A Case Report of Regressed Metastatic Brain Tumors and Prolongation of Life with Allergen Removed Rhus verniciflua Stokes after Gamma Knife Radiosurgery (감마나이프 시술 후 옻나무 추출물 투여로 전이성 뇌종양 소퇴 및 생존기간이 연장된 환자 1례)

  • Objectives: The purpose of this study is reporting the possibility of the treatment of metastatic brain tumors with allergen removed Rhus verniciflua Stokes (aRVS) after gamma knife radiosurgery. Methods: A patient with lung cancer felt a headache about one year after conventional therapies, and metastatic brain tumors were diagnosed. He received gamma knife radiosurgery twice but refused to get more conventional therapies afterwards. So he has been treating with aRVS since then. Results: During 143 weeks of administration of aRVS, the size of brain masses has decreased continuously without extracranial metastasis and the patient has maintained a good performance status. Conclusions: This report suggests that aRVS may play a therapeutic role in the treatment of metastatic brain tumors. Further studies will be needed to determine the effect of aRVS on metastatic brain tumors.

Three Cases of Pityriasis Rosea in Child treated with Eunkyo-san (은교산으로 호전된 소아 장미색 비강진 3례)

  • Objectives : The purpose of this study is to report the effect and safety of Eunkyo-San on pityriasis rosea in children and to suggest antiviral effect of Eunkyo-san. Methods : We performed a retrospective chart review of three cases of pityriasis rosea in children younger than 10 years who treated with Eunkyo-san. Results : One patient recovered after taking Eunkyo-san 10 times for 10 days. The others recovered after taking Eunkyo-san 30 times for 22 days and hyperpigmentation was observed in one of them. Conclusions : It is expected that Eunkyo-san is thought to be effective and safe methods for the treatment of pityriasis rosea in children. Based on existing research, it is thought that Eunkyo-san has antiviral effect through the activation of immunological mechanism, and thus Eunkyo-san appears to have a therapeutic effect on pityriasis rosea.

Prevention of Preterm Birth and Management of Uterine Contraction with Traditional Korean Medicine (조산 방지 및 자궁수축 관리의 한의학적 치료)

  • Objectives: The aim of this study was to demonstrate the benefit of Traditional Korean Medicine as an adjuvant therapy in management of early uterine contractions and the prevention of Preterm Birth (PTB). Methods: It is a case report of a 38 year-old-woman hospitalized for irregular uterine contractions and cervical change at 33+3/7weeks of gestation. After 7 trials of IVF and artificial insemination, conception was successful via IVF with help of traditional Korean medicines. 2 TKMs were prescribed: Gami-danggui-san, and Antae-eum. 120 ml of Gami-danggui-san was given twice a day morning and evening along with same amount of Antae-eum once a day from 31 August 2013 to 28 November 2013. Tocolytics (Ritodrine) was administered as a first aid for maintenance of pregnancy. Information regarding progress until the delivery was collected during the patient’s visit. Results: As of 34+2/7 weeks of gestation, intermittent uterine contractions appeared (5-12 min) on cardiotocography and vaginal bleeding was also smeared at 34+3/7 weeks. However, enhanced tocolytics and continuous administration of herbal medicine sustained the pregnancy to term. At 37+2/7 weeks, no sign of labor with restored cervical length was confirmed. The woman gave a term birth to a healthy infant via vaginal delivery at 39+3/7 gestational weeks. Conclusions: Our report implies the potential of herbal medicine as a adjuvant therapy for preterm labor treatment. Further studies are needed to assess the safety and efficacy of TKM herbal medicine as a therapeutic alternative for curing preterm birth.

Case Report of Essential Tremor Treated with GwiBiTang and Gagambang, Acupuncture and Moxibustion (귀비탕 및 가감방과 침구치료로 호전된 본태성 진전 환자 치험례)

  • The purpose of this study was to report the effect of GwiBiTang and gagambang, acupuncture and moxibustion on essential tremor. This study was made on 6 cases that were treated in the OO University Oriental Medicine hospital and evaluated tremor using Visual Analogue Scale(VAS). GwiBiTang was one of the most generally used herbal medication, diagnosed as dual deficiency of the heart-spleen. In five patients out of six, VAS scores decreased. Two patients's scores decreased to 0 point. Three patients's score decreased to 2, 5 and 6 point, respectively. One patient was maintained on these Korean medical treatment for 26 days with no decrease of VAS score in tremor. The overall therapeutic effect was "markedly improved" in 2 (33.3%), "moderately improved" in 3 (50%), "poor" in 1 (16.7%). The results obtained this study suggest that GwiBiTang and gagambang, acupuncture and moxibustion might be effective in the treatment of tremors. Future studies will be required to ascertain the effect of GwiBiTang.

  • The process of physical therapy uses a problem-solving approach to enhance a patients's functioning status. The International Classification of Functioning, Disability and Health(ICF) is the common concept for the functioning in the world. Physical therapists require the ability to identify problems, formulate hypothesis, and plan intervention strategies through clinical reasoning. In the clinical process, physical therapists need to use standard and common languages in speech and in documentation. The purpose of this study was to suggest the process of making strategy for efficient intervention, examining and evaluating the functional problem of the person with stroke using ICF tools. For the first step in this process model, therapists could list the information relating to functional problems used by the ICF Core set and then could identify the interaction among the problems using the ICF assessment sheet. For the next step, therapist is needed to make the hypothesis and hypothesis testing, and then set a primary functional goals and therapeutic goals in detail after prioritizing the problems to be managed based on the problem list. Finally, after setting the identified problems as the purpose of intervention through the hypothesis testing, therapist could do some intervention after making a plan to solve these problems, and find out the outcomes using the ICF evaluation display. This report illustrates how to apply the process based on ICF concept into physical therapy practice. Making a decision for the most efficient intervention requires that therapists use the clinical reasoning process based on ICF concept.

