임플란트 상부 보철물 제작 시 치은연하 변연을 가지는 맞춤형 지대주의 인상채득을 위해 전통적으로 치은 압배술이 실시되어 왔다. 하지만 치은 압배술은 치은 퇴축 등의 부작용이 있으며 출혈은 부정확한 인상채득의 원인이 될 수 있다. 본 증례에서는 이러한 문제점들을 해결하기 위해 보철물을 제작할 때 중첩 가능하도록 제작된 맞춤형 지대주를 이용하는 새로운 방법을 적용하였다. 지대주를 임플란트에 연결하기 전 형상데이터를 저장하고, 임플란트에 연결 후 치은 압배 과정 없이 광학 인상채득한 뒤 두 데이터를 중첩하여 상부보철물을 제작하였다. 치은 연하 변연을 가지는 임플란트 보철물 제작 시 심미성을 만족시키면서 기존의 인상채득 방식보다 빠르고 간편하며 우수한 변연 및 내면적합도를 보였기에 임상적 효용성에 대해 보고하고자 한다.
목적: 본 연구는 치과용 모형 스캐너를 이용하여 단일 전장관과 3본 고정성의치 모형을 스캔하는 과정에서 지대치의 중첩 과정이 최종 가상 모형에 미치는 영향을 알아보려 하였다. 재료 및 방법: 평가를 위해 단일 전장관과 3본 고정성의치를 위한 석고 모형을 제작하였다. 그리고 Pindex system을 이용하여 핀 작업된 모형을 제작하였다. 기준 스캔 데이터(CAD Reference Model)와 실험 스캔 데이터(CAD Test Model)를 획득하기 위해서 치과용 모형 스캐너(E1, 3Shape A/S, Copenhagen, Denmark)를 이용했다. 기준 스캔 데이터는 지대치를 분할한 후 탈착하지 않고 스캔하였다. 그리고 실험 스캔 데이터는 분할된 지대치를 분리하여 스캔 후, 기준 스캔 데이터에 중첩되었다(n = 20). 마지막으로 3차원 검사 소프트웨어(release 2018.0.0; Geomagic control X; 3D Systems)를 이용하여 root mean square (RMS)를 분석하였고, 통계 분석은 Mann-Whitney U test를 사용하였다 (${\alpha}=.05$). 결과: 단일 전장관 지대치의 RMS 평균은 $10.93{\mu}m$, 3본 고정성의치 지대치의 RMS 평균은 $6.9{\mu}m$가 나왔다. 두 그룹의 RMS 평균값은 통계적으로 유의한 차이를 보여줬다 (P < .001). 또한 두 그룹의 양(positive)과 음(negative)의 오류는 단일 전장관 지대치는 $9.83{\mu}m$, $-6.79{\mu}m$, 3본 고정성의치 지대치는 $6.22{\mu}m$, $-3.3{\mu}m$의 평균값이 나왔다. 두 그룹의 양과 음의 오류 평균값은 통계적으로 모두 3본 고정성의치 지대치가 단일 전장관 지대치보다 통계적으로 유의하게 낮은 값을 보여주었다 (P < .001). 결론: 핀 작업된 모형의 스캔 과정에서 지대치의 개수가 증가하여도 지대치의 중첩에 의한 오류는 증가하지 않았다. 또한 단일 전장관 지대치에서 유의하게 높은 오류를 보였지만 임상적으로 허용 가능한 스캔 정확도의 범위에 있다.
진단, 수술, 인상, 보철물 제작 등 임플란트 치료 전 과정에서 디지털 기술이 활용되고 있다. 본 증례에서는 디지털 수술 가이드를 이용하여 합병증 없이 상악동 거상술을 시행하고, 계획된 위치에 임플란트를 식립하였다. 골유착을 위한 치유 기간 이후, CAD-CAM(Computer-aided design/Computer-aided Manufacturing)으로 맞춤형 지대주 및 임시 보철물을 제작하여 장착하고, 환자의 적응도와 교합을 평가하였다. 임시 보철물상에서 교합 변화가 관찰되어, 광중합형 컴포지트 레진으로 수리하였다. 최종 보철물 제작 시, 지대주의 수직 침하, 임시 보철물의 형태와 적응된 교합 관계를 반영하기 위해, 이중 스캔과 지대주 수준의 디지털 인상을 채득하였다. 치은 압배 없이 치은 연하 변연을 인기하기 위해, CAD-CAM 소프트웨어상에서 라이브러리화된 지대주 데이터를 중첩하고, 지르코니아 최종보철물 제작하여 장착하였다. 구치부 임플란트 수복 시, 디지털 시스템을 이용하여 전통적인 방법에서 겪는 어려움을 줄이고, 수술부터 보철물 제작까지 효율적인 치료 과정과 안정적이고 예지성 있는 결과를 얻어 보고하는 바이다.
