『빌러비드』: 재기억을 통한 정체성 회복 (Beloved: Identity Recovery through Rememory)

  • The purpose of this study is to research how the writer describes characters of the text who overcome traumatic experiences and restore their identity through rememory in Toni Morrison's Beloved. The writer, Morrison gives the female characters their voices to recover their ethnic identities. By breaking silence, they establish their identities and become Americans from "unspeakable thoughts" to "speaking subjects." The ex-slave Sethe and her daughter, Denver have experienced trauma which works from traces of memory and history after slavery was abolished. Sethe and Denver are isolated from the community at the 124 Bluestone Road. When Beloved, ghost who was killed by Sethe, appears, Sethe and Denver are wondering who she is. Rememorying in Beloved is the important form of narrative that Morrison uses to recover their trauma. Morrison emphasizes the need to reconcile with the community and the aid of community for Sethe and Denver to heal their truma. Thanks to Beloved who leads Sethe and Denver to the community, they can be finally one of the community members in America.

영화에 나타난 소아, 청소년의 외상적 경험 (CHILD AND ADOLESCENT'S TRAUMATIC EXPERIENCES EXPRESSED IN MOVIES)

  • 소아와 청소년의 외상적 경험은 영화에 빈번히 다루어지는 소재 가운데 하나이다. 저자들은 소아, 청소년이 영화에 표현된 양상들을 살펴 보았다. 이들 영화는 외상적 경험이 성장영화의 중요한 모티브로 쓰인 경우, 정치사회적 이슈의 은유로 쓰인 경우, 그리고 드라마의 복선이나 소재로 쓰인 경우 등 크게 세 가지의 범주로 구분이 가능하였다. 영화는 만드는 이 혹은 그것을 보는 이들의 경험과 갈등과 무의식적 욕구가 반복되어 표현되는 장이면서, 어린 아이의 상징적인 놀이와도 같은 기능을 한다고 생각되는데, 이는 외상적 경험이라는 소재가 영화 속에 빈번히 다루어지는 원인의 일부를 설명할 수 있을 것이라 생각된다. 영화에 나타난 외상의 치유 양상도 다양하게 드러나며, 영화 자체가 교정적 감정경험을 가능케 하는 매체로 작용하여 내적 발달을 돕는 기능을 할 수 있다. 영화에 나타난 정신과적 문제들은 영화 대중의 무의식 속에 그와 관련된 전형적 이미지를 각인시킬 수 있으므로, 앞으로 영화에 드러나는 소아, 청소년의 외상적 경험이 보다 더 진실되게 그려지고 풍부한 의미를 생산해낼 수 있도록 영화를 만드는 주체와 정신과 의사간의 의사소통이 필요하다고 생각된다.

간호학생의 임상실습에서의 외상 경험과 외상후 성장 (Traumatic Experiences and Posttraumatic Growth of Nursing Students who were in the Clinical Training)

  • 본 연구는 임상실습을 하는 간호학과 학생의 외상 경험과 외상 후 성장에 대해 조사한 연구이다. 자료수집 기간은 2017년 11월20일부터 12월20일까지였으며 대상자는 4개 간호학과에 재학 중으로 임상실습 경험이 1년 이상된 4학년 학생 490명이었다. 자료는 SPSS/WIN 25.0으로 t-test, ANOVA 및 $Scheff{\acute{e}}$ test로 분석하였고, 외상경험은 내용분석법으로 분석하였다. 일상생활 중 외상경험을 한 경우는 26.5%였으나 임상실습 중 외상경험은 61.2%로 높았고, 외상 후 성장 점수는 5점 만점에 2.63점이었다. 외상경험 내용은 의료진의 폭력경험, 간호직에 대한 부정적 인식, 비교육적인 실습환경, 열악한 환경에서의 실습, 중증 환자 목격 등이 있었다. 본 연구의 결과는 향후 간호학과 학생의 외상 후 성장을 돕기 위한 임상실습교육 프로그램 개발의 기초자료로 활용 될 수 있다.

잠재프로파일분석(LPA)을 활용한 PTSD 증상과 외상 후 성장 수준의 양상: 폭식, 비자살적 자해, 문제성 음주행동에서의 차이 (Latent Profile Analysis of PTSD symptoms and PTG among Adults in South Korea: the Differences in Binge Eating, Non-Suicidal Self-Injury, and Problem Drinking Behaviors)

