Research on Improvement of Patient Quality and Perception of Hospital Operation Effect after Medical Institution Certification - For Nursing Hospitals and Western Hospitals - (의료기관 인증 후 환자의 질 향상과 병원운영효과 인식도 조사연구 - 요양병원과 양방병원 대상 -)

  • The purpose of this study was to classify the internal members of nursing hospitals and Western hospitals that received the certification system of medical institutions, and to examine the effect of certification on member satisfaction, job stress, hospital operation effect, and improvement of medical services. In nursing hospitals, the certification system was higher in member satisfaction, hospital operation effect, and medical service effect than in Western hospitals, and members of Western hospitals were higher in job stress. In both nursing hospitals and Western hospitals, the satisfaction of members, hospital operation effect, and medical service effect according to certification showed positive (+) results. It is believed that the effect of the certification system has been empirically high in nursing hospitals as mandatory certification from 2013. Therefore, this study aims to contribute to the efficiency of hospital management and improvement of patient satisfaction through continuous quality management of medical care by analyzing the effect of certification system on medical service improvement.

  • The purpose of this study was to compare and analyze the differences before and after certification for internal members of nursing hospitals and Western hospitals that received the certification system of medical institutions after conducting a survey on 297 members of nursing hospitals and Western hospitals, SPSS 26.0 was used, and to understand the effect of improvement of member satisfaction, job stress, and medical service on the hospital operation effect. As for the hospital operation effect of a nursing hospital, it was revealed that the higher the satisfaction of the members and the improvement of the medical service, the higher the effect of the hospital operation. In the case of Western hospitals, the higher the satisfaction of the members and the improvement of the medical service, the lower the job stress, the more the hospital operation effect can be seen to increase. As the basic value system of the medical institution certification system is the improvement of patient safety and quality of care, it is believed that the effect of the certification system was higher than that of Western hospitals due to mandatory certification in nursing hospitals.

  • This study briefly compares the first-cycle certification of nursing hospitals with the second-cycle certification, reviews the changes and achievements of the second-cycle certification and the third-cycle certification, and examples of challenges and solutions in the process of preparing the third-cycle certification. In this study, it is suggested as follows. First, in order to see the practical effect of certification intended by the government, the exhibition is prepared in a short period of time before certification. Second, after the implementation of the nursing hospital certification system, research on the hospital performance of the medical institution certification system is insufficient in terms of quality and quantity. Therefore, in order to see the effect on the certification system, various research support is also required so that research on this can be actively conducted. Third, design certification standard guidelines from a long-term perspective so that the standard guidelines for certification do not change significantly, and certify not only the guidelines but also individual standards and forms that can be used by medical institutions. Fourth, in the four-cycle certification, accurate and realistic guidelines for infection control and quarantine ward operation support should be developed. The importance of managing infectious diseases will be highlighted in the future due to COVID-19. Fifth, medical institutions can improve the quality of medical care in nursing hospitals and have competitiveness if their daily activities, not short-term certification preparation, are carried out in accordance with certification standards, which affects performance. Sixth, when preparing for certification, nursing hospital officials have problems in organizing documents or processing administratively in the short term as in the past. This is also based on the certification criteria for the usual business process.

Factors Influencing Turnover Intention of Nurses after Evaluation for Certification at Geriatric Hospitals: Focused on Job Stress and Burnout (요양병원 인증제 평가를 경험한 간호사들의 직무 스트레스 및 소진이 이직의도에 미치는 영향)

  • This study investigates the factors influencing nurses' turnover intentions after evaluation for certification at geriatric hospitals. Focus is centered on job stress and burnout. The study was approved by the KUIRB. Data were collected from April 1 to August 31, 2015, and analyzed using SPSS WIN 18.0 software. The participants were 205 nurses recruited from 15 long-term care hospitals in metropolitan city B in province G. The average scores for job stress, burnout, and turnover intention were 3.83, 2.92, and 3.12, respectively (range: 1-5). Nurses' turnover intention had positive relationships with job stress (r=.356, p<.001) and burnout (r=.729, p<.001). The major factor influencing turnover intention was burnout (${\beta}=.729$), which explained 53% of the intention. In conclusion, intervention programs should be developed to reduce burnout, the key influencing factor, for decreasing nurses' turnover intention.

