The Biomedical Medicalization of Depression in Korea (우울증의 '생의학적 의료화' 형성 과정)

  • This paper investigated the biomedical medicalization of "depression"which is growing fast in Korea in terms of the treatment mechanism. Depression has been regarded as a mental disease that occurs with mixing various causations because what was the cause of this disease was not clarified until a recent date. Thus, as depression treatment, the medicine and the psycho-socio therapy have been used. However, from 1990s, as the brain science was introduced in the western society, and the high-tech diagnostic equipment of the brain disease and new drugs for the mental disease were developed, depression was rapidly redefined as 'the brain nerve system illness'that is easy to be taken and is able to obtain the permanent relief with the regular care. Under the influence of the redefinition of depression and the new treatment of it, recently, 8% of depression patients per year emerge as the aggressive cure subjects in the Korean psychiatric circle. However, according to the Korean psychiatric circle's unofficial calculation, it is estimated that only 10% of depression patients are receiving the accurate treatment but over 80% of the patients are not. If so, what does this estimation mean? Based on this question, this paper critically investigated the biomedical medicalization of depression.

A Study of Research on Methods of Automated Biomedical Document Classification using Topic Modeling and Deep Learning (토픽모델링과 딥 러닝을 활용한 생의학 문헌 자동 분류 기법 연구)

  • This research evaluated differences of classification performance for feature selection methods using LDA topic model and Doc2Vec which is based on word embedding using deep learning, feature corpus sizes and classification algorithms. In addition to find the feature corpus with high performance of classification, an experiment was conducted using feature corpus was composed differently according to the location of the document and by adjusting the size of the feature corpus. Conclusionally, in the experiments using deep learning evaluate training frequency and specifically considered information for context inference. This study constructed biomedical document dataset, Disease-35083 which consisted biomedical scholarly documents provided by PMC and categorized by the disease category. Throughout the study this research verifies which type and size of feature corpus produces the highest performance and, also suggests some feature corpus which carry an extensibility to specific feature by displaying efficiency during the training time. Additionally, this research compares the differences between deep learning and existing method and suggests an appropriate method by classification environment.

Institutional Ethnography on Organization of Case Management Practice at a Mental Health Center (정신보건센터 사례관리 실천의 조직화에 관한 제도적 문화기술지)

  • This study is a research that investigated organization of case management practice at a mental health center. For organization research of case management practice, this study applied the research method of Institutional Ethnography, and analyzed 'work knowledge' and 'texts' data which were obtained through field work that lasted 3 months with 11 study participants. The analysis is not interpretation of the researcher, but it is work that assembles 'work knowledge' of field workers until they reach discovering institutions that organize case management practice. As a result of this study, case management practice of the mental health center was being standardized as 'a service that is biased towards symptom management without comprehensive and continual resources links' by new public management theory, bio-medicine discussion, 'standardization policy', 'treatment rate improvement policy', 'performance-based budget system', 'performance evaluation system' and 'continual linkage policy', mediating texts. This is a different outcome from usefulness of case management that is claimed to support politically.

