주의력결핍 과잉행동장애 아동 어머니의 대상관계와 부모양육행동 (The Relationship between Object Relations and Parenting Behavior of Mothers of ADHD Children)

  • Objectives : This study was conducted to compare the object relations and parenting behavior of the mothers of children with attention-deficit hyperactivity disorder (ADHD) with that of the mothers with normal children. Methods : In this study, we studied 64 mothers of children who were diagnosed with ADHD and 90 mothers of normal children. The mothers completed the Bell Object Relations Inventory (BORI), the Parenting Behavior Inventory (PBI), the Parenting Stress Index (PSI) and the Parenting Sense of Competence (PSOC) scale. Results : As compared with the mothers of normal children, the mothers of children with ADHD showed significantly different results for the parenting stress and parenting efficacy. For parenting behavior, the mothers of children with ADHD perceived their mothering as being more neglectful. On the other hand, the mothers of the normal control group perceived their mothering as being more affectionate and the fathers as being more reasonable and monitoring. Conclusion : This study suggests there is a significant difference of object relation between the mothers of children with ADHD and the mothers with normal children. The object relation pathology of the mothers of children with ADHD might contribute to impaired parenting behavior. The result of the present study indicate that inadequate grandparental rearing behavior affects the insecure object relation of their child (the mother), and a mother's insecure object relation affects their parental stress and parenting efficacy.

소아 성장을 위한 한약투여에 대한 만족도와 기대치 조사 (A Study for Satisfaction and Expectation of Effect on the Growth of Children Treated with Herbal Medicine)

  • Objectives The purpose of this study is to evaluate the effect of herbal medicine in children's growth and to report the children's expectation. Methods 171 children visited the department of Pediatrics, Kyung-Hee Oriental Medical Hospital from June 2009 to December 2009 were studied. They were treated more than 6 months and they visited our clinic more than 3 times. Every time when patients visited our hospital, their height, body weight and obesity degree was examined using DS-102(dszenix, Seoul). Also, height percentile of first and the last visit was measured, and those were compared by the Growth Statistics Curve made by the Korean Association of Pediatrics, 2007. Results and Conclusions The mean growth of total children increased by 4.42 percent; the increased percent for boys was 4.17 and that of girls was 5.00. Herbal medical treatment helped growth in children who took medicine for growth retardation problem. Plus, majority of parents who visited the department of Pediatrics, Oriental Medical Hospital satisfied with the service and their children's height. Although thinking genetic factor is important for their height, but they think that herbal medical treatment help the growth with changing eating habit.

초등학교 어린이의 허약상태 및 성장부진에 대(對)한 보고(報告) (The statistical study of Weakness and the Delayed Growth on primary school children)

  • Objective : As the meaning of collective medical examination, the purpose of this study is to investigate distribution of weak children by measurement according to the five group of weakness (Lung, Heart, Pancreas, Liver, Kidney), and the relationship between the five group of weakness and the Growth failure on primary school children. Methods : This study is composed of 1078 children who attend A primary school in ${\bigcirc}{\bigcirc}$ city. They are divided into 6 groups by grade and for the purpose of effective comparison, The five weak symptoms are studied to determine the association with growth stature among children and we standardize the weak children's height into percent according to age. Results: When each weak children controled grade, sex, body-weight, existence or nonexistence of personal history, physical exercise so on, relatively group of weakness for kidney is statistically less then opposite group and the rest of the people statistically have no significant difference. Conclusions: From the above result, it is suggested that the statistical relationship between the weakness for kidney and growth(p<0.001). It looks forward to have contributed to collective medical examination and children's health care by oriental medical treatment.

소아 구내염의 한약치료에 대한 임상연구 동향: 중의학 무작위대조군 임상시험을 중심으로 (A Review of Recent Clinical Studies of Herbal Medicine Treatment for Children's Stomatitis : Focusing on Randomized Controlled Trials in Traditional Chinese Medicine)

  • Objectives This study aims to find out the recent trend of the herbal medicine treatment in children's stomatitis in China, and propose better methods to treat children's stomatitis in Korea. Methods The randomized controlled trials (RCT) relevant to 'Herbal medicine treatment on children's stomatitis' were searched using China Academic Journal (CAJ). The search method we used included search formula, (SU = '口內炎' + '口腔炎' + 'stomatitis') and (SU = '儿童' + '小儿' + 'children' + 'child') from 2000 to September 2021. Results A total of 9 RCTs were selected and analyzed. All reports showed that oral administration of herbal medicine were effective and improved clinical symptoms. The most-used herbal medicine category was Heat-clearing medicine (淸熱藥). Conclusions The literatures from China have shown that herbal medicine improves children's stomatitis. These outcomes may be helpful for the treatment of children's stomatitis in Korea. However, further studies are needed to develop treatment guidelines for children's stomatitis.

