Adolescent Health Risk Behaviors in Jeju, South Korea

  • Background: Adolescent risk behaviors are of concern because they are dangerous in themselves and have negative influences on adult health. Objectives: This study explored six health risk behaviors of a sample representing the adolescent population in Jeju. Methods: Frequency and percentage of risk behaviors were calculated. Additionally, the chi-square test was used to explore whether differences in experimentation with risk behaviors exist relative to gender and schooling levels. Results: There were six major findings in this study: (a) a lower rate of lifetime and current cigarette use among female students compared to male students, (b) a substantial portion of heavy drinkers, (c) a higher rate of lifetime drug use among female students than among male students, (d) a higher prevalence of suicide ideation among female students than among male students, (e) substantial percentages of students who did not usually wear seatbelts, motorcycle helmets, or bicycle helmets, and (f) a substantial portion of students who were not involved in vigorous physical activities. Conclusions: Since a substantial portion of adolescents have been involved with diverse risk behaviors, it is urgent to implement effective intervention strategies to reduce the rates of these behaviors.

청소년 가족유형별 건강형평성, 건강행위 및 정신건강 요인이 행복감에 미치는 영향: 제16차 청소년건강행태 온라인조사 자료 활용 (Health Equity, Health Behaviors, and Mental Health Factors Affecting Subjective Happiness among Korean Adolescents by Family Type: Using Data of the 16th Korean Youth Risk Behavior Survey)

  • Purpose: The purpose of this study was to prepare basic data for education and policy proposals related to enhancement of happiness by identifying factors affecting the subjective happiness among Korean adolescents by family type. Methods: The study was a secondary analysis using the raw data of the 16th Youth Risk Behavior Web-based Survey (2020). A complex sample multiple logistic regression analysis was conducted to identify factors affecting adolescent happiness by family type. Results: As a result, the factors that commonly affect the subjective happiness of adolescents for all family types were identified as economic status, academic performance, perceived stress, and loneliness. Additional factors that affect the subjective happiness of adolescents of both Korean two-parent and single parent families were sleep and subjective health status. Financial assistance, number of toilets, breakfast, exercise, depression, suicidal ideation, and suicide attempt were also reported for Korean two-parent families. Conclusion: This study provided empirical basic data for enhancing adolescent happiness in consideration of priority targets and family composition characteristics by identifying factors affecting subjective happiness by family type. Adolescent happiness is a factor that should be actively pursued at home, school, and society, and it should be the goal of education for holistic growth of students.

양극성 장애 환자에서 불안이 질병 경과에 미치는 영향 : 전향적 추적관찰에 대한 중간분석 (Anxiety Hastened Depressive Recurrence in Bipolar Disorder : An Interim Analysis of Prospective Follow-Up Study)

  • Objectives Despite growing attention to anxiety in bipolar disorder (BD), little research has assessed anxiety symptoms in the course of BD. The current prospective follow-up study examines the influence of subjectively and objectively measured anxiety symptoms on the course of BD. Methods A total of 49 patients with BD were followed-up prospectively for average of one year at an average of four months interval. The Korean version of the Beck Anxiety Inventory (K-BAI), the Hamilton Anxiety Rating Scale, heart rate variability (HRV) were used to measure anxiety subjectively, objectively and physiologically. Participants were divided into high and low anxiety groups based on their K-BAI scores. Kaplan-Meier survival analysis was performed to compare the recurrence of mood episode, suicide attempt, emergency room visit, and psychiatric hospitalization between two groups. Mediators were investigated with Cox proportional hazards models. Results Compared to the low anxiety group, the high anxiety group reported significantly higher impulsiveness (p = 0.016) and lower high frequency component on HRV (p = 0.007) after controlling for severity of BD. Regarding survival analysis, the high anxiety group showed hastened depressive episode recurrence (p = 0.048) and suicidal ideation was the mediator of the hazard ratio (HR) 1.089 (p = 0.029) in the Cox model. Moreover, the high anxiety group showed a tendency of accelerated suicide attempt (p = 0.12) and impulsivity was the risk factor of suicide attempt (HR = 1.089, p = 0.036). Conclusions This interim analysis of prospective study suggests that high anxiety level in BD may anticipate unfavorable course. Further studies are needed to understand the multifactorial mechanism of anxious bipolar patients.

