Statistical Analysis of Clinical Nursing Competency and Self-Efficacy in Nursing Students

  • The purpose of this study is to investigate the clinical nursing competence and self-efficacy of 4th and 2nd semester nursing college students who completed most of the performance-based nursing education curriculum. It was attempted to develop competency evaluation and competency-based curriculum. The collected data were analyzed using descriptive statistics, t-test, one-way ANOVA, $scheff{\bar{e}}$ test, Pearson's correlation coefficients and Stepwise multiple regression in SPSS WIN 24.0 program. The findings of this study were as follows. 1) The knowledge level of essential basic nursing skills received a score of 88.95. The overall average score of clinical performance was 3.15 out of 5. The mean score of self-efficacy was $4.14{\pm}0.57$ points on 6 points 2) Among the general characteristics of subjects, 'motivation of major selection' and 'satisfaction of practice time' differed in the knowledge of essential basic nursing skills, 'religion' and 'health status' differed in clinical performance ability and 'interpersonal relationship', 'motivation of major selection', 'major satisfaction', 'satisfaction of practice time', 'nursing satisfaction', 'desired working period' and 'average rating' differed in self-efficacy. 3) The self-efficacy showed a significant positive correlation with the clinical nursing competency including the knowledge of essential basic nursing skills and clinical performance ability. The nursing satisfaction, clinical performance ability, the knowledge of essential basic nursing skills, interpersonal relationship and average rating influenced significantly and explained 23.7% of the subjects' self-efficacy.

Effects of Simulation-based Practice using Standardized Patients for the Care of Women with Postpartum Hemorrhage on Nursing Student's Clinical Performance Competence and Critical thinking Deposition (표준화 환자를 활용한 산후출혈 산모 간호 시뮬레이션 실습이 간호학생의 임상수행능력과 비판적 사고 성향에 미치는 효과)

  • Purpose: This study was conducted to examine the effects of simulation-based practice using standardized patients for the care of woman with postpartum hemorrhage as related to clinical performance competence and critical thinking deposition of nursing students. Methods: A nonequivalent control group pre-post test experimental design was used. Fifty four third year students were recruited, 29 students for the experimental group and 25 students for the control group. The simulation-based practice about nursing care related to postpartum hemorrhage included skill training, team-based practice, and debriefing. It was implemented with the experimental group for about 30 minutes in October 6, 2011. Data were analyzed using frequency, ratio, chi-square, Fisher's exact probability, and t-test using the SPSS/WIN program. Results: The experimental group who had the simulation-based practice showed significantly higher knowledge and skills of clinical performance competence than the control group (t=2.19, p=.003). But the experimental group who had the simulation-based practice did not show significantly higher critical thinking deposition than the control group (t=-0.32, p=.748). Conclusion: The results indicate that a simulation-based practice is an effective teaching method to improve knowledge and skills for clinical performance competence of nursing students. Further study is needed to identify the effect of a simulation-based practice on nursing attitudes.

The Relationship between the Satisfaction with Clinical Practice and Clinical Competence by Types of Self-directed Learning Ability of Nursing Students (간호대학생의 자기주도적 학습유형에 따른 임상실습만족도와 임상수행능력)

  • Purpose: The purpose of this study was to identify the relationship between the satisfaction with clinical practice and clinical performance ability by types of self-directed learning ability of nursing students. Methods: This was a triangular study that was conducted to understand clinical performance ability. The subjects were 260 junior and senior students from a university in P city. The data were collected from April 22 to December 30, 2015. Data were collected by Q-card, Q-block an assessment tool, a structured self-reporting survey and a questionnaire. Results: We classified the self-directed learning abilities into four types: Type 1: a self-reflective person; Type 2: a person who prepares for the future; Type 3: a person with a sense of responsibility and obligation; and Type 4: an enthusiastic learner. We found that clinical performance ability was higher for Type 4 than Type 3. We found that clinical performance satisfaction with clinical practice was also higher for the Type 4 individual than a Type 3 person. Conclusion: To improve students' clinical performance ability, we need plans and support to lead students toward becoming an 'enthusiastic learner' type of person with self-directed learning ability. It is necessary to increase students' satisfaction with clinical practice.

