Evaluation of Gestational Diabetes Mellitus Risk Factors Using Abdominal Subcutaneous Fat Thickness for Early Pregnancy in the US Imaging (초음파영상에서의 임신초기 복부피하지방두께를 이용한 임신성당뇨 위험인자 평가)

  • The purpose of this study was to investigate the relationship between abdominal subcutaneous fat thickness(ASFT) and maternal gestational diabetes mellitus(GDM) measured by ultrasound at period of pregnancy. We compared maternal age, pre-pregnancy body mass index, and weight gain during pregnancy in 286 pregnant women who were diagnosed with early pregnancy ASFT and high GDM screening test(50 g OGTT) of more than 140 mg/dL. ROC curve analysis was used to determine the cut-off value of ASFT for GDM prediction. Maternal age and weight gain during pregnancy were not related to GDM in the mid-trimester and pre-pregnancy body mass index and earely pregnancy ASFT were significantly different between normal and GDM high risk groups. The cut-off value of ASFT for GDM prediction was 2.23 cm(AUC 0.913. Sensitivity 76.19%, Specificity 93.72%). ASFT measured by ultrasound in early pregnancy was useful as an important index for predicting mid-trimester GDM prediction. Therefore, ASFT can be used as an auxiliary diagnostic index for early recognition of GDM.

Impact of Prepregnancy Body Mass Index on Pregnancy Outcome in Women with a Singleton Conceived by Assisted Reproductive Technology and Spontaneously Conceived Pregnancy: A Case-Control Study (생식보조술을 받은 산모와 자연임신한 산모에서 임신 전 체질량지수가 임신 결과에 미치는 영향)

  • Purpose: To compare and confirm the impact of prepregnancy body mass index on pregnancy outcome in women with a singleton conceived by assisted reproductive technology and spontaneously conceived pregnancy. Methods: A sample of 165 and 247 pregnant women with and without assisted reproductive technology were retrospectively recruited from electronic medical charts of C hospital. Results: There were significant differences between the two groups for maternal age, paternal age, length of marriage, prepregnancy body mass index, parity, spontaneous abortion experience, and preterm delivery. A prepregnancy body mass index of ${\geq}25$ was associated with higher risk for maternal and neonatal complication in the assisted reproductive technology group. Conclusion: The results indicate that a higher prepregnancy body mass index is associated with increased risks for adverse pregnancy outcomes for women using assisted reproductive technology. So these women need appropriate care to compensate for the risk.

Cord Blood Adiponectin and Insulin-like Growth Factor-I in Term Neonates of Gestational Diabetes Mellitus Mothers: Relationship to Fetal Growth

  • Purpose: The purpose of this study was to evaluate the relationship between cord blood adiponectin and insulin-like growth factor (IGF)-I and their effect on fetal growth and insulin resistance in mothers with gestational diabetes mellitus (GDM). Methods: Cord blood adiponectin and IGF-I were compared between mothers with GDM (GDM group, N=53) and controls (non-GDM group, N=101). Neonates were classified into three groups of small for gestational age (SGA, N=26), appropriate for gestational age (AGA, N=97), and large for gestational age (LGA, N=31) by birth weight. The association between cord adiponectin and IGF-I levels was evaluated in relation to maternal and neonatal clinical data. Results: Cord adiponectin was lower in the GDM group than in the non-GDM group (P<0.001). There was no significant difference in cord adiponectin among the SGA, AGA, and LGA groups in the GDM group (P=0.228). The cord adiponectin of AGA in the GDM group was significantly lower than that in the non-GDM group (P<0.001). The most powerful predictor affecting cord adiponectin was the result of maternal 75 g oral glucose tolerance test. The cord IGF-I values between the GDM group and the non-GDM group were not different (P=0.834). Neonates with the heavier birth weight had the higher cord IGF-I levels. The most powerful predictor affecting cord IGF-I was birth weight and the next was maternal parity. Conclusion: Both cord blood adiponectin and IGF-I were associated with fetal growth, but IGF-I was a more general and direct factor affecting fetal body size, and adiponectin seemed to have more association with insulin sensitivity than growth.

