  • For the successful placement of dental implants, adequate alveolar bone height and width are required. Alveolar distraction osteogenesis is an effective method that resolves insufficient alveolar bone height for dental implant placement, and thus has been clinically applied with satisfactory results. But, minor and major problems may occur during the treatment. In the following report, we studied for such problematic cases. The problems are as follows: 1) sharp edges of the transport segment, 2) infection, 3) soft tissue dehiscence, 4) limitation of distraction, 5) numbness, 6) insufficient bone formation. But, most of them were answered by simple solutions and did not jeopardize the final outcomes. Distraction osteogenesis can be considered a safe and predictable procedure for lengthening the alveolar bone.

CLINICAL STUDY ON KOREAN POSTERIOR MAXILLAE RELATED TO DENTAL IMPLANT TREATMENT (치과임플란트 치료와 관련된 상악구치부의 임상적 연구)

  • Purpose of study: The purpose of this study was to provide adequate diagnostic guideline for the maxillary sinuses prior to dental implant treatment for edentulous posterior maxillary areas. For this purpose, our procedure involves the estimation of the remaining alveolar bone height, the examination of the anatomical variation in the maxillary sinuses (e.g. sinus septum), and the evaluation of the incidence of preoperative pathological conditions in the maxillary sinuses. Materials and Methods: We selected 189 patients to undergo computerized tomography (CT) in order to account for the posterior maxillary anatomy found in patients of Korean ethnicity. We evaluated the following using Dentascan software: Remaining alveolar bone height, incidence of sinus septum, and rate of preoperative pathologic conditions in the maxillary sinus. The average amount of remaining alveolar bone height was analyzed using the student's t-test for differences according to anatomical site, and the ANOVA was used for the differences according to age group with the level of significance set at 0.05. Results: Alveolar bone heights of upper first premolar, second premolar, first molar, and second molar was 12.24 mm, 10.37 mm, 7.16 mm, and 7.15 mm, respectively with statistical significance (P < 0.05). Incidence of sinus septum as an anatomic variation was 17 out of 189 cases (9.0%). Incidence of mucosal thickening as a pathologic variation was 82 out of 189 cases (43.4%). Conclusion: In treatment planning of posterior maxillary edentulous area of Koreans, the consideration of augmentation surgery for maxillary sinus is required in maxillary molar area before dental implant installation, and preoperative screening of the asymptomatic maxillary sinuses can be regarded as a reasonable preoperative procedure in the planning of dental implant treatment on the posterior maxillary edentulous area.

THE STUDY ON RIDGE PRESERVATION FOR IMPLANT SITE DEVELOPMENT (임플란트 식립을 위한 치조제 보존술에 관한 연구)

  • Purpose : The aim of this study was to investigate healed bovine bone particles ($Bio-Oss^{(R)}$) and absorbable collagen sponge ($CollaPlug^{(R)}$) applied extraction socket site at 4-6 months' post-extraction. Material and methods : From August, 2004 to October, 2005, 17 sockets in 5 adult patients were selected out of the patients whose received ridge preservation using bovine bone particles and absorbable collagen sponges at Dept. of oral and maxillofacial surgery in Samsung Medical Center. There were 5 male patients, ages 30 to 58 years. Immediate postoperation and 4-6 months after operation study models were compared to evaluate the ridge dimension by measuring vertical height and horizontal width of alveolar ridge. Results : The measurements at 4-6 months revealed, in the ridge dimension, a loss of vertical height of 0.91${\pm}$0.40mm and horizontal width of 1.25${\pm}$0.58mm. There was no adverse reaction. Conclusion : This study suggests that treatment of extraction sockets with graft materials and collagen sponges is valuable in preserving alveolar bone in extraction sockets and preventing alveolar ridges defects.

RECONSTRUCTION OF ALVEOLAR CLEFTS WITH ILIAC CANCELLOUS PARTICULATE OR BLOCK BONE GRAFTS : A COMPARATIVE STUDY (장골의 해면 입자골 또는 블럭골을 이용한 치조열의 재건에 관한 비교 연구)

  • Objective : The objective of this study was to determine which forms of iliac cancellous bone grafts better restore alveolar clefts. Study design : Forty consecutive patients who required a unilateral alveolar cleft graft were studied. Group I (20 patients) had reconstruction with iliac cancellous particulate bone grafts and group II (20 patients) had reconstruction with iliac cancellous block bone grafts. The two groups were evaluated radiographically and clinically. Results : The group with the block bone grafts showed less postoperative problems and better incorporation of the bone graft than the group with the particulate grafts. Conclusion : Surgical reconstruction of alveolar process defects in patients with alveolar cleft using iliac cancellous block bone is a more reliable method than particulate bone grafts both for closing the oronasal fistula and for building interalveolar septal height.

