Meta-analysis of the Interventions for Caring Depression of the Elderly in the Four Countries: A Comparison of the Total Effectiveness, Short-term Effectiveness, and Long-term Effectiveness (노인 우울증 관리 프로그램의 효과성 메타 분석: 전체·단기·장기 효과성의 비교)

  • The purpose of this research is to comparatively meta-analyze the total, short-term, and long-termeffectiveness of cases involving care of the elderly depression in the age range of 60 and over in the four countries, and to identify the relevant policy implications for developing depression care programmes for Korean older people. Ten studies conducted by RCT were found via AMED, EMBASE, Ovid Medline, PsycInfo. Use of Review Manager(5.5 version) shows that the interventions for caring depression were all effective: total effectiveness is OR=0.47(95% CI), short-termeffectiveness is OR=0.37(95% CI), and long-term effectiveness is OR=0.61(95% CI). This research provides three policy implications: Firstly, elements for increasing the long-term effectiveness of depression care interventions should be applied to all new programmes for caring elderly depression. Secondly, more focused depression interventions should be applied during the first half period of care programmes for elderly men, whilst the focus should be shifted to the last half period for elderly women. Finally, new interventions for caring depression that integrate both the medical and social support model of depression should be designed for elderly Koreans.

  • 대한민국은 고령 사회로 진입하고 있을 정도로 노인인구가 빠르게 증가하고 있다. 이에 따라 각종 노인성 질환 역시 빠르게 늘어나고 있어 이것은 개인뿐 아니라 가족과 사회에 커다란 고통과 부담을 주고 있다. 그중에서도 우울증은 노인정신 질환 중에 가장 흔하며 노인자살의 원인이 되는 등 심각한 사회문제가 되고 있다.

  • Depression is one of the greatest health concerns in the world. Symptoms include emotion disorders, loss of appetite, reduction in interest, and insomnia. The prevalence of depression is increasing rapidly around the world and in South Korea. Elderly people are vulnerable to depression, resulting in serious socioeconomic problems. Depression in the elderly can cause sleep disorders, cognitive impairment, memory impairment, and many other diseases. Additionally, depression can cause frailty, which is associated with increased falls, chronic diseases, and mortality. Therefore, this study empirically analyzed sleep disorders in relation to depression, the link between exercise and the hippocampus in mitigating depression, and physical activity and exercise in the management of depression for successful aging.

Association between Depression and Sociodemographic Factors of Elderly Welfare Facility Users in a City (일도시 노인복지시설 이용자들에서 우울증과 인구사회학적 요인과의 연관성)

  • Objectives : As the prevalence of elderly depression increases, it becomes urgent problem to provide preventive and management measures. But in practice, it is difficult to detect depression in early stage. The purpose of this study is to evaluate the association between sociodemographic factors and depression in elderly welfare facility users in a city. Methods : In this research, the severity of depression and sociodemographic factors(gender, age, education, co-morbid disease, housing type, number of children, number of family members living with the subjects) was evaluated through PHQ-9 and interview. Using the data, the associations of depression and sociodemographic factors were analyzed. Based on PHQ-9 depression cut-off value(10 points), the subjects were divided into two groups, and the difference of the variables between groups were analyzed statistically. Results : 'Own house'(YES/NO), education(less than elementary school graduation/more than elementary school graduation) were sociodemographic factors which showed significance difference in mean PHQ-9 scores. Group with Over PHQ-9 10 points showed less 'having their own house' and less education level than group with less PHQ-9 10 points. Conclusions : With this research, it is expected that the risk factors for the elderly depression can be understood and the measures for early detection and invention of elderly depression would be provided.

Relationship between Sleep Disturbances and Cognitive Impairments in Older Adults with Depression (노인성 우울증 환자에서 수면 장애와 인지기능 저하의 관련성)

  • Objectives: Depression, sleep complaints and cognitive impairments are commonly observed in the elderly. Elderly subjects with depressive symptoms have been found to show both poor cognitive performances and sleep disturbances. However, the relationship between sleep complaints and cognitive dysfunction in elderly depression is not clear. The aim of this study is to identify the association between sleep disturbances and cognitive decline in late-life depression. Methods: A total of 282 elderly people who underwent nocturnal polysomnography in a sleep laboratory were enrolled in the study. The Korean version of the Neuropsychological Assessment Battery developed by the Consortium to Establish a Registry for Alzheimer's Disease (CERAD-K) was applied to evaluate cognitive function. Depressive symptoms were assessed with the geriatric depression scale (GDS) and subjective sleep quality was measured using the Pittsburg sleep quality index (PSQI). Results: The control group ($GDS{\leq}9$) when compared with mild ($10{\leq}GDS{\leq}16$) and severe ($17{\leq}GDS$) depression groups, had significantly different scores in the Trail making test part B (TMT-B), Benton visual retention test part A (BVRT-A), and Stroop color and word test (SCWT)(all tests p<0.05). The PSQI score, REM sleep duration, apnea-hypopnea index and oxygen desaturation index were significantly different across the three groups (all indices, p<0.05). A stepwise multiple regression model showed that educational level, age and GDS score were predictive for both TMT-B time (adjusted $R^2$=35.6%, p<0.001) and BVRT-A score (adjusted $R^2$=28.3%, p<0.001). SCWT score was predicted by educational level, age, apnea-hypopnea index (AHI) and GDS score (adjusted $R^2$=20.6%, p<0.001). Poor sleep quality and sleep structure alterations observed in depression did not have any significant effects on cognitive deterioration. Conclusion: Older adults with depressive symptoms showed mild sleep alterations and poor cognitive performances. However, we found no association between sleep disturbances (except sleep apnea) and cognitive difficulties in elderly subjects with depressive symptoms. It is possible that the impact of sleep disruptions on cognitive abilities was hindered by the confounding effect of age, education and depressive symptoms.

