조직공학적 연골재생을 위한 In Vitro 환경에서의 탈미네랄화 골분용액을 함유한 PLGA 지지체의 효과 (Effect of PLGA Scaffold Containing Demineralized Bone Solution for Articular Cartilage Tissue Engineering: In Vitro Test)

  • 본 연구팀은 탈미네랄화 골분(DBP)이라는 천연재료를 졸(sol)화시켜 poly(lactide-co-glycolide) (PLGA)에 함침시킨 지지체를 개발하였다. DBP를 함침시킨 PLGA 지지체 상의 세포증식과 모폴로지를 평가하기 위해 MTT 분석과 SEM을 측정하였다. 또한 sGAG와 콜라겐 함량 측정과 파종된 연골 세포의 표현형 유지에 미치는 영향을 확인하였다. 그 결과 PLGA에 DBP를 함침시킨 지지체가 PLGA 지지체보다 높은 세포 증식률을 보였다. 또한 파종된 연골세포의 표현형 유지에도 긍정적인 영향을 미치는 것을 확인하였다. 이번 연구 결과를 토대로 PLGA에 DBP를 이용한 용액을 함침시킴으로써 DBP내의 성장인자와의 상호작용을 통해 연골세포의 성장에 긍정적 영향을 미쳐 안정되게 연골을 조직화할 수 있는 연골조직공학 지지체로 적합할 것으로 예상된다.

관절 연골 손상 정도에 따른 반월상 연골 부분절제술의 결과 (The Results of Partial Meniscectomy according to the Grade of the Articular Cartilage Injury)

  • 목적: 슬관절의 퇴행성 관절 연골 손상과 동반된 내측 반월상 연골 파열에서, 관절 연골 손상의 정도를 확인하고 이에 따른 내측 반월상 연골 부분절제술의 임상적 결과를 알아보고자 하였다. 대상 및 방법: 2001년 1월부터 2004년 12월까지 슬관절의 퇴행성 관절염과 동반된 내측 반월상 연골 파열에서 관절경적 부분절제술을 시행 후 3년 이상 추시가 가능하였던 48예를 대상으로 하였다. 남자가 6예, 여자가 42예였으며, 나이는 평균 55.7세(40~78세), 관절경 소견상 Outerbridge 분류 기준으로 두 군으로 나누었으며 제 1군은 grade I과 II를, 제 2군은 grade III와 IV를 포함하였다. 제 1군이 30예였고, 제 2군이 18예였다. 임상 결과는 Tapper & Hoover분류 및 Lysholm knee score를 측정하여 평가하였다. 술전과 최종 추시 양슬관절 기립 전후면 사진에서 내측 관절 간격을 측정하였다. 결과: Tapper & Hoover분류에 따른 평가에서 우수가 28예, 양호 7예, 보통 8예, 불량 5예였다. 제 1군은 우수 25예, 양호 2예, 보통 2예, 불량이 1예다. 반면에 제 2군에서는 우수 3예, 양호 5예, 보통 6예, 불량이 4예였다. 기능적 평가상 Lysholm 점수는 1군에서는 술전 평균 62.4점에서 술후 평균 94.0점으로 향상되었으며, 2군에서는 술전 평균 58.0점에서 술후 평균 77.9점으로 향상되었다. 술후 점수의 비교에서 1군이 2군에 비하여 우수하였다. 기립 전후면 단순 방사선 사진상 관절 간격의 감소는 유의하지 않았다. 결론: 관절염과 동반된 반월상 연골 파열에서 관절경적 반월상연골 부분절제술은 증세 호전을 기대할 수 있는 효과적인 치료법이었다. 그러나 관절 연골의 손상이 Outerbridge III나 IV인 경우에는 만족스런 결과를 얻기가 어려웠다. Outerbridge III 및 IV의 관절연골 손상에서는 반월상 연골 부분절제술이 증세 호전이 만족스럽지는 않았으나 관절연골손상을 악화시키지는 않았다.