8세 초등학생의 편평족 조사 (Flat Foot Survey in 8 Year Old Primary School Children)

  • Purpose: To estimate the prevalence of flat foot in the 1st grade primary school children in the Kang-Dong Gu, Seoul, Korea. Materials and Methods: We examined 1336 8 year old the first grade children (2672 feet) in primary school at 5 primary school for prevalence of flat foot and the associating factors in Kang-Dong Gu in April 2003. The group was examined with 2 mm-pannel in physical examination at erect position to put diagnosis of flat foot which showed no plantar arch. The height, weight, foot length and foot width were estimated in all children. We used obesity grading system of Korean Pediatric Association for overweight evaluation. Results: We enrolled 728 boys and 608 girls, and prevalences of each gender were 20.8% and 14.9%. The children who had flat foot were 243 and the prevalence of flat foot was 18.2%. The foot length ranges between 152 mm and 300 mm (mean value 183.6 mm, SD 10.6), and the foot width were between 50 mm and 107 mm (mean value 2.16 mm, SD 4.8). Of the 1336 children group, 1215 children (90.0%) were in range of normal weight, 72 children (5.4%) were in grade-1 overweight, 38 (2.8%) were in grade-2 overweigh and, 11 (0.8%) were in grade-3 overweight. The prevalence of flat foot of each overweight group were 16.3%, 34.7%, 39.5% and 45.5%. Overweight in children effected increased prevalence of flat foot. But there were no significant relationship with flat foot in other factors. Conclusion: Over all prevalence of flat foot of 8 year old children was 18.2% and most of patients were flexible flat foot. The prevalence of flat foot was influenced by overweight remarkably.

어린이집 소아에서의 황색포도알균의 비강 보균율에 관한 연구 (Nasal Carriage of Staphylococcus aureus from Healthy Children Attending Day Care Center)

  • 목 적 : 국내 어린이집을 다니는 소아를 대상으로 황색포도알균의 비강 내 보균율과 이런 보균율에 영향을 끼치는 요인들에 대해 알아보고자 하였다. 방 법: 2008년 9월에서 10월까지 서울 지역에 위치한 7개 어린이집의 소아 중 서면 동의를 받은 428명을 대상으로 설문지를 통해 인구학적 자료 및 의료기관 관련 위험 요인에 대한 자료를 수집하였다. 또한 어린이집의 소아들의 비강을 도찰하여 얻은 검체를 증균 및 선택배지에 접종한 후 생화학적 검사를 통해 황색포도알균을 동정 및 분리하였고 디스크 확산법을 통해 메티실린 감수성을 결정하였다. 결 과:대상 환아의 평균 연령은 55개월 이었으며 남녀비는 1.1:1.0이었고 최근 1년 이내에 입원력 및 수술력이 있는 환아는 각각 9.2%, 3.6%였고 항생제 사용력은 약 40%에서 확인되었다. 전체 428명의 소아 중 163명에서 비강 내 황색포도알균이 분리되어 황색포도알균의 비강 내 보균율은 38.1%였으며 분리된 균주의 24.5%는 메티실린 내성 균주였다. 연령이 증가함에 따라 황색포도알균의 비강 내 보균율도 증가 추세를 보였다. 비강 내 MRSA 보균이 9.3%의 소아에서 확인되었으며 이들 대부분이 의료기관 관련 위험요소가 없는 소아였다. 결 론:국내 소아의 약 38% 및 9%가 비강 내 황색포도알균 및 MRSA 보균자임을 확인하였으며, 특히 분리된 MRSA의 95%가 CA-MRSA 균주였다. 향후 비강내 황색포도알균의 보균율의 변화 및 CA-MRSA 보균율의 변화에 대해 지속적인 연구가 필요하겠다.

The Clinical Significance of Serum Ferritin in Pediatric Non-Alcoholic Fatty Liver Disease

  • Purpose: Non-alcoholic fatty liver disease (NAFLD) in children has become an important public health issue because of its high prevalence and severity. Several noninvasive methods for estimating NAFLD are under investigation. We aimed to evaluate the usefulness of serum ferritin as a biomarker of severity of pediatric NAFLD patients. Methods: A total of 64 NAFLD patient were enrolled from Severance Children's Hospital from March 2010 to February 2013. Serum ferritin levels, liver related laboratory tests, liver magnetic resonance imaging (MRI) (2-dimensional [2D] proton density-fat fraction) and NAFLD severity markers were compared between obese group and overweight group. Correlation analyses were performed between serum ferritin and laboratory values including NAFLD severity markers. Results: In obese group, serum ferritin, alanine aminotransferase (ALT), total bilirubin, international normalized ratio (INR), MRI 2D proton density-fat fraction, aspartate aminotransferase (AST) to platelet ratio index (APRI) and fibrosis- 4 (FIB-4) (an index score calculated from platelet count, ALT, AST and age) were significantly higher than those of overweight group. NAFLD severity markers, APRI and FIB-4, and liver specific important laboratory values, AST, ALT, INR, cholesterol, triglyceride and low density lipoprotein show significant correlation with serum ferritin in NAFLD patients. Conclusion: Serum ferritin concentrations could be a candidate of useful severity marker in the pediatric NAFLD patients.

