가미자운고(加味紫雲膏)가 mouse의 피부화상 치료에 대한 분자생물학적 효과 및 기전연구 (Effects of Gamijaungo on the burn mice model and the study of hematologic, pathologic and molecular mechanism)

  • Objective: The aim of this study was to investigate the wound healing effect of herbal ointment, Gamijaungo, on the burn-induced model. Reports about Gamijaungo on the wound healing effect by local application in mice model or human study have published in the several domestic or internationally, but most are anecdotal and lack solid scientific evidence. Method: We observed the morphologic and histologic changes in the burn-induced mice model. we counted white blood cell and platelet changes. we confirmed VEGF, PI3K and pAkt protein expression by Western blot analysis. Result: In this study, we observed that Gamijaungo showed strong wound healing effects in the morphologic and histologic changes in the burn-induced mice model. Also we found that the significant changes of white blood cell and platelet changes by the treatment of Gamijaungo. In molecular mechanism, we got the strong positive effect by Gamijaungo treatment on angiogenesis, a key process in the formation of the granulation tissue during wound healing. Conclusion: These findings suggest the potential use of Gamijaungo as a therapeutic in thermal burn-induced skin injuries.

  • Research was undertaken to compare the pharmacological activity of Lithospermi radix (LR) reported as an oriental medicine for classical uses. LR contains naphthoquinone pigments : shikonin, acetylshikonin, isobutylshikonin, etc. LR is used for the treatment of excision wound, burn, eczema, blister, scarlatina and septicemia as antifebrile, antidotic and antiphlogistic. Gardeniae fructus (GF) has been used for the treatment to jaundice, hepatic disease, anti-inflammatory and analgesic effects, and it contains crocin, geniposide and its derivatives. The therapeutic effects of burn and excision wound healing from LR & GF hydrogel with $Nano-ATP^{\circledR}$ (GLN) were investigated. To evaluate the therapeutic value of various hydrogels, thermal burn model and excision wound mouse model were used. The burn and wound reduction rate and therapeutic period were measured to calculate the healing extent after 5 experiments. The 2nd degree burn was prepared on hairless mouse back skin and dressing with collagen. The burn and wound reduction rate of GLN hydrogel treated group decreased more rapidly than that of other gel group in animal model. Furthermore therapeutic periods of GLN hydrogel treated group was shorter than that of other gel group. In anti-inflammatory test, GLN hydrogel treated group decreased edema rapidly than that of other gel group. These results suggest that the GLN hydrogel treatment has an therapeutic effect on burn and excision wound healing.

동물모델을 이용한 Hydrogel Patch의 상처치유개선 능력 평가 (Efficacy of Hydrogel Patch in Wound Rat Model)

  • Sodium polyacrylate based hydrogel patch was prepared and its wound healing efficacy in comparison with control groups was evaluated in thermal burn wound ($200^{\circ}C$, 10 sec burn) and excision wound rat model. Cytotoxicity of gamma irradiated (25 kGy) hydrogel patch was investigated in human fibroblasts and showed no significant cytotoxicity. Wound closure rates (H50%) in hydrogel patch group and BAND-$AID^{(R)}$ treated group were faster than that of control group (uncovered open wound). Hydrogel patch showed an enhancement in wound healing process.

Sericin and 4-hexylresorcinol combination ointment accelerates wound healing in the diabetic burn wound model

  • Sericin has been used for the treatment of burn wound. The purpose of this study was to compare the wound healing between sericin plus 4-hexylresorcinol (4HR) ointment (SE+4HR) and base only ointment. Total 12 mice were included in this study. SE+4HR group showed significantly smaller wound size than base only group at 3 wk (P<0.05). Surface temperature was higher in SE+4HR group. In conclusion, SE+4HR group showed better wound healing than base only group.

  • Objectives The present study aimed to investigate the effects of acupuncture and herbal external preparation on wound healing procedure in a deep partial thickness burn model in rats. Methods A total of 40 male Sprague-Dawley rats were allocated into four groups with an equal number in each group: acupuncture treatment group, herbal external perparation group, Silver sulfadiazine dressing group and control group. We describe the effect of acupuncture and herbal external preparation on morphologic and histologic changes, epithelial growth factor (EGF), hematological value of the deep partial thickness burn wound in rats. Results At 21th day after wounding the wound size in acupuncture treat group and herbal external preparation group were decreased more significantly compared to control group. In addition, epidermal regeneration on acupuncture treatment was than other treatment and control group in histological finding. Hematological findings revealed that acupuncture treatment group and herbal external prepartion group was more effective than control group in reducing inflammation response induced by burn. In acupuncture treated group, neutrophil and leukocyte level were significantly decreased compared to other treatment group. Also, this study showed that EGF was obviously expressed in nascent tissue when wounds were treated with the acupuncture and herbal external preparation after injury. In particular, acupuncture treatment group had a significant increase of EGF expression in burn wound healing area when compared with the other treatment groups. Conclusions These findings suggest that acupuncture and herbal external prepration may improve burn wound healing through decreasing inflammatory reaction, increasing tissue regeneration and expression of EGF. Moreover, acupuncture treatment could be more effective in comparison with Silver sulfadiazine dressing.

