Effects of Cognitive Behavioral Therapy on Attention Deficit Hyperactivity Disorder among School-aged Children in Korea: A Meta-Analysis (한국 학령기 ADHD 아동을 위한 인지행동중재의 효과 연구: 메타분석)

  • Purpose: This study was a meta-analysis designed to identify effects of Cognitive Behavioral Therapy (CBT) interventions in alleviating main symptoms of Attention Deficit Hyperactivity Disorder (ADHD) among school-aged children in Korea. Methods: Examination of several databases including Research Information Sharing Service, Korean Studies Information Service System, Data Base Periodical Information Academic and hand-searched article references, resulted in identification of 1,298 studies done between 2000 and 2013 of which 21 met the inclusion criteria. Comprehensive Meta-Analysis version 2.0 was used to analyze effect sizes, explore possible causes of heterogeneity, and check publication bias with a funnel plot and its trim-and-fill analysis. Results: Overall effect size of CBT intervention was large (g=1.08) along with each outcome of self-control (g=1.26), lack of attention (g=1.02), social skills (g=0.92), and hyperactivity (g=0.92). For heterogeneity, moderator analysis was performed, but no significant differences were found between the RCT (Randomized Controlled Trials) group and the NRCT (Non RCT) group. Also, meta-regression was performed using sample size, number of sessions, and length of session as predictors, but no statistically significant moderators were found. Finally, a funnel plot along with trim-and-fill analysis was produced to check for publication bias, but no significant bias was detected. Conclusion: Based on these findings, there is clear evidence that CBT intervention has significant positive effects on the main symptoms of school-aged children suffering ADHD. Further research is needed to target diverse age groups with ADHD along with more RCT studies to improve the effectiveness of the CBT intervention.

  • Nursing as it is practiced and taught, historically has been viewed as natural science. There are new movements to create a paradigm of nursing in the human sciences. A natural science methodologies elicit quantitative data from observable phenomena and reveal causal relationships. Human science methodology is the study of Unitary Man's participative experience in a situation, the simultaneity paradigm. In a theory of nursing rooted in human science assumptions about man and health are synthesized, and the practice of nursing is continuously expanded through research. To find independent nursing interventions especially cognitive nursing intervention models it has been necessary to consult a multitude of journals and text sources for references, and no one nursing textbook can be used in help patients achieve nursing goals. The goals of nursing in the simultaneity paradigm focus on the quality of life from the person's perspective. Cognitive interventions based on the person's perspective were selected from those that colleagues deemed appropriate to nursing and those that were identified in the nursing literature. They were supportive nursing care, reminiscence, meditation, relaxation and imagery. Nurses have been reluctant to make diagnoses, implement actions, and assume responsibility for this actions and this is of concern but can be understood because nurses have had little exposure to action and lack an intervention armamentarium from with which to choose actions to achieve nursing goals. Efforts in nursing education and nursing service are required to remedy this problem. Nurses must be convinced of the challenge and excitement associated with autonomous functioning. It is a characteristic of the true professional. Traditionally, nursing has prescribed one method for handling a situation. Fundamental nursing texts usually only present one way to handle a situation, because alternative interventions to achieve a client goal may be available. Considerably more research is necessary before these can be prescribed. However, unless a first step is taken, progress will not be made. The quality of health care or nursing care is enhanced when nurses transform dilemas into commited action. This is apparent from widespread experiences of nurses.

A Systematic Study of Computer-Based Driving Intervention Program for Elderly Drivers (노인 운전자에게 적용한 컴퓨터 기반 운전중재 프로그램에 관한 체계적 고찰)

  • This study systematically analyzed computer-based driving intervention programs for seniors, to provide the academic background for driving intervention for seniors. Articles published from January 2009 till December 2018 were researched and analyzed. 'PubMed, Google Scholar, and Science Direct' were used to search articles published overseas, and 'RISS, KERIS, and KISS' searched for articles published in Korea. Based on the inclusion and exclusion criteria, totally 359 papers were retrieved, and 10 articles were finally analyzed; 8 articles (80%) were evidence level I, and 2 articles (20%) were evidence level III. Amongst the computer-based interventions, driving simulators (70%) were the most common, followed by two video image training (20%) and one Nintendo Wii program (10%). In most studies, driving simulators trained the cognitive and visual abilities of seniors and enhanced their abilities to cope with risk situations under various simulated circumstances. Other interventions were also reported to have a positive effect. For evaluating elderly drivers, the driving performance evaluation using a driving simulator was the most common; in addition, evaluations of attention, space-time ability, cognitive function, risk perception, depression and anxiety were also commonly used. We believe that it is appropriate to employ computer-based driving intervention programs for seniors to train and evaluate various domains. We expect that these interventions can be used as an effective tool for safe driving.

