Comparison of the accuracy of domestic dental intra-oral scanner(e-scanner) and model scanner (국산 치과용 구강스캐너(e-scanner)와 모델스캐너의 정확도 비교)

  • Purpose: The purpose of this study is to evaluate the discrepancy of scan process in dental intra oral scanner by comparing model scanner and anticipate possibility to introduce intra oral scan technique. Methods: 3D superimposition test was conducted to compare the scan discrepancy. The scanners used in this study are the e-oral scanner, the D750 model scanner, and the high precision CMM(3D Coordinate Measuring Machine). The standard of accuracy verification is ISO 5725-1; trueness and precision. Master model was manufactured by dental stone and scanned 5 times by intra oral, model scanner. Reference data was scanned 5 times by high accuracy CMM to evaluate the trueness. Results: Trueness of D750 scanner were $7.4{\mu}m$ $5.1{\mu}m$ $6.8{\mu}m$ at an abutment, an occluasal, a specific area. and trueness of e-scanner were $20.2{\mu}m$ $27.4{\mu}m$ $37.8{\mu}m$ at an abutment, an occluasal, a specific area. Precision of D750 scanner was $7.04{\mu}m$, e-scanner was $15.95{\mu}m$. Conclusion: When conducting in vitro test, The mean difference of trueness between e-scanner and D750 were $12.8{\mu}m$ at an abutment area, $22.3{\mu}m$ at an occlusal area, $31.0{\mu}m$ at a specific area and $8.91{\mu}m$ in precision. The scan discrepancies are within the range of clinical acceptance.

  • PURPOSE. The purpose of this study was to evaluate the marginal and internal gaps of Ni-Cr and Co-Cr copings, fabricated using the dental ${\mu}-SLA$ system. MATERIALS AND METHODS. Ten study dies were made using a two-step silicone impression with a dental stone (type IV) from the master die of a tooth. Ni-Cr (NC group) and Co-Cr (CC group) alloy copings were designed using a dental scanner, CAD software, resin coping, and casting process. In addition, 10 Ni-Cr alloy copings were manufactured using the lost-wax technique (LW group). The marginal and internal gaps in the 3 groups were measured using a digital microscope ($160{\times}$) with the silicone replica technique, and the obtained data were analyzed using the non-parametric Kruskal-Wallis H test. Post-hoc comparisons were performed using Bonferroni-corrected Mann-Whitney U tests (${\alpha}=.05$). RESULTS. The mean (${\pm}$ standard deviation) values of the marginal, chamfer, axial wall, and occlusal gaps in the 3 groups were as follows: $81.5{\pm}73.8$, $98.1{\pm}76.1$, $87.1{\pm}44.8$, and $146.8{\pm}78.7{\mu}m$ in the LW group; $76.8{\pm}48.0$, $141.7{\pm}57.1$, $80.7{\pm}47.5$, and $194.69{\pm}63.8{\mu}m$ in the NC group; and $124.2{\pm}52.0$, $199.5{\pm}71.0$, $67.1{\pm}37.6$, and $244.5{\pm}58.9{\mu}m$ in the CC group. CONCLUSION. The marginal gap in the LW and NC groups were clinically acceptable. Further improvement is needed for CC group to be used clinical practice.

  • 최근 몇 년간 의학 분야에서는 인체의 해부학적 조직을 컴퓨터 그래픽스 기술을 이용해 재구성하는데 관심이 증대되어 왔다. 치과 치료분야에서도 컴퓨터 그래픽스 기술에 대한 관심이 증대되고 있고, 이를 이용한 다양한 치료법들이 개발되고 있다. 자료를 측정한다거나, 시각적으로 3차원의 영상을 보여 줄 수도 있다 또한, CAD-CAM 기술을 이용하여 의치의 틀이나 금형 등을 직접 제작할 수도 있다. 본 논문에서는, 이러한 컴퓨터 그래픽스 기술을 이용한 치과 치료에 기념이라 할 수 있는 표준 치아에 대한 모델링 데이터를 만들고자한다. 그러기 위해 표준치아의 실제 모델을 3차원 스캐너로 입력받는다. 그리고 입력받는 과정에서 발생하는 오류를 수정하여, 정확한 표준 치아 모델을 모델링한다.