Current Status of Patient Safety Regulations, Guidelines and Support Mechanisms in Korean Hospitals

  • Purpose: This study was conducted to investigate patient safety regulations and guidelines in order to understand their current status, and to examine support measures to improve patient safety in Korean hospitals. Methods: The participants were the safety officers from hospitals with 200 or more beds and 112 hospitals responded to the online survey. The questions covered patient safety regulations, the performance level of patient safety activities, patient safety incident reporting systems, the dedicated professional, training, support mechanisms, and expectations of reporting systems. Results: Among preventative measures, fall prevention and hand hygiene were reported to be most widely practiced (92% and 91%, respectively). Time-out for invasive procedures showed a relatively low practice rate at 70%. Among patient care activities, transfusion, surgery and sedation, medication, and infection management were performed by 84, 74, 93 and 93% of the hospitals, respectively. Patient safety activities included patient safety committee, patient safety cooperation between decision-making bodies, patient safety workshops, seminars, lectures, and training for employees. Conclusion: Patient safety regulations and guidelines have not yet been sufficiently prepared, and a public institution such as a certification authority is of crucial importance to enforce these guidelines.

Analysis of Nursing Services of Oncology Advanced Practice Nurses from the View Point of Structure and Process (종양전문간호사가 제공한 간호서비스의 구조.과정적 측면의 분석)

  • Purpose: The purpose of this study was to investigate nursing services of Oncology Advanced Practice Nurses(OAPNs) from the view point of structure and process. Methods: Roles and practices of OAPNs for cancer patients on chemotherapy were investigated by semi-structured interview and survey. Subjects were 14 OAPNs in 3 hospitals. Results: OAPNs had high level of education and certification than registered nurses. Most subjects of OAPNs were patients on chemotherapy and OAPNs used most of their time in education and expert nursing practice. 57.1% of OAPNs used delegated order. They were satisfied with their job in general, but satisfaction in communication with nurse manager or resident doctor was low. Conclusion: These results will be the concrete explanation for the nursing services of OAPNs. On the basis of theses results, more study about effects of OAPNs will be needed.

A Survey on Nursee' Needs for ICU Clinical Nurse Specialists (중환자 전문간호사제도에 관한 요구조사)

  • The purpose of this study is to investigate the nurses' needs in the general hospitals as a basic study to develop ICU clinical nurse from Aug. 3 to Sept. 26 1998 at 13 general or university hospitals that have ICU nurse training course and are accredited at KNA. The subjects of study were ICU nurses and nurse managers at the department of nursing. The questionnaire items were developed from literature review, interviews with 30 ICU nurses at an university hospital in Pusan. The results were as follows : The subjects of study educated in ICU nurse training course were 44.3% of total. The 93.1% responded the necessity of ICU CNS and 89.1% wanted to have ICU CNS certification. The 43.8% answered that ICU should be a center of CNS education and 32.0% answered the department of nursing. Most of subjects responded that the clinical experience of ICU should be needed prior to CNS education. Regarding the treatment of ICU CNS, the 34.2% of total subjects responded it would be suitable to pay additional allowance and the 28.3% answered to grade-up salary step, and then the 13.7% to promote level position. Concerning the assignment department, the 63.5% answered that the charge nurse would be proper than general nurse or head nurse. As to the working time of ICU CNS, the 93.2% responded that D-duty is needed, the 79.5% to E-duty and the 64.4% to N-duty. It is suggested that the consensus of ICU nurses for the concept, the role and the system of ICU CNS is needed.

Influence of the Awareness of Healthcare Accreditation on Job Stress and Turnover Intention in Tertiary Hospital Nurses (상급종합병원 간호사의 의료기관인증제에 대한 인식이 직무 스트레스와 이직의도에 미치는 영향)

  • Purpose: The aim of this study was to investigate the influence of nurses' awareness of healthcare accreditation on their job stress and turnover intention. Methods: Across-sectional correlation study design was used. Participants consisted of 143 nurses from two tertiary hospitals in Seoul and Gyunggido, South Korea. Data were collected in October 2016 using self-report questionnaires and analyzed using descriptive statistics, t-test, an ANOVA, Pearson's correlation coefficient, and multiple regression in SPSS/WIN 21.0. Results: Awareness of health care accreditation correlated negatively with job stress (r=-.63, p<.001) and turn over intention (r=-.50, p<.001). Awareness of health care accreditation was the factor that most influenced job stress, explaining 38% of the variance (F=23.10, p<.001). Awareness of health care accreditation and duty pattern were the factors that most influenced intention to leave, explaining 32% of the variance (F=10.35, p<.001). Conclusion: These findings suggest raising nurses' awareness of certification systems to reduce their job stress and turnover intentions. It will be necessary to provide support for the aggressive work nurses do and improve their work structure, highlighting the need for both manpower and institutional support. Accordingly, providing regular education programs and appropriate compensation schemes, by raising nurses' awareness of medical institutions' certification systems, is necessary.

