간호⋅간병통합서비스 병동 간호사의 감정노동 파악 및 개선방안: 초점집단인터뷰 적용 (The Emotional Labor Status and Improvement Plans of Nurses Working in the Integrated Nursing Service Ward: Applying Focus Group Interviews)

  • Purpose: The purpose of this study was to investigate the status of emotional labor of nursing personnel working in comprehensive nursing service ward and to suggest the way service improvement can be achieved. Methods: A total of 28 nurses working in comprehensive nursing service ward were divided into four groups to conduct focus group interview. All interviews were recorded and transcribed after the interview to perform data analysis in the order of data classification, topic categorization, and keyword derivation. Results: The five categories of subjects and relating keywords drawn from the focus group interviews are as follows: 1) Emotional labor experience: suppressing emotions, expressing emotions or actions that are different from reality, 2) Situations of emotional labor: verbal abuse and assault, sexual harassment, personal needs and errands, 3) Responses to emotional labor: responding directly, responding directly, receiving senior's help, using the organizational system, persevering, 4) Problems caused by emotional labor: work exhaustion, job change intention, job stress, 5) Protection plan against emotional labor: manual or education for nurses, education for patients and carers, compensation, tough sanctions though system strengthening. Conclusion: This study shows that although nurses working in comprehensive nursing service ward generally experience high levels of emotional labor, the problem solving of them relies mainly on personal response. Therefore, it is necessary to develop various measures to protect nurses in an organizational level response, thus to improve the comprehensive nursing service system.

간호·간병통합서비스의 간호업무 인식, 수행도, 중요도에 대한 조사연구 (The Survey of Perception, Performance and Importance in Nursing Practice of Comprehensive Nursing Care Service)

  • 2016년부터 전국적으로 확대되고 있는 간호 간병통합서비스 인식과 제공되는 간호업무에 대한 수행도와 중요도를 파악하고자하는 서술적 조사 연구이다. 연구대상자는 임상실습을 경험한 간호학생과 간호사들이며, 간호 간병통합서비스에 대한 인식과 114개 업무분류, 235문항으로 구성되어있는 구조화된 설문지를 이용하였다. 연구 결과로 연구대상자의 간호 간병통합서비스에 대한 인식정도는 '알고 있다'가 가장 높았고, 간호 간병통합서비스 도입 시 간호의 질과 업무의 양과 간호사 책임정도는 증가하는 것으로 나타났으며, 수행도는 '환자 간호 관리 및 정보관리', '투약 및 수혈' 등의 순이었으며, 중요도는 '투약 및 수혈'이 가장 높았다. 본 연구를 통해 간호 간병통합서비스사업의 확대 실시의 필요성은 인식하고 있으나, 간호업무의 양적 확대와 책임에 대한 부담을 높게 인식하고 있으며, 직접 간호시간의 확대가 예상되므로 앞으로 간호 간병통합서비스의 효과적인 간호업무 관리에 대한 논의가 필요할 것이다.

A RURAL HEALTH SERVICE MODEL FOR KOREA BASED OH A PRIMARY CARE NURSING SERVICE SYSTEM

  • This study concerns itself with the development of a new model of comprehensive health service for rural communities of Korea. The study was conceived to resolve the problems of both underservice in rural communities and underutilization of valuable health manpower, namely the nurses, the disenchanted elite health personnel in Korea. On review of the current situation, the greatest deficiencies in the Korean health care system were found in the availability of primary care at the peripheries of md communities, in the dissemination of knowledge of disease prevention and health care, and in the induction of and guidance for active participation by the clientele in health maintenance at the personal, family and community level Abundant untapped health resources were identified that could be brough to bear upon the national effort to extend health services to every member of the Korean Population. Therefore, it was Postulated that the problem of underservice in rural communities of Korea can be structurcturally resolved by the effective mobilization and organization of untapped health resources, and that. a primary care Nursing Service System offers the best possibility for fulfillment of rural health service goals within the current health man-power situation. In order to identify appropriate strategies to combat the present difficulties in Korean rural health services and to utilize nurses and other health personnel in community-centered health programs, a search was made for examples of innovative service models throughout the world. An extensive literature survey and field visits to project sites both in Korea and in the United States were made. Experts in the field of world health, health service, planners, administrators, and medical and nursing practitioners in Korea, in the United States as well as visitors from other Asian countries were widely consulted. On the basis of information and inputs from these experts a new rural health service model has been constructed within the conceptual framework of community development, especially of the innovation diffusion Model. It is considered especially important that citizens in each community develop capacities for self-care with assistance and supports from available health professionals and participate in health service-related decisions that affect their own well-being. The proposed model is based upon the regionalization of health care planning utilizing a comprehensive Nursing Service System at the immediate delivery level The model features: (1) a health administration unit at each administrative level; (2) mechanisms for community participation; (3) a continuous source of primary health care at the local community level; (4) relative centralization of specialty care and provision of tertiary or super-specialty care only at major national metropolitan centers; and (5) a system for patient referral to the appropriate level of care. This model has been built around professional nurses as the key community health workers because their training is particularly suited and because large numbers of well-trained nurses are currently available and being trained. The special element in this model is a professional nurse-guided, self-care facilitating primary care Community Nursing Service System. This is supported by a Nursing Extension Service as a new training and support structure. (See attached diagrams). A broad spectrum of programs was proposed for the Community Nursing Service System. These were designed to establish a balance of activities between the clinic-centered individual care component and the field activity-centered educational and supportive component of health care services. Examples of possible program alternatives and proposed guidelines for health care in specific situations were presented, as well as the roles and functions of the key health personnel within the Community Nursing Service System. This Rural Health Service Model was proposed as a real alternative to the maldistributed, inequitable, uncoordinated solo-practice, physician-centered fee-for-service health care available to Koreans today.