Acceleration of Wound Healing on Scald Burn Skin Using Irradiation of TDP and Skin Spread of Myrrha

  • The present study was conducted to determine whether skin spread of Myrrha and Tending Diancibo Pu (TDP) irradiation have a remarkable effect on the cell regeneration as well as wound healing following dermal scald burn injury. Burn injury was induced on dorsal surface $(TBSA\;15\~20\%)$ by scald burn in rats. Postburn concentration of serum protein was significantly decreased compared with sham-treated, double treatment with Myrrha and TDP was significantly increased the protein concentration compared with that of burn control. The content of keratinocyte growth factor (KGF) at 48 h is higher than that of at 24 h, and double treatment with Myrrha and TDP was the most effective to increase the production of KGF in all experimental groups. Morphologically, epithelial regeneration and dermal collagen synthesis by fibroblasts were accelerated in Myrrha and TDP treated group compared with bum control at same time postburn. At 48 h after burn, all dermal connective tissues are recovered to new collagen fibers in case of Myrrha and TDP double treated group. The data suggest that double treatment with skin spread of Myrrha and TDP radiation have a remarkable effect of to accelerate cell regeneration and wound healing in case of scald burn skin.

화상 및 창상 치료용 외용제제 개발에 대한 기초 연구 (Studies on the Wound Healing by the PVA-soft hydrogel)

  • A PVA-soft hydrogel, which is a semi-solid form in container, whereas after applying on the skin, it formed a thin layer within a few minutes. In this study, we prepared a novel type PVA-soft hydrogel containing 6-methyluracil as active drug, and the therapeutic value was characterized. To evaluate the therapeutic value of the PVA-soft hydrogel containing drug, various animal models, thermal burn model, incision & excision wound rat model were used. We also measured the wound size and breaking strength to calculate the wound healing extent after single or multiple administration. The wound size of soft hydrogel treated group decreased rapidly than that of control group after multiple dosing in excision wound model. And, the breaking strength of the soft hydrogel treated group was greater than that of control group in incision wound model.

Recombinant human granulocyte macrophage colony-stimulating factor (rhGM-CSF) could accelerate burn wound healing in hamster skin

  • Burns are one of the most devastating forms of trauma and wound healing is a complex and multicellular process, which is executed and regulated by signaling networks involving numerous growth factors, cytokines, and chemokines. Recombinant human granulocyte macrophage colony-stimulating factor (rhGM-CSF) was specifically produced from rice cell culture through use of a recombinant technique in our laboratory. The effect of rhGM-CSF on promotion of deep second-degree burn wound healing on the back skin of a hamster model was evaluated through a randomized and double-blind trial. As macroscopic results, hamster skins of the experimental groups showed earlier recovery by new epidermis than the control groups. Immunohistochemical reactions of proliferating cell nuclear antigen and transforming growth factor-b1, which are indicators of cell proliferation, were more active in the experimental group, compared with the control group. On electron microscopy, basal cells in the epidermis of the experimental group showed oval nuclei, prominent nucleoli, numerous mitochondria and abundant free ribosomes. In addition, fibroblasts contained well-developed rough endoplasmic reticulum with dilated cisternae. Bundles of collagen fibrils filled the extracellular spaces. Particularly, ultrastructural features indicating active metabolism for regeneration of injured skin at 15 days after burn injury, including abundant euchromatin, plentiful free ribosomes, and numerous mitochondria, were observed. These findings suggest that use of rhGM-CSF could result in accelerated deep second-degree burn wound healing in animal models.

Controlled Partial Skin Thickness Burns: Rabbit Ear as a 2nd Degree Burn Wound Model for Studies of Topical Therapy

  • This study was designed to prepare an animal model for partial thickness bum wound which can be employed for testing topical therapy. We first evaluated whether rabbit ear and mouse back skin wound model could differentiate the wound healing process in terms of degree of re epithelialization, required days for complete wound closure, presence of scarring. $2^{nd}$ degree wet bum were prepared on mouse back skin and rabbit ear by applying 5 mL hot water($85{\pm}0.1^{\circ}C$) for 7 see followed by 5 mL ice-cold 0.5% acrynol solution for cooling and disinfecting the inflicted area. After removing the dead epidermis layer at 24 hr, tested dressings were applied for specified time and wound progression was investigated. In mouse model, wound contraction was the primary wound closing mechanism, which is quite different from human wound healing process. In rabbit ear model, epidermal regeneration was the major wound healing process rather than wound contraction and the difference in wound healing property among tested dressings could be clearly demonstrated. A rabbit ear model could differentiate the wound progression among open, occluded and epidermal growth factor(EGF) treated wound. Four sites of circular wound(diameter: 1 cm) on the anterior part of rabbit ear could be employed for the comparative wound healing study. For obtaining reproducible bum wound, degree of bum depth and bum sites should be carefully controlled in addition, employing rabbits of same strain and weight. The result suggests that rabbit ear could be employed as a reliable and human-resembled wound model.

Effect of Curcuma xanthorrhiza Gel on Methicillin-Resistant Staphylococcus aureus-Infected Second-Degree Burn Wound in Rats

  • Methicillin-resistant Staphylococcus aureus (MRSA) infection often complicates burn wounds. Mupirocin is the antibiotic of choice for superficial MRSA infection, and its resistance is on the rise due to its frequent and widespread use. This study aimed to develop and evaluate Curcuma xanthorriza extract (CXE)-containing gel as a topical agent against MRSA-infected second-degree burn wound in rats. CXE was obtained using maceration with 96% ethanol. Xanthorrhizol level, antibacterial, and antioxidant activity were evaluated using a standardized method. In vivo, the wound's healing and bacterial load were evaluated every three days, whereas the histopathology of the wound was examined on day 12 of treatment. One-Way ANOVA and Kruskal-Wallis test were used to analyze the data. In this study, 27.0% and 7.10% of the obtained CXE were xanthorrhizol and curcumin, respectively. Additionally, an IC50 of 64.27 ppm was shown in antioxidant activity measurement, and MIC against MRSA was 5 mg/ml. Treatment with CXE-containing gels showed a significant reduction in bacterial load and proliferation of connective tissue in a dose-dependent manner. In conclusion, CXE-containing gel showed a greater reduction of bacterial load and more advanced wound healing phase than mupirocin.
