Recently, restoring implants in the esthetically demanding region, zirconia-based materials are widely used due to their superior mechanical properties, accuracies, and esthetics. The purpose of this study was to investigate the load transfer and mechanical stability of zirconia and titanium implant abutments by using the three-dimensional finite element analysis model. The internal conical joint type and external butt joint type implant system was selected as an experimental model. Finite element models of bone/implant/prosthesis complex were constructed. An load of 250N was applied vertically beside 3mm of implant axis. Stress distribution of zirconia and titanium implant abutment is similar. The maximum equivalent stress of titanium implant abutment is lower than zirconia implant abutment about 15%. Howevere considering a high mechanical strength that exceed those of titanium implant abutment, zirconia implant abutment had similar mechanical stability of titanium implant abutment clinically.
Purpose: The aim of this study was to evaluate the fit accuracy of two zirconia and titanium abutments in internal hexagonal implants. Materials and methods: One titanium abutment and two zirconia abutments were tested in internal hexagonal implants (TSV, Zimmer). Prefabricated zirconia abutments (ZirAce, Acucera) and customized zirconia abutments milled by the Zirkonzahn system (Zirkonzahn Max, Zirkonzahn) were selected and prefabricated titanium abutments (Hex-Lock, Zimmer) were used as a control. Eight abutments per group were connected to implants with 30 Ncm torque. The marginal gaps at abutment-implant interface, the internal gaps at internal hex, vertical and horizontal gaps between screws and screw seats in abutments were measured after sectioning the embedded specimens using a scanning electron microscope. Data analysis included one-way analysis of variance and the Scheffe test (n=16, ${\alpha}=0.05$). Results: The mean marginal gap of customized zirconia abutment was higher than those of two prefabricated zirconia and titanium abutments. The internal gaps at internal hex showed no significant differences between customized and prefabricated abutments and were higher than those of prefabricated titanium abutments. The mean vertical and horizontal gaps at screw in prefabricated zirconia abutment were higher than those of prefabricated titanium abutment. In the case of customized zirconia abutment, the mean horizontal gap at screw was higher than those of both the prefabricated zirconia and the titanium abutment but the mean vertical gap was not even measureable. The screw seats were clearly formed but did not match with abutment screws in prefabricated zirconia abutments. They were not, however, precisely formed in the case of customized zirconia abutments. Conclusion: Within the limitations of this study, the prefabricated titanium abutments showed better fit than the zirconia abutments, regardless of customized or prefabricated. Also, the customized zirconia abutments showed significantly higher marginal gaps and the fit was less accurate between screws and screw seats than the prefabricated abutments, titanium and zirconia.

Titanium and gold-alloy abutments have been used for a long-time in the clinical situations, but the use of zirconia abutments also increased. This study was designed to compare and evaluate the microgap differences according to types of abutment and dynamic loading. Titanium abutment, zirconia abutment and gold-alloy abutment (UCLA plastic) were connected into titanium implants of external hexagonal structure US II ${\phi}$$3.75{\times}11.5$ mm (Osstem Co., Seoul, Korea) with the tightening torque of 30 Ncm. A sine type dynamic loading of 25-250 N and $30^{\circ}$ inclination from long axis was applied for $10^5$ times. Using the SEM both before and after the loadings, implant-abutment interfaces were analyzed on the labial, palatal, mesial and distal surface. The microgaps before and after the loading were compared, no statistically significant difference was observed caused by the dynamic loading on the labial, palatal, mesial or distal surface. Statistically significant difference was observed between UCLA and titanium group and between UCLA and zirconia group on both before and after the loading(p<0.05). No statistically significant difference was found between titanium and zirconia group. Loadings for $10^5$ times did not show significant effect to the microgaps between implants and abutments.
