Changes of symphysis morphology after chincup treatment (이모장치 착용 후 하악 이부의 헝태변화)

  • Although it is well known that the chincup, used to correct a skeletal class III malocclusion in growing children, reduce the mandibular prognathism by arresting the growth of the mandibular length and rotating the mandible posteroinferiorly, the majority of the studies about chincup is focused on condylar head that plays an Important role in mandibular growth. The aim of this study was to evaluate the morphologic change of the mandibular symphysis where extraoral force is applied directly during chincup treatment. The data lot this study were obtained from lateral cephalometric radiographs of 62 growing children(chincup group:32, control group:30) with mixed dentition who had been accepted lot the orthodontic treatment at Chonbuk National University Dental Hospital. The results were as follows : 1. Symphysis height was increased both in chincup therapy group and control group during treatment. Symphysis depth was decreased or maintained the initial values in chin cup therapy group, whereas increased in control group. Posterior symphysis depth was decreased both in chin cup therapy group and control group, but anterior svmphysis detph was increased in control group, whereas decreased in chincup therapy group. 2. Chin depth and chin curvature were increased in control group, whereas maintained or decreased in chincup therapy group during treatment. Chin angle, menton ang1e and symphysis angle were decreased in control group, whereas increased in chincup therapy group. It suggested that bone deposition in pogonion area that occur normally with mandibular growth was supressed by direct contact of chincup. 3. When growing children wear chincup, symphysis morphology was maintained due to inhibition of forward growth at mandibular symphysis. It may be due to the suppression of bone deposition in anterior part of symphysis.

Treatment of Frozen Shoulder under the Arthroscopic Capsular Release Combined with Manipulation (관절경하 관절낭 유리술과 강압교정술을 병용한 동결견의 치료)

  • Purpose: To evaluate effectiveness of arthroscopic capsular release combined with manipulation in frozen shoulder. Material and Method: 15 patient who had failed to respond to physical therapy were evaluated, which were treated with arthroscopic capsular release combined with manipulation, from July 1998 to March 2000. Result: At a mean of fifteen months(range, six to twenty four) after the combined procedure, the improvement in the score of Constant and Murley averaged 45 points. The mean improvement in motion was 76 degrees for abduction; 40 degrees and 65 degrees for external rotation in adduction and abduction. Conclusion : Arthroscopic capsular release with manipulation is useful method to treatment the frozen shoulder which was not respond to conservative treatment.

A STUDY ON THE EFFECT OF CHINCAP IN JUVENILE SKELETAL GLASS III MALOCCLUSION (유년기 III급 부정교합자에서 이모장치의 치료 효과에 관한 연구)

  • The purpose of this study was to investigate the changes in the craniofacial skeleton subsequent to chincap therapy in the juvenile skeletal Class III malocclusion with more appropriate control samples. The experimental group consisted of 29 Korean children(14 males, 15 females) who had skeletal Class III malocclusion with prognathic mandible and were undergone chincap thorny from the beginning of treatment. The control group was composed of 21 Korean children(10 males, 11 females) who had no orthodontic treatment, but with similar skeletal discrepancies to experimental group. Lateral cephalometric radiographs at the age of 7, and 2 years later were analyized and compared with student's t-test(p<0.05). The results of this study were as follows; 1. The control group without chincap therapy had not shown any improvement of the skeletal discrepancies, but had grown to be much severe. This means that the untreated Class III patient with prognathic mandible would not be corrected by growth. For the experimental group with chincap therapy, the anterior-posterior skeletal discrepancies and mandibular prognathism were both improved. 2. Neither significant restraint nor acceleration of growth was found in the cranial base and maxilla by chincap treatment. 3. The inhibition of mandibular growth could not be accepted, but the changes of the direction of growth and morphological changes were found. 4. Vertical growth tendency was increased with chincap therapy. 5. When Putting together the results of the analyses , it seems to be the rotation and displacement of the mandible that the major treatment effects of chincap we. The changes of the direction of growth and the morphological changes also seems to contibute to the treatment effect partly. In summary, the chincap doesn't restrain the mandibular growth. But, it is considered as a useful treatment modality for correction of skeletal discrepancies and reduction of mandibular prognathism in growing Class III patients with madibular prognathism.

