Case Studies of Vital Pulpotomy for Treatment of Open Fractured Canines in Dogs (개에서 개방골절된 견치에 대한 치수절단술의 적용 증례)

  • Two cases of vital pulpotomy for treatment of open fractured canine teeth in dog were referred to the Veterinary Teaching Hospital of College of Veterinary Medicine, Chonnam National University. In case 1, a 2-year-old male Locker. Spaniel was fractured left mandibular canine and the pulp was exposed. In case 2, an 1-year-old male Border Collie was open fractured both mandibular canine teeth. In both cases, we made sure of pulp vitality by oral examination and radiological findings. Therefore, we performed vital pulpotomy in these patients. Two months after therapy, radiological examinations were performed for evaluation of success of therapy. In both cases the radiographs showed normal periapical anatomy and no changes in the root resorption.

A Study on the Prediction Percentile Tabulation of the size of the Uperupted Permanent Canines and Premolars in Korean Population (미맹출 영구견치 및 소구치 크기 추정에 관한 연구)

  • The primary objective of this study is to make the prediction percentile tabulation of the sun of mesiodistal width of the unerupted permanent canines and premolars derived from the sum of mesiodistal width of the erupted permanent mandibular incisors in Korean population. The subjects were 316 individuals with normal occlusion aged 11 to 23 years. The sum of mesiodistal width of mandibular incisors, and the sum of mesiodistal width of maxillary canine, first and second premolars, and the sum of mandibular canine, first and second premolars were measured from the dental cast models using the sliding caliper (Mitutoyo Co.). From the study, the results are as follows: 1. The sum of M-D width of mandibular incisors and the sum of M-D width of maxillary canines, first and second premolars, and the sum of M-D width of mandibular canine, first and second premolars were smaller than those of American Caucasians and Negros. 2. The correlation coefficient between the sum of M-D width of mandibular incisors and that of maxillary or mandibular canines, first and second premolars was found to be 0.598 and 0.586, respectively. 3. The regression constants were determined to evaluate the sum of M-D width of the unerupted permanent canines and premolars derived from the sum of M-D width of the erupted permanent incisors. 4. The prediction percentile tabulation were made in an attempt to predict the total M-D width of the unerupted permanent canines and premolars derived from the total M-D width of the erupted permanent mandibular incisors.

  • This study was aimed to observe the effect of Anterior J hook headgear on the craniofacial structures in mixed dentition with Class II malocclusion. The laterial cephalograms of 20 children treated by Anterior J hook headgear were traced, digitized and statistically analyzed. The results were as follows : 1. Forward growth of maxilla was inhibited. 2. Rotational effect of maxilla was not observed. 3. There was distal movement of maxillary dentition. 4. Maxillarly_dentoalveolar growth changes were more effective in anterior portion than posterior portion. 5. Mandible maintained a normal growth and mandibular plane angle was maintained during treatment period. 6. The ratio of anterior facial height to posterior facial height was almostly not changed.

  • Impaction of permanent incisor occurs rare than the canine & third molar. But it's often observed in school age child. The causes of impaction are trauma, space deficiency, mesiodens, infections of root apex, etc. In spite of elimination of cause, normal eruption of impacted tooth is rare. Though eruption is normal, the position of tooth will be incorrect. Because the impacted tooth results in malocclusion, root resorption of adjacent tooth, pathologic cystic change, it should be confirmed the precise position by clinical and radiographic exam and found the correct location by appropriate treatment plan. In case of pathologic change of impacted tooth and injury to adjacent tooth, it will be extracted. But through orthodontic retraction, the function and esthetics of tooth can be restored. It is important that impacted tooth should be detected early and diagnosed correctly, and appropriate treatment plan should be made. Before impacted tooth is retracted, the considerations of space for alignment and anchorage should be preceded and through appropriate force and mechanics, the side effects, for example, a root resorption should be minimized. In this study, we guided impacted tooth to normal position by using a forced eruption.

  • Orthodontic problems of the oral maxillofacial area could be classified into skeletal and dental problems. Dental problems might cause various occlusal disharmony and among them congenital missing or eruption failure might cause not only functional problems but also esthetic problems. Additional psychological problems are also one of the reasons for patients seeking treatment. In cases showing eruption failure of many teeth in the maxilla, not only occlusal and esthetic problems but also difficulty in alveolar bone maintainment could occur. Therefore, successive approach is necessary when multiple teeth show eruption problems. In this case, a patient with eruption problems of #13, 12, 11, 23, 43 was successfully treated by surgical exposure and successive orthodontic extrusion resulting perfect occlusion. By additional periodontal treatment, gingival recession that occurred along with teeth eruption was solved. The impacted right mandibular canine was erupted successfully only by natural arch expansion and leeway space without any special surgical treatment.