Prevalence and Risk Factors for the Weight Loss during Hospitalization in Children: A Single Korean Children's Hospital Experience

  • Purpose: Undernutrition during hospitalization increases the risk of nosocomial infection and lengthens the disease courses. The aim of this study was to evaluate the risk factors of weight loss during hospitalization in children. Methods: All the patients who were admitted in general wards between April and May 2014 were enrolled. Patients aged >18 years and discharged within 2 days were excluded. Weight loss during hospitalization was defined as a decrease in body weight of >2% in 8 hospital days or on the day of discharge. Patients who lost body weight during hospitalization were compared with patients who maintained their body weights. Significant parameters were evaluated by using the multivariate logistic regression analysis. Results: We enrolled 602 patients, of whom 149 (24.8%) lost >2% of their body weight. Complaint of pain (p=0.004), admission to the surgical department (p=0.001), undergoing surgery (p=0.044), undergoing abdominal surgery (p=0.034), and nil per os (NPO) durations (p=0.003) were related to weight loss during hospitalization. The patients who had high weight-for-age tended to lose more body weight (p=0.001). Admission to the surgical department (odds ratio [OR], 1.668; 95% confidence interval [CI], 1.054-2.637; p=0.029) and long NPO durations (OR, 1.496; 95% CI, 1.102-2.031; p=0.010) were independent risk factors of weight loss during hospitalization. The patients with high weight-for-age tended to lose more weight during hospitalization (OR, 1.188; 95% CI, 1.029-1.371; p=0.019). Conclusion: Greater care in terms of nutrition should be taken for patients who are admitted in the surgical department and have prolonged duration of nothing by mouth.

Predictive Factors of Postoperative Pain and Postoperative Anxiety in Children Undergoing Elective Circumcision: A Prospective Cohort Study

  • Background: Although circumcision for phimosis in children is a minor surgical procedure, it is followed by pain and carries the risk of increased postoperative anxiety. This study examined predictive factors of postoperative pain and anxiety in children undergoing circumcision. Methods: We conducted a prospective cohort study of children scheduled for elective circumcision. Circumcision was performed applying one of the following surgical techniques: sutureless prepuceplasty (SP), preputial plasty technique (PP), and conventional circumcision (CC). Demographics and base-line clinical characteristics were collected, and assessment of the level of preoperative anxiety was performed. Subsequently, a statistical model was designed in order to examine predictive factors of postoperative pain and postoperative anxiety. Assessment of postoperative pain was performed using the Faces Pain Scale (FPS). The Post Hospitalization Behavior Questionnaire study was used to assess negative behavioral manifestations. Results: A total of 301 children with a mean age of $7.56{\pm}2.61$ years were included in the study. Predictive factors of postoperative pain measured with the FPS included a) the type of surgical technique, b) the absence of siblings, and c) the presence of postoperative complications. Predictive factors of postoperative anxiety included a) the type of surgical technique, b) the level of education of mothers, c) the presence of preoperative anxiety, and d) a history of previous surgery. Conclusions: Although our study was not without its limitations, it expands current knowledge by adding new predictive factors of postoperative pain and postoperative anxiety. Clearly, further randomized controlled studies are needed to confirm its results.

만성(慢性) 변비(便秘)를 주소(主訴)로 하는 소아(小兒) 환아(患兒) 4례에 대한 육미지황탕(六味地黃湯) 가미방(加味方)의 치료효과에 대한 임상적 고찰 (A Clinical study on the Effects of Ryoumijihawangtang gamibang on 4 Constipation Children)

  • Introduction : constipation is the condition of being unable to empty the bowels frequently enough and effectively. It happens many times to children. however it is usually tempoary and not a serious disease. therefore, the parents don't need to worry. Purpose : The purpose of this study is to find out weather Ryoumijihawangtang gamibang has effects on children's constipation. Methods The subjects are severe constipation children who visited Dong-sand oriental Medicine hospital from March 2002 to July 2002. The clinical period is three months and one week. Results and Conciusion : 1)After on medication, three children showed positive a result, being empty the bowels frequently and enough and effectively, but one child stated the same, the children's parents showed diverse satisfaction regardless of their children's result. 2)This study is not satisfactically correlational. however, the effect of the medicine is clinically positive to alleviate the constipation. 3)After on medication, the subject tried to alleviate constipation by diet and the change of lifestyle(eating more fiber and getting more exercise)