Intravitreal Anti-vascular Endothelial Growth Factor Injections to Treat Neovascular Age-related Macular Degeneration: Long-term Treatment Outcomes (삼출 나이관련황반변성에 대한 항혈관내피성장인자 유리체내주사 치료의 장기 임상 결과)

  • Purpose: We assessed the visual and anatomical outcomes, and the safety profile of long-term intravitreal anti-vascular endothelial growth factor (VEGF) injections (aflibercept, ranibizumab, and bevacizumab) given to treat neovascular age-related macular degeneration (NAMD). Methods: We analyzed medical records collected over 7 years of treatment-naive NAMD patients who received outpatient clinic-based intravitreal anti-VEGF injections. All were treated employing either "treat-and-extend" or "as needed" protocols at the discretion of the retinal specialist. The number of injections, adverse events associated with injection, and measures of visual acuity (VA), central foveal thickness (CFT), and intraocular pressure (IOP) were recorded. Results: Overall, we assessed 196 eyes of 196 patients (average age $68.6{\pm}9.6years$; 77 females). Patients received an average of $17.3{\pm}13.5$ injections over $78.0{\pm}16.5months$ of clinical follow-up. The initial mean VA (logMAR) was $0.75{\pm}0.58$ and the CFT was $349.7{\pm}152.6{\mu}m$. Both parameters exhibited maximal improvements at the 6-month visit (p < 0.05). However, the clinical outcomes worsened over the 7-year clinical course; the best-corrected visual acuity (BCVA) was $0.91{\pm}0.78$ and the CFT was $284.5{\pm}105.8{\mu}m$ at 7 years. The BCVA at 7 years was significantly correlated with the initial BCVA. IOP-related events increased 11-fold and anterior chamber reactions increased 3-fold over the years, but no significant complications such as endophthalmitis were recorded. Conclusions: The use of intravitreal anti-VEGF agents was associated with initial visual improvements over 6 months but did not prevent the worsening of NAMD over 5 years. The BCVA at the initial visit was a strong predictor of the final BCVA. A more intensive injection schedule might improve long-term outcomes.

Effects of compressive stress on the expression of M-CSF, IL-$1{\beta}$, RANKL and OPG mRNA in periodontal ligament cells (압박력이 치주인대 세포의 M-CSF, IL-$1{\beta}$, RANKL 및 OPG mRNA 발현에 미치는 영향)

  • Objective: The aim of this study was to determine if human PDL cells can produce osteoclastogenic mRNA and examine how compressive stress affects the expression of osteoclastogenic mRNA in human PDL cells. Methods: Human PDL cells were obtained from biscupids extracted for orthodontic treatment. The compressive force was adjusted by increasing the number of cover glasses. PDL cells were subjected to a compressive force of 0.5, 1.0, 2.0, 3.0 or $4.0\;g/cm^2$ for 0.5, 1.5, 6, 24 or 48 hours. Reverse transcription polymerase chain reaction (RT-PCR) analysis was performed to examine levels of M-CSF, IL-$1{\beta}$, RANKL, OPG mRNA expression. Results: Human PDL cells could produce M-CSF mRNA. Human PDL cells under compressive stress showed increased M-CSF, IL-$1{\beta}$ and RANKL mRNAs expression in a force (up to $2\;g/cm^2$) and time-dependent manner. However, OPG mRNA expression was constant regardless of the level and duration of stress. Conclusions: Continuous compressive stress induced the mRNA expression of osteoclastogenic cytokines including M-CSF, RANKL, IL-$1{\beta}$ in PDL cells. Together with an unchanged OPG mRNA level, these results suggest that compressive stress-induced osteoclastogenesis in vivo is partly controlled by M-CSF, RANKL and IL-$1{\beta}$ expression in PDL cells.

