범이론모형(Transtheoretical Model) 기반의 운동행위강화 프로그램이 고혈압 노인의 혈압 및 신체활동량에 미치는 효과 (Effects of a Transtheoretical Model Based Exercise Behavior Improving Program on Blood Pressure and Physical Activity for Older Adults with Hypertension)

  • 본 연구는 범이론모형에 기반하여 운동행위강화 프로그램을 개발하고 이를 고혈압 노인에게 적용하여 혈압 및 신체활동량에 미치는 효과를 규명하고자 하였다. 연구대상자는 서울시 소재 노인복지관에 내원하며 연구에 참여를 허락한 65세 이상 고혈압 노인 41명으로 실험군 20명, 대조군 21명이었다. 실험군에게는 범이론모형에 기반한 운동행위강화 프로그램을 8주 동안 적용하였다. 연구결과 실험군은 대조군에 비하여 운동행위 변화단계, 전체 변화과정 점수, 행위적 변화과정 점수, 운동 자기효능감, 신체활동량이 유의하게 증가하였으나, 경험적 변화과정 점수, 의사결정균형, 혈압 등은 유의한 차이가 없었다. 이러한 결과를 바탕으로 운동행위강화 중재는 비활동 고혈압 노인의 운동행위로의 이행과 신체활동량을 증진시키는데 효과가 있음을 확인하였다. 본 프로그램이 지역사회에 거주하는 고혈압 노인에게 적용된다면 운동습관을 형성하게 하여 고혈압 합병증을 예방하는데 기여할 것으로 기대된다.

범이론적 모형을 적용한 간호여대생의 운동행동 변화단계 분석 (Analysis of The Stages of Change in Exercise Behavior of Women in Nursing College based on Transtheoretical Model)

  • 본 연구는 범이론적 모형을 적용하여 간호여대생의 운동행동 변화단계에 따른 변화과정, 의사결정균형, 자기효능감 및 사회적지지 간의 관련성을 파악하기 위한 서술적 조사연구이다. 일개 간호학과 재학생 223명을 대상으로 설문지를 이용하여 자료를 수집한 후 IBM SPSS 21.0 program으로 분석하였다. 연구결과, 대상자의 운동행동 변화단계는 계획전단계 17.9%, 계획단계 54.3%, 준비단계 16.6%, 행동단계 4.5%, 유지단계 6.7%이었다. 운동행동 변화단계에 따른 변화과정에서 의식고양, 극적전환, 자아재평가, 사회적해방, 대체행동형성, 협력관계, 강화관리, 자아해방, 자극통제와 의사결정균형의 운동이득요인, 운동장애요인 그리고 자기효능감은 유의한 관련성이 있었다. 그러나 사회적지지는 유의한 차이가 없었다. 범이론적 모형은 간호여대생의 운동행동을 설명하는데 적합한 이론이며, 본 연구결과는 간호여대생의 운동행동 변화단계를 고려하여 효과적인 운동프로그램을 개발하는데 유용한 정보를 제공해줄 것이다.

범이론적 모형을 적용한 대면적 금연상담 프로그램의 효과 (Effectiveness of Public Health Center Smoking Cessation Counseling Program using the Transtheoretical Model)

  • Purpose: The purpose of this study was to identify the effectiveness of public health center smoking cessation counseling program using the transtheoretical model on the process of change, smoking temptation, decisional balance and stage change transition. Methods: A nonequivalent control group non-synchronized design was used for this study. The study population was 115 adult smokers. The counseling program was administered to the experimental group and a smoking cessation program from the Korean Ministry of Health & Welfare was administered to the control group. Descriptive analysis, $\chi^2-test$, t-test, and ANCOVA were used with the SPSS 12.0 program to analyze the data. Results: After treatment with this program, the experimental group showed significantly higher scores for process of change (t=4.148, p<.001), smoking temptation (t=-2.988, p=.003), and stage change transition ($\chi^2=5.871$, p=.031) compared to the control group. Experimental group members also showed significantly lower score for Pros of smoking (t=-3.151, p=.002). Conclusion: The findings indicate that this program could have positive effect on process of change, smoking temptation, decisional balance and stage transition for adult smokers. Based on these findings, the authors suggest additional counseling program focusing on smokers in specific stages.