  • 본 연구는 외상 사건을 경험한 국내 성인의 외상 후 스트레스(Post Traumatic Stress Disorder) 증상과 외상 후 성장(Post Traumatic Growth) 양상을 토대로 잠재계층(latent class)을 도출하고, 각 집단 구분에 영향을 미치는 특성을 탐색하며, 집단에 따른 자기파괴적 행동의 차이를 탐색함으로써, 외상 사건을 경험한 개인에 대해 보다 심층적으로 이해하고자 한다. 이를 위해 본 연구에서는 DSM-5 진단기준에 해당하는 외상 사건을 경험한 국내성인 860명을 대상으로 잠재프로파일분석(Latent Profile Analysis)과 R-3STEP 방식을 이용하였다. 집단 예측 변인으로 개인의 인구사회학적 변인(성별, 연령, 최종 학력, 종교유무, 종교활동 정도, 월 평균 소득)과 사회적 고립, 외상 경험 빈도가 포함되었으며, 종속변인으로 자기파괴적 행동(폭식 정도, 비자살적 자해 유무, 음주의 양, 음주 형태, 음주로 인한 정신 사회적 문제)을 포함하였다. 연구결과 집단은 '저PTSD/중 PTG집단', '저 PTSD/고 PTG집단', '고 PTSD/고 PTG집단'으로 분류되었다. 또한 성별, 최종학력, 사회적 고립, 외상 노출 빈도가 집단 예측 변인으로 영향을 미치는 것으로 나타났으며, 폭식 정도, 비자살적 자해 유무, 음주 형태, 음주로 인한 정신사회적인 문제에서 집단 간 차이가 있는 것으로 나타났다. 마지막으로 본 연구의 결과를 바탕으로 논의와 시사점을 제시하였다.

사회복지전담공무원의 외상사건 및 스트레스 대처방식과 외상후스트레스증상 관련성 (Relationship between Traumatic Events, Stress Coping Strategies and Post-Traumatic Stress Symptoms among Social Workers in Public Sector)

  • Objectives Social workers can suffer from occupational stress resulting from dealing with clients, which might lead to metal health problems. We aimed to investigate the association of duty-related traumatic experiences and stress coping strategies with post-traumatic stress symptoms among social workers in public sector. Methods A total of 110 social workers in public sector (men 30.9%, $36.5{\pm}7.6$ yrs) participated in this study. All subjects were evaluated the frequency and the impact of duty-related traumatic events. Additionally, they completed questionnaires including the Impact of Event Scale-Revised for post-traumatic stress symptoms, the Beck Depression Inventory-II for depressive symptoms, the Scale for Suicidal Ideation for suicide symptoms and the Ways of Coping Checklist for stress coping strategies. Results The most frequent traumatic events were "Violent or abusive language from a client" (95.0%) and "Client made a fuss" (94.5%). The most distressing traumatic event was "Seeing a dead body on duty" (7.6 out of 10), which predicted post-traumatic stress symptoms (odds ratio 4.04 ; 95% confidence interval, 1.79-9.11). Among 4 types of stress coping strategies, the emotion-focused coping was positively correlated with post-traumatic stress symptoms after controlling age and sex (${\beta}$ = 0.50, p < 0.001). Conclusions Social workers in public sector showed high level of post-traumatic stress symptoms. Duty-related traumatic events and the emotional-focused coping strategies were associated with the severity of post-traumatic stress symptoms. The modification of stress coping strategies would alleviate post-traumatic stress symptoms in social workers in public sector.

Blunt Traumatic Cardiac Rupture: Single-Institution Experiences over 14 Years

  • Background: Blunt traumatic cardiac rupture is rare. However, such cardiac ruptures carry a high mortality rate. This study reviews our experience treating blunt traumatic cardiac rupture. Methods: This retrospective study included 21 patients who experienced blunt traumatic cardiac rupture from 1999 to 2015. Every patient underwent surgery. Several variables were compared between survivors and fatalities. Results: Sixteen of the 21 patients survived, and 5 (24%) died. No instances of intraoperative mortality occurred. The most common cause of injury was a traffic accident (81%). The right atrium was the most common location of injury (43%). Ten of the 21 patients were suspected to have cardiac tamponade. Significant differences were found in preoperative creatine kinase-myocardial band (CK-MB) levels (p=0.042) and platelet counts (p=0.004) between the survivors and fatalities. The patients who died had higher preoperative Glasgow Coma Scale scores (p=0.007), worse Trauma and Injury Severity Scores (p=0.007), and higher Injury Severity Scores (p=0.004) than those who survived. Conclusion: We found that elevated CK-MB levels, a low platelet count, and multi-organ traumatic injury were prognostic factors predicting poor outcomes of blunt cardiac rupture. If a patient with blunt traumatic cardiac rupture has these factors, clinicians should be especially attentive and respond promptly in order to save the patient's life.