  • Arthritis may be broadly classified as degenerative - related to defects in cartilage and other joint constituents, often age-associated - or inflammatory disease. Inflammatory arthritis called as rheumatoid arthritis (RA) is a chronic inflammatory arthropathy and characterized by a destructive arthritis. RA encompasses infectious arthritis, arthritis caused by intra-articular deposits of crystalline material (gout), syndromes associated with genetic defects (familial Mediterranean fever), and the immune-mediated inflammatory arthropathy. Degenerative arthritis called as osteoarthritis (OA), which is most frequently occurring, causes degenerative figures of knee, waist and knuckle, and accompanies severe pain around the cartilage. Also, it may cause morning stiffness, gelling effect, tenderness, bone swelling, crepitus, and motion disorders.

Current perspectives in stem cell therapies for osteoarthritis of the knee

  • Mesenchymal stem cells (MSCs) are emerging as an attractive option for osteoarthritis (OA) of the knee joint, due to their marked disease-modifying ability and chondrogenic potential. MSCs can be isolated from various organ tissues, such as bone marrow, adipose tissue, synovium, umbilical cord blood, and articular cartilage with similar phenotypic characteristics but different proliferation and differentiation potentials. They can be differentiated into a variety of connective tissues such as bone, adipose tissue, cartilage, intervertebral discs, ligaments, and muscles. Although several studies have reported on the clinical efficacy of MSCs in knee OA, the results lack consistency. Furthermore, there is no consensus regarding the proper cell dosage and application method to achieve the optimal effect of stem cells. Therefore, the purpose of this study is to review the characteristics of various type of stem cells in knee OA, especially MSCs. Moreover, we summarize the clinical issues faced during the application of MSCs.

개 슬개골 탈구 교정술에 있어서 활차구 성형술의 평가 (Evaluation of trochleoplasty for correction of patellar luxation in dogs)

  • This study was performed to investigate the best surgical technique for normal stifle mobility by comparison with clinical signs and histopathological changes of articular cartilage after femoral trochleoplasty, trochlear chondroplasty and trochlear wedge recession. Twelve small mixed dogs who had grade I or II medial patellar luxation were used. The days that the dog had partial and full weight bearing were checked and histopathological changes 49 and 90 days after surgery were observed. The dogs had partial weight bearing in 3~6 days postoperatively and full weight bearing in 20~24 days. After femoral trochlepoplasty, the articular surface was irregular and rough, but smooth after trochlear chondroplasty and trochlear wedge recession. Histopathologic examination performed on the 49th and 90th days after femoral trochleoplasty revealed that articular surface was occupied by fibrocartilage but the hyaline cartilages are preserved after trochlear chondroplasty and trochlear wedge recession. In trochlear wedge recession, the wedges of all cases were firmly attached to underlying subchondral bone which were indicating complete healing. The results of this study suggested that the trochlear wedge recession was the best chondroplasty for correcting patellar luxation.

슬관절 부분결손에 대한 혈관부착 비골근위 관절면을 이용한 재건술 (Partial Knee Joint Defect Reconstruction with Vascularized Proximal Fibular Articular Surface)

  • It has been very difficult to managing partial joint defect in any etiologies, especially in children. Unicondylar defect of the tibial condyle in children reconstructed with proximal fibular head with articular cartilage from 1995. Two kinds of transfering methods were used, peroneal artery pedicled ipsilateral fibula head transposition to defective lateral tibial condyle defect that revealed poor prognosis with gradual absorption of transposed fibular epiphysis. Free vascularized fibular head transplantation with microvascular anastomosis underwent in the case with medial condyle defect of tibia which revealed very satisfactory results. Author can conclude with these clinical experiences: 1. Tranposition without epiphyseal vesssels intact is not sufficient in fibular head osteochondral transplantation in reconstruction of tibial condyle defect. That means peroneal arterial vascular pedicle is not enough for transplanted proximal epiphysis maintains its function on articular surface and growth activity in children. 2. The anterior recurrent tibial artery is one of the most important and easy to utilizing vessel in proximal fibular epiphyseal transplantation. 3. Free vascularized fibular head transplantation is hopeful method in reconstruction of the knee joint in the patient with partial joint defect which has no effective solution in conventional methods.