도농복합지역 재가복지대상자의 기능상태평가 (Functional Status Evaluation of Disabled Persons in Urban and Rural Areas)

  • Purpose: The purpose of this study was to provide the basic techniques in utilizing the systematic home-rehabilitation service with a Minimum Data Set for Home Care (MDS-HC) 2.0 navigator system. Methods: The present study was conducted with 50 persons receiving home-care services from a Welfare Center. Respondents were selected from urban and rural areas in and around the Wonju area. Results: The results showed that MDS-HC2.0 was useful to evaluate the functional status of disabled persons in the fields of health and home-care services. Furthermore, this navigator system provided basic information about the functional health problems of respondents and therefore can provide guidance for health and home care services for disabled persons. Conclusions: Through the present study, a comprehensive model for health and home-rehabilitation was developed. The author believes that respondents will have the satisfaction of high quality service if the developed model is used as the standard in planning and providing home-rehabilitation services.

  • 본 연구는 간호 간병 통합서비스 품질이 의료서비스 성과에 미치는 영향을 알아보기 위해 가설 검정을 실시했다. 연구 결과는 간호 간병 통합서비스 품질의 반응성, 확신성, 공감성, 계산적 몰입, 신뢰성, 감정적 몰입은 의료서비스 성과의 지각된 성과, 충성도, 지각된 성과, 성과의 충성도와 의료서비스 성과의 지각된 성과는 충성도에 긍정적인 영향은 채택으로 나타났다. 즉, 통합서비스 품질은 의료서비스 역량에 매우 중요함을 검증하였다. 이를 위해서는 간호팀원의 인력확충을 위한 제도적 노력이 우선되어야 하며, 우수한 간호팀원 양성을 위해서 전문적 간호능력 외에도 환자와의 공감을 통해 긍정적 관계 형성을 위한 인성 교육에도 중점을 두어야 할 것이다. 본 연구에서 통합서비스 품질이 의료서비스 성과에 미치는 영향을 검중 하였다는데 연구의 의미를 찾을 수 있다.

간호·간병통합서비스병동 간호사의 위임준비성과 직무만족 간의 관계 (Relationship between Delegation Preparedness and Job Satisfaction of Nurses in Comprehensive Nursing Care Service Units)

  • 본 연구의 목적은 간호·간병통합서비스병동 간호사를 대상으로 위임준비성과 직무만족 간의 관계를 파악하기 위함이다. 자료 수집은 2017년 5월 26일부터 2017년 6월 7일까지 서울과 경기 지역에 위치한 5군데 종합병원의 간호·간병통합서비스병동에서 근무하는 간호사 126명을 대상으로 시행되었다. 자료는 independent t-test, one way ANOVA, Pearson's correlation coefficients을 이용하여 분석되었다. 연구결과 간호사의 위임 교육 여부(t=-2.77, p=.006)가 위임준비성에 유의한 차이를 나타냈고, 위임준비성과 직무만족 간에 유의한 양적 상관관계(r=.43, p<.001)가 있는 것으로 나타났다. 위임준비성 하부 영역과 직무만족에서는 업무 내용에 대한 이해(r=.26, p=.003), 위임업무구분(r=.45, p<.001), 위임 내용에 대한 이해(r=.35, p<.001), 위임 기술(r=.34, p<.001) 간에 유의한 양적 상관관계가 있는 것으로 나타났다. 위임준비성과 직무만족의 하위 영역 중에서는 상호작용(r=.46, p<.001), 직무과업(r=.36, p<.001), 전문직업적 수준(r=.33, p<.001), 행정(r=.31, p<.001), 보수(r=.20, p=.026)가 유의한 양적 상관관계가 있는 것으로 나타났다. 이상의 결과는 간호·간병통합서비스병동 간호사가 효과적인 위임을 위해서는 학교와 간호현장에서 위임 기술을 포함한 교육을 강화시킬 필요성을 제시한다.

