Temperament characteristics of children with persistent and recovered stuttering: A longitudinal study (말더듬이 지속된 아동과 회복된 아동의 기질 특성 비교: 종단연구)

  • The purpose of this study was to investigate the temperament characteristics associated with stuttering subtypes (persistent and recovered) over time and the relationship between those characteristics and stuttering severity. This four-year longitudinal study covered 41 preschool children who stutter (CWS) and 30 preschool children who do not stutter (the CWNS group). At the final visit, 27 CWS were classified as the Recovered group and 14 CWS were classified as the Persistent group. Using the Children's Behavior Questionnaire-Short Form, each participant's temperament characteristics were measured twice: at one year and two years after the initial visit. The three subscale scores (Extraversion, Negative Affectivity, and Effortful Control) and the 15 component scores were analyzed, and they were used for between-group and between-visit comparisons. The Persistent group showed a significantly higher Negative Affectivity subscale score at every visit than the Recovered and CWNS groups. Within this subscale, significant group differences were found in the 'Fear' and 'Anger/Frustration' components, demonstrating that the Persistent group scored higher than the Recovered and CWNS groups. There was no significant correlation between the subscale and component scores and the stuttering severity scores within the Persistent group at any visit. These results support the proposition that these two stuttering subtypes have different temperament characteristics; they also imply that temperament might be influenced by stuttering experience over time.

Development of stuttering treatment practice device using stretch sensors (스트레치 센서를 이용한 말더듬 치료 훈련기의 개발)

  • Using stretch sensors, a stuttering treatment training device that improve the abnormal breathing of stutterer was designed and developed. To improve stutterer's inadequate breathing method that is one of principal reason of stammering, the device estimates breathing method by checking the changes of the stretch sensor's resistances those are put on the chest and abdomen of user. And a vocal exercise program that carry out exercises only when the user maintains the abdominal breathing was designed. Using a PIC16C711 device that includes an A/D convertor, a main controller was designed and the vocal exercises software was developed using Director and C program with graphic user interface for user convenience. The controller sends the resistance data of sensors to PC through the serial port and the software verifies the breathing method. And the device was designed that the RTS (request to send) pin of serial port in PC is used as a power source so that it can work without any battery or other power source. Three stutterers have carried out the clinical experiments using the implemented device for two months and the results showed it was excellent to alleviate the stuttering.

A Case Report of treating Stuttering Priapism (재발성 음경지속발기증 1례)

  • Priapism is a persistent penile erection that continues for more than 4 hours beyond, or is unrelated to, sexual stimulation. Because priapism is a medical emergency, all patients with priapism should be evaluated and treated urgently. Subtypes of priapism are ischemic, non-ischemic and stuttering priapism. Stuttering type is a recurrent form of ischemic priapism in which unwanted painful erections occur repeatedly with intervening periods of detumescence. The etiology of this type is often idiopathic and the treatments are still not clearly established. We present one case of stuttering priapism which occurred after suppression of his sexual desire. A 23-year-old man visited our clinic complaining of recurrent pain and erections persisting for about 5 weeks. We performed evaluation and confirmed the diagnosis of Rigid swollen penis. Jibaekjihwang-tang gamibang (ZhiBaiDihuang-Tang Jiaweifang) was administerd to the patient for 9 days. After the treatment, duration of erections and pain showed remarkable improvements. This will be recorded as the first case report of treating priapism with a Korean medical approach and suggests that Korean medicine therapy can be efficient to treat priapism.

  • This study was aimed of the effects of the Parent's Gestalt Speech Intervention for stuttering development of Fluency disorder Child. The Parent's Gestalt Speech Intervention was made up of a program understand phase, an awareness phase, a change phase and, finally, an arrangement and termination phase. The subjects 6 (female 2, male 4) of this research were developed a stuttering behavior without any apparent neurological damage or other speech or developmental impediments. To access their stuttering behaviors, I used methods for observing levels of behavioral in each phase. The results of the study are as follows: In regard to stuttering behavior, word repetition frequency decreased in the interim assessments, showing that the learning of fluent speech was acquired early in the therapy process. In conclusion, the results of the study show that Parent's Gestalt Speech Intervention for stuttering development of Fluency disorder Child.

