Full mouth rehabilitation in patient with deep bite, inter-dental arch discrepancy and loss of vertical dimension: a case report (과개교합과 치열궁부조화 및 수직고경 감소를 가진 환자의 전악수복증례)

  • Loss of posterior bite support might cause unstable occlusal relationship and when the mandible slides forward in the centric or habitual occlusion, excessive load is applied to the anterior region followed by causing the occlusal plane to collapse or leading to a decrease in occlusal vertical dimension. In addition, disorder of temporomandibular joint function may occur. The inter-dental arch discrepancy causes a mismatch in the vertical and horizontal overlap of the anterior and posterior regions. The deep bite in the anterior region and the scissor bite in the posterior region cause unstable occlusal contact and insufficient occlusal contact area. This report was to rehabilitate a patient with above-mentioned complex problems. Physiologic adaptation to increased vertical dimension and new occlusal plane were evaluated using provisional prostheses, and definitive prostheses was fabricated using cross-mounting technique. Stable occlusion, harmonious teeth overlap and adequate occlusal plane were established, so functionally and aesthetically satisfactory results are obtained.

A Study Practice of Infection Control for Dental Office and Dental Hygienist and Bacterial Contamination of Dental Office Surface (치과 의료기관 및 치과위생사의 감염관리 실천도와 진료실내의 표면 세균오염도에 대한 연구)

  • The purpose of this study was to investigate the status of infection control in dental office and the degree of infection control practice of dental hygienists. A self-reported questionnaire was answered by 193 dental hygienists working in dental hospitals or dental clinics in Gyeonggi and Incheon areas. The number of bacteria was measured sample of surface dental unit chair shoulder backrests and light handles from 19 dental hospitals and 28 dental clinics. It was responded that impression or occlusion bodies were routinely disinfected in 52.6% of the dental hospitals and dental prosthesis were routinely disinfected in 46.4% in dental clinics. The dental hospital and clinics disinfect the surface each 26.3%, 25% after the patient treatment. The dental hospital and clinic were detected bacteria $5.02^*10\^3CFU/mL$, $1^*10\^4CFU/mL$, from dental unit chair backrest, respectively and $8.32^*10\^3CFU/mL$, $4.26^*10\^4CFU/mL$ from light handles, respectively. At the conclusion of this investigation, it can be kept from infection source in dental office is to make the right selection and use of the personal protective equipments and to improve the practice level by regular and active education. The same infection control practice should be done by institutionalizing and mandating infection control practice.

Prosthetic rehabilitation by obturator considering the biomechanics in partially edentulous patient after maxillectomy (상악골 절제술을 받은 부분 무치악 환자에서 생역학을 고려해 제작한 구개 폐색장치를 이용한 보철 수복 증례)

  • Patients who went through maxillectomy can have severely impaired swallowing, mastication, and pronunciation functions because of palatal defects. Leakage occurs through the nasal cavity while eating, chewing becomes difficult due to the loss of teeth and alveolar ridges, and oral and nasal passages are not separated, leading to hyper-nasal sound, and significantly reducing the quality of life. To prosthetically reconstruct the defect, the weight of the obturator should be reduced as much as possible to minimize dropout because of gravity, and the bulb of the obturator should be properly extended into the defect to get additional retention and stability. In this case of a partially edentulous patient who underwent additional maxillary resection because of tumor recurrence, a metal framework was designed by applying the basic design principles of removable partial dentures. An obturator with improved retention, stability, and support was fabricated through functional impressions. The patient was satisfied with the improved facial expression, mastication, swallowing, and pronunciation, and showed stable occlusion and oral hygiene management during the follow-up period.

Assessment of the Caries Detection Ability of Quantitative Light-induced Fluorescence (QLF) in Primary Teeth in vitro (유치의 치아우식증 확인을 위한 정량형광분석(QLF) 검사법의 우식탐지능력 평가)

  • The aim of this study was to evaluate QLF (quantitative light-induced fluorescence) technology for the caries detection in primary teeth and validate the relationships between the cavity volume of carious lesions and QLF analysis results. Total 125 tooth surfaces include 53 occlusal surfaces, and 72 proximal surfaces were investigated with the portable QLF device for detection of dental caries in primary molars. Micro-CT radiograph was also performed to classify carious lesions and calculate the cavity volume. QLF showed good accuracy and reliability (sensitivity 0.75 - 0.94, specificity 0.82 - 0.95, and AUROC 0.88 - 0.98) for the caries detection in primary teeth except 𝚫R average results of proximal caries which showed relatively low values. Statistically significant relationships were found between 𝚫F average, QS-Index and the cavity volume according to Spearman rank-order correlation coefficients (r = 0.805 - 0.832, p < 0.001). QLF detection method would be a harmless and reliable way for children to diagnose dental caries without the concern about radiation exposure.