VERTICAL DISTRACTION OF ALVEOLAR BONE FOR PLACEMENT OF DENTAL IMPLANT (치과 임플란트 식립을 위한 치조골의 수직적 신장술)

  • Adequate alveolar bone height and width are required for the successful placement of dental implants. Conventional therapeutic regimens for alveolar atrophy are bone grafts or augmentation using allografts and membrane (GBR). Conventional graft techniques have some limitations and complications such as infection, soft tissue problem and high resorption rate. Recently, distraction osteogenesis of alveolar bone is considered as a new alternative for ridge augmentation. Distraction osteogenesis was originally defined and popularized by Ilizarov for lengthening of long bone. Some clinicians have tried to apply distraction osteogenesis in treatment of maxillofacial discrepancies. It was also used to augment alveolar bone. Cologne study group successfully applied the technique for augmentation of alveolar bone and designed several miniplate-distractor systems fabricated by Martin Medizintechnik GmbH in Germany. Vertical distraction of alveolar bone was successfully completed in 104 patients with miniplate-distractor systems. The mean distance of distraction was 10.2mm (range: 6-15 mm) and the mean length of segment was 45 mm (range: 6-127 mm). 162 dental implants in 54 patients were placed immediately or 4 weeks later after removal of the distractor. The results of our study show that vertical distraction of alveolar bone is an effective and reliable technique to restore alveolar atrophy and alveolar vertical defect caused by trauma or tumor.

Comparative study on alveolar bone height of pantomography and multi planar reformatted computed tomography (파노라마방사선사진과 다면상재구성 CT상의 치조골 높이 계측에 대한 비교 연구)

  • Purpose: To compare alveolar bony height of pantomograph with bony height of thin slice, multiplanar reformatted (MPR) Computed Tomograph. Materials and Methods : Panoramic radiograms of 12 young adult patients had been taken by one radologic technitian and the measurements were corrected by magnification ratio (1.20). The slice thickness of Multi-detector Computed Tomography (CT) was at least 1mm for the accuracy. The raw CT datas were imported into the V-works 4.0 (CyberMed Corp., Seoul, Korea) and transformed to MPR images. Pantomographic measurements of alveolar bone were compared to CT values by average mean bony height measurements for the accuracy. Inter-, and Intra-observer variability was evaluated. Results : There was no significant differences between height measurement of pantomography and that of CT (P>0.05). There were no significant differences in either inter-or intra-observer measurements (P>0.05). Conclusion : Pantomography showed relatively high accuracy and precision in measuring alveolar bony height.

A 2-plane micro-computed tomographic alveolar bone measurement approach in mice

  • Purpose: This study introduces a standardized 2-plane approach using 8 landmarks to assess alveolar bone levels in mice using micro-computed tomography. Materials and Methods: Bone level differences were described as distance from the cemento-enamel junction (CEJ) to alveolar bone crest (ABC) and as percentages of vertical bone height and vertical bone loss, comparing mice infected with Porphyromonas gingivalis (Pg) to controls. Eight measurements were obtained per tooth: 2 in the sagittal plane (mesial and distal) and 6 in the coronal plane (mesiobuccal, middle-buccal, distobuccal, mesiolingual, middle-lingual, and distolingual). Results: Significant differences in the CEJ-to-ABC distance between Pg-infected mice and controls were found in the coronal plane (middle-lingual, mesiobuccal, and distolingual for the first molar; and mesiobuccal, middle-buccal, and distolingual for the second molar). In the sagittal plane, the distal measurement of the second molar was different. The middle-buccal, mesiobuccal, and distolingual sites of the first and second molars showed vertical bone loss relative to controls; the second molar middle-lingual site was also different. In the sagittal plane, the mesial sites of the first and second molars and the distal site of the second molar showed loss. Significantly different vertical bone height percentages were found for the mesial and distal sites of the second molar (sagittal plane) and the middle-lingual and distolingual sites of the first molar(coronal plane). Conclusion: A reliable, standardized technique for linear periodontal assessments in mice is described. Alveolar bone loss occurred mostly on the lingual surface of the coronal plane, which is often omitted in studies.