청소년정서행동발달검사 표준화연구 자료를 활용한 학교폭력 피해 전국유병률 및 관련요인 조사 (Victims of Bullying among Korean Adolescents: Prevalence and Association with Psychopathology Evaluated Using the Adolescent Mental Health and Problem Behavior Screening Questionnaire-II Standardization Study Data)

  • Objectives : This study was conducted to investigate the prevalence of victims of bullying and the demographic characteristics of victims, and their related psychopathology, in a Korean nationwide sample of youths in middle and high school over a one month period. Methods : During the autumn of 2009, students in the 7th to 12th grades at 23 secondary schools participated in a nationwide, cross-sectional study. The study subjects completed the Adolescent Mental Health and Problem Behavior Screening Questionnaire-II (AMPQ-II) and Symptom Checklist-90-Revision (SCL-90-R). Based on the data acquired, descriptive statistics, correlation coefficients and multiple logistic regression analysis were performed. Results : Among the 3364 participants, 2272 (67.54%) completed the questionnaire. The prevalence of victimization was 28.9%. Male gender was positively associated with victimization, and grade level was negatively related to victimization. The AMPQ-II bullying score (Factor 4) was significantly (p<.001) and positively correlated to the AMPQ-II student total score (r= 0.50), Worry and thought (Factor 1 ; r=0.38), Mood and suicide (Factor 2 ; r=0.31), Academic and Internet-related problems (Factor 3 ; r=0.24), Rule violations (Factor 5 ; r=0.23), and AMPQ-II teacher total score (r=0.11). Somatization (r=0.23), Obsessive-compulsive behavior (r=0.24), Interpersonal sensitivity (r=0.30), Depression (r=0.33), Anxiety (r=0.26), Hostility (r=0.30), Phobic anxiety (r=0.22), Paranoid ideation (r=0.36), and Psychoticism (r=0.31) results from the SCL-90-R were also found to be positively related to the AMPQ-II bullying score, and remained significant after adjusting for age and gender. A total of 26% of the victims reported suicidal ideations as compared to 9% of non-victims over the month prior to the evaluation ($x^2$=119.595, df=1, p<.001). The multiple logistic regression analysis indicated that the AMPQ-II bullying score significantly increased the risk of suicidal ideation [Exp(b)=1.55, df=1, p<.001] after adjusting for age and gender. Conclusion : School bullying was highly prevalent among Korean middle and high school students. This study provided strong evidence that suicidal ideation and psychopathology were serious problems among the victims of bullying.

지역의 사회·경제적 요인과 노인의 자살생각 간의 관련성 연구 -다수준 분석을 활용하여- (A research on the Relationship between the Socio-economic Factors of the Regions and Suicidal Ideation of the Elderly -By utilizing the multi-level analyses-)

  • 본 연구는 생태학적 관점에서 노인이 생활하고 있는 지역의 경제 사회적 요인이 노인의 자살생각에 실제 영향을 미치는지 실증적으로 분석한 것이다. 분석 자료는 미시와 거시 데이터로 구분하여 미시 데이터는 "2014년도 노인실태조사" 10,248사례가 분석에 사용되었다. 거시데이터는 국가통계포털(KOSIS)에서 제공되는 각 지역별 사회적, 경제적 상황을 나타내는 지표들을 선별, 16개 시도의 2014년도 지역별 경제수준과 자연재해 및 보건과 노인의 여가, 복지 수준을 나타내는 6개 지표가 활용되었다. 분석방법은 분석단위가 개인차원과 지역차원의 2수준 이상의 위계적 특성과 이질성을 고려하여 위계적(다층) 분석 모형(Hierarchical or multi-level analysis model)을 적용하였다. 분석 결과, 노인의 자살생각 여부에는 지역의 생활물가지수와 국가기초수급률, 자연재해피해규모, 그리고 노인인구 대비 노인 대상의 여가 및 복지시설 수가 통계적으로 유의미한 영향을 미치는 것으로 분석되었다. 이상의 분석 결과들을 기반으로 지역사회에서는 자살생각을 증가시키는 주요 요인들을 주목하여 지역차원의 예방과 관리할 수 있는 시스템을 갖추고 노력이 강구 되어야 한다.

