Mechanical removals in decayed teeth have been performed using drill and sharp hand instruments. These methods have some disadvantages such as pain, local anesthesia and overextended cavities Therefore chemo-mechanical excavation of dentin carious lesions has been introduced. The purpose of this study was to evaluate the efficacy of traditional mechanical methods using burs and chemo-mechanical methods (Carisolv) of caries dentin. Mechanical caries removal was carried with low speed round bur Chemo-mechanical caries excavation was performed with Carisolv (Medi-team), using the Carisolv hand instruments. The mean time to remove caries with two different methods was evaluated and the data analyzed with SPSS software (ver 11.5) by t-test (p < 0.05). For histomorphometry of caries removal were also carried with mechanical or chemo-mechanical (Carisolv) methods from 20 extracted caries permanent molars. Complete caries removal was verified with a $\#$23 sharp explorers, Caries Detector (Kuraray Co. Japan), and standard apical radiography. 1. Chemo-mechanical method was taken more times than mechanical method (1.5 fold) (p < 0.05) 2. Excavation for caries took more time for molar lesion than premolar lesion, and the least time was taken to remove the caries in incisor lesion (p < 0.05). 3. There were no significant differences to remove the caries between the maxilla and mandible (p > 0.05). 4. The remaining carious dentin was detected after the ckemo-mechanical removal of the carious dentin, and no smear layer were seen after the mechanical and chemo-mechanical removal of the carious dentin.

Common dental procedures(dental extraction & minor operation) are potentially stress-inducing in many patients, especially medically compromised patients. The body's response to dental stress involves the cardiovascular system(an increase in cardiovascular workload), the respiratory organ and the endocrine system(change in metabolism). To minimize the stress to the medical risk patient, the stress reduction protocol was established. The obtained contents were as follows: (1) Recognize the patient's degree of medical risk (2) Complete medical consultation before dental therapy (3) Schedule the patient's appointment in the morning (4) Monitor and record preoperative and postoperative vital signs (5) Use psychosedation during therapy (6) Use adequate pain control during therapy (7) Short length of appointment : do not exceed the patient's limits of tolerance (8) Follow up with postoperative pain/anxiety control (9) Telephone the higher medical risk patient later on the same day that treatment was given Though the stress reduction protocol above was applied to the dental extraction in medically compromised patients with the advanced infected teeth, the final responsibility for the complications(syncope, bleeding & infection, etc.) in a patient rests with the dentist who ultimately treats him. For the prevention of postextraction complications & poor prognosis, the authors treated the advanced infected teeth with the pulp extirpation, opening drainage through the canal and complete occlusal reduction. The final extraction and wound closure were then done after $1{\sim}2$ weeks. The final prognosis was comfortable without common complications.
Statement of problem: when using resin for class II restoration, micoleakage by instrumentation can be regarded as the primary negative characteristic. A review of the available literature suggests that using flowable resin as liner to decreased microleakage. Purpose: The aim of this study was to determine the influence of the nanofilled flowable resin lining on marginal microleakage after load cycling in class II composite restoration fillings using nanofiller resin. Material and methods: 24 extracted premolars were prepared with class II cavity. F group was restored the nanofilled resin with the nanofilled flowable resin as liner. NF group was restored the nanofilled resin only. After restoration, an experiment was performed on 2 groups using a 300N load at 104, 105 and 106 cycles. Prior to and before each load cycling, it was gauged length on total marginal microleakage, axial marginal microleakage and buccal, gingival, lingual marginal microleakage. Data were analyzed with the Mann-Whitney test & Kruskal-Wallis test. Results: There were statistically significant differences between 2 groups and between individual groups. (P <.05) The result showed less microleakage in teeth restored by the nanofilled resin, which was lined by the nanofilled flowable resin. Conclusion: There was significant reduction in microleakage when the nanofilled flowable resin lining was placed underneath the nanofilled resin in class II composite restoration fillings.

When attempting to restore the oral function of a partially edentulous patient, there are a number of prosthetic treatment options available, depending on the structure of remaining teeth. For example, when only one set of maxillary and mandibular teeth are diagonally in place across from each other, it is difficult to gain stable occlusion. In this case, implants can be put in place at the corresponding edentulous area to achieve balance. By doing so, a stable occlusion can be achieved. For this case report, a patient with crossed occlusion after extraction was treated with maxillary RDP (removable dental prosthesis) and mandibular implant-supported RDP (removable dental prosthesis). Moreover, an implant fixture was placed under the posterior molar of the distal extension base diagonally across from the remaining maxillary teeth. Then, magnetic attachment was implemented. According to the patient who received the treatment, the result was functionally and aesthetically satisfactory.

Traumatic injury on tooth occurs frequently among trauma patients, and mainly occurs on tooth with premature roots which influences pulp tissue, periodontal ligament, alveolar bone, and Hertwig's epithelial root sheath. According to the degree of trauma, a number of kinds of healing process can be observed, such as complete re-vascularization of pulp, root canal obliteration, growth suspension of root apex, and invasion of alveolar bone into root canal, and there can be some complications such as necrotic change of inflammatory root resorption and partial pulp necrosis due to pulp necrosis toward complete necrosis. In this clinical case, 3 patients who had traumatic injury showed root growth suspension and alveolar bone invasion into root canal due to proliferation of periodontal ligament cell and osteocyte at the base of extraction socket into pulp chamber because of the injury on Hertwig's epithelial root sheath. If intrusion of alveolar bone into root canal due to injury on Hertwig's epithelial root sheath after having traumatic injury doesn't show any complication, the pulp may be considered to have normal vitality and doesn't need any further treatment, therefore differential diagnosis is very necessary. However, it may be accompanied with suspension of root growth, therefore, additional trauma during the treatment of injured tooth should not be applied.

