-PLGA coated stent (PSS). In this study, it was confirmed that endothelialization was promoted without being significantly affected by the coating order (SPS or PSS). The sequence of drug and peptide coating may affect the development of restenosis and PSS was effective in the prevention of restenosis compared to that of using SPS.

Fabrication and Evaluation of Polyelectrolyte Complexes of Dextran Derivatives for Drug Coating of Coronary Stents

  • The aim of this study was to fabricate a dextran polyelectrolyte multi-layer on a bare metal stent (BMS) and to evaluate bio-physical properties of the layer. Diethylaminoethyl-dextran (DEAE-D) as a polycation and dextran sulfate (DS) as a polyanion were successively coated on the bare metal stent by a well-known layer-by-layer procedure. The morphology of the stent surface and its cell adhesion were studied after each coating step by scanning electron microscopy. The stent showed more blotched and slightly rougher morphology after dextran-DS coating. The contact angle of the DEAE-DS group ($39.5{\pm}0.15^{\circ}$) was significantly higher than that of the BMS group ($45.16{\pm}0.08^{\circ}$), indicating the improvement of hydrophilic. The SMC proliferation inhibition in the DEAE-DS-coated stent group ($20.9{\pm}0.04%$) was stronger than that in the control group ($21.7{\pm}0.10%$ in DS-coated group only). The DEAE-DS coating is desired for stent coating materials with biocompatibility and anti-restenosis effect.

  • The drug-eluting stent (DES) implantation is a widely acceptable treatment for coronary heart disease. It was reported that iron chelator had anti-proliferative effect on human vascular smooth muscle cells (HA-VSMCs). In this study, tetraphenylporphine (TPP) was selected as an iron chelator and drug for DES. MTT assay showed that TPP had antiproliferative effect on HA-VSMCs. TPP and polycaprolactone (PCL) were coated onto stainless steel plate using a spraycoating method. From the surface morphology examination of the coated plate by SEM, smooth polymer coating layer could be observed. The thickness of coating layer could be controlled by changing repeating time of coating. From in vitro release test, sustained release of TPP was observed from plate during two weeks. Thus, TPP as iron chelator can be used as drug for stent coating because of its antiproliferative effect and sustain release profile.

Spontaneous resolution of new coronary artery aneurysm following guideline-directed medical therapy after drug-eluting stent implantation

  • Here, we present a case of a 56-year-old man with acute myocardial infarction. The patient underwent percutaneous coronary intervention (PCI) at the left main bifurcation and mid-left anterior descending artery using drugeluting stents. Four months after the PCI, the patient was readmitted for cardiac arrest. Coronary angiography (CAG) revealed stent thrombosis in the left main-to-proximal left anterior descending artery and in-stent restenosis in the left main-to-proximal left circumflex artery. We performed balloon angioplasty at the left main to mid-left anterior descending artery and left main to proximal left circumflex artery stents; subsequently, blood flow was fully restored. However, contrast agent extravasation was observed outside the mid-portion of the left main artery to the proximal left anterior descending artery stent, indicating the presence of a coronary artery aneurysm (CAA) outside the stent. After guideline-directed medical therapy with dual antiplatelet agents and high-intensity statins, follow-up CAG revealed near-resolution of the CAA, absence of stenotic lesions, and good blood flow.

관동맥질환 치료의 최신 지견 (Updates in the Management of Coronary Artery Disease)

  • Coronary artery disease (CAD) has been increasing during the last decade and is the one of major causes of death. The management of patients with coronary artery disease has evolved considerably. There are two main strategies in the management of CAD, complementary, not competitive, each other; the pharmacologic therapy to prevent and treat CAD and the percutaneous coronary Intervention (PCI) to restore coronary flow. Antiplatelet drugs and cholesterol lowering drugs have central roles in pharmacotherapy. Drug eluting stent (DES) bring about revolutional changes in PCI. In the management of patients with 57 segment elevation acute myocardial infarction (AMI), there has been a debate on the better strategy for the restoration of coronary flow. Thrombolytic therapy is widely available and easy to administer, whereas primary PCI is less available and more complex, but mote complete. Recently published evidences in the pharmacologic therapy including antiplatelet and stalin, and PCI including DES and reperfusion therapy in patients with ST segment elevation AMI were reviewed.

