A Study on the Private Investigator System in U.S.A. (미국의 탐정제도에 관한 연구)

  • Private detectives and investigators offer many services, including executive, corporate, and celebrity protection; pre-employment verification; and individual background profiles. They also investigate computer crimes, personal injury cases, insurance claims and fraud, child custody and protection cases, missing persons cases, and premarital screening. This paper focuses on a private detective or investigator system in most developed U.S.A, but it is not easy to describe that system. Licensing requirements vary in U.S.A. Some States have few requirements, and many other States have stringent regulations. A growing number of States are enacting mandatory training programs for private detectives and investigators. Employment and need of private detectives and investigators is expected to grow faster than the average for all occupations. Thus, it is reasonable that korean government should permit private investigation service find the way to minimize the side effects of private investigation service instead of banning the service totally.

Three-dimensional conformal radiotherapy for portal vein tumor thrombosis alone in advanced hepatocellular carcinoma

  • Purpose: We sought to evaluate the clinical outcomes of 3-dimensional conformal radiation therapy (3D-CRT) for portal vein tumor thrombosis (PVTT) alone in patients with advanced hepatocellular carcinoma. Materials and Methods: We retrospectively analyzed data on 46 patients who received 3D-CRT for PVTT alone between June 2002 and December 2011. Response was evaluated following the Response Evaluation Criteria in Solid Tumors. Prognostic factors and 1-year survival rates were compared between responders and non-responders. Results: Thirty-seven patients (80.4%) had category B Child-Pugh scores. The Eastern Cooperative Oncology Group performance status score was 2 in 20 patients. Thirty patients (65.2%) had main or bilateral PVTT. The median irradiation dose was 50 Gy (range, 35 to 60 Gy) and the daily median dose was 2 Gy (range, 2.0 to 2.5 Gy). PVTT response was classified as complete response in 3 patients (6.5%), partial response in 12 (26.1%), stable disease in 19 (41.3%), and progressive disease in 12 (26.1%). There were 2 cases of grade 3 toxicities during or 3 months after radiotherapy. Twelve patients in the responder group (15 patients) received at least 50 Gy irradiation, but about 84% of patients in the non-responder group received less than 50 Gy. The 1-year survival rate was 66.8% in responders and 27.4% in non-responders constituting a statistically significant difference (p = 0.008). Conclusion: Conformal radiotherapy for PVTT alone could be chosen as a palliative treatment modality in patients with unfavorable conditions (liver, patient, or tumor factors). However, more than 50 Gy of radiation may be required.

Effects of total body irradiation-based conditioning on allogeneic stem cell transplantation for pediatric acute leukemia: a single-institution study

  • Purpose: To evaluate the effects of total body irradiation (TBI), as a conditioning regimen prior to allogeneic stem cell transplantation (allo-SCT), in pediatric acute leukemia patients. Materials and Methods: From January 2001 to December 2011, 28 patients, aged less than 18 years, were treated with TBI-based conditioning for allo-SCT in our institution. Of the 28 patients, 21 patients were diagnosed with acute lymphoblastic leukemia (ALL, 75%) and 7 were diagnosed with acute myeloid leukemia (AML, 25%). TBI was completed 4 days or 1 day before stem cell infusion. Patients underwent radiation therapy with bilateral parallel opposing fields and 6-MV X-rays. The Kaplan-Meier method was used to calculate survival outcomes. Results: The 2-year event-free survival and overall survival rates were 66% and 56%, respectively (71.4% and 60.0% in AML patients vs. 64.3% and 52.4% in ALL patients, respectively). Treatment related mortality rate were 25%. Acute and chronic graft-versus-host disease was a major complication; other complications included endocrine dysfunction and pulmonary complications. Common complications from TBI were nausea (89%) and cataracts (7.1%). Conclusion: The efficacy and toxicity data in this study of TBI-based conditioning to pediatric acute leukemia patients were comparable with previous studies. However, clinicians need to focus on the acute and chronic complications related to allo-SCT.

Analysis on Oh, hein-kuhn's Portrait Photograph (오형근 초상사진 분석)

  • Oh, hein-kuhn is a representative portrait photographer in Korea. His works, , , , which treated portraits of women are a bit different from those of the past that described the appearance of characters. The artist expressed the figure of inner side that conflicts and feels uneasy between 'family and society' and 'child and woman' along with the look of character. That is, he included specific aura which the whole of aunts and girls' group radiates by showing similar photos repeatedly, that we can be aware of 'aunt' and 'girl' by seeing them. The important technique of Oh, hein-kuhn's Portrait Photograph is 'repetition' of image. The artist delivers the meaning of the photo by looking at the character upright in the front and showing photos with similar frame repeatedly. Such form and content of the work as this might feel a bit boring, since it's very simple just like identification picture. However, the photos in the frames which do not move in the fixed position can deliver the meaning of work more clearly rather than the image which expresses flowing time and moving space, we can grasp the message that the artist intends to deliver rather quickly. In the environment of photograph production mainly with image transformation, Oh, hein-kuhn's works of authentic original work method definitely stand forth. Through the portraits of aunts and girls, the artist tells an aspect of our society and we reconfirm the reality through the portraits made by him.