Garre's osteomyelitis is associated with bacterial infection and bone necrosis resulting from obstruction of blood supply. The most common cause for Garre's osteomyelitis is odontogenic infection that originates from periodontal tissue or dental pulp. Subperiosteal abscess may also cause Garre's osteomyelitis in the progress of the infection. Mandible is more often affected than maxilla, most commonly in the permanent first molar region of mandible. Clinically, it results in a hard swelling over the jaw, producing facial asymmetry. Meanwhile, radiograph shows a characteristic feature of irregular pulpal cavity, showing new periosteal proliferation located in successive layers to the condensed cortical bone on stimulated site. The treatment method for Garre's osteomyelitis are removal of the infection source, root canal treatment, antibiotic medication, and incision and drainage. This report presents a case of Garre's osteomyelitis under 15 years old. The patient was successfully treated by antibiotic medication accompanied with root canal treatment. Since the symptom of pediatric patients is less severe than adult, careful diagnosis with history taking and clinical examination is necessary. Furthermore long-term follow-up examination is needed to prevent recurrence even after the symptom disapears.
The objective of this study was to investigate the effect of excessive occlusal loading on stress distribution on four type of cervical lesion, using a three dimensional finite element analysis (3D FEA). The extracted maxillary second premolar was scanned serially with Micro-CT. The 3D images were processed by 3D-DOCTOR. ANSYS was used to mesh and analyze 3D FE model. Four different lesion configurations representative of the various types observed clinically for teeth were studied. A static point load of 500N was applied to the buccal and lingual cusp (Load A and B). The principal stresses in lesion apex, and vertical sectioned margin of cervical wall were analyzed. The results were as follows 1. The patterns of stress distribution were similar but the magnitude was different in four types of lesion 2. The peak stress was observed at mesial corner and also stresses concentrated at lesion apex. 3. The compressive stress under load A and the tensile stress under load B were dominant stress. 4. Under the load, lesion can be increased and harmful to tooth structure unless restored.
The purpose of this study was to observe the effect of canal filling on the bacteria left in the dentinal tubules and to compare the sealing ability between Gutta-percha and Resilon. The bovine dentin block models were prepared E. faecalis was inoculated to dentin blocks and incubated. The dentin blocks were divided into 5 groups. Group 1 was the negative control. Group 2 was the positive control. Group 3 was filled with ZOE based sealer and Gutta-percha, Group 4 with resin based sealer and Gutta-percha, and Group 5 with resin based sealer and Resilon. After 24 hour, the blocks were incubated at $37^{\circ}C$ for 1, 2, 3 and 4 weeks on BHI agar plates. The internal dentin portion of the blocks was removed using ISO 027, 029, 031, 035 round burs and the dentin chips were incubated at $37^{\circ}C$ for 24 hour Following incubation, the optical density of the medium was measured. The data were statistically analysed using repeated measures ANOVA and one-way ANOVA. The results were as follows, 1. There. was statistically significant reduction in the number of E. faecalis of the group where dentinal tubules were completely sealed with nail varnish in comparison with the groups obturated with gutta-percha or resilon (p < 0.05). 2. In group 5, the number of E. faecalis in the dentinal tubules decreased significantly with time (p < 0.05). whereas in Group 3 and 4, there was no reduction in its number (p > 0.05). 3. Under the conditions of this experiment, E. faecalis survived up to 4 weeks after obturation with gutta-percha or resilon (p > 0.05).

An in vitro study was performed to evaluate the effect of four variables on the temperature rise produced by polishing of restorations. The four variables were : restorative material, base, thickness of remaining dentin, continuous polishing or intermittent polishing. Class V cavities were cut on extracted molar and restored with composite resin, resin-modified glass ionomer cement, compomer, amalgam on the various bases (glass ionomer cement, zinc oxide eugenol cement, zinc phosphate cement) Dentin thickness under the restoration was 0.5mm, 1.5mm. Polishing was done with an aluminum oxide-coated disc. Polishing time was continuous or intermittent for up to 1 minute. Intra-pulpal temperature increased almost linearly in all cases. Amalgam produced highest temperature rises at the pulp, while the composite resin, resin-modified glass ionomer cement and compomer were not different for each other. The rate and extent of temperature rising of amalgam restoration was reduced by presence of a cement base. Zinc oxide eugenol cement bases showed the highest temperature rise, while glass ionomer cement, zinc phosphate cement were not different to the untreated tooth Thickness of remaining dentin was only significant for the amalgam restoration. Continuous polishing produced higher temperature rise than intermittent polishing.

The aim of this study was to evaluate the effect of temporary restorative and filling material on bonding strength between lithium disilicate glass-ceramic and dentin. 60 extracted human molars were cross-sectioned at occlusal third and were embedded into self-cure acrylic resin. Then the teeth were randomly divided into four groups of 15 each. Lithium disilicate glass-ceramic is cemented to dentin as follows: after no any application of the provisional materials (Group A), after application of ALIKETM (GC America Inc.)(Group B), after application of Luxatemp$^{(R)}$ Automix plus (DMG, Germany)(Group C), after application of Fermit$^{(R)}$ (Ivoclar Vivadent, Leichtenstein)(Group D). After the specimens were stored in distilled water for 24 hours, the shear bond strength of the specimens were measured using UTM (Zwick 1456 41, Zwick, Germany) at a crosshead speed of 1mm/min. The data were analysed by one-way ANOVA and Tukey HSD tests. There were no statistically significant differences of bond strength among the groups. Fracture type was showed mixed type of adhesive and cohesive fracture in most of specimens. Within the limitation of this study, bond strength of adhesive resin cement between lithium disilicate glass-ceramic and dentin was not affected by provisional restorative and filling materials.