A Study on the Nursing Performance of the Home Care Nurses in Seoul (서울지역 가정간호사의 업무수행능력 실태조사)

  • The purpose of this research was to find out the state of the nursing performance of the home care nurses in Seoul in order to provide the data for the practical work guide. The data were collected from fifty home care nurses working in 22 institutions located in Seoul. The research tool used for this research was modified and tested by Song Jong-Rye(1999) which was originally modified the standard tool of American Nurses Association (1998) and was partly supplement and adjusted for this research. The reliability of this tool was Cronbach's $\alpha$=.0982. The collected data were statistically processed using SAS for t-test, ANOVA. $Scheff\'{e}$ test. Among the surveyees, 50% have been working as a home care nurse more than three years and 74.0% of them were married. And 52% have B.S degree with certification of home care nurse from the certification program for Home Care Nurses affiliated at nursing schools. Half of the home care nursing organizations were based on the general hospitals, and most(78%) of the responsible senior personnels of those organizations were nurses. The following results are drawn from this research. 1) The level of nursing performance by the task sectors General performance level of home care nurses was relatively high in grade of 3.06 from total 4.00. Among the task sectors. the ethical field scored the highest points, and the next were nursing intervention, professional training, and data gathering, and the research sector scored the lowest point. 2) The level of nursing performance by general features of home care nurses Statistically relevant correlation between performance according to the duration of working experience as a nurse(p=0.8951) and performance according to the duration of working as home care nurse(p=0.2263) did not emerge. Also, performance by marriage status(p=0.2218), education(p=0.5733), and taking the certification program for home card nurse(p =0.1560) has no statistically meaningful correlation. 3) The level of nursing performance by the type of home care nursing organizations There exists a significant difference(p=0.002) between performances by the types of organizations. Most of the responsible senior personnels of the home care nursing organization were nurses. The level of nursing performance of the home care nursing organization under nurse management was relatively higher than that of home care nursing organizations led by medical doctors or non-medical professional, but this was not proved as statistically meaningful(p =0.3617). 4) The level of nursing performance by task sectors according to the characteristic of home care nursing service organization There exists a significant difference between nursing performances by task sectors according to the characteristics of home care nursing organization(p=0.002). In case of model research center of one College of Nursing, the nursing performance in the sectors of organization, theory, and data gathering were lower than that of in hospital based home care nursing service. And in case of local home care centers, performances in sectors of organization, theory, data gathering, nursing intervention. professional training, and research sectors were significantly low. Based on the obtained results, overall performance of home care nurses can be appraised as relatively good. Especially, performances in sectors of the nursing intervention, nursing plan. and data gathering including the in direct nursing were recorded high scores. From this, it can be concluded that high quality of nursing is relatively practicing for home patients at these days. Since the high quality of nursing for patients was directly related to the level of nursing performances of home care nurses, it is required to improve practical performance level of them by making constant evaluation and running continual education program and supplementing curriculum for the sectors with low scores.

A Systematic Review on the Certified Geriatric Pharmacist Program of the U.S.: Certification, Education and Outcomes (미국의 노인전문약사 제도에 대한 체계적 고찰: 인증, 교육 및 성과)

  • Background: As elderly population has been increasing, pharmacists need to possess special knowledge and skills to provide enhanced pharmaceutical care for senior patients. Purpose: This study aims to systematically review on (1) the certified geriatric pharmacists (CGP) system, (2) the curriculum related to geriatric pharmacy education, and (3) the CGP's performance in terms of clinical, economic and humanistic outcomes in the U.S. Method: The information related to CGP system and curriculum was obtained through the official websites of 'American Society of Consultant Pharmacists', 'Commission for Certification in Geriatric Pharmacy' and the selected pharmacy school samples. Articles about on the outcomes of pharmacist-provided cognitive services including Medication Therapy Management were searched through PubMed. Results: To gain the CGP credential, pharmacists need 2-year experience as pharmacist in advance and take the CGP examination. This certification must be renewed every 5 year. Most pharmacy schools provide geriatric-related curriculum through didactic or pharmacy practice classes for pharmacy students and certificate or dual degree in gerontology are given on completion. Most previous outcomes research reported that pharmacists have played a role in yielding favorable results regarding clinical, economic and humanistic outcomes for nursing facilities, hospitals and community pharmacy settings. Conclusion: Considering the organized CGP certification system and concrete educational courses established the basis for pharmacists to exert their ability for senior patients of the U.S., it is suggested to build a geriatric pharmacist credential model for pharmacists to provide the increasing elderly patients with the requisite safe and effective pharmacy care in Korea.