Effect of Simulation-based Practice on Clinical Performance and Problem Solving Process for Nursing Students (시뮬레이션을 활용한 실습교육이 간호학생의 간호수행능력과 문제해결 과정에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effect of a simulation-based practice on clinical performance and problem solving processes for nursing students. Method: The study used a one group pre-post test design. Students experienced a simulation-based practice that included team base learning, skill training, taking a high-fidelity simulation with SimMan 3G, and also being debriefed for 12 weeks (August 2010 to December 2010). The pre-test and post-test were conducted to compare the differences in knowledge, clinical nursing skills, and problem solving processes. Result: After students had received the simulation-based practice, they showed statistically significant higher knowledge (t=14.73, p<.001) and clinical nursing skills (t=15.47, p<.001) than before. However, there was no significant difference in the problem solving process score (t=1.53, p=.127). Conclusion: This study showed that knowledge and clinical nursing skills were significantly improved by the simulation-based practice. Further research would be required to identify how the problem solving process that uses simulation-based practice could be developed further.

A Study on the Experience of Clinical Practice and the Performance Confidence (치위생과 학생의 임상실습 경험정도와 수행자신감에 관한 연구)

  • To find out the relationship between the experience of clinical practice and the performance confidence, some 2nd and 3rd grade students in D health college who completed clinical practice in 2008. Sept. were surveyed. The results are as follows. 1. During the clinical practice of the dental hygiene students, the performance experience was highest in the basic prep. part among every part. As for the dental hygiene part, the students showed more than 70%, rather higher performance experience in the following parts: arrangement before and after treatment, suction, basic instruments setup, local anesthesia setup, extraction setup, resin filling setup, temporary filling setup, cements mixing, and ligation and removal setup. 2. As for performance confidence, those with performance experience showed higher performance confidence than those with only observation experience or with no experience in every treatment field except basic prep. (p < 0.001). 3. It was proven that the hygiene students have seldom had performance experience in other treatment parts except in assisting treatment part in clinical practice. In order to improve future performance experience on dental hygiene activities, subsequent research is needed to set more specific and objective criterion about the dental hygiene students' clinical practice.

A Study on the Efficiency of Clinical Practice for Nursing Education in the Junior College of Nursing in Korea (전문대학 간호과의 임상 실험 효율화를 위한 연구)

