Aplastic anemia and dental implant rehabilitation: a clinical trial

  • The purpose of this study was to investigate implant-supported restoration as a technique for restoring missing teeth in patients with aplastic anemia. Recurrent bleeding from wound sites leads to persistent release of iron in the tissue. Excessive iron in tissue is related to clinical findings, including fibrosis, poor wound healing, and high level of angiogenesis, which are possible etiological factors of reduced osseointegration. A 44-year-old female patient with aplastic anemia was treated with multiple endosseous implants throughout the mandible and in the posterior region of the maxilla. After 14 implants were placed, radiological and clinical parameters were assessed during the follow-up period. Marginal bone did not change significantly during the follow-up period. The fine trabecular bone in intimate contact and enclosing the implant fixture was sufficient for successful osseointegration. None of the 14 implants were associated with compilations during the seven-year experimental period. This study suggests that dental implant procedures are a safe and reliable treatment option for restoration of missing dentition in patients with aplastic anemia.

Morphological Changes of Bones and Joints with Rheumatoid Arthritis and Osteoarthritis

  • Arthritis is a common disease in aged people, and is clinically divided into rheumatoid arthritis (RA) and osteoarthritis (OA). Although common symptoms such as pain are present, the underlying pathological mechanisms are slightly different. Therefore, the objectives of the present study were to compare joint damage induced by RA and OA by analyzing the major morphological and molecular differences, and to propose a suitable therapeutic intervention based on the pathophysiological conditions of bones and joints. For the RA animal model, 8-week-old DBA1/J mice were immunized with bovine type II collagen emulsified in complete Freund's adjuvant (CFA). Normal C57BL/6 mice (over 2 years of age) were used for OA. The clinical arthritis score was calculated using a subjective scoring system, and paw thicknesses were measured using calipers. The serum TNF ${\alpha}$ level was analyzed using an ELISA kit. Micro-CT was used to identify pathological characteristics and morphological changes. In collagen-induced RA mice, there were increased ankle joint volumes and clinical scores (p<0.01). The concentration of TNF ${\alpha}$ was significantly increased from 3 to 7 weeks after immunization. Micro-CT images showed trabecular bone destruction, pannus formation, and subchondral region destruction in RA mice. OA among aged mice showed narrowed joint spaces and breakdown of articular cartilage. This study suggests that a careful therapeutic intervention between RA and OA is required, and it should be based on morphological alteration of bone and joint.

자성백서의 난소절제시 악관절 변화에 관한 조직학적 연구 (THE HISTOLOGIC STUDY OF THE RAT TEMPOROMANDIBULAR JOINT CHANGE FOLLOWING OVARIECTOMY)

  • This investigation studied the effect of ovariectomy on the temporomandibular joint (TMJ) of rat. Forty female Sprauge-Dawley strain white rats, each weighing about 200grams were used and divided into two groups. The one group was performed ovariectomy, and the other was performed sham operation. Both group were killed at the intervals of 3 days, 1, 2, 4, and 8 weeks postoperatively, paraffin section were stained with H&E, MT and histomorphometric study of the TMJ was performed in a synchronous manner with an sham operated control group The result were as follows: 1. At 4 weeks after ovariectomy, an adhesion between the inferior surface of the articular disc and the condyle was observed in the central portion. At 8 weeks after ovariectomy, a focal thickening of the bone (osteophyte) was observed in anterior portion and there was a marked decrease in the trabecular bone in the posterior region. 2. The thickness of cartilage zone was increased at 2 weeks postopratively in the central portion, and decreased at 4 weeks postopratively in the posterior portion. The thickness of transitional zone was decreased at 1 week postoperatively in the anterior and proliferating zone was decreased postoperatively. The thickness of articular zone was decreased at 3 days postopratively in the anterior portion, and decreased at 1 week postoperatively in the central portion. From the above finding, we could conclue that estrogen deficiency in rats causes alteration of TMJ in marked early stage.