유흥업소 여성종업원의 죽음에 대한 태도가 자살충동억제의지에 미치는 영향 -룸살롱과 바 여성종업원 태도 비교조사- (Korean Hostess' Attitudes toward Death and its Effects on Their Capacities to Resist Suicidal Impulses -Comparative study of Korean Hostess Working at the Room Salon and Bar-)

  • 목적: 본 연구에서는 부산지역 소재 유흥업소 여성종업원(룸살롱/바)을 대상으로 이들의 죽음에 대한 태도와 자살관련 요인들과의 관계를 조사하였다. 방법: 120명의 유흥업소 여성종업원을 대상으로 설문 조사를 실시하였다. 설문지는 죽음에 대한 태도에 관한 총 7개의 요인으로 구성되어있으며, 죽음해방인식, 죽음자연현상수용, 죽음공포, 죽음생각회피, 나의 죽음이 가족에게 주는 충격, 죽음생각빈도, 자살충동억제의지로 구분된다. 결과: 룸살롱 여성종업원들은 '월수입 만족도'가 높을수록 죽음에 대해 생각하는 빈도가 낮았으며, 자살충동억제의지가 높아지는 것으로 나타났다. 또한 룸살롱 여성들의 '자살충동억제의지'에 유의미한 영향을 주는 요인은 '나의 죽음이 가족에게 주는 충격'과 '월수입 만족도'로 나타났다. 그러나 바 여성종업원들의 경우 '죽음해방인식' 요인만이 '자살충동억제의지'에 유의한 영향을 주는 것으로 조사되었다. 결론: 본 연구의 결과에 따르면, 룸살롱 여성종업원들의 경우 '월수입 만족도'가 죽음에 대한 태도와 자살에 관한 생각을 정립하는데 있어 중요한 역할을 하는 것으로 보인다. 이처럼 개인이 가진 죽음에 대한 태도와 자살과의 관계를 이해하는 것은 어려운 상황에서 자살이라는 최악의 선택을 하지 않게끔 도울 수 있는 또 하나의 방법이 될 것이다.

Factors Related to Suicidal Thinking in Cancer Patients of Korean Adults Over 19 Years in Accordance with the Lapse of Time: Data from the Korea National Health and Nutrition Examination Survey (KNHANES) 2009, 2017

  • 본 연구는 국민건강영양조사의 자료를 이차분석하여 시간적 추이에 따른 19세 이상 한국 성인 암환자의 자살생각 관련 요인을 파악하여 암환자 관리의 기초자료를 제공하기 위해 시도되었다. 자료는 IBM SPSS WIN 22.0 program을 이용하여 2009년, 2017년 2개년도 연구대상자의 인구사회학적 특성, 건강관련 특성, 정신관련 특성을 분석하기 위해 복합표본 빈도분석, 복합표본 교차분석, 복합표본 다중 로지스틱 회귀분석을 이용하여 분석하였다. 연구결과 암환자 자살생각 관련요인은 2009년 활동제한, 우울증상 경험으로 나타났고(Negelkerke R2=.384) 2017년 연령, 결혼상태, 활동제한, 우울증상 경험으로 나타났다(Negelkerke R2=.525). 본 연구는 시간적 추이에 따른 암환자 자살생각 관련요인을 파악하여 관련요인의 변화를 확인했다는 점에서 의의가 있으며, 암환자의 자살을 예방할 수 있는 중재 프로그램의 개발 및 적용이 필요하다.