The effects of maxillary protraction appliance (MPA) depending on vertical facial patterns (수직적 안모 형태에 따른 상악골 전방 견인 장치의 효과 비교)

  • Preadolescent children with deficient maxillae are suitable candidates for the maxillary protraction appliance(MPA). The theoretical effect of the MPA is protraction or anterior displacement of the maxilla. However, it is known that complex effects such as anterior displacement of the maxillary teeth, downward and backward rotation of the mandible, linguoversion of the mandibular anterior incisors, are known to play a role in improving the Cl III malocclusion. There have been much studies with regard to maxillary protraction, but the different effects of MPAs depending on the vertical facial pattern are not known precisely. This study was based on 67 patients (31 males, 36 females) aged from 6 years 6 months to 13 years 3months, who visited the Dept. of Orthodontics at Yonsei Univ., Dental Hospital and diagnosed as skeletal Class III with maxillary deficiency. They were divided into 3 groups (low, average, high angle groups) depending on genial angle and the SNMP (Go-Gn) angle, respectively. Pretreatment and post-treatment lateral cephalograms were used to compare the effects of MPA and the following conclusions were obtained: 1) A significantly large amount of backward movement of the B point was observed in patients with a low SNMP angle. Those with a high SNMP angle had significant forward movement at A point. 2) The patients with low genial angle had the least forward movement at the A point, and those with a high angle had more forward movement. 3) In comparing the arcTan of the A point, the high angle group showed more horizontal movement while the low angle group showed more vertical movement. 4) There was no significance between the treatment duration of the SNMP and the Genial angle groups.

Friction of calcium phosphate brackets to stainless steel wire (인산칼슘재 브라켓과 강선사이의 마찰저항에 관한 연구)

  • Esthetic brackets which resemble the color of natural teeth have been widely used. But the frictional resistance of ceramic brackets, a typical esthetic bracket, is greater than that of metal brackets. The purpose of this study was to measure the frictional resistance of the new calcium phosphate brackets (CPB) which were recently developed and to evaluate its clinical usability by comparing the frictional differences of CPB with metal brackets and metal slot inserted ceramic brackets. Methods: Experimental groups were CPB (Hyaline II, Tomy, Tokyo, Japan), metal bracket (Kosaka, Tomy, Tokyo, Japan) and metal slot inserted ceramic bracket (Clarity, 3M Unitek, Monrovia, CA, USA). All of the brackets had 0.022-inch slot sizes. The brackets were tested with $0.019\;{\times}\;0.025$ inch stainless steel wire (3M Unitek, Monrovia, CA, USA). A biologic model was used to simulate the situation which would occur during orthodontic treatment with fixed appliances. Retraction force was applied at a speed of 5 mm/min for 30 seconds. The frictional resistance was measured on a universal testing machine (Instron 4467, Instron, Norwood, MA, USA). Results: CPB showed significantly higher friction than metal brackets (p < 0.05) and lower friction than metal slot inserted ceramic brackets (p < 0.01). Conclusions: CPB can be considered to be a useful orthodontic esthetic bracket with respect to frictional resistance, as its friction is remarkably lower than that of metal slot inserted ceramic brackets.