Safflower Bud Dietary Prevents Ovariectomy-induced Osteoporosis in Rats

  • Safflower (Carthamus tinctorius L.) seeds have long been clinically used in Korea to promote bone formation and prevent osteoporosis. In addition, the safflower buds (SB) were found to have more useful functional ingredients than safflower seed. Thus, we investigated the preventive effects of SB diet in ovariectomized (OVX) rats. The rats were divided into five groups; sham operated group, OVX alone group, OVX plus 17β-estradiol (E2 10 μg/kg, i.p.) and OVX plus SB diet feeding group (0.3% or 1%). Feeding of SB diet (0.3% or 3%) to OVX rats markedly increased trabecular formation in femur compared to OVX rats. Feeding of SB diet (0.3% or 3%) to OVX rats also decreased TRAP activity compared to OVX rats. These results suggest that SB diets have bone sparing effects by the decrease of osteoclast activity. We also observed that OVX rats fed with SB diet (0.3% or 3%) exhibited the decrease of calcium and phosphorus in serum compared to OVX-induced rats. Therefore, SB may be beneficial for the patients of osteoporosis, especially in postmenopausal women.

족부 설상골에 발생한 거대세포종 - 증례보고 1례 - (Giant cell tumor of Cuneiform - A Case Report -)

  • 골의 거대세포종은 원발성 골 종양의 5-10%를 차지한다. 주로 20-40세 사이의 연령에서 많으며 90% 이상이 장골에 발생하지만 아주 드물게 수부나 족부 같은 편평골이나 단골에도 발생한다. 주요 증상은 동통이며 종종 병적 골절이나 심지어 연부 조직의 침범이 동반될 수있다. 방사선학적으로 골간단에 음영이 감소된 균질의 골 용해 소견이 보이며 주위에 반응성골 형성은 없다. 치료는 골 종양의 병기에 따라 차이가 있으며 재발율을 줄이고 침범 부위의 기능을 보존할 수 있는 수술적 치료가 필요하다. 비진행성 병기는 소파술후 골이식이나 골시멘트 충전을 할 수 있고 진행성 병기는 광범위 절제술후 관절 고정술이나 종양 대치물로 사지 구제술을 시행한다. 전이, 국소재발을 주기적으로 관찰해야 한다. 저자들은 족부의 외측 설상골에 발생한 거대 세포종을 경험하였기에 보고하고자 한다.

Psammomatoid Juvenile Ossifying Fibroma of the Maxilla Misdiagnosed as Fibrous Dysplasia: A Clinicopathologic Case Report

  • Juvenile ossifying fibroma (JOF) is a variant of the ossifying fibroma and includes two histopathological subtypes: trabecular and psammomatoid. Psammomatoid JOF (PJOF) in craniofacial structures should be distinguished from other fibro-osseous lesions, such as fibrous dysplasia (FD), considering the difference in the treatment protocols. Here, we present a rare case of PJOF that was initially misdiagnosed as a case of FD and emphasize the importance of considering JOF in the differential diagnosis of patients with craniofacial fibro-osseous lesions. A 4-year-old boy demonstrated progressive enlargement of the zygomaticomaxillary area on his left side for the last 6 months. The patient was diagnosed as a case of FD based on the clinical features and radiographic findings, and was operated considering the rapid progression. To achieve facial symmetry, contouring of the zygomatic bone and arch was performed. However, the patient demonstrated rapid enlargement at the 3-month postoperative follow-up. The decision was made to surgically remove the tumor due to visual field impairment. Intraoperatively, a rubbery mass, which was separated from the surrounding cortical bone, was identified and excised. The lesion was confirmed as PJOF by histopathological examination. The possibility of PJOF should not be ruled out in the differential diagnosis of patients with fibrous-osseous lesions. In the event of suspected PJOF, accurate diagnosis should be made through definitive biopsy.

치과에서 디지털 x-선 영상의 이용 (Digital X-ray Imaging in Dentistry)

  • In dentistry. RadioVisioGraphy was introduced as a first electronic dental x-ray imaging modality in 1989. Thereafter. many types of direct digital radiographic system have been produced in the last decade. They are based either on charge-coupled device(CCD) or on storage phosphor technology. In addition. new types of digital radiographic system using amorphous selenium. image intensifier etc. are under development. Advantages of digital radiographic system are elimination of chemical processing, reduction in radiation dose. image processing, computer storage. electronic transfer of images and so on. Image processing includes image enhancement. image reconstruction. digital subtraction, etc. Especially digital subtraction and reconstruction can be applied in many aspects of clinical practice and research. Electronic transfer of images enables filmless dental hospital and teleradiology/teledentistry system. Since the first image management and communications system(IMACS) for dentomaxillofacial radiology was reported in 1992. IMACS in dental hospital has been increasing. Meanwhile. researches about computer-assisted diagnosis, such as structural analysis of bone trabecular patterns of mandible. feature extraction, automated identification of normal landmarks on cephalometric radiograph and automated image analysis for caries or periodontitis. have been performed actively in the last decade. Further developments in digital radiographic imaging modalities. image transmission system. imaging processing and automated analysis software will change the traditional clinical dental practice in the 21st century.