THE FITNESS OF ADJUSTABLE DENTAL IMPRESSION TRAYS ON THE KOREANS (한국인에 대한 가변형 치과 인상용 트레이의 적합성에 관한 연구)

  • Purpose : this study was to evaluate the fitness of adjustable dental impression trays on the Koreans : the trays used in the previous study by Kim et al. as part of the dental adjustable tray development project were improved and modified. Material and method: The patterns of tray were made through CAD-CAM process, and a simple silicone-base molds were made from them. The trial products were reproduced by pouring polyurethane into these molds. 30 male students(Wonkwang University, Dental College) and 30 female students (Wonkwang Health Science College, Department of Dental Hygiene) were selected and Reversible hydrocolloid impression materials were used for this study. The fitness of the trays was evaluated by measuring the width and length of impression materials of each measurement sites. Results and conclusion : 1. In adapting the trays inside the mouth, a uniform width of impression material(3 $\sim$ 6mm) was obtained in most sites due to the tooth stops and the inclined planes accommodating the width of the tray 2. The thickness of impression material in the central part of the palate was a mean 9.8mm, which turned out to be somewhat thick. 3. In the mandible. the thickness of the impression material in the lingual side inferior to the contact point of the 1st and 2nd molars was 2.7mm, and the thickness of the material in the lingual side of the rearmost margin was 2.5 mm. The thickness of the impression material of these areas was relatively thin.

  • The purpose of this study is to research how to effectively export the dental prosthetic restoration and the service to the overseas. The study showed the present status of dental prosthetic restoration service in South Korea and some suggested ways to improve the export of the dental prosthetic restoration service to the overseas. The size of the medical service market has been gradually increased because of the globalization and the development of the information-oriented tendency. The medical industry is being perceived as a highly valuable service industry that emphasizes the importance of quality. Through the efforts of the nations attempting to develop the market and create new employment, the medical field is becoming a prospective field of trade. In order to develop the medical industry, the strategies; firstly, medical institutions need to explore overseas markets and strengthen globalization; secondly, medical service should be upgraded through specialization; and finally, specialized management system has to be established and medical management system has to be innovated.

Evaluation of internal adaptation of PMMA 3-unit bridge manufactured by 5-axis milling machine (5축 밀링으로 가공한 PMMA 3본 브릿지의 내면 적합도 평가)

  • Purpose: The purpose of this study was to assess the internal fitness of the PMMA 3-unit bridge that was fabricated with 5-axis milling machine and to verify the clinically allowable values. Methods: For fabrication of the crown bridge in this study, 25-27 abutment teeth were used. The prepare abutment teeth were scanned with a scanner and 3-unit bridge was designed by using design software. Upon the completion of the design, the 3-unit bridge was fabricated by using a PMMA block with 5-axis milling machine. The internal surface of the fabricated 3-unit bridge was scanned by using a scanner and the difference between the 3-unit bridge and the abutment teeth was assessed by merging them together. Results: $RMS{\pm}SD$ values for PRE group, MOL group, and BRI group were $51.2{\pm}18.2$, $44.8{\pm}10.0$, and $52.1{\pm}8.3{\mu}m$, respectively. The mean of the PRE group was bigger than that of the MOL and BRI group; however, statistically significant difference was not found (p>0.05). Conclusion: The PMMA 3-unit bridge that was fabricated with 5-axis milling machine presented stable internal values for each crown and overall internal values were within the range of clinically allowable values.