관상동맥질환자의 금연변화단계와 관련된 요인 및 장애요인에 대한 전향적 연구 (Prospective Study on the Relating Factors to the Stages of Change in Smoking Cessation and Barriers in Coronary Artery Disease Patients*)

  • Purpose: The main purpose of this study was to investigate that the stages of change in smoking cessation behavior among coronary artery disease patients for six months progressed following the stages of change suggested by the transtheoretical model. Method: Subjects for this descriptive survey were 59 coronary disease patients who were smoking or who had stopped smoking for less than six months. Result: In the baseline, the distribution of the subjects’ stages of change was as follows: pre-contemplation stage 25.4%, contemplation stage 25.4%, preparation stage 22%, and action stage 27.1%. After six months, more subjects in the contemplation(33.3%) and preparation stages(30.8%) progressed to the action stage than those of the pre-contemplation stage(0%). Eighty-one percent of the subjects in the action stage at baseline progressed to the maintenance stage. The relationship between the numbers of smoking cessation attempts for six months and stages of change at baseline was significant(p=.001). However, the relationships between self-efficacy and nicotine dependence at baseline and progression in stages of change after six months were not significant. Conclusion: Progression in the stages of change for six months among subjects corresponded to the stages of change suggested by the transtheoretical model. Hence, future development and evaluation of intervention programs should be tailored individually considering each patient's stage of change.

산업재해 근로자의 변화동기에 영향을 미치는 개인, 가족, 산업재해 요인 : 범이론 모형의 적용 (Individual, Family and Industrial Factors Related to Injured Workers' Motivation for Change : Application of the Transtheoretical Model)

  • 본 연구는 범이론을 적용하여 산업재해 근로자(이하 산재근로자)의 변화동기 수준과, 이에 영향을 미치는 요인을 개인, 가족, 산업재해 요인으로 구분하여 살펴보았다. 산업재해로 요양 치료를 받고 있는 305명의 근로자로부터 자기보고식 설문조사를 통해 수집한 자료를 분석한 결과는 다음과 같다. 첫째, 산재근로자의 변화동기 수준은 초이론 모형에서 대부분 전숙고, 숙고 단계에 머물러 있었다. 둘째, 산재근로자의 변화동기에 긍정적 영향을 미치는 개인 요인은 심리안정성, 장애수용도, 독립성이었고, 가족 요인은 가족의 지지도였으며, 산재 관련 요인은 주관적 재해상태, 재해 당시 직업, 직업 복귀 상태였다. 이러한 결과는 산재근로자의 재활을 위해 개인의 심리와 가족의 지지에 초점을 둔 개입이 필요함을 시사한다.

An Exploratory study of compliance with dietary recommendations among college students majoring in health-related disciplines: application of the transtheoretical model

  • Compliance with food group and nutrient recommendations, and self-efficacy, stage of change, perceived barriers and benefits for healthy eating were assessed among a convenience sample of college students majoring in health-related disciplines. Dietary and psychosocial data were collected using three-day food records and scales, respectively. Means (SD), frequencies, and percents were calculated on all data, and logistic regressions were used to determine whether any of the psychosocial correlates predicted the stage of change for healthy eating. Noncompliance with food group recommendations ranged from 53% for the meat/meat alternates group to 93% for the vegetables/juice group, whereas noncompliance with nutrient recommendations ranged from 26% for cholesterol to 99% for potassium. A majority of students (57%) self-classified in the preaction and 40% in the action stages of change for eating healthy. The students' self-efficacy to eat healthy was highest in positive/social situations and lowest when experiencing emotional upset. The most important perceived barrier to healthy eating was that friends/roommates do not like to eat healthy foods, and the most important perceived benefit was that eating healthy foods provides the body with adequate nutrients. The difficult/inconvenient self-efficacy subscale predicted the stage of change for healthy eating. These students would benefit from interactive learning opportunities that teach how to purchase and prepare more whole grain foods, fruits, and vegetables, enhance their self-efficacy for making healthy food choices when experiencing negative emotions, and overcome perceived barriers to healthy eating.