A STUDY ABOUT ALVEOLAR CREST BONE HEIGHT BEFORE AND AFTER ORTHODONTIC TREATMENT BY USING BITEWING FILM (교익사진을 이용한 교정치료 전후의 치조골 높이 변화에 관한 연구)

  • Alveolar bone grows with development of tooth germs and roots; bone deposition occurs with tooth eruption. Bone components undergoes processes of resorption and deposition, and when the balance between them is disrupted, decrease in alveolar bone height or excessive bone deposition result. It has been hon that repositioning of teeth through orthodontic treatment can cause alveolar bone resorption which result in decreased alveolar bone height, and there have been many studies to evaluate such effects. X-ray films that could be replicated and standardized were chosen in clinical studies, and among them, bitewing films were used for objective evaluation of changes in alveolar bone level. Twenty subjects, 10 to 13-year- old (average 12.2) children with Cl I molar key, healthy oral condition, no congenital missing, no periodontal disease, and pre-and post-orthodontic bitewing films, were randomly selected for comparison of alveolar bone heights. Amounts of tooth and changes in alveolar bone heights were analyzed. The following results were obtained: 1. Amount of tooth movement in canine, premolar, and molar regions, changes in tooth axis, and changes in alveolar bone heights were measured, and the mean and median values were obtained. 2. When pre-and post-orthodontic alveolar bone levels were compared, larger changes were noticed in maxilla than mandible. 3. When mesio-distally compared, larger changes were observed in the distal sides of 3D3 and 4M3, mesial sides of 4M3 and 4D3, distal sides of 4D3 and 5M3, mesial sides of 5M3 and 5D3, md distal sides of 5D3 and 6M3. 4. When the amounts of tooth movements(TX, TY)and changes in tooth axis(A) were compared,34TX, 34TY, 34A of both sides in maxilla were greater, iud changes in alveolar bone level were greater than any other region.

Evaluation of augmented alveolar bone with vertical alveolar distraction osteogenesis and implant installation (수직 치조골 신장술 후 증대된 치조골과 임플란트의 예후 평가)

  • Introduction: The purpose of this study was to evaluate the clinical result of vertical alveolar distraction, especially the distracted alveolar bone and installed implants. Materials and Methods: Twenty-one patients who have been received the vertical alveolar distraction and implant installation on 22 areas (3 maxilla and 19 mandible) using intraoral alveolar distraction device were examined. After consolidation period of 3-4 months, distraction devices were removed and 91 implants were installed in the distracted alveolar bone. The distracted bone and implants were evaluated clinically and radiographically. Results: Mean height of distracted alveolar bone was $7.5{\pm}3.2$ mm (range: 2.5-15.0 mm). Mean follow-up period after completion of the distraction was 3.1 years (range: 1.4-11.5 years). Mean resorption of distracted alveolar bone was $1.6{\pm}1.8$ mm. The success and survival rates of implants was 95.3% and 100%, respectively. Conclusion: Results of this study indicate that vertical alveolar distraction procedure is a useful and stable method for alveolar ridge augmentation and implantation.

A Prognosis Evaluation after Iliac Bone Graft in Cleft Alveolus Patients (치조열 환자의 장골이식술 후 예후 평가)

  • Alveolar cleft exists in 75% of cleft patients, In alveolar cleft patients, alar base is widening, palatal fistular formation, maxillary growth disturbance & tooth loss of adjacent area is raised, Alveolar bone grafting, especially iliac bone grafting, is a general treatment method. As operation timing, bone grafting is classified with primary, early secondary, secondary, & late secondary, Here we report cleft width, marginal bone height, bone resorption rate, grafted shape & bone densities after secondary iliac bone grafting was done in the Dept. of oral and maxillofacial surgery of chonbuk national university hospital. We compared cleft width to bone resorption rate and grafted shape. Also, alveolar bone densities of grafted and contralateral site was compared with Emago 3 package? (Oral Diagonostic System, The Netherlands), The data obtained were analyzed using Spearman's rho coefficients and sign test with SPSS for window, The results were obtained as follows. 1. As alveolar cleft width is increase, bone resorption rate is, too. This relation showed significant difference(P<.01). 2, In proximal & distal area, alvolar cleft width and bone graft contour after bone grafting had a reverse proportional difference. It was not significant difference(P>.05). 3. After 3 month, in bone density results by using Emago 3 package? with periapical standard view, occlusal view & panoramic view, differences between grafted bone and alveolar bone of contralateral site didn't show a significant difference(P>.05). Thus, differences of bone densities in the alveolar bones didn't exist.