The Effects of an Individual Cognitive Improvement Program on the Elderly with Mild Cognitive Impairments (경도인지장애 노인의 개인별 인지기능강화 프로그램적용의 효과)

  • Purpose: This study was conducted to investigate changes in cognitive functions, depression and life-satisfaction for elderly participants with mild cognitive impairments participating in an individual cognitive improvement program. Methods: A quasi-experimental with non-equivalent control group pretest-posttest design was implemented. The participants were senior citizens over 65 years of age who had been clinically diagnosed with mild cognitive impairments in G City and J Province (experimental group: 29, control group: 27). The experimental group participated in the individual cognitive improvement program based on Korean traditional tales and games for nine weeks. Results: After the intervention, only the participants in the experimental group reported significant improvement in K-MMSE (t=-2.5, p=.016), MoCA-K (t=-2.6, p=.008), depression (t=3.51, p=.001), and life satisfaction (t=-2.75, p=.008) when compared to those in the control group. Conclusion: These results indicated that the individual cognitive improvement program was effective in improving cognitive functions, depression, and life-satisfaction among the elderly with mild cognitive impairments. Therefore, the application of this individual cognitive improvement program developed in this study by visiting nurses may strengthen the cognitive functions of seniors with mild cognitive impairments.

Predictors of Burnout among Informal Caregivers of Older Adults ; Using Discriminant Analysis (노인대상자의 비전문 간호제공자가 경험하는 소진예측요인 : Discriminant Analysis를 이용한 분석)

  • The purpose of this study was to examine the predictors of burnout among informal caregivers of older adults. One hundred thirty seven caregivers who had provided care to older adults over 60 years of age for more than one month were included in the study. Most of the caregivers were white (91%) and female (78%). Mean ages of the caregivers were 64 years and 78% of them were married. Seventy percent of the older adults suffered from Alzheimer and related disease. In this study, caregivers reported that they experienced burnout once a month. The scores of emotional exhaustion and reduced personal accomplishment of the caregivers were in the moderate burnout range. To examine the predictors of burnout, discriminant analysis was used. Caregivers were divided by two groups based on the sum of scores (cutpoint=6) on three dimensions of burnout after each dimension was categorized into high(3), moderate(2), and low(1). Nine predictors were included in the analysis : Caregiver's age, employment status, Instrumental Activities of Daily Living (IADL) of the older adult, presence of dementia of the older adult, caregiver's empathy toward the older adult (emotional and cognitive), and 3 coping strategies (negative, problem-focused, cognitive reconstructioning). Caregivers who reported high burnout showed higher cognitive empathy toward the older adults than those who showed low burnout. Caregivers who experienced high burnout used negative coping more often, and cognitive reconstructioning coping and problem focused coping less often than their counterparts. Wilks' Lambda was .78 indicating that differences between the two groups were significant. Cognitive empathy and cognitive reconstructioning coping showed high standardized canonical discriminat function coefficients over .40. Discriminant function with 9 predictors correctly classified 71% of the sample. In conclusion, informal caregivers also experienced certain level of burnout. Cognitive empathy and coping strategies predicted burnout experience of the caregivers. Based on this study, nursing intervention to the informal caregivers to improve their cognitive empathy toward the older adults and to modify their coping strategies in a way to reduce burnout experience can be applied. Further research to develop effective nursing interventions for the purpose of reducing burnout experience by modifying predictors was suggested.

Effectiveness of a Comprehensive Health Promotion Program based on the Primary Health Care Post for Rural Elders (농촌 노인을 위한 보건진료소 중심의 포괄적 건강증진 프로그램의 효과 검증)

  • Purpose: This study was done to develop a comprehensive health promotion program based on the primary health care post, and then to examine the effectiveness of the program for rural elders. Methods: A single group pre-test post-test design was used, for 3 months and the 51 participants received the comprehensive health promotion program based on the primary health care post. Effectiveness of the intervention was measured immediately after the comprehensive health promotion program. Results: There were significant differences between pre-test and post-test on cognitive function (p=.001), depression (p=.001), systolic blood pressure (p=.001), blood sugar (p=.013), and cholesterol (p=.018). Conclusion: The comprehensive health promotion program based on the primary health care post was found to be effective for rural elders by improving cognitive function and physiological indicators and decreasing their depression.

