The influence of the burden of nurse's work and health problems on presenteeism (간호사의 건강문제와 업무에 대한 부담감이 프리젠티즘에 미치는 영향)

  • The purpose of this study was to investigate the effects of nurses' perceived burden from work and health problems on the presentism of nurses. The study subjects were recruited from four general and tertiary hospitals in K province and D metropolitan city. A quarter of nurses had shoulder, back, and neck pain and average number of health problems was 3.62. The nurses' presenteeism score was $43.37{\pm}12.43$ points out of 100 points. There was a statistically significant relationship between nurses' perceived burden, numbers of health problems, and presenteeism. The numbers of health problems and perceived burden of nurses had significant effects on presenteeism of nurses while controlling demographic factors such as length of work experience, job position, types of hospital, and gender. These results suggested that the less burden from work and the lower number health problems could result in the lower level of presenteeism of nurses. Therefore, it is necessary to develop diverse strategies to reduce the burden of nurses and health problems at both individual and organizational levels as a way to increase productivity of hospital.

Posttraumatic Stress by Work in Firefighters (소방공무원의 직무별 외상 후 스트레스)

  • To investigate a posttraumatic stress, social support and work burden and to identify high risk group and related factors which were exerted influence on posttraumatic stress of firefighters. Data were gathered from total 264 firefighters consisting of 85 fire distinguishers & rescue workers, 96 emergency medical personnel and 83 in ambulance & fire truck drivers in C province and were analyzed 22 items by IES-R, 20 work burden items by Choi(2000), 8 social support items by Oh(2006) using SPSSWIN 14.0 program. Posttraumatic stress of general characteristics by work was showed significant difference in age, marriage status, education, position, office duration, mobilization frequency and experience of traumatic events. Posttraumatic stress were showed significant difference in 3 groups and fire distinguishers & rescue workers is the highest group. Posttraumatic stress was correlated with work burden(r=.317, p<0.01) for fire distinguishers & rescue workers, social support(r=-.331, p<0.01) and work burden(r=.522 p<0.001) for emergency medical personnel and work burden(r=.454, p<0.01) for ambulance & fire truck drivers. The high risk groups are fire distinguishers & rescue workers(60%), emergency medical personnel(55.2%)and ambulance & fire truck drivers(45.8%). Related factors are the experience seeing victim's danger for fire distinguishers & rescue workers(odds ratio=1.216, 95% confidence interval:1.068-1.383), and are work burden(odds ratio=1.100, 95% confidence interval:1.043-1.159) and office duration(odds ratio=1.010, 95% confidence interval 1.001-1.018) for emergency medical personnel, and are the experience seeing victim's danger(odds ratio=1.178, 95% confidence interval:1.010-1.373), age(odds ratio=1.129, 95%confidence interval:1.020-1.249), work burden(odds ratio=1.103, 95% confidence interval:1.034-1.177) for ambulance & fire truck drivers.

Factors Affecting Posttraumatic Stress of Emergency Medical Personnels (구급대원의 외상성 스트레스 영향요인)

  • This study not only investigates emergency medical personnel's posttraumatic stress(PTS), social support, work burden, and coping style but also identifies related factors which were exerted influence on PTS and active coping method of emergency medical personnels among the firefighters. The data were gathered from 143 emergency medical personnels who were receiving training. The data were collected by IES-R, consisting of twenty-two questions that determine the level of PTS, method used in Cho's research(2000) that deal with work burden, method used in Oh's article (2006) relating to social support, and sixty-two questions that deals with coping methods. Then the data gathered were analyzed using SPSSWIN 14.0 program. PTS of general characteristics showed significant difference in age, marriage status, education, and position. High risk group of PTS was 74(51.7%). Work burden, active and passive coping method, and three symptoms in PTS are significantly high(p=0.000) in high risk group. PTS was correlated with work burden(r=0.508, p=0.000), active coping method(r=0.375, p=0.000), and passive coping method(r=0.505, p=0.000) but not with social support. Related factors of PTS were work burden(0.371) and passive coping method(0.366). Also related factors of high risk group of PTS were work burden(odds ratio=1.064, 95% confidence interval:1.031-1.103) and passive coping(odds ratio=1.050, 95% confidence interval:1.022-1.080). Related factors of active coping method were PTS(0.392) and social support(0.158). To minimize the PTS of emergency medical personnels, the new policy should decrease their work burden and passive coping method and strengthen the social support to encourage active coping method.

Different Patterns of Behavioral and Psychological Symptoms of Dementia and Caregiver Burden according to Dementia Type in Nursing Home Residents (요양시설 치매노인의 치매유형에 따른 행동심리증상 특성과 간호제공자의 업무 부담감)

  • The purpose of this study was to examine different patterns of behavioral and psychological symptoms of dementia (BPSD) and caregiver burden according to dementia type. Data were collected from June 2014 to December 2014 from five nursing homes. In all, 214 patients [131 patients with Alzheimer's disease (AD) and 83 patients with Vascular dementia (VD)] were included in the study. BPSD and caregiver burden data were examined using NPI-NH (Neuropsychiatric Inventory Nursing Home Version). According to the results, both the AD and VD group showed the highest frequency in apathy/indifference and the frequency of anxiety, elation/euphoria, and irritability/lability was significantly higher in the AD group than in the VD group. The difference in total BPSD composite scores between the AD and VD groups was not significant but agitation/aggression was significantly higher in the AD group than in the VD group. The mean score for caregiver burden was significantly higher in the AD group than in the VD group, particularly in agitation/aggression. In conclusion, BPSD differed according to dementia type, and nurses should provide specific interventions to control BPSD; a program to reduce caregiver burden according to dementia type is also needed.