생의학 의용을 위한 마이크로 웨이브 대기압 플라즈마 장비

  • 대기압 플라즈마는 멸균과 살균, 지혈, 피부재생, 치아 미백 등 여러 의학 분야를 대상으로 그 효과를 나타내고 있으며, 플라즈마 장비를 만들어 내기 위해 부피가 큰 진공 장비가 필요하지 않다는 점에서 대기압 플라즈마는 그 활용과 효과에 있어 큰 기대를 받고 있다. 대기압에서 플라즈마는 다양한 주파수를 이용하여 만들어져 왔으며, 본 연구실에서 연구하고 있는 수백 MHz-수 GHz 대역의 파워를 사용하는 플라즈마의 경우 대기압 플라즈마를 의학 분야에 사용할 때 만족해야 할 조건들에 만족하는 특성을 보여준다. 기존의 고주파를 사용하는 장비의 경우 추가적인 Matching 장비로 인해 플라즈마를 만들기 위해 큰 장비와 높은 파워가 필요한 단점이 있었다. 하지만 이 마이크로웨이브 장비는 전송선 이론을 기반으로 장비 자체가 구조적인 Matching이 이루어 지도록 설계되었다(그림 1). 즉, 추가적인 Matching 장비의 필요 없이 외부에서 파워를 주는 것만으로 플라즈마를 발생 시킬 수 있으며, 50% 이상의 파워 효율을 보여준다. 또한 그 크기도 손에 쥐고 사용할 수 있을 볼펜 정도의 크기이며, 3W의 정도의 저 전력으로 플라즈마를 발생 시켰다. 높은 에너지를 가지는 전자들은 공급되는 기체뿐만 아니라, 주변 공기와의 반응하여 여러 응용분야에 적합한 활성 종을 다량 만들어내게 된다. 본 연구실의 강점인 플라즈마 시뮬레이션으로 얻은 결과에서 주파수가 올라 갈수록 높은 에너지를 가지는 전자들이 많아지는 것을 보여준다. 그리고 발생시킨 플라즈마의 광학 특성에서도 생의학 분야에 적합한 많은 활성 종들이 발생 되는 것을 확인하였다. 일반적으로 의학 분야에 사용되는 플라즈마의 경우 플라즈마에서 발생하는 열에 의한 피해를 최소화 하는 것이 중요하다. 마이크로웨이브 플라즈마의 경우, 그 플라즈마의 온도가 50$50^{\circ}$C 미만으로 의학 분야에 사용하기 적합하다. 또한 구동 주파수가 올라갈수록 플라즈마를 유지하는데 필요한 전압이 상대적으로 낮아지게 되는데, 이는 전기적 쇼크 등 플라즈마 의용에서 발생하는 안전성 문제에 있어서도 마이크로 웨이브 장비가 좋은 점이다. 본 플라즈마 장비를 구동하기 위한 손바닥 크기 정도의 소형의 전용 파워 장치를 개발함으로써 저전력 소형 플라즈마 장치를 개발하는 것을 목표로 하고 있다. 마이크로 웨이브 장비는 여러 가지 분야에서 그 효과를 검증 받았다. 혈액 응고 실험에서 30초 정도의 짧은 처리만으로도 자연 응고에 비해 탁월한 지혈 효과를 보여줬다 (그림 2). 충치를 발생시키는 대표적인 구강균인 S.mutans의 살균 실험에서 Ti02와의 복합적인 처리를 통해 30초 미만의 처리로 처리하지 않은 것에 비해 10-6 만큼의 줄어드는 살균 효과를 보여줬다. 뿐만 아니라 치아의 미백에 있어서도 탁월한 효과를 나타냈다. 현재 본연구실에서는 마이크로 웨이브 장비의 기본적인 구조를 응용하여, 좀더 넓은 영역을 처리할 수 있는 대면적 마이크로 웨이브 장비를 위한 연구를 수행 중이다.

What Should We do with Korea's Biomedical Model of Medicine? - From Biomedical to Biopsychosocial Model - (우리나라 의료의 생의학적 모델 어떻게 할 것인가? - 생의학적 모델에서 생물정신사회적 모델로 -)

  • Understanding the biopsychosocial model of illness is crucial for any meaningful advance of health. The maintenance and promotion of health is achieved by different combinations of physical, mental, social and spiritual well-being. Health is not an objective of living. It is not only a state, but also a resource for everyday life. Health is a positive concept that emphasizes personal and social resources, as well as physical capacities. Understanding the biopsychosocial model of health and disease is very important in the medical system. George Engel challenged the medical profession to reconsider a strict biomedical approach to medical education and care, and to embrace a "new medical model," the biopsychosocial model. He argued that humans are at once biological, psychological, and social beings who behave in certain ways that can promote or harm their health. Although understanding the biopsychosocial model of illness is important, Korea's medical system have mainly been focusing on the biomedical model of illness. I would like to highlight the importance of biopsychosocial model of illness for Korea's medical system and real clinical field according to the 20th anniversary of Korean Society of Psychosomatic Medicine.

