  • This study investigated factors affecting the bone mineral density (BMD) of Korean adult men. Data on BMD and anthropometric (height, weight, body mass index, BMI, waist circumference, body fat) and biochemical(total cholesterol, vitamin D, alkaline phosphatase, ALP) measurements were obtained from the Fourth and Fifth Korea National Health and Nutrition Examination Surveys (KNHANES, 2008~2011). Overall, BMD of the subjects was decreased from year to year (T-score of 30~39 yr decreased to 0.447 (2010~2011) from 0.106(2008~2009) and in 50~59 yr decreased to 0.234 from 0.033. Age was negatively associated with BMD (T-score 0.361 of 20~29 yr group and -0.894 of ${\geq}80yr$ group in total femoral). According to increase of weight, BMI and waist circumference continuously increased BMD. High value of total cholesterol (T-score 0.157 of 201~<230 mg/dL group and 0.064 of ${\geq}230mg/dL$ group in total femoral) and alkaline phosphatase (T-score 0.337 of ${\leq}102IU/L$ group and -0.270 of ${\geq}336IU/L$ group in total femoral) were associated with lower BMD. Overall height, weight and BMI were positively associated with BMD, and ALP were negatively associated with BMD. Finding of the present study showed that bone loss may be associated with various factors such as age, weight, BMI, total cholesterol, vitamin D and ALP density and that much attention should be paid to bone health of adult men. Therefore, practical and systematic programs are required to improve the BMD of adult men as well as to maintain bone health.

  • Objectives: Obesity is known to influence physical and mental health as well as the general quality of life. The aim of this study was to evaluate the effect of obesity related quality of life on selecting a goal for weight management in overweight and obese female patients. Methods: A total of 140 overweight or obese (Body mass index $23kg/m^2$) female outpatients aged ${\geq}20$ and ${\leq}60$ years from one clinic participated in this study. Patients' desired weight (goal weight, ideal weight, satisfactory weight, acceptable weight and disappointed weight) and obesity related quality of life measures were evaluated. Univariate and multivariate analysis were performed to evaluate the effect of obesity related quality of life on goal weight reduction (%) and goal body mass index (BMI). Results: Mean BMI of overweight group, mild obesity group and severe obesity group were $62.0{\pm}4.8kg$, $68.5{\pm}5.5kg$ and $83.5{\pm}9.6kg$, respectively. Mean weight loss expectations of the three groups were $16.4{\pm}4.7%$, $19.5{\pm}5.3%$ and $30.2{\pm}6.8%$, respectively and goal weight was significantly different among the three groups. Severe obesity group had a lower total quality of life score including physical, work-related, daily living domains than overweight or mild obesity groups. In univariate and multivariate regression analysis, psychosocial domain of quality of life had an effect on goal weight reduction (%) and goal BMI. Conclusions: The results of this study demonstrated that the obese patients with poorer quality of life and psychosocial health tended to choose higher goal weight reduction and lower goal BMI.

  • This study was conducted to investigate the effect of weight control by use of commercial formula diet and nutrition education on 36 obese women over a period of 6 months. During the study it motivated ways for women to control their weight and improve their health, thus analyzing variables that influence weight control. To help weight control, for the first 3 weeks, along with 1 regular mea, 2-3 liquid formula diets of 135㎉ per pack a day were provided within the 1,200-800㎉ range. For 6 months since then, all 3 meals were taken regularly and it was recommended that energy which is 500㎉ less than energy requirement be taken. To evaluate the effect of weight change by the weight control program, anthropometric measurements, biochemicl parameter, and food behavior are measured. The mean body mass index(BMI) was 30.1$\pm$3.8(25.0-43.6) and it was in the upper 5 percentile of the mean BMI of Korean women. The percent of body fat was significantly reduced from 29.1$\pm$2.4 to 26.4$\pm$2.3, showing that the weight control program induced not only a change in weight but also a change in body composition. The body circumference such as waist and hip circumference and skinfold thickness measurements of biceps, triceps, subscapular and surprailiac were reduced significantly, but the mean waist-to-hip ratio didn't change. Singnificantly improved food behavior score measured by the questionnaire indicated that the weight-contorl program induced the change of the food behavior, as well. Weight loss also induced biochemical parameters of the subject, thus total cholesterol, LDL-Cholesterol, and triglycerol which were in the high range of the normal values were reduced to 20.2%(p<0.01), 22.2%(p<0.01), 25.8%(p<0.01), repectively. HDL-cholesterol didn't change significantly, but the ratio of HDL to total cholesterol increased(p<0.01).

  • BACKGROUND/OBJECTIVE: The goal of the present study was to investigate the effects of moderate caloric restriction on ${\beta}$-cell function and insulin sensitivity in middle-aged obese Korean women. SUBJECTS/METHODS: Fifty-seven obese pre-menopausal Korean women participated in a 12-week calorie restriction program. Data on total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), triglycerides (TG), and fasting serum levels of glucose, insulin, C-peptide, blood pressure, leptin and anthropometrics were collected. A dietary intake assessment was based on three days of food recording. Additionally, ${\beta}$-cell function [homeostasis model assessment of ${\beta}$-cell (HOMA-${\beta}$), insulinogenic index (ISI), C-peptide:glucose ratio, and area under curve insulin/glucose ($AUC_{ins/glu}$)] and insulin sensitivity [homeostasis model assessment for insulin resistance (HOMA-IR), Quantitative insulin-sensitivity check index (QUICKI) and Matsuda index (MI)] were recorded. RESULTS: When calories were reduced by an average of 422 kcal/day for 12 weeks, BMI (-2.7%), body fat mass (-10.2%), and waist circumference (-5%) all decreased significantly (P < 0.05). After calorie restriction, weight, body fat percentage, hip circumference, BP, TC, HDL-C, LDL-C, plasma glucose at fasting, insulin at fasting and 120 min, $AUC_{glu}$ and the insulin area under the curve all decreased significantly (all P < 0.05), while insulin sensitivity (HOMA-IR, QUICKI and Matsuda index) measured by OGTT improved significantly (P < 0.01). CONCLUSIONS: Moderate weight loss due to caloric restriction with reduction in insulin resistance improves glucose tolerance and insulin sensitivity in middle-aged obese women and thereby may help prevent the development of type 2 diabetes mellitus.