Modified Twin Blocks에 의한 성장기 아동의 II급 부정교합의 치료증례 (A CASE REPORT ON TREATMENT OF CLASS II MALOCCLUSION WITH TWIN BLOCKS IN GROWING CHILD)

  • The Twin Blocks technique was developed by Dr. William Clark of Scotland during the early 1980's. Twin Blocks are an uncomplicated system that incorporates the use of upper and lower bite blocks. These blocks reposition the mandible and redirect occlusal forces to achieve rapid correction of malocclusions. They are also comfortable and the patients wear them full-time-inducing eating time. Occlusal forces transmitted through the dentition provide a constant proprioceptive stimulus to influence the rate of growth and the trabecular structure of the supporting bone. The features of Twin Blocks mean easier and quicker treatment. The occlusal inclined plane is the fundamental functional mechanism of the natural dentition. Twin blocks are bite blocks that effectively modify the occlusal inclined plane to induce favorably directed occlusal forces by causing a functional mandibular displacement. Upper and lower bite blocks interlock at a $45^{\circ}$ angle and are designed for full-time wear to take advantage of all functional forces applied to the dentition including the forces of mastication. The patients who were treated with modified Twin Blocks, and following results were observed: 1. Large overjet and deep overbite were corrected. 2. Class II molar relationship was changed into Class I. 3. Labial inclination of upper incisors was corrected by adjustment of labial bow of upper bite block. 4. The profiles of two patients were improved by anterior displacement of mandible.

척추성형술 시술에서 PMMA주입에 대한 흉추의 생체역학적 평가 (Biomechanical Evaluation of PMMA Injection in Vertebroplasty)

  • 본 논문에서는 골다공증성 척추압박골절에 대한 척추성형술시 골강화제인 PMMA 주입방법을 제안하였다. 척추성형술의 정량적 분석을 위해서는 유한요소해석을 사용하였으며 1m간격의 척추체 CT를 바탕으로 보다 정교한 척추체 모델을 제시함으로써, 단순화된 척추의 기하학적 형상과 간단한 재료 물성치를 사용하는 기존의 연구의 한계를 극복하고자 하였다. 본 연구 결과를 토대로 추체내 망상골에 주입되는 PMMA의 주입형태, 주입위치를 정량적으로 평가하여 이상적인 수술방법을 제시하였다. PMMA의 구입위치는 망상골의 앞쪽-위쪽에 많은 양이 집중하여 주입되었을 때, 가장 효율적으로 추체를 보완해 주는 것으로 나타났다.

ddY 마우스를 이용한 골다공증 모델에 대한 비교연구: 난소적출 및 신경절단 모델 (A comparative study on the osteoporotic animal models in ddY mouse: ovariectomized and neurectomized models)

  • In order to compare the induced time of osteoporosis between ovariectomized and neurectomized models in ddY mice. Experimental groups were divided into Sham, ovariectomized (OVX group) and neurectomized (NX group) group. The changes of body weight, tibia weight and histomorphometry of epiphyseal regions of tibia that were generally used as criteria index in osteoporosis, were evaluated at 2 and 4 weeks after operations with other generally used index-changes of serum osteocalcin. Also, calcium and phosphorus levels in the ash tibia were demonstrated with their ratio (Ca/P ratio). From the result of this study, evidences which reflect osteoporotic states of animals such as decrease of absolute and relative tibia weight, histomorphometrical index of epiphyseal region of tibia including trabecular bone volume %, and calcium and phosphorous contents in tibia, were generally detected from 4 weeks after ovariectomy and 2 weeks after neurectomy with increase of serum osteocalcin levels. In conclusion, it is considered that more rapid and favorable osteoporosis was induced in neurectomized model compared to that of ovariectomized model.