운동행위변화단계에 근거한 노인 운동 동기화 중재프로그램의 효과에 관한 연구 (The Study on the Effect of Stage Based Exercise Motivational Intervention Program for the Elderly)

  • This study aims at confirming exercise effects on obesity, mobility, self-efficacy, process of change, and decisional component by stage based exercise motivational intervention program for the elderly. The stage based exercise intervention program was constructed based on Transtheoretical Model. Methods: The design of this study is nonequivalent control group with repeated measuring by quasi-experimental study. The subjects of this study, composing of experimental group of 32 and control group of 28 were selected at one institution for the aged in Seoul. Results: 1) The body fat (weight, BMI and circumference of waist), of the intervention group was significantly decreased than the control group. 2) The mobility of the intervention group was not significantly increased than control group. 3) The self-efficacy, Pros, Process of Change for exercise of the intervention group was not significantly increased than the control group. 4) The Cons for exercise of intervention group was not significantly decreased than the control group. Conclusion: The above result have informed us that a stage-based exercise motivational intervention program for the elderly has the effect of decreasing old persons' body fat and has value as an effective means of nursing for the elderly.

범이론적 모형에 근거한 일부 초등학교 고학년 아동의 운동행위 변화단계 관련요인 (Factors Associated with Stages of Exercise Behavior Change Applying the Transtheoretical Model in Upper Grade Elementary Students)

  • Objectives: The purpose of this study was to identify the factors influencing the stages of exercise behavior change after adjusting for related covariates. Methods: Participants included 362 of fourth to sixth graders in 3 elementary schools in a metropolitan city in Korea. The data were collected using structured questionnaire included Korean Stages of Change Scale for Exercise. Results: Logistic regression results showed that the motivational factors associated with transition from precontemplation to contemplation were cognitive process of change, self-reevaluation, conscious raising; regarding that from contemplation to preparation, cons of the decisional balance; regarding those from preparation to action, behavioral process, counter-conditioning, stimulus control, which were very similar to the results of previous researches subjected other age groups. But, there was no motivational factors associated with from action to maintenance, and self-efficacy had no influence on forwarding stages of change. Conclusions: TTM would be applicable to explain the exercise behavior of some children in Korea, which suggested that it be useful in developing the programs to improve physical activities of Korean school children.

범이론과 여가모형을 적용한 절주프로그램이 문제음주행위와 인지에 미치는 효과 (The Effects of the Drinking-Reduction Program Adopting Transtheoretical and Leisure Models on Problem Drinking Behavior and Cognition)

  • Purpose: This study is to examine the effects of the drinking-reduction program that utilizes transtheoretical model and leisure ability model on the problem drinking behavior, alcohol outcome expectancy, and abstinence self-efficacy of. problem drinkers at workplaces. Method: The subjects consisted of 54 male problem drinkers who scored over 12 points in the AUDIT-K with 23 persons in the experimental group and 31 persons in the control group. The research results were analyzed using the SAS program with Chi-square test, t-test, repeated ANOVA, and post-hoc comparison. Results: 1) The experimental group showed a significantly lower score in problem drinking behavior than the control group. 2) The experimental group showed a significantly lower score in alcohol outcome expectancy than the control group. 3) The experimental group showed a significantly higher score in abstinence self-efficacy than the control group. Conclusion: These results demonstrate that the drinking-reduction program effectively reduces the drinking behavior of problem drinkers at workplaces, reduces alcohol outcome expectancy, and enhances abstinence self-efficacy. Accordingly, it is considered that a drinking-reduction program can be a useful nursing intervention method for problem drinkers at workplaces to reduce drinking and improve the quality of life.

일 농촌지역 고등학생의 금연 변화단계 및 변화과정 (Stages and Processes of Smoking Cessation among High School Students in Rural Community)

  • Purpose: The purpose of this study was to describe stage distribution of current and former smokers and to analyze significant variables that used by adolescents at each stage using the Transtheoretical Model. Methods: A cross-sectional study was performed with 145 students attended to five high schools in one rural area. The survey instruments included stages and processes of change. decisional balance, self-efficacy, and knowledge related to smoking. Results: Among the participants, 47.6% were in the precontemplation and contemplation stages, 20.7% were in the preparation stage, and the other 31.7% were in the action and maintenance stages. Analysis of variance tests revealed that behavioral processes, pros and cons of smoking, and self-efficacy were significantly associated with the stages of smoking cessation, while among 10 processes of change, self-reevaluation, self-liberation, and reinforcement management were significantly different by the stages. Conclusions: The study results suggest that smoking cessation programs should emphasize behavioral strategies, self-efficacy, and decisional balance to help students adopt and maintain smoking cessation.