Efficacy of retreatment NiTi files for root canals filled with calcium silicate-based sealer (칼슘실리케이트 계열 실러로 충전한 근관에서 재치료용 NiTi 파일의 효율성)

  • Purpose: The efficacy of the amount of sealer in the root canal and two retreatment NiTi file systems in removing filling materials. Materials and Methods: Extracted premolars with a single root canal were selected for this study. After access opening and root canal preparation up to size #40/.06, the specimens were randomly divided into four groups. Gutta percha (GP) tapers of .06 or .04 were used for each group and filled using a single-cone filling technique with CeraSeal, a calcium silicate-based sealer. Each group was retreated either using the ProTaper Universal Retreatment System (PTUR) or the Hyflex Remover (HR). The time taken to remove the filling material, the amounts of apically extruded debris, and canal cleanliness were measured and compared. Results: The amount of sealer did not affect the efficiency when removing the filling materials. However, the filling material was removed faster in the HR group than in the PTUR group. Two types of NiTi files showed similar retreatment effects in the amounts of apically extruded debris and in the degree of canal cleanliness. Conclusion: The amount of sealer in canal filling had no significant effect on retreatability. Retreatment with HR removed filling materials is faster than PTUR. There was no difference in other removal efficiencies according to the type of retreatment NiTi file.

Positional deviation between CBCT-based digital facebow transfer and analog facebow transfer: case series (CBCT 기반 디지털 안궁이전과 아날로그 안궁이전의 위치 편차: 증례보고)

  • Facebow transfer is essential for accurately mounting a dental cast onto a semi-adjustable articulator. The precision of traditional analog facebow transfer is influenced by both the accuracy of the equipment used and the skill level of the operator. Considering that substantial positional deviations can adversely affect the quality of a fabricated dental prosthesis; it is critical to assess the positional accuracy of casts mounted using analog facebow transfer. This case report evaluates the linear and angular deviations of the occlusal plane for maxillary casts mounted through both analog facebow transfer and cone-beam computed tomography-based methods. The findings indicate that analog facebow transfer produced a linear deviation ranging from 3 to 16 mm and an angular deviation of the occlusal plane between 5 to 7 degrees. This case report confirms that, across two patients, analog facebow transfer can result in varying degrees of positional deviation, thereby potentially leading to inaccuracies in the fabrication of dental prostheses. These results suggest that, in clinical practice, the use of analog facebow transfer may yield significant deviations during the process of mounting maxillary casts.

Status of Mongolian dentistry viewed from information resources and selection of adhesive dental restorative materials and continuing education (접착성 치과수복재료에 대한 정보습득과 선택 및 평생교육을 통해 본 몽골 치과계의 현황)

  • Purpose: To investigate the information acquisition, selection criteria and selection methods of Mongolian dentists regarding adhesive agents for bonded restorations, and to provide future direction for continuing education. Materials and Methods: One hundred Mongolian dentists were interviewed and asked to complete a questionnaire containing 7 questions on general information about the responder, 8 questions on information acquisition and selection of bonded restoration agents, and 10 questions on continuing education. Results: Objective and credible information regarding bonded restoration materials were not being acquired, and logical material selection was not being made. The extent of continuing education was inadequate and not enough information regarding education was being acquired. The participants responded positively to online supplementary education. Conclusion: A systemized approach needs to be established in Mongolian dentistry to enable organized delivery of evidence-based guidelines and information, and logical selection of the numerous and various bonded restoration agents. Furthermore, the education of dentists, through various means, is required to enable proper use of the selected materials.

A Study on Knowledge of and Attitude to the Elderly among Some Dental Hygienist (일부 치과위생사의 노인에 대한 지식과 태도 연구)

  • The purpose of this study was to examine the knowledge of dental hygienists about the elderly and their attitude toward them as dental hygienists played vital roles in oral health care for elderly patients. It's ultimately meant to help improve their right understanding of the elderly to provide quality oral health care service to the elderly population in preparation for an aging society. The subjects in this study were 241 dental hygienists, on whom a self-administered survey was implemented. After the collected data were analyzed, the following findings were given: They got a mean of $12.40{\pm}2.99$ out of possible 22 points on knowledge of the elderly. Their general characteristics and characteristics related to the elderly made no statistically significant differences to their knowledge. As for attitude toward the elderly, they got a mean of $3.13{\pm}0.28$. Concerning links between their general characteristics and attitude toward the elderly, the better-educated dental hygienists took a more favorable attitude to the elderly.

The Relation between Dental Technician's Social Support and Individual Competency (치과기공사가 인식하는 사회적 지지 척도와 개인역량과의 상관관계 조사)

  • This study analyzed the dental technician's social support and individual competency as well as its correlation. 200 dental technicians in Seoul, Incheon, and Chunchungnamdo area were selected. The survey was carried out from April 28, 2012 to May 25, 2012 using a self-administered questionnaire. Regarding the tools for this study, a structured questionnaire was used with proven reliability and feasibility, and the contents of the questionnaire consisted of 55 questions based on the related references. The subjects completed the questionnaire regarding the general characteristics, social support, and individual competency. Data analysis was processed using computerized system with SPSS 18.0. Statistical analysis techniques including frequency, percentage, T-test, ANOVA analysis and correlation. An analysis of the research subjects' social support level revealed a significant difference in the item of marital status and residence area (p<0.05). The average in the social support was indicated to be high with 3.33. An analysis of the research subjects' individual competency revealed a significant difference in the items of age, marital status, residence area, carrier, job position, education (p<0.05). The average in the individual competency was indicated to be high with 3.68. The correlation between social support and individual competency showed that most of the variables had an association, showing that more social support leads to more individual competency.

As a result of analyzing the research subjects' organizational commitment level, there was difference in the item of gender, age, marital status, residence area, carrier, job position, education(P<0.05). The average of organizational commitment was indicated to be high with 3.46. Correlation between individual competency and organizational commitment, most of variables were indicated to have correlation, thereby having been shown that the more individual competency leads to the more organizational commitment.