  • The dentigerous cyst involving maxilla is rare disease in Otolaryngologic field. A case of 22-year-old male patient involving dentigerous cyst of the maxilla with chronic inflammatory signs has been presented. The occurrence of odontogenic epithelium in the wall of dentigerous cyst is a well known entity. This epithelium usually remains inactive and does not have clinical significance, but these small inactive islands of epithelium may be stimulated, resulting in chronic inflammation, ameloblastoma and squamous cell carcinoma. Therefore correct diagnosis and proper treatment are very important. The authors have recently experienced a case of dentigerous cyst causing a mild disfigulation of face. The cyst was surgically enucleated through sublabial approach.

  • The dentigerous cyst originates through alteration of the reduced enamel epithelium after the crown of the tooth has been completely formed, with accumulation of fluid between the reduced enamel epithelium and the tooth crown. The dentigerous or follicular cyst comprises about 20% of all dental cysts. It usually occurs between 20 and 30 years of age but is occasionally seen in children or in old people. The most common sites of this cyst are the mandibular third molar and maxillary cuspid areas, since these are the most commonly impacted teeth. Many of these cysts give no clinical symptoms until noticeable asymmetry of the face develops. Rarely it develops to an ameloblastoma, epidermoid carcinoma and mucoepidermoid carcinoma, so early removal leads to better result. Roentgenographic examination of the jaw involved by a dentigerous cyst will reveal a radiolucent area. Recently the authors have experienced a giant dentigerous cyst which produced the bony deformity of the maxillary and hard palate bone. The cyst was completely removed under the local anesthesia by Caldwell-Luc approach.

  • The purpose of this study was to evaluate the stress patterns developed in abutment and residual ridge according to removable denture design in case of remaining mandibular canines. The removable denture designs in this study were as foolows : 1. Removable partial denture with non-splinted abutment 2. Removable partial denture with splinted abutment 3. Overdenture with telescopic crown 4. Overdenture with O-Ring attachment 5. Overdenture with combination bar attachment Photoelastic stress analysis was used to record the isochromatic fringe patterns and to calculate principal stress components at measuring points. The results were as follows : 1. In case of removable partial denture with non-splinted abutment, stress of root area at the loaded side was the largest. No significant differences in stress of root area were observed between loaded side and unloaded side. 2. No significant differences in stress of residual ridge at the loaded side were observed between removable partial denture with splinted abutment and removable partial denture with non-splinted abutment. 3. In case of combination bar attachment retained overdenture, stress of root area was the largest and in case of telescopic crown retained overdenture, stress of root area was the lowest. 4. In case of attachment retained overdenture, stress of residual ridge was lower but stress of root area was larger than in case of removable partial denture.

  • Mandible with severe alveolar bone atrophy poses a significant challenge in terms of reproducing clinically acceptable anatomy for a removable prosthesis. To overcome this potential complication, altered cast impression technique is often recommended to capture accurate and functional gingiva tissues. It becomes possible to get proper anchors functional impression by placing 2 implants crowns which were impossible in previous implant overdenture impression technique. In this case, an 80-year old female patient with severe mandibular ridge atrophy was treated with an implant-assisted removable partial denture with two implant crowns on the canine area. An altered cast impression was taken with an individual tray on a metal framework of removable partial denture on both posterior edentulous areas. The patient was satisfied with the final prosthesis after failure of 2 previous prostheses. Clinician had a difficult time to manage disabled patient and patient were suffered with ill-fitting denture due to inaccurate impression in conventional overdenture condition. The oral rehabilitation was completed with placing 2 implants as proper anchor.

  • Compared with traditional full dentures, mandibular implant overdentures have the advantage in that good support and retention can be obtained even with two implants. When manufacturing a mandibular implant overdenture using two implants, it is important to place the implant in the correct position. The long-term prognosis of overdenture is good when two implants are placed in the direction vertical to the occlusal plane and parallel to each other at the canine position. However, it is difficult to place two implants in the correct position in edentulous patients, and if you use surgical guides in these cases, you can get help in placing the implants in a prosthetically advantageous position. This case, a 57-year-old male patient, came to our hospital with all upper and lower teeth removed due to periodontal disease. Therefore, the maxilla was restored with a traditional full denture and the mandible was restored with an implant overdenture using two implants, which resulted in satisfactory functional and esthetic results.