  • The purpose of this study was to find out the present condition of clinical practice and to develop a scheme on the efficiency of clinical practice for nursing education in junior college of nursing in korea. This study was conducted by 2 sections. Ist section was to find out the present condition of clinical practice to 42 directors of nursing collegd and data were collected July 8 to September 30, 1988. 2nd section wat to develop a scheme on the efficiency of clinical practice for nursing education and subjects were nursing professors 258: and clinical nurses 223 in 42 junior nursing colleges their clinical settings in korea. So total subjects were 481. Data were collected july 8, 1988 to June 30, 1988 and were analysed to get the mean, standand deviation, frequency, percentage, t-test, x-test used by SPSS - pc. Major findings were as follows: 1. The present condition of clinical education in junior college of nursing in Korea. 1) 32 colleges (76.2%) were managed by a-yeas system. 2) 25 colleges (59.5%) were performed by individual practice for each subject. 3) 4 weeks interval between class education and clinical education was a major type among total colleges(36.6%, J5 colleges) 4) 30 colleges (71.4%) provided clinical education for all subjects that should be practiced. Nursing administration wes not practiced in 5 colleges (41.9%) among the remainder(12 colleges). The main cause that all practice subjects were not practiced was the lack or absence of suitable clinical settings(8 colleges. 66.7%) 5) 18 colleges (42.9%) responded that a clinical educator was, subject-charged professor. 6) 12 colleges (29.3%) responded that a clinical instructor was in charge of 6~10 students. 7) The evaluation ration ratio(professor to head nurse) by each evaluator was mostly 50% to 50 % and 60% to 40%, respectively 11 colleges(27.5%) The most common evaluation methods were evaluation by head nures, report, presence, conference (11 colleges, 27.5%) 8) The field carrier of professor was mostly 2 years (79 persons, 20.7%) and mean was 3.2 years. The education carrier of a professor was mostly over than 6 years (261 persons, 66.4%) and mean was 9.2 years. The charge hours per-week of a professor were mostly 16-18 hours (16 persons, 131.8%) 9) 34 colleges (82.9%) approved that clinical practice hour was class hour and 18 colleges (43.9 %) counted that 2 hours of clinical education equaled 1 hour of class education. 2. A study 'on the efficiency of clinical practice for nursing education. L) general characteristics of subjects were as follows: kung-sang province (145 persons, 30.5%), 30-34 years (190 persons, 39.8%), graduated degree (245 persons, 51.5%), 6-10 years of carrier (199 persons, 41.4%) were the majority. 2) suitable clinical setting was responded the systematic ward with responsible clinical educator by 210 persons(43.8%) The response by working field of subjects showed a significant difference (p< 0.01) 3) 259 subjects (54.0%) responded that the desirable qualfication of clinical instructor was 3-5 years of clinical experience with master degree or higher. 4) The mean score of desirable quality degree of clinical instructor was 3.43 professors, score (3.54) was significantly higher than clinical nurses' (3.28) (p<0.01) 412 subjects (86.0%) responded that the insufficient guality of instructor was improved by continuing to seek more new information in reference. 5) 196 subjects (41.4%) responded that desirable qualification of head nurse was more than 2 years of head position among 5 years of clinical experience. The response by working' field of subjects showed a significant difference (p<0.05) 6) The mean score of desirable quality degree of head nurse was 3.18 Clinical nurses' score(3.38) was significantly higher than professors' (3.01) (p<0.01) 419 subjects (87.8%) responded that the insufficient of head nurse was improved by continuing relationship with instructor and being responsible from planing of clinical education. 7) The mean score of performance level of the desirable clinical education guide incollege was 2.91 Professors' score (2.96) was significantly higher than clinical nurses' (2.84) (p<0.01) 340 subjects (71.1%) responded that the possible resolution for poor performance was the more specified syllabus of clinical education and the satisfiable orientation for students. 8) The mean score of performance level of the desirable clinical education guide in hospital was 3.03 9) 141 subjects (29.6%) responded that the desirable clinical evaluator was the group of professor, head nurse, staff nurse. Response by working field of subjects was a significant difference (p< 0.05) 10) The mean score of performance level of the evaluation content needed in clinical education was 3.50 Clinical nurses' score (3.56) was significantly higher than professors' (3.45) (p<0.01) 11) 433 subjects (90.2%) responded that6 desirable evaluation method for clinical education was the presence. 12) The mean score of performance level about how personal difference among clinical educators was minimized was 2.89 and response by working field of subjects was not significant. The cause of poor performance was too much workload at clinical settings and too many students st colleges by 386 subjects (81.1%).

Clinical Practice Ability and Satisfaction of Clinical Training of Health-Medical Information Management Major Students (보건의료정보관리 전공 학생의 임상실습 수행능력과 실습 만족도)

  • Objectives : This study aimed to investigate the clinical practice ability and satisfaction of clinical training of health-medical information management major students. Methods : The data were collected from 68 persons from students finished clinical training at medical record (information) team using self administered questionnaires. The data were analyzed using t-test, ANOVA and correlation with SPSS 22.0 version. Results: Performance of data collection, data management, and data analysis were analyzed in three areas of the job area. In terms of academic characteristics and correlation, they were not related to the level of satisfaction with the practical experience. Conclusions : Research on a virtuous cycle clinical practice program that analyzes the factors by assessing the satisfaction level of clinical practice in each area of health care information management will be conducted continuously.