Purpose: The purpose of this study was to compare the screw joint stability between the CADCAM custom-made implant abutment and the prefabricated implant abutment by measuring the reverse torque value after cyclic loading. Materials and methods: Twelve screw type implants (Implantium, Dentium Co., Seoul, Korea) were embedded in aluminum cylinder with acrylic resin. The implant specimens were equally divided into 3 groups, and connected to the prefabricated titanium abutments (Implantium, Dentium Co., Seoul, Korea), CADCAM custom-made titanium abutments (Myplant, Raphabio Co., Seoul, Korea) and CADCAM custom-made zirconia abutments (Zirconia Myplant, Raphabio Co., Seoul, Korea). The CAD-CAM milled titanium crown (Raphabio Co., Seoul, Korea) was cemented on each implant abutment by resin cement. Before cyclic loading, each abutment screw was tightened to 30 Ncm and the reverse torque value was measured about 30 minutes later. After the crown specimen was subjected to the sinusoidal cyclic loading (30 to 120 N, 500,000 cycles, 2 Hz), postloading reverse torque value was measured and the reverse torque loss ratio was calculated. Kruskal-Wallis test was used for statistical analysis of the reverse torque loss ratio. Results: The CADCAM custom-made titanium abutments presented higher values in reverse torque loss ratio without statistically significant differences than the prefabricated titanium abutments ($P$>.05). Reverse torque loss ratio of the custom-made zirconia abutments was significantly higher compared to that of the prefabricated titanium abutments ($P$=.014). Conclusion: Within the limitation of the present $in-vitro$ study, it was concluded that there was no significant difference in screw joint stability between the CADCAM custom-made titanium abutments and the prefabricated titanium abutments. On the other hand, the CADCAM custom-made zirconia abutments showed lower screw joint stability than prefabricated titanium abutments.
Purpose: The purpose of this study was to evaluate the preload maintenance of the retaining screw when using anaerobic sealing agents in implant fixture and abutment components. Specifically, the study examines the effects of anaerobic sealing agents on different types of screws. Materials and methods: External hexagon implants made of titanium and anti-rotational abutments were used. Titanium abutment screws and ebony abutment screws from the same manufacturer were used. The experiment was divided into four groups (n = 10 in each group). In the control group, no sealing agent was used at the implant fixture and abutment screw interface. All abutment screws were tightened according to the manufacturer's recommended torque (30 N.cm). After 24 hours, the removal torque (detorque) of each screw was measured using a digital torque gauge device. The data were analyzed by two-way ANOVA test according to normality distribution satisfaction. Results: Looking at the results for each group, titanium screws and no treatment showed detorque values of 20.3 ± 1.6 N.cm. titanium screws and applied anaerobic sealing agent showed detorque values of 32.4 ± 6.7 N.cm. Ebony screws and no treatment showed detorque values of 20.2 ± 1.5 N.cm. ebony screws and applied anaerobic sealing agent showed detorque values of 30.4 ± 4.5 N.cm. Conclusion: The detorque value was higher in the case of using anaerobic sealing agents in both the titanium screw and ebony screw groups. But there was no difference between the two screws.

The purpose of this study was to clarify the clinical efficiency of tungsten carbide/carbon coating on obtaining optimal preload of abutment screw compared with non-coated screw in external-hex implant system. In the present work, rotational value and the compressive force between abutment and fixture of abutment screws without coating and with coating tightened to 30Ncm were measured. Mean compressive force of coated screw was 504.6N. Then uncoated screw was tightened to clamping the abutment and the implant to 504.6N, and the tightening torque value and the rotational value was recorded. The following conclusions were drawn within the limitation of this study. Compressive force of coated screw was higher than that of uncoated screw after tightening at 30Ncm. The tightening torque that was nedeed to clamping the uncoated screw equal to coated screw tightened to 30Ncm was 55.6Ncm. In case of equal compressive force, there was no significant difference in rotation value between coated and uncoated screw.