Effects of Body Weight Control Therapy using Herb Mixture to Pregnant Women on Postpartum Weight (한방복합제에 의한 산후체중 조절요법이 출산후 체중감소에 미치는 영향)

  • This studies were performed to functional food consisted of herb mixture having an effect on postpartum body weight control in 105 subjects(control group n=54, experimental group n=51). Mean age of control and experimental group were 29.3 and 29.1 Mean pregravida body mass index of control and experimental group were 22.8 and 21.5. Total weight gain during the period of pregnancy of control and experimental group were 19.1kg and 17.8kg. In this studies, changes in postpartum weight was measured until one year after parturition. Until 1 month after parturition, there almost had no changes in weight all of two groups. But after 2 month, change in weight loss between two groups was shown definitely. Mean weight loss of control group and experimental group were 2.3kg and 6.5kg after 2 month, 3.7kg and 9.8kg after 3 month, 5.0kg and 10.4kg after 1 year(p<0.01).

  • The purpose of this study was to examine obstetrical & gynecological health status and body composition for married female immigrants. The participants were 154 immigrant women living in Busan. Data were collected from July 2008 to July 2009 using a questionnaire and a bioelectrical impedance analysis tool (Inbody 720) for analysis of body composition. Most subjects were in the normal range for body composition, but some of those who answered the questionnaire had experienced some gynecological problems such as dysmenorrhea, or irregular menstruation. There was a significant difference in experiences with in vitro fertilization, pregnancy and abortion according to age and in premensturaion syndrome, as well as differences in regulation of menstruation during the past year, leukorrhea odor, experiences in in vitro fertilization, and delivery and breastfeeding according to nationality. There were significant differences in body mass index and waist-hip ratio according to age, as well as differences in percent body fat, body mass index and waist-hip ratio according to nationality. There were also significant differences in body mass index and waist-hip ratio according to period of stay in Korea. Procreative care protocols concerning married female immigrants' characteristics should be developed.

Factors associated with the bone mineral density in Korean adults: Data from the 2010-2011 Korean National Health and Nutrition Examination Survey (KNHANES) V (우리나라 성인남녀의 골밀도 관련요인: 국민건강영양조사 제 5기 1, 2차 년도(2010년, 2011년))

  • Objective: The purpose of this study was to identify the correlates of bone mineral density of Korean adults. Methods: This study was based on the data from the 2010-2011 Korean National Health and Nutrition Examination Survey (KNHANES, 2010~2011) V. Among 8,473 subjects of the survey, who were adults 19 years old or over with bone mineral density data, the current study excluded those whose data include a missing study variable or the response of 'I don't know', and finally analyzed the data of 5,986 subjects (2,692 males and 3,294 females). This study employed the following statistical methods of analysis: t-test, analysis of variance (ANOVA), and hierarchical multivariate linear regression analysis. Results: In multivariate analysis, in the case of men, the significant associated factors in bone mineral density were age, education level, body mass index (BMI), waist circumference, total muscle mass, exercise, and weight control; in the case of women, the significant associated factors in bone mineral density were age, education level, economic activity, BMI, total muscle mass, exercise, weight control. fat intake, uptake female hormone, menopause, and age of menarche. Conclusion: For both sexes, the strongest modifiable factor in influencing bone mineral density was total muscle mass. Therefore, to prevent osteoporosis and promote health, active health education and interventions such as regular exercise for total muscle mass increase should be implemented from an early growth period.

Effects of Herb Mixture to Postpartum Body Control and Arthralgia (한방 복합제가 산모의 체중조절 및 관절통에 미치는 영향)

  • We have studies in 38 subjects (19 control and 19 experiment) for development of functional food for postpartum body control. Mean age of control and experimental subjects were each other 30.4 and 29.0 years old, and mean body mass index were 20.2 and 21.4. Total weight gain during pregnancy of control and experimental subjects were 14.0kg and 19.8kg. We measured change of weight and liver activity after parturition for 3 months. It shows that weight not reduced until 30 days in postpartum but significantly decreased during 30 days to 90 days. The mean weight of control and experimental subjects were decreased 3.15kg and 6.46kg at 60 days(p<0.05), 4.47kg and 7.44kg at 90 days(p<0.05). In this study, hemoglobin, GOT and CPT level were not changes before and after of experiment, respectively.