Development of PTSD Web-based learning (소방공무원을 위한 외상후스트레스장애(PTSD) 웹기반교육 개발)

  • 본 연구의 목적은 소방공무원이 현장에서 겪는 충격 스트레스로 발생하는 외상 후 스트레스장애 (posttraumatic stress disorder, PTSD) 분석을 통해 위기상황 스트레스 해소 교육 프로그램 기초 자료를 제시하고자 하는 데 있다. 연구 목적을 달성하기 위하여 독립변인(업무부담감, 스트레스, 스트레스 대응) 3개, 매개변인(현장충격 스트레스) 1개, 종속변인(신체적 증상)으로 구성하여 영향력을 파악하였다. 본 연구 대상자는 전국 970명 소방공무원으로 2007년 3월부터 12월까지 구조화된 설문지를 이용하여 조사하였다. 자료분석은 SPSS 14.0과 구조방정식 모형인 AMOS 7.0 통계패키지를 사용하였으며, 정확한 코딩데이터의 입력확인을 위해 데이터클리닝(data cleaning) 작업을 실시하였다. 가설검증을 위하여 구조방정식 모형분석을 실시한 결과, 업무부담감, 스트레스, 현장충격 스트레스가 낮고, 스트레스 대응이 높을 때 신체적 증상이 낮아지는 것으로 나타났다. 서울소방학교와 공동으로 총 10개 차시로 웹기반 교육을 구성하였으며, 2010년 1월부터 소방공무원 전체를 대상으로 웹기반 교육을 실시하고자 한다.

The Effect of Managerial Leadership on the Care Burden of Care Workers -Focus on the Leadership of Top-leader and Leadership of Mid-level Leader- (관리자의 리더십이 요양보호사의 케어부담감에 미치는 영향 -시설장의 리더십과 중간관리자의 리더십을 중심으로-)

  • The purpose of this study was to investigate the influence of the leadership of the top-leader and the mid-level leader on the care burden of care workers. A survey was conducted on 400 care workers working in long-term care facilities in Seoul and Gyeonggi area. We conducted a hierarchical regression analysis to grasp the effect of the leadership of top-leader and mid-level leader on the care burden of care workers. Monthly income of care workers was significantly associated with care burden. In next step, the leadership of the top-leader affected the care burden of care workers. But, In last model 3, only the leadership of mid-level leader affected the care burden with monthly income. This findings suggested that it is urgent to stabilize the salary level of care workers, and to develop the leadership skills of mid-level leader. The improved leadership of Mid-level Leader will help to reduce jop chaos for care workers.

  • This study attempts to explain phenomenologically the shadow work that occurs among Kwangju·Jeonnam business support counselors as video consultation increases due to the counseling management systems used in business consulting for SMEs and digital transformation since COVID-19. Since digital shadow work is widespread, it has been considered basic and mandatory work, not recognized as shadow work itself, assuming that it is continuously used for a long time. We found that, in the case of the newly expanded video consulting systems, most of the users complain of burden and inconvenience and tend to choose face-to-face despite the time and cost savings associated with the former. These study results practically suggest that, in order to implement such digital systems successfully, a well-prepared system, an elaborate way of reducing the burden and inconvenience, and an adaptation period are considered critical as they can minimize the shadow work of users.

  • 해상교통관제 업무는 선박사고 발생 위험에 대한 부담감 및 교대근무 등 불규칙한 근무형태로 인해 스트레스가 많은 직종이다. 특히나 관제 업무는 고도의 주의와 예측능력을 요구하는 업무로 이를 저해하는 인적요인 분석 및 관리 필요성이 절실한 분야지만 관제사의 직무스트레스에 관한 연구가 전무한 실정이다. 관제사의 스트레스로 인한 피로증상은 관제업무수행에 필요한 인지능력이나 의사결정 능력을 저하시킬 수 있고 이로 인해 업무수행에 차질을 빚을 경우 안전사고로 이어질 수 있는 위험이 존재한다. 그러므로 안전과 직결된 관제사의 스트레스는 과학적으로 연구되어야 하며 제도적 마련과 더불어 체계적 관리가 필요할 것이다. 이에 본 연구에서는 한국인 직무스트레스 측정도구를 이용하여 관제사의 직무 스트레스 수준을 파악하고 직무스트레스 관리의 필요성에 대하여 제고해보고자 한다.

An FGI Study on the Adaptation of Beginning Health Teachers During the COVID-19 Era (코로나-19 유행 시기 신규 보건교사의 교직 적응에 대한 FGI연구)

  • In this study, three health teachers assigned to J-do were analyzed through the FGI research method to find out the difficulties, resolution process, and implications of new health teachers in adapting to teaching during COVID-19. As a result of the study, first, there were difficulties in relationships with parents, especially in the course of numerous calls and daily school status reports in the course of handling infectious diseases. After getting used to the COVID-19 work to some extent, beginning health teachers were burdened with having to think about health room work alone, difficulties in health classes for various students, and the burden of showing expertise as the only medical personnel in emergency situations. Second, new health teachers continued to share and communicate related information through online health teacher groups and YouTube to solve the difficulties of teaching. Third, this teaching adaptation process suggests that specific training to provide practical help to new health teachers is needed at the education office level, and it is necessary to support infrastructure such as class contents related to health classes.