PTSD Symptoms in Elementary School Children After Typhoon Rusa

  • Purpose. A natural disaster negatively affects children's emotional and behavioral adjustment. The purpose of this paper was to examine the prevalence, symptoms, and correlates of PTSD after the occurrence of Typhoon Rusa. Method. 261 elementary school children living in Kimcheon, which was a devastated rural area in South Korea by Typhoon Rusa, were selected. Data were collected 4 months after the disaster using the PTSD Reaction Index categories recommended by Frederick, severity of PTSD. Results. 12.3% of the children had either moderate or severe PTSD symptoms; 22.7% reported mild symptoms; and the remaining 65% had sub-clinical symptoms of PTSD. The most frequent symptom was recurrenct fear(67.0%). 13% to 17.2% of children exhibited difficulty in concentration, sleep disturbance, and guilt feeling. The regression model of severity of PTSD was composed of the level of exposure to traumatic experiences, grade in school, gender, negative coping style, and social support, and explained 34.3% for PTSD symptoms. Exposure to traumatic experiences was the strongest factor of all predictors. Conclusion. Emotional support from friends and coping style were correlated with PTSD severity. School-based interventions that emphasizes coping with disaster related problems and problem-solving may prove to be useful, and may aid in building close and supportive ties with teachers, classmates, and friends.

Experiences of Emergency Surgical Treatment for a COVID-19 Patient with Severe Traumatic Brain Injury at a Regional Trauma Center: A Case Report

  • Various medical scenarios have arisen with the prolonged coronavirus disease 2019 (COVID-19) pandemic. In particular, the increasing number of asymptomatic COVID-19 patients has prompted reports of emergency surgical experiences with these patients at regional trauma centers. In this report, we describe an example. A 25-year-old male was admitted to the emergency room after a traffic accident. The patient presented with stuporous mentality, and his vital signs were in the normal range. Lacerations were observed in the left eyebrow area and preauricular area, with hemotympanum in the right ear. Brain computed tomography showed a contusional hemorrhage in the right frontal area and an epidural hematoma in the right temporal area with a compound, comminuted fracture and depressed skull bone. Surgical treatment was planned, and the patient was intubated to prepare for surgery. A blood transfusion was prepared, and a central venous catheter was secured. The initial COVID-19 test administered upon presentation to the emergency room had a positive result, and a confirmatory polymerase chain reaction (PCR) test was administered. The PCR test confirmed a positive result. Emergency surgical treatment was performed because the patient's consciousness gradually deteriorated. The risk of infection was high due to the open and unclean wounds in the skull and brain. We prepared and divided the COVID-19 surgical team, including the patient's transportation team, anesthesia team, and surgical preparation team, for successful surgery without any transmission or morbidity. The patient recovered consciousness after the operation, received close monitoring, and did not show any deterioration due to COVID-19.

청소년의 학교폭력 피해 경험이 복합 외상 후 스트레스에 미치는 영향 - 자아존중감의 조절효과를 중심으로 - (The Effects of School Violence in Adolescents on Complex Post-traumatic Stress Disorder Symptoms - Focus on Moderating Effect of Self-esteem -)

  • 본 연구는 청소년의 학교폭력 피해 경험이 복합 외상 후 스트레스에 미치는 영향과 이 관계에서 자아존중감의 조절효과를 검증하고자 하였다. 이를 위해 291명의 청소년을 대상으로 조사된 자료를 연구에 활용하였으며, 조절회귀분석을 통해 연구모형을 검증하였다. 연구 결과, 학교폭력 피해 경험과 자아존중감은 복합 외상 후 스트레스에 유의미한 영향을 가진 것으로 나타나 학교폭력 피해 경험의 수준이 높을수록, 자아존중감이 낮을수록 복합 외상 후 스트레스가 증가하게 됨을 확인하였다. 또한 자아존중감은 학교폭력 피해 경험이 복합 외상 후 스트레스에 미치는 영향을 조절하고 있음을 확인하여 같은 수준의 학교폭력 피해 경험이 있더라도 자아존중감이 높은 경우 복합외상 후 스트레스의 문제를 완화할 수 있음을 확인하였다. 연구 결과를 바탕으로 청소년의 학교폭력과 이로 인해 발생하는 복합 외상 후 스트레스의 문제를 완화하기 위한 방안을 제시하였다.

  • In psychodynamic therapy, trauma related symptoms are considered as an attempt to overcome the traumatic experiences. Therefore, psychodynamic psychotherapy regards the symptom not as a patient's defect but as an adaptive reaction. In contrast to other therapies, psychodynamic theory places heavy emphasis on the understanding and resolution of the patient's symptoms. In addition, transference is a distinctive feature of this theory, which plays a significant role of reflecting the realistic evaluation on the therapist's characteristics, and in guaranteeing the therapeutic alliance. The psychodynamic therapist inducts meaning from the patient's unconscious mind. The therapist's task is to help the patient to better understand defense mechanisms guide their management of experiences and reactions, and facilitate a better understanding of their personal experiences.