Acoustic Features of Phonatory Offset-Onset in the Connected Speech between a Female Stutterer and Non-Stutterers (연속구어 내 발성 종결-개시의 음향학적 특징 - 말더듬 화자와 비말더듬 화자 비교 -)

  • The purpose of this paper was to examine acoustical characteristics of phonatory offset-onset mechanism in the connected speech of female adults with stuttering and normal nonfluency. The phonatory offset-onset mechanism refers to the laryngeal articulatory gestures. Those gestures are required to mark word boundaries in phonetic contexts of the connected speech. This mechanism included 7 patterns based on the speech spectrogram. This study showed the acoustic features in the connected speech in the production of female adults with stuttering (n=1) and normal nonfluency (n=3). Speech tokens in V_V, V_H, and V_S contexts were selected for the analysis. Speech samples were recorded by Sound Forge, and the spectrographic analysis was conducted using Praat. Results revealed a stuttering (with a type of block) female exhibited more laryngealization gestures in the V_V context. Laryngealization gesture was more characterized by a complete glottal stop or glottal fry both in V_H and in V_S contexts. The results were discussed from theoretical and clinical perspectives.

Measurements of Speaking Rate and Fluency in Stuttering Adults (유창성장애 성인의 말속도와 유창성 측정에 관한 연구)

  • The purpose of this study was to investigate speech rate and fluency in stuttering adults. It was suggested that a measurement guideline of speech rate and fluency for collecting clinically meaningful data be used. Subjects included 10 adults who stutter (mean age=25;8). Syllables were used as the unit of measurement for analyzing the duration of speech. The mean rate was 241 SPM (syllables per minute) for reading, and 196 SPM for spontaneous speaking. Fluency was also measured in both cases. The correlation between rate of speech and fluency was high (r=0.92). A strong positive correlation was found between different investigators in measuring speech rates and fluencies.

  • Laryngeal DDK involve with the rate, pattern, and regularity (periodicity) in opening and closing of vocal fold. This study was aimed at investigating the characteristics of laryngeal DDK between nonfluent and fluent speakers. One with an ataxic dysarthria (with cerebellar lesion) and the other with stuttering, and 13 normal speakers were evaluated. L-DDK were analyzed with MSP (motor speech profile, CSL 4400). Measures of DDK included: DDKavr, DDKcvp, DDKjit, DDKavp. An ataxic dysarthric speaker and a stutterer showed more reduced rate and aperiodic L-DDK (both adductory and abductory movement) than normal speakers. But the average L-DDK period (ms) in adductory movement in a speaker with stuttering showed more decreased than the other. Results from this study are preliminary. Nonetheless, results of L-DDK produced by nonfluent speakers suggested the possibility to have relation with slow rate of phonatory initiation and connected speech. In the future, perceptual studies are needed in conjuction with acoustic and speech production.

Effects of Background Noises on Speech-related Variables of Adults who Stutter (배경소음상황에 따른 성인 말더듬화자의 발화 관련 변수 비교)

  • This study was mainly aimed at investigating on the effects of background noises (i.e., white noise, multi-speaker conversational babble) on stuttering rate and other speech-related measures (i.e., articulation rate, speech effort). Nine Korean-speaking adults who stutter participated in the study. Each of the participants was asked to read a series of passages under each of four experimental conditions (i.e., typical solo reading (TR), choral reading (CR), reading under white noise presented (WR), reading with multi-speaker conversational babble presented (BR). Stuttering rate was computed based on a percentage of syllables stuttered (%SS) and articulation rate was also assessed as another speech-related measure under each of the experimental conditions. To examine the amount of physical effort needed to read, the speech effort was measured by using the 9-point Speech Effort Self Rating Scale originally employed by Ingham et al. (2006). Study results showed that there were no significant differences among each of the passage reading conditions in terms of stuttering rate, articulation rate, and speech effort. In conclusion, it can be argued that the two different types of background noises (i.e., white noise and multi-speaker conversational babble) are not different in the extent to which each of them enhances fluency of adults who stutter. Self ratings of speech effort may be also useful in measuring speech-related variables associated with vocal changes induced under each of the fluency enhancing conditions.

Botulinum Toxin Injection for the Treatment of Voice and Speech Disorders (보툴리눔독소 주입에 의한 음성장애 및 언어장애의 치료)

  • Botulinum toxin, a neurotoxin derived from Clostridia Botulinum, has been injected into the target muscle(s) for the treatment of several kinds of voice and speech disorders at the Voice Clinic, Yonsei Institute of Logopedics and Phoniatrics since December 1995. Criteria for the diagnosis and method of injection for spasmodic dysphonia, mutational dysphonia, muscle tension dysphonia, dysphonia after total laryngectomy, and stuttering were summarized. Among 144 patients with adductor type spasmodic dysphonia, who were injected one time to maximum 8 times during the 27 months, 90% were recognized as having better than slight improvement. Even though the injected cases were small, not only the abductor type spasmodic dysphonia, but also the intractable mutational dysphonia or muscle tension dysphonia resistant to voice therapy revealed that botulinum toxin injection would be another options for treatment. Patients who cannot phonate after total laryngectomy and some forms of adulthood stutterers can also be candidates for the injection of botulinum toxin.