  • The purpose of this study was to validate knowledge outcomes included Nursing Outcomes Classification(NOC) developed by Johnson and Maas at the University of Iowa. A sample of 71 nurse experts working in university affiliated hospitals participated in this study. They were asked to rate indicators that examplified the outcomes on a scale of 1(indicator is not all characteristic) to 5(indicator is very characteristic). A questionnaire with an adaptation of Fehring's methodology was used to establish the content validity of outcomes. The results were as follow: 1. All indicators were considered to be 'supporting' and no indicators were considered to be 'nonsupporting'. 2. 'Knowledge: Treatment Regimen' attained and OCV score of 0.816 and was the highest OCV score among outcomes. 3. 'Knowledge: Energy Conservation' attained an OCV score of 0.748 and was the lowest OCV score among abuse outcomes. 4. 'Knowledge: Breastfeeding' attained an OCV score of 0.790 and was the highest indicator was 'description of benefits of breastfeeding'. 5. 'Knowledge: Child Safety' attained an OCV score of 0.778 and was the highest indicator was 'demonstration of first aids techniques'. 6. 'Knowledge: Diet' attained an OCV score of 0.779 and was the highest indicator was 'performance of self-monitoring activities'. 7. 'Knowledge: Disease Process' attained an OCV score of 0.815 and was the highest indicator was 'description of signs and symptoms'. 8. 'Knowledge: Health Behaviors' attained an OCV score of 0.800 and was the highest indicator was 'description of safe use of prescription drugs'. 9. 'Knowledge: Health Resources' attained an OCV score of 0.794 and was the highest indicator was 'description of need for follow-up care'. 10. The Effects of Educational Activity in Relation with Nuri Curriculum in Green Growth Education Programme for Young Children on Their Knowledge in Environmental Conservation, Sensitivity to the Natural Environment and Attitudes in Environmental Conservation (누리과정과 연계한 유아녹색성장 교육활동이 유아의 환경보전지식, 자연환경감수성 및 환경보전태도에 미치는 효과)

  • This study's aims at seeking ways to support educational activity in 'Green-Growth' for children, in order for children to practice green life from their infancy and studying effects of their educational activity in knowledge of environmental conservation and receptivity to the natural environment, attitudes in environment conservation. This study conducted a survey of fifty-four children of five years of age attending the kindergartens in the Metropolitan City of Incheon. Twenty-seven were classified in the experimental group and the remaining Twenty-seven were in the comparative group. For the statistical analysis of the data collected, the t-test and the analysis of covariance(ANCOVA) were conducted using the SPSS WIN 17.0 program. The findings of this research are as follows: the children of the experimental group who experienced educational activity in green growth, demonstrated higher score than those of the comparative group in knowledge of environmental conservation and receptivity to the natural environment, attitudes in environment conservation. The result of the research indicates that educational activity in green growth, for children, enhances their knowledge in environment conservation, the receptivity to natural environment and the positive changes of attitudes in environmental conservation. It is then advisable to integrate and apply educational activity in green growth to the curriculum of kindergartens from early childhood.

The Effects of Self-Control, Parental Behaviors, and Marital Conflict on Boys' Aggression in Korean Upper Elementary School Students (초등학생 고학년 남아의 자기통제력, 부모의 양육행동, 부모간 갈등이 공격성에 미치는 영향)

  • This research was conducted in order to find out the effects of boys' self-control, their parents' behaviors and marital conflict on boys' aggression. Subtypes of aggression in the present research consist of dimensions of the functions and forms (proactive-overt, proactive-relational, reactive-overt, reactive-relational). The participants of this study were 237 boys in the fifth and sixth grades in elementary schools located in Seoul and Gyeonggi province. The questionnaires were derived and modified from Lee Hong (2009) for boys' aggression, from Ju So Young and Lee Yang Hee (2008) for boys' self-control, from Jun Joo Ram (2008) and Heo Seung Yeon (2009) for maternal behaviors and from Kang Na Jung (2008) for marital conflict. In order to grasp the general characteristics of the participants, frequency and descriptive statistics were used. Also, Pearson's correlation was applied to the relationships amomg boys' self-control, marital behaviors and marital conflict on boys' aggression. Lastly, the variables that can predict boys' aggression were analyzed through multiple regression analysis by SPSS 19.0. The major results of this study were as follows. This research showed that this model fit well for boys. To summarize the important results, what is interesting in the present study is the effect of the mother's role in rearing their sons. It was found that mothers' criticism, negative behaviors, and excessive interference can cause boys' high level of aggressiveness. Also, having parents who fight frequently was strongly associated with boys' relational aggression. An interesting finding is that the mother's support influenced boys' reactive-overt aggression. Lastly, boys' self-control was highly associated with boys' proactive-relational aggression. This research may contribute to understanding boys' aggression' subtypes. To that end, the current results suggest that the dimensions of the functions and forms may be a useful tool in an effort to inform developmental theory and intervention. Also, this research may provide guidance for the resolution of problems caused by boys' aggression. Lastly, it was found that self- control training and parental education programs might prove beneficial for helping aggressive boys to overcome their aggressive impulses.

