  • Purpose: The aims of this study were to develop a motivational interviewing program for exercise improvement in persons with physical disabilities and to examine the effect of this motivational interviewing intervention. Methods: The study employed a nonequivalent control group pretest and posttest design. A total of 62 persons with physical disabilities (30 in the experimental group, 32 in the control group) were recruited from 2 community rehabilitation centers. The experimental group received 8 sessions of a group motivational interviewing program, scheduled once a week, with each session lasting 60 minutes. Test measures were completed before the intervention, immediately after the end of the intervention, 2 weeks later, and 6 weeks after the end of the intervention. Measures included self-efficacy for exercise, decisional balance for exercise, stage of change for exercise, regularity of exercise, exercise maintenance, and independent living ability. Data were analyzed using the ${\chi}^2-test$, Fisher's exact test, Independent samples t-test, and repeated measures ANOVA, conducted using IBM SPSS Statistics version 18. Results: The experimental group showed a significant increase in self-efficacy for exercise (F=50.98, p<.001), benefit (pros) of exercise (F=24.16, p<.001), and independent living ability (F=50.94, p<.001), and a significant decrease in loss (cons) of exercise (F=26.50, p<.001). There were significant differences between the two groups in stages of change for exercise (p<.001), regularity of exercise (p<.001), and exercise maintenance (${\chi}^2=26.61$, p<.001). Conclusion: The motivational interviewing program has the potential to improve exercise levels in persons with physical disabilities.

  • Purpose: This paper presents the development process and the final contents of the sellf- management education program integrated with exercise training (Healthy Aging Happy Aging, HAHA program) for community residing older adults with chronic diseases. Methods: The program evaluation methodology was applied which is an interactive program development process based on needs assessment, formative evaluation, process evaluation and outcome evaluation. The program was developed and revised while the program was implementing to 22 hypertension (HT) and 32 diabetic (DM) participants. Results: The final program has two sub-programs for HT and DM participants utilizing self-efficacy resources. They share four common components; 1) health screening of exercise risks, 2) weekly 1-hour group self-management education classes, 3) biweekly 1-hour group exercise training and 4) a mid-term individual counseling. Both sub-programs were 12-weeks long but have different education and exercise contents. Participants-rated mean satisfaction scores were 3.47/4 and 3.61/4 for HT and DM program respectively. Attendance rate were 83.1% ~ 92.3% for the classes. Conclusion: The HAHA program developed by multidisciplinary team which reflected participants needs was accepted well by participants evidenced by high attendance rate and perceived satisfaction level.

  • Objectives: The purpose of this study was to specify major tongue diagnostic indicators and evaluate their significance in discriminating pattern identification subtypes in stroke patients. Methods: This study used a community based multi-center observational design. Participants (n=1,502) were stroke patients admitted to 11 oriental medical university hospitals between December 2006 and February 2010. To determine which tongue indicator affected each pattern identification, a decision tree analysis of the chi-square automatic interaction detector (CHAID) algorithm was performed. The chi-squared test was used as the criterion in splitting data with a p-value less than 0.05 for division, which is the main procedure for developing a decision tree. The minimum sample size for each node was specified as n =10, and branching was limited to two levels. Results: From the 9 tongue diagnostic indicators, 6 major tongue indicators (red tongue, pale tongue, yellow fur, white fur, thick fur, and teeth-marked tongue) were identified through the decision tree analysis. Furthermore, each pattern identification was composed of specific combinations of the 6 major tongue indicators. Conclusions: This study suggests that the 6 tongue indicators identified through the decision tree analysis can be used to discriminate pattern identification subtypes in stroke patients. However, it is still necessary to re-evaluate other pattern identification indicators to further the objectivity and reliability of traditional Korean medicine.

  • Purpose: To survey the functional status, self-esteem and depression of a community of indwelling clients with total laryngectomy, and to evaluate the correlation among these three variances. Methods: From May 2002 to October 2002, 76 clients who had undergone total laryngectomy more than three months ago were surveyed with using Baker's functional status in head & neck cancer-self report, Rosenberg's self rating self-esteem scale and Zung's self rating depression scale. Results: 1. The functionaI status score was $48.67{\pm}10.12$. The subjects aged 70 years or older had significantly higher scores than the subjects aged $50{\sim}59$ years. The group of clients who were operated on more than five years ago showed higher scores than the patients operated on within a year. 2. The self-esteem score was $30.47{\pm}6.44$. 3. The depression score was $47.26{\pm}8.01$. The subjects who bad three family members and the subjects who had five family members had significantly higher scores than the subjects who had two family members. 4. A significant correlation found between the functional status and self-esteem. Significant negative correlations were found between the functional status and depression and also between self-esteem and depression. Conclusion: Remarkable correlations were observed among functional status, self-esteem and depression. Nurses should consider the status of layngectomy patients and pay more attention to their pre- and post- operative nursing care and rehabilitation.

Methods: A nonequivalent control group pre-posttest design was employed. Participants were 46 chronic mentally ill persons who agreed to participate in this study, and were assigned to an experimental group (n=23) and a control group (n=23). The weight reduction program was modified by the author, based on the Wellness program (Lilly Pharmaceutical Company, 2003), Lee (2007) and Kim (2013). Data analysis with frequency, ${\chi}^2$-test, Fisher's exact test, and t-test was performed using SPSS/Win 18.0 version. Results: The results showed a significant decrease in body weight and BMI and an increase in self-esteem in the experimental group compared to the control group. Conclusion: The weight reduction program had a positive effect on body weight, BMI, and self-esteem. Evaluation of drug compliance and vocational rehabilitation is recommended for long term effect of the program and conduct of further studies will be required for use of various resources for self-efficacy for inclusion in drug education in a program for improved health outcome.

  • It is a recent trend to emphasize community services for the rehabilitation of the mentally ill. However, it is also necessary to provide social work services for those living in the institutions, who is the most underprivileged in our society. To deal with interpersonal skill deficits which is one of the main characteristics of the mentally ill, social skills programs are being utilized, usually in the communities, but not in the institutions. Therefore, the objective of this study is to develop and evaluate institution-relevant program for the improvement of assertiveness, which is the basic ability among the interpersonal skills, and self-esteem, which is the emotional basis of assertiveness. Because there has been little literature on such a program, a preliminary program had been implemented in order to include the peculiarities of the life in the institutions on the main program. The evaluation of the program effect was done by pretest-post test control group design, and its results are as follows. Firstly, in regard to program effectiveness, the pre and post test showed that self-esteem improved in a statistically meaningful way, but that assertiveness did not so. However, the staff verified the improvement of the group members' assertiveness to some degree in their actual lives. Secondly, in regard to the group process, the group interaction improved in a statistically meaningful way between 2nd and 5th session, and the improvement maintained till the program termination. Also the group members evaluated the program as helpful in their everyday lives. Therefore it can be said that the result of this program is encouraging. In light of this, several suggestions were made: utilizing more dynamic techniques in the program; developing the advanced level of the program; making an effort to change environmental structures as well as individuals; applying this kind of program to another living facilities such as those for the homeless or handicapped.