Purpose: The purpose of this study was to evaluate the level of electro-chemical corrosion and surface roughness change for the cases of Ti abutment connected to restoration made of base metal alloys. Materials and methods: It was hypothesized that Ni-Cr alloys in different compositions possess different corrosion resistances, and thus the specimens ($13{\times}13{\times}1.5\;mm$) in this study were fabricated with 3 different types of metal alloys, commonly used for metal ceramic restorations. The electrochemical characteristics were evaluated with potentiostat (Parstat 2273A) and the level of surface roughness change was observed with surface roughness tester. Paired t-test was used to compare mean average surface roughness (Ra) changes of each specimen group. Results: All specimens made of nickel-chromium based alloys, average surface roughness was increased significantly (P < .05). Among them, the Ni-Cr-Be alloy ($0.016{\pm}.007\;{\mu}m$) had the largest change of roughness followed by Ni-Cr ($0.012{\pm}.003\;{\mu}m$) and Ni-Cr-Ti ($0.012{\pm}.002\;{\mu}m$) alloy. There was no significant changes in surface roughness between each metal alloys after corrosion. Conclusion: In the case of galvanic couples of Ti in contact with all specimens made of nickel-chromium based alloys, average surface roughness was increased.
Purpose: The purpose of this study was to investigate the effect of heat applied to disintegrate cement on the removal torque value and fracture strength of titanium abutment and abutment screw. Materials and methods: Implants, titanium abutments and abutment screws were prepared for each 20 piece. Implant abutments and screws were classified as the control group in which no heat was applied and the experimental group was heated in a vacuum furnace to $450^{\circ}C$ for 8 minutes and cooled in air. The abutments and screws were connected to the implants with 30 Ncm tightening torque at interval 10 minutes and the removal torque value was measured 15 minutes later. And the fracture strength of abutment screw was measured using universal testing machine. Results: The mean removal torque value was $27.84{\pm}1.07Ncm$ in the control group and $26.55{\pm}1.56Ncm$ in the experimental group and showed statistically significant difference (P < .05). The mean fracture strength was $731.47{\pm}39.46N$ in the control group and $768.58{\pm}46.73N$ in the experimental group and showed statistically no significant difference (P > .05). Conclusion: The heat applied for cement disintegration significantly reduced the removal torque value of the abutment screw and did not significantly affect fracture strength of the abutment screw. Therefore, in the case of applying heat to disintegrate cement it is necessary to separate the abutment screw or pay attention to the reuse of the heated screw. However further studies are needed to evaluate the clinical reuse of the heated screw.

This study was to evaluate the stress distributions of prefabricated, customized abutments and fixtures according to their material and shape by three-dimensional finite element analysis. And to investigate the fatigue life and fracture characteristics. Mandibular models were fabricated by reconstruction of the CT scan of patients with normal occlusion. A total of six finite element models were designed, a load of 100 N was applied on the buccal cusps vertically, and 30 degree obliquely. 10 specimens each were fabricated for the more clinically widely used 4 type abutments and were loaded according to ISO 14801. Differences in stress distribution patterns were not found according to the materials of the abutments and fixtures. But a slight difference in the stress level was detected. Customized abutment groups showed lower crown stress levels. One-piece zirconia implant showed the lowest bone stress levels. In the fatigue test, highest values were measured in group 7. Prefabricated abutments showed less variation of fatigue life (P<0.05). Use of customized abutments can improve the fracture resistance of restorations. Especially, use of customized zirconia abutments reinforced by titanium screw connecting parts is recommended.

Purpose : This study was designed to evaluate the influence of implant abutment materials on detorque value and screw joint stability before and after dynamic fatigue test. Materials & Methods : The external hexagonal fixture and three different groups of abutment (titanium abutments, zirconia abutments, and UCLA abutments) were used. The detorque value before loading and after loading (cyclic loading up to $10^5$ cycles) of the abutment screw were measured. Result : 1. There was no significant difference in detorque value before loading among the each group. 2. There was no significant difference in detorque value after loading among the each group. 3. Detorque values before and after cyclic loading in each group were not significantly different. 4. There was no significant difference in loss percentage of removal torque before loading among the each group. 5. There was no significant difference in loss percentage of removal torque after loading among the each group. 6. There was no significant difference in loss percentage of removal torque according to loading among the each group. Conclusion : Short term screw loosening of three types of abutment was not significantly different. When bite force was applied, there was no significant difference in screw loosening between before loading and after loading.
