Purpose: This study was undertaken to evaluate the clinical usefulness of Tensilon test, repetitive nerve stimulation test(RNST), single fiber EMG(SFEMG) test and acetylcholine receptor antibody(AchR Ab) assay for making diagnosis of myasthenia gravis(MG). Method: These tests were performed in 21 MG patients which were classified into 11 ocular, 5 mild generalized, 4 moderate generalized, and 1 chronic severe MG. Result: The overall positivity of Tensilon test, SFEMG and AchR Ab was 95%, 87%, and 76% respectively. The overall positivity of RNST was 67%; 38% on flexor carpi ulnaris, 43% on adductor digiti quinti and 62% on orbicularis oculi muscles. The positivity of each test was higher in generalized MG group than in ocular MG group. But we could observe the statistically significant difference only in the RNST(p<0.05). Conclusion: Tensilon test showed the highest positivity in all MG groups. So we would like to recommend the, Tensilon test for the diagnosis of MG at first, followed by RNST and AchR Ab assay, and SFEMG would be indicated to MG group which showed relatively low postivity in other tests.
Hepatitis B virus(HBV) infection is one of the major health problems in Korea and HBsAg positive rate was known to be about $5\sim15%$ in general population. This study was conducted to identify the positive rates of serum HBsAg and anti-HBs among community population regarded as having hish HBV vaccination rate than in previous decade, using EIA(Enzyme immunoassay) method, in Seo-Gu, Taegu, Korea. The study subjects were 1,160 who visited Seo-Gu Health Center for check-up serologic markers of hepatitis 3. The data were obtained from the serologic test for hepatitis markers and questionnaire survey was conducted to obtain the general characteristics, vaccination history, past history of hepatitis and other liver disease, and exposure history to risk factors of hepatitis of the study subjects. The positive rates of HBsAg and anti-HBs were 5.2% and 62.4% respectively. The positive rates of HBsAg for male and female were 6.6% and 4.3% respectively. The age was divided into two groups as group I (less than 15 years old), group II (more than 16 years old) according to the hypothesis that these two groups might be different in HBV vaccination rate. HBV vaccination rates for group I and II were 83.1% and 52.3%. The positive rates of HBsAg for group I and II were 2.6% and 6.5%. The positive rates of HBsAg for the vaccinated people of the group I and II were 2.2% and 3.5%, the positive rates of anti-HBs for the vaccinated people of the group I and II were 70.1% and 71.1% respectively. The most significant factor in positive rate of HBsAg was 'hepatitis carrier in family'. Multiple logistic regression analysis revealed that 'hepatitis history' and 'hepatitis carrier in family' were significant variables for positivity of HBsAg, and 'hepatitis B vaccination' was only a significant variable for positivity of anti-HBs.
Purpose: We measured anti-H. pylori IgG in Korean elementary school children living in Shinchon area of Seoul, Korea to evaluate the influence of environmental living standards on H. pylori infection. Methods: IgG antibodies to H. pylori were measured in plasma using a commercial ELISA kit (GAP IgG Helicobacter pylori, Bio-Rad Laboratories Inc., Hercules, CA, USA). Information on environmental status such as place of birth, parental income, type of housing, number of persons in the household, parents' occupation, family history of peptic ulcer disease and gastric cancer was obtained. Statistical analysis was done by Chi-square and logistic regression test using SPSS $7.0^{TM}$ for Windows. Results: Study subjects consisted of 571 children, and the age distribution ranged from 6.0 to 13.6 years with a mean of $9.6{\pm}1.8$ years. Male-to-female ratio was 1.1:1. The seropositive rates of H. pylori infection ranged from 10.4% in children aged 6 years to 30.9% in 12 year-old group, overall 16.8%. The prevalence of H. pylori infection progressively increased with age, but there was no significant difference in seropositive rates among children in different age groups (p=0.06). Seropositive rates of anti-H. pylori IgG on the basis of gender, place of birth, parental income, type of housing, parents' occupation, family history of peptic ulcer disease and gastric cancer showed no statistically significant difference. Interestingly, however, seropositive rate of anti-H. pylori IgG showed statistical significance in relation to number of persons in the household (p=0.003; Odds ratio 1.50 by logistic regression test). Conclusion: These results suggest that number of persons in the household is the most important factor among environmental living standards, and that risk of H. pylori infection increases by increment of 1.5 times as the number of persons in the household increases by one.