The Effect of Tailored Occupational Intervention on the Elderly with Mild Dementia using the Day Care Center (주간보호센터 이용 경도 치매노인에게 적용한 맞춤형 작업 중재의 효과)

  • Objective : This study identified the effect of tailored occupational intervention applied to the elderly with mild dementia using the day care center. Methods : This study applied the single-group experimental design to 29 elderly with mild dementia who use the day care center in B area, and conducted a total of 10 sessions once a week, 60 minutes. Intervention consisted largely of group activities and individual activities, and individual activities were conducted one-on-one with various activities necessary to select occupational goals and achieve the goals through consultation between the elderly with mild dementia and the person in charge. The effects of tailored occupational intervention were confirmed through CERAD, BBS, and GDS. Results : Significant differences were shown in the total score of MMSE-KC, time orientation, attention, constructional praxis delayed recall, and trail making test B in the cognitive function changes, and standing to sitting, standing unsupported with eyes closed of BBS in the physical function changes, and in the GDS score in the depression changes (p<.05). Conclusion : Tailored occupational intervention has been shown to be effective in improving cognitive and physical functions of the elderly with mild dementia and reducing depression. This provided a basis for proposing a tailored occupational intervention as an intervention that can be applied to the elderly with mild dementia.

The Effects of Instrument-Activities Daily Living Training through Client-Centered Home Visitation on Cognitive Functions, Occupational Performance, and Instrument-Activities Daily Living among Elderly at the Cognitive Support Grade (클라이언트 중심 가정방문 일상생활훈련이 인지지원등급, 노인의 인지기능, 작업수행, 일상생활수행도에 미치는 영향)

  • Purpose : This study aims to investigate the effect of instrument-activities daily living training through client-centered home visitation on the cognitive functions, occupational performance, and instrument-activities daily living of elderly at the cognitive support grade(Grade6). Methods : The subject of this study was a 66-year-old woman living in G Metropolitan City, who has been diagnosed with Alzheimer's and mild dementia. The study period was from March 17, 2020 through June 12, 2020, and the A-B-A' design, among the individual case experiments, was adopted as the study design. For the data analysis, descriptive statistic and visual analysis using graph were used for the change of cognitive functions, occupational performance, and instrument-activities daily living. Results : The instrument-activities daily living provided through client-centered home visitation improved the subject's cognitive functions, occupational performance(performance, satisfaction) and instrument-activities daily living. Conclusion : This study showed that daily life training through client-centered home visitation can help elderly people at the cognitive support grade select for themselves the problems of daily life caused by cognitive decline and practice specific action plans, thereby enabling them to maintain and improve the cognitive functions necessary for the performance of activities, such as comprehension, memory, and thinking skills. In addition, it is thought that the activities based on the subject's preferences, performance, and sense of importance assured the subject of feelings of motivation and the possibility of participation, and had a positive effect on the subject's performance speed and rate. With the above in mind, Instrument-activities daily living client-centered home visitation is proposed as a potential practical intervention program for individuals. It can help elderly people at cognitive support grade to maintain and improve their functions, thereby delaying the progress of their condition to severe dementia.

  • Objectives: The aim of this study was to investigate the association between cognitive function and upper extremity function in the elderly. The articles were analyzed based on patient, intervention, comparison, and outcome using the P.I.C.O. principle. Methods: We systematically examined papers from January, 2000 to November, 2015 published papers through the foreign journals which were Medline & Pubmed for three months. mainly used key words were elderly, dementia, Alzheimer's disease, Mild cognitive impairment, age-related, aging, cognitive, upper extremity function, hand function, hand-grip strength, grip force, complex motor function, bimanual, dexterity, UE performance, and coordination. Results: The number of discovered outcomes for association between cognitive function and upper extremity function in the elderly was 7; grip strength & sex are impact on manipulation object, 1. The results show that cognitive function is associated with upper extremity function in the elderly. Conclusion: This study is expected to help selecting intervention, assessment tools according to the individual's degree of cognitive level and upper extremity function. In future domestic research, variety assessment tools need to be used and more qualitative level experiment will be carried out.

Mitigation of Adverse Effects of Malicious Users on Cooperative Spectrum Sensing by Using Hausdorff Distance in Cognitive Radio Networks

  • In cognitive radios, spectrum sensing plays an important role in accurately detecting the presence or absence of a licensed user. However, the intervention of malicious users (MUs) degrades the performance of spectrum sensing. Such users manipulate the local results and send falsified data to the data fusion center; this process is called spectrum sensing data falsification (SSDF). Thus, MUs degrade the spectrum sensing performance and increase uncertainty issues. In this paper, we propose a method based on the Hausdorff distance and a similarity measure matrix to measure the difference between the normal user evidence and the malicious user evidence. In addition, we use the Dempster-Shafer theory to combine the sets of evidence from each normal user evidence. We compare the proposed method with the k-means and Jaccard distance methods for malicious user detection. Simulation results show that the proposed method is effective against an SSDF attack.