  • Many stroke patients undergoing rehabilitation therapy require a quantitative indicator for the evaluation of body composition in paretic and non-paretic regions. In this study, the body fluid alteration in the paretic and non-paretic regions of stroke patients with hemiplegia caused by cerebral hemorrhage and cerebral infarction was analyzed using bioelectrical impedance analysis (BIA). Alterations in body fluids were investigated to assess the physical status of the paretic and non-paretic regions of 20 stroke patients with hemiplegia caused by cerebral hemorrhage (7 patients) and cerebral infarction (13 patients). Extracellular water (ECW), intracellular water (ICW), ICW/ECW, total body water (TBW), ECW/TBW, and TBW/fat-free mass were utilized to evaluate the functional status of the paretic and non-paretic regions. Compared with the non-paretic region, the paretic region had high ECW and low ICW. Due to the loss of motor function and nutritional imbalance caused by the stroke, the amount of fat increased while the muscle quantity and quality significantly decreased in the paretic region. Thus, BIA can be a useful tool for quantitatively assessing paretic and non-paretic regions in stroke patients with hemiplegia.

  • Purpose: Laparoscopic total gastrectomy (LTG) for gastric cancer is still uncommon because of technical difficulties, especially in esophagojejunostomy (EJ). There are many reports for various laparoscopic procedures of EJ using linear or circular staplers. On the other hands, there has been no report for hand-sewn anastomosis. We report successfully performed intracorporeally hand-sewn EJ after LTG. Materials and Methods: The clinicopathologic data and short-term surgical outcomes of 6 patients who underwent totally laparoscopic total gastrectomy for upper gastric cancer from December 2010 and July 2011 were retrospectively reviewed. Results: The mean age was 66.5 years and mean body mass index (kg/$m^2$) was 24.6. All patients had medical comorbidities. The mean patient ASA score was 2.17. Among the 6 patients, previous abdominal operation was performed for 2 patients and combined operation was performed for 3 patients. The mean blood loss, operation time, and EJ anastomosis time was 130 ml, 379.7 minutes, and 81.5 minutes, respectively. The mean time to first flatus, first oral intake, and postoperative hospital stay was 3.0, 3.0, and 12.5 days, respectively. There was no 30-day mortality case. Postoperative aspiration pneumonia and multiple periventricular lacunar infarctions developed in 1 patient. There were no anastomosis-related complications and other major surgical complications. Conclusions: When the intracorporeal anastomotic technique becomes popular in LTG the intracorporeally hand-sewn EJ may be accepted as one method among the various laparoscopic procedures of EJ.

  • The purpose of this study was to investigate the relationship between nutritional status of iron and bone minernl density in premenopausal women. In the study, we classified the subjects into osteopenia (-2.5-I, n=29) groups according to their lumbar spine bone mineral density. Anthropometric measurements, dietary intake analysis and blood biochemistry measurements were performed on the subjects. The average ages of those in the osteopenia and normal groups were 22.2 yrs and 23.0 yrs, respectively, with no significant difference. The average body mass index (p<0.05) of those in the osteopenia group (19.6) was significantly lower than that of the normal group (21.3). The mean protein intake of those in the osteopenia group was significantly lower than that (p<0.05) the subjects in the normal group. The osteopenia group consumed a significantly lower amount of iron (p<0.05) and non-heme iron (p<0.05) compared to the normal group. The intakes of total food, vegetables and milk of those in the osteopenia group were significantly lower than those of the subjects in the normal group. The serum ferritin (p<0.001) level of those in the osteopenia group was significantly lower than those of the subjects in the normal group. In conclusion, a balance of iron status may be helpful in the prevention of bone mass loss in premenopausal young women.

  • This study was designed to find the optimum conditions for converting isoflavone glycoside to aglycone by ${\beta}-galactosidase$. Three different forms of the enzyme were tested and the optimum enzyme concentration, incubation temperature, pH, and incubation time were determined. Before treatment with enzyme, isoflavone contained 89.4% glycoside including daidzin, glycitin and genistin, and only 10.6% aglycone including daidzein, glycitein and genistein. Among the enzymes tested, the highest rate of isoflavone hydrolysis to aglycone, 35%, was observed when 3 unit/g Fungal Lactase (Amano Enzyme) was used. Higher incubation temperatures resulted in a higher rate of hydrolysis along with a greater loss of isoflavone mass. Therefore, body temperature $(37^{\circ}C)$ may be adequate for isoflavone conversion, with 44.9% hydrolysis and less than 10% loss of mass. As expected, a higher amount of aglycone was produced at pH 7 compared with other pH values. During 5hr of incubation, the conversion of glycoside to aglycone increased dramatically from 0 to 1hr, and plateaued thereafter. In addition, commercial soy-based milk was hydrolyzed more effectively with ${\beta}-galactosidase$ when incubated for 5hr. Based on the above results, the optimum conditions for isoflavone hydrolysis by ${\beta}-galactosidase$ were for 3 hr at $37^{\circ}C$, pH 7 with 3 unit/g Fungal Lactase (Amano Lactase), yielding an average total amount of aglycone ranging from 40 to 47%.