An epidemiological study was undertaken to evaluate the prevalence of E. vermicularis infestation among children in primary schools and orphanages. A total of 2,966 children in primary schools and orphanages was examined for F. vermicularis by means of cellotape anal swab technique. The purpose of the present studies is to determine the prevalence of enterobiasis among children and to make a comparison with the rate of infestation by survey areas. The result could be summarized as follows : 1) The egg positive rate of cellotape anal swab method was 16.3% out of 2,966 children. 2) The egg positive rate of urban area was 6.9% out of 1,144 children and the egg positive rate of rural area was significantly higher than that of urban area. 3) The egg positive rate of boys and girls were 15.7% out of 1,528 children and 16.9% out of 1,438 children. 4) The egg positive rate of primary schools, orphanages and mental retardation child group was 11.3% (out of 2,412), 32.7% (out of 416) and 55.1% (out of 138) respectively. There was noted a tendency of decrease in prevalence by increase of the age.

Objectives: Since 1983, vaccination for Hepatitis B virus has been recommended for all neonates in Korea. The objectives of this study was to assess the changing trend of the positive rate of HBsAg and anti-HBs in university students in one province. Methods: The study population consisted of one university students who taken physical exam in 2004, which included 8,527 students (3,391 males and 5,136 females). Serum HBsAg and anti-HBs were detected by immunochromatography method. Results: The positivities of HBsAg and anti-HBs were 2.7% and 61.1%. The positivity of HBsAg was significantly higher in males(p< .05), while that of anti-HBs was significantly higher in females(p< .05). There was some differences of HBsAg positivities among age groups. But there was no correlation between age and the positivities. There was some differences of anti-HBs positivities among age groups. But there was no correlation between age and the positivities. Conclusion: The positivity of HBsAg has significantly decreased and that of anti-HBs has increased with years. It was suggested that the changes of positivity were caused by nationwide hepatitis B vaccination program.

Toxocariasis is produced by the migration of Toxocara canis larvae into the extra- intestinal tissue of unnatural hosts or natural hosts under unsuitable conditions. Soil contaminated with T. canis embryonated eggs in the main source of infection of man. In the present study, ELISA with T. canis adult crude antigen was used for determination the seroprevalence of T. canis infection in two areas of Korea. It was found that antibody positive rate was 15.7% in Keoje- Island. In the analysis according to sex, female group presented significantly higher positive rate than male group (23.8% in female and 7.4% in male, p<0.05). In Inchon city, the positive rate was 13.1%, and there was no significant difference between female and male group. Immunoblot analysis was performed to some positive patient sera. As the results, 9 cases of 15 cases were positive in Keoje-Island, and 22 cases of 27cases were positive in Inchon city by immunoblotting.

Purpose: Cytomegalovirus (CMV) infection is mostly asymptomatic but can be detrimental to certain hosts. We investigated changes of CMV seroprevalence in Koreans before and after the year 2000. Methods: We reviewed laboratory values of patients who were tested for CMV immunoglobulin G (IgG) at Samsung Medical Center, Seoul, Korea, from January 1995 to December 2015. Changes in seroprevalence were analyzed by gender, age, region, and tested year period (period 1, 1995-2005 vs. period 2, 2006-2015). Results: Overall CMV seropositivity was 94.1% (10,900/11,584). There was no significant difference for CMV seropositivity among the two periods (94.2% vs. 94.1%) (P=0.862). CMV seropositivity in the 11 to 20-year age group in period 2 (78.8%) was significantly lower than that of period 1 (89.9%) (P=0.001). The seropositivity of individuals aged 31-40 years (97.4%) was significantly higher than that of younger age groups (P<0.001) and lower than that of older age groups (P<0.001). Of 2,441 females of reproductive age (from 15 to 49), CMV seropositivity was 97% (2,467/2,441). The seropositivity in women aged 20-24-years was higher than that of men in the same age group (97.6% vs. 85.6%, P=0.003). No significant difference was observed among different regions. Conclusions: Overall CMV seropositivity of Koreans was estimated to be 94% and the average seropositivity of reproductive women was 97%. Monitoring of the changes in seroprevalence including the reproductive age group is needed in the future.