The Relationship between Nursing Professionalism, Clinical Decision Making Abilities, and Job Performance in Advanced Practice Nurses (전문간호사의 전문직업성, 임상적 의사결정능력 및 간호업무성과의 관계)

  • Purpose: To identify the extents of nursing professionalism, clinical decision making abilities and job performance of advanced practice nurses and investigate the relationship among the variables Methods: Participants, selected by a convenience sampling method, were 135 advanced practice nurses working in 4 tertiary care general hospitals in 'B' Metropolitan City and in 1 tertiary care general hospital in 'J' city. Data collection was done from January 15 to February 28, 2013 using self-reporting questionnaires. Results: The average score for nursing professionalism of study participants was $81.19{\pm}7.56$. for clinical decision making abilities, $140.42{\pm}9.62$ and for job performance, $102.54{\pm}10.30$. These averages are relatively high. The relationship between the extent of nursing professionalism and the extent of clinical decision making abilities showed an intermediate level positive correlation (r=.45, p<.001). The relationship between the extent of nursing professionalism and the extent of job performance was also an intermediate level positive correlation (r=.42, p<.001). The extent of clinical decision making abilities and the extent of job performance was an intermediate level positive correlation (r=.41, p<.001). Conclusion: Developing a program, which can improve nursing professionalism and clinical decision making abilities of nurse, is required to enhance their job performance.

Job Performance of Advanced Practice Nurses, Perceived Difficulty and Importance, and Willingness to Legally Delegate Clinical Practices to Advanced Practice Nurses by Health Care Professionals (전문간호사의 업무 수행 빈도 및 의료인이 인식하는 업무 난이도와 중요도, 법제화 시 업무 위임 의향)

  • Purpose: To identify the clinical role of Advanced Practice Nurses (APN), and evaluate how other medical personnel perceive their work(difficulty, importance). Methods: A questionnaire survey was performed with 277 health care providers (APN 52, nurses 88, and medical doctors 137 [professors 51, fellows 44, & residents 42]) in a single, tertiary hospital. The questionnaire was categorized into 6 domains (total 40 tasks): 6 tasks on identifying health issues (A); 3 tasks on prescribing and conducting diagnostic tests (D); 18 tasks on disease treatment (T); 4 tasks regarding prescribing medicine (M); 3 tasks regarding medical collaboration (C); 6 tasks regarding patient education (E). The survey measured the frequency, difficulty, and importance of APN's clinical tasks, and evaluated the willingness of authorizing clinical tasks to APN. Results: The most frequent tasks for APN were A domain, lowest were T domain. The scores for perceived job difficulty were lower than those for job importance in all groups. The proportion of willingness to legally delegate clinical practices to APN was higher in A and E domains, but lower in D and T domains. However, professors, who spent the most time with APN, showed a higher willingness to legally delegate clinical practice. The participants favored medical doctors as substitutes for tasks which were not legislated for delegated job performance. Conclusion: In this study identified clinical roles that medical doctors considered possible for legal delegation to APN were identified. The results can be used as evidence for the legalization of the practice of APN.

The Effect of Nursing Students' Clinical Practice Stress, Performance Ability, Satisfaction, and Critical Thinking on Nursing Professional Self Concept (간호학생의 임상실습 스트레스, 수행능력, 만족도, 비판적 사고성향이 간호전문직 자아개념에 미치는 영향)

  • This study was to examine the structural model of nursing student 's Professional self-concept, Critical thinking, Clinical Practice Satisfaction, Clinical competency and Clinical Practice Stress. The subjects consisted of 227 nursing students and data was collected through Structured questionnaires. Results, Critical thinking, Clinical Practice Satisfaction, Clinical competency and Clinical Practice Stress showed a direct effect on Professional self-concept for nursing students. Clinical competency and Clinical Practice Stress showed an indirect effect on nursing Professional self-concept. Clinical competency and Clinical Practice Stress showed a direct effect on Critical thinking and Clinical Practice Satisfaction. In this paper, we propose a new approach to nursing students 'self-concept in nursing. In this paper, we propose a new approach to nursing students' self-concept. And to develop and apply an intervention program that can reduce the stress of clinical practice.