  • 최근에 내시경 관련기술의 진보에 따라서 시술 편리성, 회복시간 단축, 환자 고통 경감 등의 장점으로 인해 스텐트 삽입술이 빠르게 진보하였다. 본 연구에서 양성담관 협착 치료를 목적으로 파클리탁셀을 이용한 약물방출 스텐트를 전기분사 방법에 의해 제조하였으며, 이 때 사용된 고분자는 polyether-based polyurethane(상표명 : PELLETHANE 2363-80AE$^{(R)}$)과 첨가제로서 Pluronic F127, 약물로서 파클리탁셀을 사용하여 금속스텐트 표면에 코팅하였다. 그 결과로서, 약물이 코팅된 고분자 필름의 물리적 특성은 SEM, FTIR, 접촉각 측정기, XRD에 의해 확인하였으며, 약물방출속도는 약물함량이 높을수록 감소하였음을 알 수 있었다.

미만성 긴 병변이 있는 급성심근경색환자에서 긴 단일 용출 스텐트 시술과 중첩 스텐트 시술의 효과 비교 (Comparison of Efficacy of Overlapped Versus Long Eluting Stent in Acute Myocardial Infarction Patients with Diffuse Long Lesion)

  • 미만성 긴 병변이 있는 급성심근경색증 환자에서 장기적인 임상 결과에 대해 중첩된 약물용출스텐트와 긴 단일 약물용출스텐트라는 두 가지 중재 방법으로 시술한 환자의 관상동맥조영술 결과와 임상결과를 비교·분석하였다. 2008년 6월부터 2019년 3월까지 급성심근경색환자 (65.9±11.0세, 남성 306명)가 미만성 긴 관상동맥 병변 30 mm이상 병변에 대해서 두 그룹으로 나뉘었습니다. 그룹I (중첩 약물용출스텐트 그룹; n=140) 및 그룹II (긴 단일 약물용출스텐트 그룹; n=298). 주요심장사건의 발생률을 비교하였다[주요심장사건; 두 그룹 사이에서 12개월 동안 심장사망률, 심근경색, 표적병변 재개통술 및 스텐트 혈전증]. Everolimus-용출 스텐트는 그룹I보다 그룹II에서 더 일반적으로 사용되었고(28.1% vs. 51.8% p<0.001), 시술 전 평균 병변직경은 그룹II에서 약간 굵었고 (3.1±0.3mm vs. 3.2±0.3mm, p=0.042), ACC / AHA 병변 유형C의 유병률은 그룹I에서 더 높았다(41.7% vs. 25.4%, p<0.001). 12개월 동안 주요심장사건의 발생률은 그룹II보다 그룹I에서 더 높았고(18.5% vs. 14.4%, p=0.034), 심장 사망률 (2.1% vs. 4.4%, p=0.667), 심근경색 (5.0% vs. 2.7%, p=0.260) 및 스텐트 혈전증률 (0.7% vs. 1.7%, p=0.669)은 두 그룹 차이가 없었다. 그러나 표적병변 재개통술 비율은 그룹I에서 더 높았다(10.7% vs. 5.6%, p=0.041). 다변량 로지스틱 회귀 분석에서 당뇨병의 존재[위험률 2.383, 95 % 신뢰 구간 1.332-4.260, p=0.003]과 파클리탁셀-용출 스텐트[위험률 2.367, 95 % 신뢰 구간 1.371-4.086), p=0.002]는 12개월 주요심장사건의 독립적 예측인자였으며, 두 그룹 사이의 유병률에 큰 차이가 없었다. 미만성 병변이 있는 급성심근경색 환자에서 12개월의 추적 관찰 기간 동안 중첩 약물용출스텐트 그룹에서 표적병변 재개통술 비율이 더 높았고 당뇨병의 존재 및 파클리탁셀-용출 스텐트의 사용은 주요심장사건의 독립적인 예측인자였다.

A Novel Deposition Method of PLGA Nanoparticles on Coronary Stents

  • Bare metal stents which were used to treat coronary artery disease have several biochemical problems. Polymerbased drug-eluting stents (DES) have opened up a new paradigm in the treatment of in-stent restenosis. Many studies and research programmes have proved that DES can prevent restenosis. In our study, paclitaxel-loaded poly (lactic-co-glycolic acid) (PLGA) nanoparticles have been deposited along the three dimensional scaffold of coronary stents by a method using self-assembling properties of colloidal particles. We found that the nanoparticles were deposited uniformly and closely packed. The amount of paclitaxel was easily controlled by the drug content of the nanoparticles and the deposition count.
