Usefulness of Triglyceride and Glucose Index to Predict the Risk of Hyperuricemia in Korean Adults (한국 성인에서 고요산혈증 위험을 예측하기 위한 중성지방-혈당 지수의 유용성)

  • The purpose of this study was to evaluate the usefulness of the triglyceride and glucose(TyG) index to predict the risk of hyperuricemia in Korean adults. This study included 14,266 men and 9,033 women over 20 years old who underwent health screenings from 2017 to 2019 at a general hospital in Seoul. To confirm the risk of hyperuricemia and predictive ability of the TyG index, logistic regression analysis and ROC curves were obtained. The accuracy of the TyG index for predicting hyperuricemia was 0.68, 0.61 for men and 0.67 for women(respectively p<0.001). The risk of hyperuricemia in the TyG index was 1.69 times higher in the fourth quartile than in the first quartile, 2.03 times higher in men and 2.07 times higher in women(respectively p<0.05). Thus the TyG index was not of high diagnostic usefulness as a screening test for hyperuricemia, but it was related to the TyG index and hyperuricemia.

  • This study aims to examine the relationship between hyperuricemia and metabolic syndrome, which is a risk to health, and to analyze the effect of hyperuricemia on the body. The analysis data were downloaded and used for the 8th 2nd (2020) data of the National Health and Nutrition Survey, and in this study, 2,320 men and 2,893 women were finally analyzed. For the analysis of the data, Chi-square test and t-test were used for the difference values according to collected general characteristics and hyperuricemia, and the risk of eGFR rise was analyzed by regression analysis, and Pearson correlation was used to confirm the correlation with each variable. Through this study, it was found that hyperuricemia is significantly related to metabolic syndrome, and through this, preemptive management is needed to prevent metabolic syndrome from worsening into vascular diseases including kidney diseases. Therefore, it is proposed to develop a health program suitable for the patient's age through this study.

  • This research is a data analysis result of a future gout treatment was carried out in order to take advantage of basic data by Planned Population Federation of Korea in 2012, classified by gender. Using SPSS 18.0 analysis, frequency analysis, chi-squared analysis and logistic regression analysis were conducted. As a result, there were exceptional number of male subjects that required hyperuricemia thorough complete examination, and in case of hypouricemia, it is notable to find much more female subjects (p<0.001). Subjects that required thorough complete examination per age group and hypouricemia were significantly higher in those above the age of 70 years(p<0.001). Conclusions that required hyperuricemia thorough complete examination were a bit higher in the city areas and hypouricemia was a bit higher in the county areas, hyperuricemia was a bit higher in ratio in inland areas and hypouricemia was a bit higher in ratio in the coastal areas. I believe that continuous observation of gout prevalence rate based on this research by year, age group, and gender, would be extremely useful in making decisions regarding the changing trend of gout prevalence rate. In addition, hyperuricemia increased in postmenopausal women, as well as research on the causes of hypouricemia study.

Association between Hyperuricemia and Hypertension in Korean Adult Women: Using the Eighth KNHANES(2019) (한국 여성의 고요산혈증이 고혈압에 미치는 요인: 국민건강영양조사 제8기 자료(2019년))

  • This study was conducted to investigate the association between hyperuricemia and the risk of hypertension in Korean adult women using data from the 2019 Korea National Health and Nutrition Examination Survey (KNHANES) 8th wave. Study subjects were 2,693 and descriptive analysis was used to investigate the general characteristics of the subjects. The effects of hyperuricemia on the risk of hypertension were evaluated using binomial logistic regression and it was shown that the risk of hypertension in the hyperuricemia group was higher than in the normal group (OR:1.94, p<.001), after adjusting socioeconomic factors (OR:1.81, p=.001) and even after additional adjustment for health behavioral factors, the risk of hypertension was high (OR:1.54, p=.019). Therefore, in order to prevent hypertension in adult women, it is necessary to develop programs and policies to effectively manage their uric acid levels.

A Study on the Prevalence and Risk of Hyperuricemia according to Sitting time and High-Risk Drinking by Occupational group (직업군별 좌식시간과 고위험음주에 따른 고요산혈증의 유병률과 위험도에 관한 연구)

  • This study was conducted to analyze with a focus on gender whether the prevalence of hyperuricemia varies depending on sitting time and high-risk drinking by occupational group. For this study, the Korea National Health and Nutrition Examination Survey data were used, and 16,366 people were selected. The chi-square independence test and logistic regression model were used as the analysis method. The prevalence and risk of hyperuricemia by sitting time were different in the 'agricultural, forestry and fishery skilled workers' only in men. On the other hand, the prevalence with high-risk drinking, both men and women showed differences in 'managers, experts and related workers' and 'office worker'. Also, only women have differences in 'service and sales workers', 'technicians, equipment, machine operation and assembly workers' and 'unemployed'. These results inform men have a higher prevalence and risk of hyperuricemia and suggest that health care policies and medical services are needed to prevent it by occupational group.