THE ERUPTION GUIDANCE OF IMPACTED MAXILLARY ANTERIOR TEETH (맹출장애를 보이는 상악 전치의 맹출유도)

  • Unerupted maxillary anterior teeth is not a common case, However it may present practitioners with management problem. The cause of impaction is considered to be multifactorial, and local cause is the most common. These impacted teeth require surgical intervention, removal, transplantation, or surgical exposure, with or without orthodontic traction to align the malpositioned tooth. The preferred option is surgical exposure and orthodontic correction. Surgical intervention and orthodontic correction should not be delayed to avoid unnecessary difficulties in aligning the tooth in the arch. Surgical exposure should be performed with the intent of providing sufficient attached gingiva rather than simply uncovering the crown, which results in only alveolar mucosal attachment. Attached gingiva is essential to secure the gingival tissues to the adjacent teeth at the dentogingival junction. Thus preventing loss of periodontal tissues as a result of the pull of the surrounding soft tissues and facial muscles. Labially impacted maxillary anterior teeth uncovered with an apically positioned flap technique have more un- esthetic sequelae than those uncovered with a closed-eruption technique. In the case of severly displaced impacted teeth, autotransplantation ensures preservation of the alveolar bone and will facilitate future placement of an osseointegrated implant once growth has ceased or if ankylosis/resorption of the transplant occurs.

A Cognitive Degree on the Operative Dentistry, Prosdontis (치과보존 보철에 대한 인식도)

  • This study was carried out to investigate and analyze the congnitive degree of dental subjects and general pubic who have an experience to be treated for their teeth on the dental knowledge, operative dentistry, and dental prothesis in the oral health care through the questionnaires. The results are as follows: 1. In the knowledge on the care of dental prothetic stuff, there isn't any significant difference, but the degree of therapeutic knowledge was significantly high as showing 71.6% in I can prevent a gingivits;(P<0.01) Therefore, they have well known an importance of prevention. 2. In the knowledge on the orthodontics, it was appeared to cure their teeth due to an esthetic reason(47.9%). There is not any significance in the Statisics, but it appeared the relatively high knowledge in the time and period of orthodontics(P<0.01) 3. In the congnition on the necessity of dental prothesis, it appeared relatively high, but 'Be necessary' didn't show the significance as 77.2%. The proferance to dental prothesis material showed the similar difference of porcelain(45.6%) and gold(43.7%), and the moderns tend to ptefer porcelain teeth similar to the color of matural teeth in the modern, getting out of the existing cognition of preferring the gold. Accordingly it is considered to be preferentialy necessary the motivae the general public for the oral thealth education on the dental knowledge, operative dentistry, and dental prothesis the knowledge and practice of general public have an important effect upon the oral health as the abovementioned results.

Therapeutic Effect of Anti-Rotavirus Chicken Egg Yolk Immunoglobulin(IgY) on Diarrhea by Infection of Rotavirus (로타바이러스 감염성 설사에 대한 항-로타바이러스 난황항체의 치료 효과)

  • Purpose : Rotavirus is an enteric pathogen that affects millions of children globally each year. But no specific therapy is available for the management of rotavirus diarrhea. Due to the clear need to define improved modality for treatment of rotavirus diarrhea, we evaluated the efficacy of antirotavirus IgY in the treatment of infants and children with gastroenteritis. Methods : First, the amount of viral particle in the stools of thirteen patients(seven were given IgY, 6 placebo) infected by rotavirus were evaluated for 3 days with the quantitative RT-PCR method. Second, 36 children with known rotavirus infection identified by ELISA or semi-quantitative RT-PCR were evaluated. We gave 5 g anti-rotavirus egg yolk daily in two equally divided doses for 3 days to two groups(an 18 IgY group and an 18 placebo group), respectively after parenteral consent. Daily vomiting frequency, stool frequency, oral intake and urine output were monitored for 3 days, and electrolyte and blood chemistry were checked at the first and third days. Results : First, in the placebo group, the amount of virus particles increased daily, but in the IgY group it decreased daily. Second, when IgY and placebos were given to children infected with rotavirus, diarrhea on the third day decreased significantly in the IgY group, compared with the placebo group. Conclusion : Treatment with antirotavirus immunoglobulin from immunized chicken's egg resulted in a decrease in the amount of viral particles in stools and diarrhea frequency in children. These results suggest that anti-rotavirus IgY is effective in the treatment of rotavirus gastroenteritis.