  • Purpose: The purpose of this study was to investigate the effect of the development and application of the new nurses' core skill program on the clinical skill, confidence, knowledge and performance of new nurses. Methods: This study was a nonequivalent control group post-test design on 46 new nurses with 3 to 6 months work experience in a general hospital. Data were collected from August 2015 to August 2016. A training program of core fundamental nursing skills was applied on 23 new nurses from 2016 in the experiment group, and an existing training method was applied on 23 new nurses from 2015 in the comparison group. The collected data were analyzed using SPSS/WIN 23.0. Results: The experimental group ($89{\pm}4.29$) showed higher score in practical skill assessment compared to the control group ($85.09{\pm}3.99$) and showed a statistically significant difference (p<001). Conclusion: A training program of core fundamental nursing skills for new nurses proved effective in improving practical skills. Further research on the development of a systemic training program that can improve clinical nursing knowledge and work ability for new nurses according to the extended enforcement of a Nursing Care Integrated Service ward is needed.

간호·간병통합서비스 병동 간호사의 감정노동, 강인성이 직무만족도에 미치는 영향 (Influences of Emotional Labor, Hardiness on Job Satisfaction of Nurses in Comprehensive Nursing Care Service Units)

  • 본 연구는 간호·간병통합서비스 병동 간호사의 직무만족도에 미치는 영향을 파악하기 위해 수행된 서술적 조사연구이다. 본 연구는 3개 도시 250병상이상 종합병원 2개와 800병상이상 상급종합병원 3개로 총 5개 병원에서 수행하였다. 연구 대상자는 간호·간병통합서비스 병동에 근무하고 있는 간호사로150부를 배포하여 최종 146부를 회수하였다. 자료수집 기간은 2019년 8월 1일부터 2019년 11월 15일까지였고, x2-test, t-test, ANOVA, Pearson 상관관계, Multiple linear regression으로 분석하였다. 감정노동과 강인성은 음의 상관관계(r=-.20, p=.018), 감정노동과 직무만족도는 양의 상관관계가 나타났다(r=.25, p=.004). 대상자의 직무만족도에 영향을 미치는 예측변인은 병동업무의 적성여부(β=-.35, p=<.001)와 강직성(β=.21, p=.009)으로 나타났다. 직무만족도의 설명력은 18.2%였다(F=3.54, p=.009). 본 연구 결과 간호·간병통합서비스 병동 간호사의 직무만족도에 영향을 미치는 예측변인은 강직성과 병동업무의 적성여부로 나타났다. 따라서 해당 병동 간호사의 직무만족도를 높이기 위해서는 간호사의 강직성 향상과 적성을 고려한 인력배치 등의 행정적인 뒷받침이 요구된다.

가정간호대상자의 삶의 질에 영향을 미치는 요인 (A Study on the Quality of Life for Home Care Nursing Patient)

  • Purpose : This study purposed to conduct a comprehensive survey of home care nursing clients' quality of life based on the PRECEDE model. Method : This study selected 74 home care nursing clients registered at a university hospital in Incheon and performed face-to-face interviews by structured questionnaire. The research period was two months from the $2^{nd}$ of February to the $30^{th}$ of March in 2004. Result : According to the result of assessment at each stage of the PRECEDE model, home care nursing clients' quality of life was 13.88 out of 25 points, health level 15.22 out of 21, abilities to perform activities of daily living 29.26 out of 100, cognitive abilities 16.00 out of 30, social support 13.68 out of 20, and satisfaction with home care nursing service 33.26 out of 40. According to the result of stepwise regression in order to identify factors influencing home care nursing clients' quality of life, social support and abilities of daily living were found to be significant variables among the characteristics of each assessment stage. Conclusion : It is necessary to develop nursing intervention strategies for strengthening social support and enhancing abilities to perform activities of daily living in order to improve home nursing clients' quality of life.

Mediating Effect of Job Burnout in the Relationship between Role Ambiguity and Organizational Effectiveness among Nurses in Comprehensive Nursing Care Service Units

  • 본 연구의 목적은 간호간병통합서비스 병동에 근무하는 간호사의 역할모호성, 직무소진과 조직유효성 간의 관계를 확인하고, 역할모호성과 조직유효성의 관계에서 직무소진의 매개효과를 파악하는데 있다. 대상자는 10개 종합병원에 근무 중인 149명의 간호사이다. 데이터는 자기보고식 설문지를 사용하여 수집하고 IBM SPSS Statistics 25.0 program과 PROCESS macro Program의 model 4를 이용하여 분석하였다. 분석 결과, 조직유효성은 역할모호성, 직무소진과 유의한 부(-)의 관계가 있는 것으로 나타났으며 직무소진은 역할모호성이 조직유효성에 미치는 영향을 완전매개 하는 것으로 확인되었다. 연구결과에 기초하여 직무소진을 관리하고 조직유효성을 제고하기 위한 방안을 제안하였다.