Purpose: To evaluate the hepatitis B vaccination program which has been conducted since 1980, Korea. Methods: This study was carry out self reported questionnaire and serologic test covering 2,072 elementary school students who were born between 1980 and 1987, selected by cluster sampling. The HBV serologic markers (HBsAg, Anti-HBs and Anti-HBc) were tested by radioimmunoassay (RIA). The contents of questionnaire include demographic data of students and parents, vaccination status, vaccination frequency, vaccination age, past history of mother's HBV test. Results: 1) The HBsAg positive rates by sex showed 3.7% for male and 2.7% for female, representing an average rate of 3.4%. The HBsAg positive rates by age group showed 5.6% for 13 years and l.5% for 6 years, representing a tendency of lowering rate as ages being younger. 2) The pre-natal HBV test rate was 10.2%, while post-natal HBV test rate was 42.5%. The test showed that the parents' educational level being higher than others, the pre- and post-natal HBV test showed higher rates. In case the fathers occupation being office worker, the post-natal HBV test showed a higher rate compared with other occupation. 3) Overall vaccination rate was 82.6%, complete vaccination rate 69.8%, booster injection rate 42.8%. The vaccination rate, complete vaccination rate and booster injection rate increased as the age being younger. If the educational level of parents were higher, the vaccination rate, complete vaccination rate and booster injection rate showed higher rates. Younger students showed younger vaccination age, and higher educational background of family showed younger vaccination ages. 4. With regard to positive rate of HBV markers by vaccination age, HBsAg and Anti-HBc positive rate showed higher degrees in the following order; preschool age (1-6 years), school age (6-13 years) and infancy (0-1 year). Anti-HBs positive rate was increased as the frequency of vaccination increased. Five years after initial complete vaccination, minimum protective rate was lowest, 69.6%.
Background: Most of malignant pleural effusions are serous but 8-33% of them are bloody. We wanted to evaluate the relationships between gross appearance and pleural CEA level or results of histocytology in malignancy associated pleural effusions. We also tried to reevaluate the meaning of CEA measurement in histocytologically proved or unproved malignancy associated pleural effusions. Methods: We studied 98 cases of malignancy associated pleural effusions, 50 cases of histocytologically proven malignant effusions and 48 cases of histocytologically unproven paramalignant effusions. We had observed gross appearance and conventional laboratory values and CEA levels for pleural effusions. Results: 44.9% of malignancy associated effusions were bloody(63.6% of bloody effusions were histocytologically proven malignant effusion). 65.0% of malignancy associated pleural effusions which have RBCs numbers over $100,000/mm^3$ were cytologically proven malignant effusions. 72.7% of cytologically proven malignant effusions had increased pleural fluid CEA level over 10 ng/ml. 58.2% of cases with pleural CEA over 10 ng/ml had positive results in pleural bistocytology. There was no definable relationships between pleural fluid CEA elevation and RBCs numbers and results of pleural fluid cytology. Conclusion: About half of the cases with malignancy associated pleural effusions were bloody. Histocytologically proven malignant effusions were more common in bloody effusion than non-bloody effusion(63.6% Vs 38.9%). But increased red blood cell numbers was not associated with positivity of pleural histocytology. Pleural fluid CEA elevation(over 10 ng/ml) was not correlated with positive pleural histocytology. But pleural fluid CEA elevation was rare in nonmalignant pleural effusions, and than pleural CEA measurement in uncertain pleural effusions maybe helpful to distinguishes its origin.
Ten farrow-finish farms participated in this seromonitoring that was conducted to investigate the porcine brucellosis situation in Korea. In total, eight (80.0%) of the 10 farms and 139 (24.0%) of 578 pigs tested showed a positive response in the Rose Bengal test (RBT). Seroprevalence levels were determined using RBT according to age; 35 (14.6%) of 239 piglets, 36 (31.3%) of 115 growing pigs, and 68 (30.4%) of 224 finishing pigs and sows were positive, respectively. All positive samples in RBT were tested with the tube agglutination test (TAT) and competitive ELISA (C-ELISA), simultaneously. Although 48 samples came up positive in the TAT, all samples tested with C-ELISA were negative. Among 26 rectal swab samples from the TAT positive-pigs, Yersinia enterocolitica O:9 was isolated from seven samples (26.9%). Therefore, we speculated that the positive reaction of RBT and TAT in this study might be induced by the serologically cross-reacting bacteria with Brucella abortus.