Evaluation of the marginal and internal gap of metal-ceramic crown fabricated with a selective laser sintering technology: two- and three-dimensional replica techniques

  • PURPOSE. One of the most important factors in evaluating the quality of fixed dental prostheses (FDPs) is their gap. The purpose of this study was to compare the marginal and internal gap of two different metal-ceramic crowns, casting and selective laser sintering (SLS), before and after porcelain firing. Furthermore, this study evaluated whether metal-ceramic crowns made using the SLS have the same clinical acceptability as crowns made by the traditional casting. MATERIALS AND METHODS. The 10 study models were produced using stone. The 20 specimens were produced using the casting and the SLS methods; 10 samples were made in each group. After the core gap measurements, 10 metal-ceramic crowns in each group were finished using the conventional technique of firing porcelain. The gap of the metal-ceramic crowns was measured. The marginal and internal gaps were measured by two-dimensional and three-dimensional replica techniques, respectively. The Wilcoxon signed-rank test, the Wilcoxon rank-sum test and nonparametric ANCOVA were used for statistical analysis (${\alpha}$=.05). RESULTS. In both groups, the gap increased after completion of the metal-ceramic crown compared to the core. In all measured areas, the gap of the metal cores and metal-ceramic crowns produced by the SLS was greater than that of the metal cores and metal-ceramic crowns produced using the casting. Statistically significant differences were found between cast and SLS (metal cores and metal-ceramic crown). CONCLUSION. Although the gap of the FDPs produced by the SLS was greater than that of the FDPs produced by the conventional casting in all measured areas, none exceeded the clinically acceptable range.

Fracture load and marginal fitness of zirconia ceramic coping by design and coloration (유색 및 백색 지르코니아 세라믹 코핑의 코핑 디자인에 따른 파절 하중과 변연 적합성)

  • Purpose: The purpose of this study was to compare the marginal fitness and fracture load of the zirconia copings according to the design with different thickness and coloration. Material and methods: The evaluation was based on 80 zirconia copings. Zirconia copings were fabricated in design with different thicknesses using CAD/CAM system (Everset, KAVO dental GmbH, Biberach, Germany). The designs of copings were divided into four groups. The first group consisted of copings with uniform thickness of 0.3 mm. The thickness in the second group was 0.3 mm on the buccal surface and 0.6 mm on the lingual surface. The third group consisted of coping with uniform thickness of 0.6 mm. The thickness in the fourth group was 0.6 mm on the buccal surface and 1mm on the lingual surface. Each group consisted of 10 colored and 10 uncolored copings. Half of the copings (40) processed with a milling system according to the specific design were sent to be given a color (A3) through saturation in special dye by a manufacturing company. Just after sintering, the marginal discrepancies of copings were measured on the buccal, lingual, mesial and distal surfaces of metal die, under a Video Microscope System (sv-35, Sometech, Seoul, Korea) at a magnification of $\times$ 100. It was remeasured after the adjusting of the inner surface. Next, all copings were luted to the metal dies using reinforced cement {GC FujiCEM (GC Corp. Tokyo, Japan)} and mounted on the testing jig in a Universal Testing Machine (Instron 4467, Norwood, MA, USA). The results were analyzed statistically using the one-way ANOVA test. Results: The obtained results were as follow: 1. The measured value of marginal discrepancy right after sintering was the greatest in the contraction of the buccal area in all groups, except for group I2. 2. There was no significant difference of marginal fitness among the groups in the colored zirconia group (P<.05). 3. When the marginal fitness among the groups in the uncolored zirconia group was considered, group II2 had the smallest marginal discrepancy. 4. When the colored and uncolored groups with the same design were compared, there was a significant difference between I1 and II1 groups. In group 2, 3, and 4, the uncolored zirconia had the greatest marginal fitness (P<.05). 5. After adjustment of inner surface, there was no significant difference in the marginal fitness in all groups when color and design of the zirconia coping were compared. 6. The fracture load of CAD/CAM zirconia copings showed significant difference in group 1, 2, 3, and 4. I4 and II4 had the strongest fracture load. 7. When groups with different color and same design were compared, all colored groups showed greater fracture load (P>.05), with no significance. Conclusion: There was difference in the marginal fitness according to the design and coloration of zirconia copings right after sintering, but it was decided that the copings may well be used clinically if the inner surface are adjusted. The copings should be thick enough for the reinforcement of fracture strength. But considering the esthetics of the visible surfaces (labial and buccal surface), the thickness of copings may be a little thin, without giving any significant effect on the fracture strength. This type of design may be considered when giving priority to preservation of tooth or esthetics.