꽃송이버섯 추출물의 Monosodium Iodoacetate로 유도된 골관절염 억제 효과 (Inhibitory Effect of Sparassis crispa (Wulf.) Extract on Monosodium Iodoacetate Induced Osteoarthritis)

  • Sparassis crispa (Wulf.) is an edible/medicinal mushroom and has been reported to biological activities such as antitumor, anti-angiogenesis, antioxidant and wound healing. However, there have not been many researches on osteoarthritis of S. crispa. The aim of this study was to investigate the effects of S. crispa extract on rats with osteoarthritis induced by MIA. Osteoarthritis is a gradually developmental disease that early stage, causes joint stiffness and complains of joint pain. In addition, it gives rise to edema and hypo-function. The results of this study, S. crispa extract effectively inhibited ROS production, increased the production of antioxidant protein SOD and catalase in knee joint cartilage tissue. In addition, S. crispa extract inhibited the expression of pro-inflammatory cytokines and enzymes such as NOX4 and $P47^{phox}$, which are involved in the expression of COX-2, iNOS and the production of ROS. Also, S. crispa extract inhibited the destruction of synovial tissue, cartilage tissue and proteoglycans in articular cartilage in rats.

우슬과 초음파가 퇴행성관절염 백서의 관절 연골에 미치는 영향 (Effect of Achyrantis Radixs and Ultrasound in Osteoarthritis Rats Articular Cartilage)

  • Osteoarthritis(OA) diseases are characterized by joint pain, tenderness, limitation of movement, crepitus, occasional effusion, and variable degrees of inflammation without systemic effects. We investigated the effects of Achyrantis radixs cream treatment and low intensity ultrasound in monosodium iodoacetate(MIA) induced experimental osteoarthritis rat. Sprague-Dawley 40 rats of 7-8 weeks, weight $250\;{\pm}\;50$ g were divided into four groups including the control group and ostoarthritis group(30 rats). Histopathological examination, Mankin's score, and immunohistochemical were performed. Histological findings in control group that are similar to those observed in human osteoarthritis, such as disorganization of chondrocytes, erosion and fibrillation of cartilage surface, and subchondral bone exposure. Safranin O-fast green staining revealed that marked diffuse reduction of proteoglycans and chondrocyte treated with MIA. The Mankin's score were closely correlated to the grade of histological findings. The level of Bax and caspase-3 expression decreased experimental groups. This study shows that a Acyranthes Radix cream treatment and low intensity ultrasound exerts a beneficial influence on the severity of chondral lesion in osteoarthritis rats. This treatments could related to a reduced level of chondrocyte apoptosis through anti-apoptotoc capacities of MIA-induced apoptotic protein overexpression.

Biomimetics of the extracellular matrix: an integrated three-dimensional fiber-hydrogel composite for cartilage tissue engineering

  • The native extracellular matrix (ECM) consists of an integrated fibrous protein network and proteoglycan-based ground (hydrogel) substance. We designed a novel electrospinning technique to engineer a three dimensional fiber-hydrogel composite that mimics the native ECM structure, is injectable, and has practical macroscale dimensions for clinically relevant tissue defects. In a model system of articular cartilage tissue engineering, the fiber-hydrogel composites enhanced the biological response of adult stem cells, with dynamic mechanical stimulation resulting in near native levels of extracellular matrix. This technology platform was expanded through structural and biochemical modification of the fibers including hydrophilic fibers containing chondroitin sulfate, a significant component of endogenous tissues, and hydrophobic fibers containing ECM microparticles.