Effects of Administration of Oxytocin Antagonist on Implantation and Pregnancy Rates in Patients with Repeated Failure of IVF/ICSI Treatment (체외수정시술의 반복적인 실패 환자에서 옥시토신 길항제 주입이 착상 및 임신에 미치는 영향)

  • Objective: This study was performed to evaluate the effect of oxytocin antagonist on the outcome of IVF/ICSI cycles in infertile patients with repeated failure of IVF/ICSI treatment. Method: Forty patients who had experienced two or more failures of IVF/ICSI treatment without low ovarian reserve, were recruited for this prospective randomized study. All patients received controlled ovarian stimulation (COS) using GnRH antagonist multidose protocol (MDP). For the intervention group, intravenous administration of atosiban (mixed vasopressin $V_{1A}$/oxytocin antagonist) started with a bolus dose 6.75 mg one hour before embryo transfer (ET) and continued at an infusion rate of 18 mg/hour. After ET, administered atosiban was reduced to 6 mg/hour and continued for 2 hours. The main efficacy endpoints were clinical pregnancy rate and implantation rate. Results: Patients' characteristics were comparable in the intervention and control groups. COS parameters and IVF results were also similar. The number of uterine contractions for 3 minutes measured just before ET was significantly lower in the intervention group than control group ($3.5{\pm}1.4$ vs $8.7{\pm}2.2$, p<0.001). While there was no statistically significant difference in the clinical pregnancy rate between control group and intervention group (20.0% and 40.0%, p=0.168), the implantation rate was significantly higher in the intervention group, with 16.9% (11/65) compared with 6.0% (4/67) in the control group (p=0.047). There were no differences in ectopic pregnancy rate and miscarriage rate between the two groups. Conclusion: This study demonstrates that administration of oxytocin antagonist during ET can improve the implantation rate probably by decreasing the frequency of uterine contractions in infertile patients undergoing IVF/ICSI treatment.

Residual Consistency and Interrelationship Estimation of PCDDs, PCDFs, Dioxin-like PCBs in Colostrum of Primipara and Multipara (초산부, 경산부 초유 중 PCDDs, PCDFs, dioxin-like PCBs의 축적수준 및 상관성 평가)

  • Polychlorinated dibenzo-p-dioxins (PCDDs), polychlorinated dibenzofurans (PCDFs) and dioxin-like polychlorinated biphenyls (dl-PCBs) are bioaccumulative chemicals that are considered to be toxic contaminants based on several epidemiological studies. These chemicals in colostrum were investigated and estimated for their residual consistencies by maternal characteristics like parity and maternal ages. Test subjects were healthy primipara and multipara mothers with a mean age of 31.5 (S.D=3.6) in 2007. Seven isomers of PCDDs, 10 of PCDFs, 4 of non-orthopolychlorinated biphenyls(non-ortho PCBs) and 8 of mono-orthochlorinated polychlorinated biphenyls (mono-ortho PCBs) were analyzed by HRGC/HRMS. From the analyzed data, the mean level of total WHO-TEQs was 9.41 pg TEQ/g lipid, which is significantly lower than the level found in individuals from other countries. The main contributors to the total WHO-TEQs with increasing percentages were 2,3,4,7,8-PeCDF, 1,2,3,7,8-PeCDD and 3,3',4,4',5-PCB (#126), and they accounted for more than 60% of the total WHO-TEQs. PCDFs concentrations and total WHO-TEQs were negatively associated with parity (p<0.05), and maternal age was positively associated with total WHO-TEQs (p<0.01). However, the associations with body mass index (BMI) and fish intake during pregnancy were not significant. These results were suggested that parity and maternal age are an important factor affecting the concentrations of PCDD/DFs and dl-PCBs in these specimens.