Uterine Artery Embolization as an Effective Management and Diagnostic Tool for Puerperal Uterine Inversion with Severe Postpartum Bleeding: A Case Report (자궁내번증으로 인한 중증 산후 출혈 환자에서 효율적인 조치 및 진단적 도구로서의 자궁동맥색전술: 증례 보고)

  • Acute puerperal uterine inversion is a rare postpartum obstetric complication; however, without rapid diagnosis and appropriate management, it is life-threatening. Substantial bleeding hinders the verification of a partially inverted uterus, possibly delaying the treatment. Herein, we present the report of a 32-year-old female presenting with massive postpartum bleeding managed by uterine artery embolization. The peculiar course of the uterine artery bowing inferiorly along the inverted fundus during embolization could uncover the uterine inversion, which was not diagnosed by physical examination and CT. In conclusion, uterine artery embolization is not only an effective therapeutic strategy for postpartum hemorrhage but also a valuable tool for diagnosing uterine inversion.

  • Intrahepatic portosystemic venous shunt (IPSVS) is a rare vascular abnormality that involves abnormal communication between the intrahepatic portal vein and systemic veins, such as the hepatic vein or inferior vena cava. Patients with IPSVS are typically asymptomatic, and IPSVS is incidentally discovered via imaging while evaluating other diseases. However, endovascular closure of the shunt should be considered in symptomatic patients with a high-flow shunt. This report presents a patient with congenital IPSVS with sudden onset hepatic encephalopathy treated using percutaneous transhepatic embolization.

Interventional Treatment of Chemical Pleuritis and Hemothorax Caused by Iatrogenic Internal Jugular Vein Perforation after Central Venous Port System Implantation: A Case Report (중심정맥포트 삽입 후 의인성 속목정맥 천공에 의한 화학적 늑막염과 혈흉의 중재적 치료: 증례 보고)

  • The perforation of the intrathoracic internal jugular vein during the placement of an implantable central venous chemoport is a rare complication that is manifested by hemothorax or hemorrhagic shock. Furthermore, inappropriate instillation of a chemotherapeutic agent in the chemoport can cause chemical pleuritis, and the diagnosis of these complications prior to the instillation of chemotherapeutic agents and open thoracic surgery is mandatory. We report a patient with chemical pleuritis and hemothorax following an inappropriate instillation of a chemotherapeutic agent, through the perforated right internal jugular vein after placement of an implantable central venous chemoport. Treatment by embolization using coils and N-butyl cyanoacrylate, after percutaneous drainage, was successful.

Delayed Hemorrhage of the Hepatic Artery Caused by Biliary Stenting after Concurrent Chemoradiotherapy (동시항암화학방사선요법 후 담도 스텐트에 의해 발생한 지연성 간동맥 출혈)

  • Neoadjuvant concurrent chemoradiotherapy has been increasingly used to obtain secondary resectability for locally advanced pancreatic cancers. Although most patients require biliary decompression, only a few studies have investigated the safety of biliary stenting with chemoradiotherapy. Herein, we report a rare case of delayed hemorrhage of the hepatic artery caused by biliary stenting after chemoradiotherapy. The serial follow-up CT demonstrated that the biliary stent was approaching the right hepatic artery and eventually caused acute angulation and indentation. Diagnostic catheter angiography revealed contrast extravasation at the right hepatic artery, and endovascular embolization was performed. This report highlights the relevance of anatomical deformation after chemoradiotherapy, which can result in fatal complications. Indentation of the hepatic artery caused by biliary stents should be recognized as a warning sign of vascular injury.

Effects of a Tongue Self-Resistance Exercise on the Swallowing Function of Patients With Stroke: Case Report (자발적 혀 저항 운동이 뇌졸중 환자의 삼킴기능에 미치는 효과: 사례연구)

  • Objective: This study aims to discover how tongue self-resistance exercise affects the swallowing function of patients with stroke. Method: The subjects of this study were two patients who were treated at the Y hospital in Gyeonggido. Data were gathered by VFSS regarding the degree of swallowing disability and oral intake before and after intervention. This study analyzed case studies of two patients. One patient's intervention was applied in September 2012 for three weeks. The other's intervention was applied in February 2014 for three weeks. At the first session, the VFSS examination was administered. Then, intervention began after the patients gave their consent for participation. Each session was practiced 10 times per set, but one session had 3 sets. Intervention frequency consisted of five sessions per week, and it was conducted for three weeks. Tongue self-resistance exercise included tongue protrusion, tongue lateralization, and tongue elevation on the hard palate. Results: The first tongue self-resistance exercise had a positive effect on swallowing function because of the decrease in VFS score. The second tongue self-resistance exercise showed improved oral intake based on FOIS scores. Conclusion: This study's results suggest that there are many benefits of tongue self-resistance exercise on the swallowing function when it is applied to patients with stroke.