Relationship between Serum Uric Acid Levels, Insulin Resistance and Components of Metabolic Syndrome in Korean Adults (한국 성인의 혈청 요산 수치와 인슐린저항성 및 대사증후군 구성요소간의 연관성)

  • The purpose of this study was to investigate the relationship between serum uric acid levels, insulin resistance and components of metabolic syndrome. It was conducted on 4,428 adults over the age of 20 who had undergone health checkups at a general hospital in Gyeonggi-do from June 2018 to May 2020. As a result of the study, uric acid levels were higher in the metabolic syndrome subjects than normal subjects in both men and women. and the incidence of metabolic syndrome and its components was higher in the hyperuricemia group than in the normal group. Hyperuricemia was found to increase the risk of elevated blood pressure(p=0.006) and hypertriglyceridemia(p<0.001) in men and metabolic syndrome(p=0.012) and low HDL-cholesterol(p<0.001) in women. Thus, in both men and women, hyperuricemia was associated with metabolic syndrome and its components, and it was confirmed that it was an independent predictor of the onset of metabolic syndrome in women.

  • 저자들은 4년 전 모야모야병으로 진단되어 수술 받았던 16세의 여아에서 지속적으로 고지질혈증이 있어 검사를 시행한 결과 가족 중 여동생에게서도 고지질혈증이 있음이 확인되었고 대사성산증, 고요산혈증, 젖산혈증 및 간비대가 지속되어 시행했던 염기 서열 분석에서 당원병 type Ia로 진단된 1례를 경험하였기에 보고하는 바이다.

Protective effects of Sasa quelpaertensis Leaf Residue Extract against Potassium Oxonate-induced Hyperuricemia in Mice (생쥐에서 제주조릿대 잎 잔사 추출물의 고요산 혈증 저감 효과)

  • Leaves of Sasa quelpaertensis Nakai are used in folk medicine for their anti-inflammatory, antipyretic, and diuretic properties. To ensure efficient utilization of S. quelpaertensis leaf, we previously reported a preparation method for phytochemical-rich extract (PRE) using the leaf residue, which was produced after hot water extraction. This study was undertaken to evaluate the hypouricemic potential of S. quelpaertensis leaf PRE in potassium oxonate (PO)-induced hyperuricemic mice. The administration of PRE significantly reduced serum uric acid (UA), blood urea nitrogen (BUN), and serum creatinine levels and increased urine UA and creatinine levels in the PO-induced hyperuricemic mice. It also reduced liver UA levels and xanthine oxidase (XA) activity. A histological analysis revealed that PRE administration protected against PO-induced liver damage, pointing to anti-inflammatory and cytoprotective effects in PO-induced hyperuricemic mice. We analyzed the transcriptome response to PRE administration in PO-induced hyperuricemic mice using RNA sequencing (RNA-Seq) in kidney tissues. The administration of PRE mainly enriched genes involved in mediating immune and inflammatory responses and the metabolic pathway. A Kyoto Encyclopedia of Genes and Genomes (KEGG) analysis showed that the metabolic pathway, purine metabolism, and antibody biosynthesis were the major pathways altered in the PRE and PO groups. These results suggest a potential role for PRE in the prevention and treatment of hyperuricemia with inflammation.

Hypouricemic Activity of Actinidia polygama (개다래의 고요산혈증 개선 활성)

  • Ethanol extract of Actinidia polygama Max. (AP) was examined for hypouricemic activity in potassium oxonateinduced hyperuricemic animal model. Ethanol extract of AP lowered the plasma uric acid level by 20% when compared to the hyperuricemic control group. AP ether fraction at a dose of 100 mg/kg showed 27% reduction, whose efficacy was comparable to the probenecid-treated group. AP ether fraction-treated group also showed 23% reduction of urate in urine and this result suggests that AP ether fraction has anti-gout activity probably due to xanthine oxidase inhibition.

  • A 67-year-old male patient with a history of epididymectomy and anti-tuberculosis treatment for epididymis tuberculosis was admitted for acute low back pain and radiating pain. The patient had no history of gout but showed hyperuricemia and a bone destruction lesion in the facet joint and lamina of the lumbar spine. A histology examination was performed after a computed tomography-guided needle biopsy, and the findings were compatible with gout spondyloarthropathy and tuberculous spondylitis. The acute symptoms improved after conservative treatment for gouty arthritis. When patients with hyperuricemia risk factors, such as taking anti-tuberculosis drugs, complain of acute low back pain, gout spondyloarthropathy should be considered in a differential diagnosis.