An absolutely passive fit at the interface with the superstructure and the abutment cylinders is need for implant longevity. In this study, a method of cast framework correction using spark erosion technology was evaluated. Electric discharge machining(EDM) is a process that uses electrical discharges, or sparks, to machine metal, The surface being machined is bombared with high-intensity electrical energy pulses that gradually melt away the stock until the desired configuration is obtained. Master model was fabricated by using metal block impression analogue, which fired 5 Branemark squared impression coping in an arc shaped metal block. Then framework using 4mm standard gold cylinder and type IV gold alloy was fabricated. In order to measure the fit of the framework we used both contact and noncontact coordinate measurement machine and data was processed by computer program. After superimposition of gold framework and master model numeric image data the distances between gold cylinder bearing surfaces and abutment replica bearing surfaces, and the angle deviations between gold cylinder and abutment replica centroid points were calculated. The results were as follows 1 The total mean distance (p<0.01) and standard deviation (p<0.001) between the gold cylinders and abutment bearing surfaces were significantly decreased after EDM 2. The total mean of maximum distances was significantly decreased after EDM (p<0.05). 3. After EDM, the mean angle deviation between centroid points was decreased.
Purpose: The purpose of this study was to assess the fitness of anterior and posterior interim crowns fabricated by three different additive manufacturing technologies. Methods: The working model was digitized, and single crowns (maxillary right central incisor and maxillary right first molar) were designed using computer-aided design software (DentalCad 2.2; exocad). On each abutment, interim crowns (n=60) were fabricated using three types of additive manufacturing technologies. Then, the abutment appearance and internal scan data of the interim crown was obtained using an intraoral scanner. The fitness of the interim crowns were evaluated by using the superimposition of the three-dimensional scan data (Geomagic Control X; 3D Systems). The one-way analysis of variance and Tukey posterior test were used to compare the results among groups (α=0.05). Results: A significant difference was found in the fitness of the interim crowns according to the type of additive manufacturing technology (p<0.05). The posterior interim crown showed smaller root mean square value than the anterior interim crown. Conclusion: Since the fitness of the posterior interim crown produced by three types of additive manufacturing technology were all within clinically acceptable range (<120 ㎛), it can be sufficiently used for the fabrication of interim crowns.
Purpose: The purpose of this study is to evaluate the discrepancy of scan process in dental intra oral scanner by comparing model scanner and anticipate possibility to introduce intra oral scan technique. Methods: 3D superimposition test was conducted to compare the scan discrepancy. The scanners used in this study are the e-oral scanner, the D750 model scanner, and the high precision CMM(3D Coordinate Measuring Machine). The standard of accuracy verification is ISO 5725-1; trueness and precision. Master model was manufactured by dental stone and scanned 5 times by intra oral, model scanner. Reference data was scanned 5 times by high accuracy CMM to evaluate the trueness. Results: Trueness of D750 scanner were $7.4{\mu}m$$5.1{\mu}m$$6.8{\mu}m$ at an abutment, an occluasal, a specific area. and trueness of e-scanner were $20.2{\mu}m$$27.4{\mu}m$$37.8{\mu}m$ at an abutment, an occluasal, a specific area. Precision of D750 scanner was $7.04{\mu}m$, e-scanner was $15.95{\mu}m$. Conclusion: When conducting in vitro test, The mean difference of trueness between e-scanner and D750 were $12.8{\mu}m$ at an abutment area, $22.3{\mu}m$ at an occlusal area, $31.0{\mu}m$ at a specific area and $8.91{\mu}m$ in precision. The scan discrepancies are within the range of clinical acceptance.