The Effectiveness of Problem-Solving Treatment on Geriatric Depression (노인 우울증에 대한 문제해결치료(Problem-Solving Treatment)의 효과성 연구)

  • Research was undertaken to examine the effectiveness of PST for the depression of Korean Elderly. The program and manual for korean elderly was developed based on PST procedures for depression as outlined by Nezu and D'Zurilla(2007). For the purpose, 150 old people over 65 who resided in chunchun were randomly selected. and screened with PHQ-9. 42 elderly who had depression symptom at middle level, were chosen for study and randomly assigned into experimental group(n=23) and control group(n=19). PST is a six session, manual-based conducted over an 6-week period by trained interviewers in the elderly's home. As a results, CES-D and KGDS of the experimental group have been significantly decreased and total SPSI-R has been significantly increased between pre-and post measures while no statically significant change was found in control group. This results suggest that problem-solving therapy program might decrease the depression and enhance the problem solving ability of the elderly and that PST can be suitable to home visit program with social work perspectives.

Concert Oriented Music Therapy(COMT) Alleviates Depression: Validated on Elderly Persons with Visual Impairments (연주회 중심의 음악치료에 대한 시각장애 노인의 우울 개선 효과)

  • Purpose: This study proposes a concert oriented music therapy (COMT) program which takes into account the personal and social factors of depression, and its clinical effects were validated on elderly persons with visual impairments who were susceptible to depression. The experiment was conducted on 34 elderly persons with visual impairments, randomly allocated to either the experimental group (n = 15) or the control group (n = 19). The COMT program was applied 20 times in total to the experimental group for 110 minutes every other week, for a duration of 10 months. The depression level was evaluated using the Korean form of the Geriatric Depression Scale (KGDS). The experimental results showed that the average KGDS score in the experimental group decreased by 4.1 points (19.4%), from 21.0 to 16.9, whereas it increased by 1.16 points (5.8%), from 19.95 to 21.11, in the control group. The reduction in the depression level was significant for the following 4 depression factors : decreased social interest (40%), negative thinking and the unhappiness feeling (20%), emotional discomfort (16.3%), and cognitive dysfunction (7.6%), whereas it was negligible in the factors of physical weakening and decreased vitality. The COMT program reduced the KGDS scores of the experimental group regardless of their experience in musical performance. In conclusion, the proposed COMT program proved to be efficacious in alleviating depression in elderly persons with visual impairments. In particular, it proved highly effective in helping with decreased social interest, namely, the social factor of depression which is not properly considered in existing music therapy methods.

  • It was conducted that yoga practice for the visually impaired Elderly has an influence on boby composition, flexibility and depression disease. Also, It has a purpose to provide a method of movement and basic materials for them. The 18 seventy-year-old men or women without any experience of yoga from the visually impaired association of the elderly in G city were participated in a training yoga session. This experiment was performed by simple random sampling with the nine of treat group (yoga training) and the nine of control group. The data was analyzed by covariance analysis with 0.05 significant level. The treat group were conducted twice a week to exercise (60mins) for twelve weeks. In conclusion, it is proved that yoga exercise has a positive effect on body composition, flexibility and depression disease.

Geriatric Depression and Suicidality According to Residence Type among the Elderly in a Rural City Area (거주형태에 따른 노인 우울증과 자살경향성 비교 연구)

  • Objective : This study aimed to investigate and compare the prevalence of depression and suicidality among the elderly in a rural city according to their residence type. Methods : Participants were 311 elderly people (109 male and 202 female) in Icheon City, whom trained researchers interviewed and examined Geriatric Depression Scale, Social Support Scale, Instrumental Activities of Daily Living, Activities of Daily Living, Quality of Life Scale, and Mini International Neuropsychiatric Interview (MINI), module C. Using these data, we analyzed participants' risk factors associated with depression and suicidality. Results : The prevalence of depression was 28.0%. The prevalence in the elderly living alone and those admitted to residential facilities was significantly higher than that among the elderly living with family. Suicidality frequency was 19.6%. The suicidality frequency among the elderly living alone and those admitted to facilities was significantly higher than that among the elderly living with family. The risk factors for depression were age, admission to a facility, and low economic status. The suicidality risk factors were living alone, admission to a facility, poor social support, and a history of headache. Conclusion : These results showed the prevalence of depression and frequency of suicidality among the elderly in such a city was quite high. The results of this study remind psychiatrists of the importance of early detection and therapeutic intervention for the elderly with a high risk of depression and suicidality.