TREATMENT OF SHELL TEETH IN CHILD : A CASE REPORT (소아에서의 shell teeth의 치험례)

  • Shell teeth, a rare dysplastic condition of dentin, was first described by Rushton in 1954. It is characterized by normal enamel, extremely thin dentin, correspondingly large pulp chambers, and shortened roots. This case report is of a male 3 years old. He is refered to the Chosun University dental hospital Pediatric Dentistry because of dental caries and dentin hypoplasia. Intra-oral examination showed attrition of all primary teeth. Radiographic examination showed that the pulps were extremely large with only a shell of surrounding hard tissue. The permanent premolars were missed congenitally. The diagnosis was shell teeth. Because of behavior problem, all dental treatment was undertaken with general anaesthesia. Extration, endodontic treatment and SS crown were performed. The patient has now been wearing the space maintainer and manages it well. The patient is seen intervals for supervision and follow-up care.

A COMPARATIVE EVALUATION OF ORAL CHLORAL HYDRATE VS. ORAL MIDAZOLAM IN SEDATIVE EFFECT (Chloral hydrate와 Midazolam의 경구투여시 진정효과에 대한 비교)

  • The purpose of present study was to compare the sedative effect of chloral hydrate and oral midazolam treatment. The study was conducted on twenty two child patients who required at least two visits. All the patients showed a good physical status (ASA-I). The patient was randomly assigned to receive drug either chloral hydrate 60mg/kg and hydroxyzine 25mg(Group I) or midazolam 0.5mg/kg and hydroxyzine 25mg(Group II) at each visit. Treatment procedures were divided into six stages. In each stage, sleep, crying, movement and overall behavior score were evaluated. The overall results can be summarized as follows: 1. Success rate was 59.9% in group I and was 77.8% in Group II. 2. Mean sleep score was $1.81{\pm}0.93$ in group I and was $1.44{\pm}0.69$ in group II (P<0.05). 3. Through all treatment procedures, no significant difference was observed between group I and group II in terms of crying, movement and overall behavior score(P>0.05). 4. There was no significant difference between the sex and the age(P>0.05). In regard to above results, oral midazolam considered to be a valuable drug as sedative agent in pediatric dentistry.

Risk Factors for Malignancy of Pheochromocytoma and Abdominal Paraganglioma in Children: Clinicopathologic Perspectives (소아에서 갈색세포종과 복강내 부교감신경절종의 악성화 예측인자)

  • 목적 Pheochromocytoma of the Adrenal gland Scaled Score (PASS) 시스템과 면역화학염색 등을 통한 갈색세포종 및 부교감신경절종의 악성화 예측인자가 제시되고 있으나 명확한 병리학적 또는 분자생물학적 예측인자는 밝혀진 바 없다. 본 연구에서는 임상적, 병리학적 분석을 통해 갈색세포종 및 복강내 부교감신경절종의 악성화 예측인자를 확인하고자 하였다. 대상 및 방법 1990년 1월부터 2010년 12월까지 서울대학교어린이병원에서 수술적 절제 후 병리학적으로 갈색세포종 및 복강내 부교감신경절종으로 확진된 20명의 18세 이하 소아 환자를 대상으로 임상적 특징을 분석하였고, PASS 시스템에 따른 병리 슬라이드 판독하였다. 세포활성도를 반영한다고 알려진 유전자에 대한 항체 중 Ki-67, p53, bcl-2, mdm-2, cycline D1, p21, p27을 이용해 면역 화학검사를 한 후 결과를 확인하였다. 결과 20명의 환자 중 갈색세포종은 14명, 복강내 부교감신경절종은 6명이었다. 악성화는 각각 4명, 3명에서 관찰되었다. 혈관 침범, 주변부 지방조직 침습, 세포분열 증가가 통계적으로 유의한 악성화 예측인자였으며(각각 p=.007, .031, .031), 갈색세포종만 분석하였을 때도 통계적으로 유의하였다(각각 p=.033, .003, .019). PASS 시스템은 악성화를 예측하는데 있어 통계적으로 유의하지 않았으며, 혈관 침범, 주변부 지방조직 침습, 세포분열 증가를 항목으로 하여 새롭게 만든 병리 스코어 시스템은 악성 환자군과 양성 환자군 사이에 통계적으로 유의한 차이를 보였다(p< .001). 악성과 양성 질환 사이의 면역화학염색 결과에서 유의한 차이는 없었다. 결론 소아에서 갈색세포종 및 복강내 부교감신경절종의 악성화 예측인자로 혈관 침범, 주변부 지방조직 침습, 세포분열 증가를 이용할 수 있다. 소아에서 PASS시스템으로 악성화를 예측할 수 없었으나, 새로운 병리스코어 시스템으로 악성 환자군을 예측할 수 있었다. 면역화학검사 결과 세포 활성도를 반영하는 인자들은 악성화를 예측할 수 없었다.