구강 스캐너를 이용한 디지털 인상과 CAD-CAM (Computer-aided design-computeraided manufacturing) 기술은 점차 발전하고 있다. 전통적인 인상 채득, 작업모형의 제작, 왁스 납형 제작 및 주조의 복잡한 과정이 단축되었으며 환자의 방문 횟수도 줄일 수 있게 되었다. 구강 스캐너 기술의 발전으로 디지털 인상의 정밀도와 정확성이 향상되었으며, 그 적응증은 보다 광범위한 부위의 고정성 치과보철물의 수복으로 점차 확대되어지고 있다. 본 증례 보고에서는 광범위한 부위의 고정성 임플란트 보철물의 수복을 위하여, 컴퓨터로 계획하고 가이드 수술용 템플레이트로 완전히 가이드된 수술을 하고, 즉시/조기 임플란트 보철물을 장착하였으며, 임시 보철물에서 최종 보철물로 전환하는 과정에서 구강스캐너의 지대주중첩 알고리즘을 활용하였다. 임플란트 수술 당일 획득한 구강스캔으로 맞춤형 지대주를 포함한 임시 보철물을 제작하여 활용하였으며, 최종 보철물은 임시 보철물에서의 맞춤형 지대주를 낀 채로 구강스캔하여 제작되었다. 이 과정에서 임시 보철물 장착 전에 미리 스캔해서 라이브러리화한 맞춤형 지대주 데이터를 구강스캐너 소프트웨어 '지대주 자동중첩 기능'으로 최종 디지털 인상에 자동적으로 매칭하였고, 치은연하마진인 부분도 지대주를 탈거하지 않고, 치은압배사 없이 정밀하게 획득할 수 있었다. 구강스캐너와 소프트웨어의 다양한 기술을 응용하여 임플란트 치료 과정을 디지털 워크플로우로 변화시킴으로써, 환자 불편감 및 치료 시간을 단축하였으며, 환자와 술자에게 모두 이롭고 예지성 있는 치료가 가능하였다.
최근 CAD-CAM과 지르코니아를 이용한 보철물의 제작이 증가하고 있으며 구강스캐너의 발달로 임상 및 기공과정이 단순화되고 있다. 균열치 혹은 치아의 일부가 파절된 경우, 구강스캐너를 사용하여 치아 삭제 전에 치아의 형태를 미리 스캔하고, 추후 수복물 제작 시 중첩을 통해 단일체 지르코니아 전장관을 제작하면 원래 환자의 자연치 형태와 교합을 재현할 수 있다. 본 증례에서는 균열 및 파절된 치아에서 구강 스캐너, CAD-CAM 및 단일체 지르코니아 전장관을 사용하여 삭제 전 치아의 형태와 교합을 재현한 수복물을 제작하였으며, 교합조정을 최소화할 수 있었다. 또한 임상적으로 기능적이고 심미적인 결과를 얻을 수 있었다.
Purpose: To quantify the effect of the crystallization process on lithium disilicate ceramic crowns that are fabricated using a computer-aided design/computer-aided manufacturing(CAD/CAM) system, and to determine whether they are clinically acceptable by comparing values before and after the crystallization process. Methods: The maxillary first molar was selected as the abutment for the experiments. Ten working models were prepared. Marginal and internal gap of 4 groups of lithium disilicate crowns(n=10) fabricated with conventional method. Comparison was performed using the silicone replica technique and 3D superimposition analysis. The marginal and internal gaps of the restoration were measured before and after the crystallization process of this prosthesis. The average value of each part(the average of values measured before and after the crystallization) was statistically analyzed using paired t-test(α=0.05). Results: The results from the second phase of this research, which compared the average value of the gap between the marginal and internal fits of the lithium disilicate single crown before and after the crystallization process, indicated that the marginal gap was larger and the internal gap was smaller after the crystallization process, and this difference was statistically significant (P<0.05) in all the parts evaluated. Conclusion: While the shrinkage that occurs during crystallization does affect the marginal and internal fit of the prosthesis, it cannot be concluded to be a major effect because the resultant distortion was within the clinically acceptable range.

PURPOSE. The purpose of this study was to evaluate the reliability of computer-aided replica technique (CART) by calculating its agreement with the replica technique (RT), using statistical agreement analysis. MATERIALS AND METHODS. A prepared metal die and a metal crown were fabricated. The gap between the restoration and abutment was replicated using silicone indicator paste (n = 25). Gap measurements differed in the control (RT) and experimental (CART) groups. In the RT group, the silicone replica was manually sectioned, and the marginal and occlusal gaps were measured using a microscope. In the CART group, the gap was digitized using optical scanning and image superimposition, and the gaps were measured using a software program. The agreement between the measurement techniques was evaluated by using the 95% Bland-Altman limits of agreement and concordance correlation coefficients (CCC). The least acceptable CCC was 0.90. RESULTS. The RT and CART groups showed linear association, with a strong positive correlation in gap measurements, but without significant differences. The 95% limits of agreement between the paired gap measurements were 3.84% and 7.08% of the mean. The lower 95% confidence limits of CCC were 0.9676 and 0.9188 for the marginal and occlusal gap measurements, respectively, and the values were greater than the allowed limit. CONCLUSION. The CART is a reliable digital approach for evaluating the fit accuracy of fixed dental prostheses.