The Relationship among Learning Engagement, Emotional Intelligence, and Academic Resilicence of Nursing Students : The Moderated Mediating Effect of Self Regulation (간호대학생의 학습참여, 감성지능, 학업탄력성과의 관계 : 자기조절의 조절된 매개효과)

  • The purpose of this study was to examine mediating effect of emotional intelligence on relationship between nursing students' learning engagement and academic resilience, find whether self-regulation would moderating the relationship between learning engagement and emotional intelligence. The data were collected from 277 nursing students from three colleges in J province and were analyzed with a regression analysis and bootstrapping. As a result of the study, first, the fit of the causal model between learning engagement, emotional intelligence, academic resilience and self-regulation of nursing students was found to be good, and the causal relationship between variables was predicted appropriately. Second, partially mediating effect of emotional intelligence on the path of nursing students' learning engagement affecting academic resilience. Third, self-regulation had moderating effect on learning engagement affecting emotional intelligence. Finally, the significance of this study is that the influence of various variables that can affect the academic resilience of nursing college students was verified, and in order to improve academic resilience, a strategy that considers the subjects' learning engagement, emotional intelligence, and self-regulation.

The Mediating Effects of Major Satisfaction in the Relationship of Nursing Professionalism and Career Preparation Behavior in Nursing Students (간호대학생의 간호전문직관과 진로준비행동의 관계에서 전공만족도의 매개효과)

  • The purpose of this study was to examine the mediating effects of perceived nursing professionalism and career preparation behavior on the effects of major satisfaction of nursing students. For this purpose, 231 nursing students from 4 university in 4 city were sampled for convenience. The result of this study were as follows:First, nursing professionalism of nursing students directly affected career preparation behavior and major satisfaction, which was statistically significant. Second, nursing professionalism showed mediating effects in relationship between career preparation behavior. Third, the major satisfaction of nursing students directly affected nursing professionalism and career preparation behavior. Based on the results of the research, it is necessary to develop an intervention program that strengthen major satisfaction of nursing students.

Influencing Factors the Metacognition and Learning Motivation on Problem-Solving Ability in Nursing Students (간호대학생의 메타인지, 학습 동기가 문제해결력에 미치는 영향요인)

  • The purpose of this research is to identify how the metacognition and learning motivation of nursing students affect problem-solving ability. The subjects of this study were 160 students attending nursing universities, and the data were collected from 20 January to 10 March 2021. Data were analyzed using the SPSS/WIN 24.0 program. As a result of this study, there was a significant positive correlation between problem-solving ability, metacognition (r=.44, p<.001), and learning motivation(r=.45, p<.001). factors affecting the problem-solving ability of nursing students were identified in order of learning motivation, metacognition. Based on the results of this study, it is hoped that programs will be developed and applied to improve the problem-solving ability of nursing students.

The Causal Relationship among Academic Resilience, Covert Narcissism, Interpersonal Competence and Self-Efficacy of Nursing Students (간호대학생의 학업탄력성과 내현적자기애, 대인관계능력 및 자기효능감과의 인과적 관계)

  • This study intended to investigate the structural relationship and the pathway of academic resilience, covert narcissism, interpersonal competence and self-efficacy for nursing students. For this purpose, 460 nursing students from three colleges in J province were sampled for convenience. The result of this study were as follows: First, the fix indexed of causal model among academic resilience, covert narcissism, interpersonal competence and self-efficacy were identified suitably. Second, covert narcissism, interpersonal competence and self-efficacy of nursing students has direct effect on academic resilience, it was statistically significant. Third, interpersonal competence and self-efficacy of nursing students showed mediating effects on the relationship between covert narcissism and academic resilience, but they were competitive effects. Self-efficacy mediated the relationship between interpersonal competence and academic resilience. Based on the results of the study, the necessity of developing an effective education program considering the academic resilience and related factors of nursing students was suggested.

The Effect of Covert Narcissism of Nursing Students on Nursing Professionalism : Mediating effect of Perceived Social Support and Interpersonal Competence (간호대학생의 내현적 자기애가 간호전문직관에 미치는 영향 : 지각된 사회적 지지와 대인관계능력의 매개효과)

Third, interpersonal competence showed mediating effects in relationship between covert narcissism and nursing professionalism. Fourth, the interpersonal competence of nursing students directly affected perceived social support and nursing professionalism. Based on the results of the research, it is necessary to develop an intervention program that can enhance interpersonal competence in order to strengthen nursing professionalism of nursing students.