다문화 가정과 한국 가정 청소년의 정신 건강 비교 (Comparative Study of Adolescents' Mental Health between Multicultural Family and Monocultural Family in Korea)

  • Objectives : With the observation of factors influencing depression, self-reported happiness, self-reported stress and suicidal attempt, this study compared and evaluated the mental health of adolescents from multicultural families and monocultural families in Korea. Methods : This study compared the characteristics of adolescents from multicultural and monocultural families based on the analysis of data of The Tenth Korea Youth Risk Behavior Web-based Survey and analyzed the factors influencing adolescent depression, self-reported happiness, self-reported stress and suicidal attempt. Results : There was no significant difference in the levels of depression, self-reported happiness, and self-reported stress between adolescents from multicultural and monocultural families. However, suicidal ideation and suicidal attempt were significantly higher in adolescents from multicultural families. Conclusion : There was no significant difference between the mental health of adolescents from multicultural and monocultural families. Because depression is the greatest factor for suicidal attempt, and violence has stronger relationship with depression of adolescents from multicultural families, further study is needed to prevent this trend.

청소년들의 우울 및 자살관련 행태와 스마트폰 사용과의 관련성 (The association of depression and suicidal behaviors with smartphone use among Korean adolescents)

  • Objectives: This study aims to examine the association of depression, suicidal behaviors with smartphone use behaviors among Korean adolescents. Methods: We analyzed data from 'Youth Health and Life Skills Survey' in 2013. The survey was based on self-reported questionnaires distributed to 915 grade 7th and 8th students at four middle schools in Gangdong-gu, Seoul, Korea in 2013 to evaluate the effects of 'Youth Health and Life Skills' program. Depression and suicidal behaviors were measured in terms of the experience rate, and smartphone use behaviors were measured with average hours of smartphone use a day and overindulged behaviors in smartphone use by using 5-point Likert scales. We examined the association of depression and suicidal behaviors with smartphone use behaviors by using t-test and regression analyses. Results: 21.5% of respondents have ever felt depressed or hopeless to lose interest in school life almost everyday for 2 weeks or longer in the past year. The experience rate of suicidal ideation, suicide plan, and suicidal attempt were 21.1%, 6.1%, and 5.7%, respectively. The mean of average hours of smartphone use a day was 3.9 hours, and the mean score of overindulged behaviors in smartphone use was 2.99. The students who have experienced depression and suicidal behaviors were more likely to have more smartphone using time and higher scores of overindulged behaviors in smartphne use than those who have not. Conclusion: We concluded that for preventing addictive or problematic smartphone use behaviors among adolescents we should consider and develop more positive strategies for the moderate use of smartphone than regulatory measures, which include the implementation of skill-based health education program including life skills at schools and the development of smartphone applications useful for resolving mental stress and promoting mental health.

The Nonlinear Association Between Internet Using Time for Non-Educational Purposes and Adolescent Health

  • Objectives: This study was performed to consider the association between Internet using time for non-educational purposes and adolescent health, and to examine how health status differs between Internet users and non-users. Methods: We analyzed 2009 data from the Korea Adolescent Risk Behavior Web-Based Survey, conducted on a nationally representative sample of students in grades 7 to 12. A total of 75 066 adolescents were categorized into four groups according to their Internet using time excluding using for educational purposes: non-Internet users (NIUs), occasional Internet users (OIUs) (<1 h/d), moderate Internet users (MIUs) (${\geq}1$ and <2 h/d), and heavy Internet users (HIUs) (${\geq}2$ h/d). Health factors included eight health risk behavior indices, four mental health indices and six physical health indices. Results: The distribution of Internet use was as follows: NIUs 17.4%, OIUs 68.1%, MIUs 12.7%, and HIUs 1.7%. In multivariate analysis, using OIUs as a reference, U- or J-shaped associations were observed for five health risk behavior indices (current smoking, current drinking, drug abuse, sexual intercourse, sedentary behavior on weekdays) and four mental health indices (stressed, depressed, suicidal ideation, attempted suicide) in both genders. After removing confounding effects, including age, region, school type, subjective school record, subjective economic status, presence of parents, living with family, and sedentary behavior, these associations were still observed. Conclusions: Health professionals should consider both Internet non-users (for non-educational purposes) and heavy users to be high-risk groups in terms of health status. Also, more well-designed studies are needed to clarify what factors are working in these nonlinear associations.
