보건소 방문간호사의 직무만족 관련요인 (Job Satisfaction related Factors of Home Visiting Nurses in the Public Health Centers)

  • Purpose: This study was aimed to investigate predictors of job satisfaction of home visiting nurses at the 16 public health centers in Busan. Method: There are two groups of independent factors: non-work related (age. educational level. working duration as nurses in hospitals. certificate). and work related (working duration as nurses in public health centers. working duration as home visiting nurse. position. number of households visited per week, workload, cooperation among staff, support by supervisors, supply of vehicles, supply of materials). The participants were 88 home visiting nurses from 16 public health centers in Busan. Data were collected with self-administrated questionnaires and analysed using an univariate logistic regression and multiple logistic regression analysis. Result: support by supervisors (good vs not-good, OR=3.70. p=0.025), and supply of materials (good vs not-good, OR=3.33, p=0.038) had significants effects on job satisfaction. Conclusion: The results were similar to those of other studies on the predictors of job satisfaction of clinical nurse at hospitals, and were helpful in developing nursing interventions to increase job satisfaction among home visiting nurses in public health centers. 의료 인력의 확보가 환자 입원일수에 미치는 영향 (The Effects of Medical Staffing Level on Length of Stay)

  • Purpose: The objective of this study was to analyze the effects of medical staffing level as bed-to-medical staff ratio on patient outcomes as length of stay (LOS) among hospitals in Korea. Methods: Two hundred and fifty one hospitals participated in the study between January and March 2008. Data for the study was requested by an electronic data interchange from the Health Insurance Review Agency in 2008. In data analysis, SPSS WIN 15.0 program was utilized for descriptive statistics, t-test, ANOVA, Pearson correlation coefficients, and multiple regression. Results: The mean score for length of stay was 13.6 days. The mean of operating bed-to-nurse ratio was 7.93:1. The predicting factors for LOS were bed-to-nurse's aide ratio, bed-to doctor's ratio, severely ill patient rate, and hospital type. These factors explained 28.9% of the variance in patient outcomes. Conclusion: This study results indicate that the relationship between medical staffing level and patient outcomes is important in the improvement of the quality of patient care. Thus, improvements in the quality of the nurse practice environment could improve patient outcomes for hospitalized patients.

간호관리자의 진정성리더십이 팀 창의성에 미치는 영향 : 긍정심리역량의 매개효과 (The Influence of Authentic Leadership on Team Creativity : Mediation Effect of Positive Psychological Capital)

  • 본 연구는 간호사가 인식한 간호관리자의 진정성리더십과 팀 창의성 관계에서 긍정심리역량의 매개효과를 확인하기 위해 시행되었다. 연구참여자는 서울과 경기도 소재 4곳의 종합병원에 근무하는 124명의 일반간호사였다. 자료수집은 진정성리더십, 긍정심리역량, 팀 창의성 측정도구를 사용하였다. 자료분석은 SPSS 25.0을 활용하여 간호관리자의 진정성리더십, 긍정심리역량, 팀 창의성 간의 상관관계는 Pearson's correlation coefficient로, 진정성리더십과 팀 창의성 사이의 긍정심리역량 매개효과는 Baron과 Kenny의 3단계 위계적 회귀분석을 실시하였다. 연구결과는 일반간호사가 지각하는 진정성리더십이 높을수록 팀 창의성이 높아지고, 간호사의 긍정심리역량도 높았다. 또한 진정성리더십과 팀 창의성과의 관계에서 긍정심리역량은 부분매개효과를 갖는 것으로 분석되었다. 이러한 결과를 근거로 팀 창의성 향상을 위해서 간호관리자의 진정성리더십과 일반간호사의 긍정심리역량을 강화시킬 수 있는 융복합적이고 조직행동론적 관리전략이 필요하다.

병동선임간호장교의 간호관리역량 격차분석과 원인조사 (An Analysis of Nursing Managerial Competencies;Military Hospital Head Nurses)

  • The purpose of this study was to identify the gap between need-level and demonstration-level in nursing managerial competencies. In addition, the study proposes solutions to narrow this gap. The results of this study are as follows : 1) The mean score for need-level of each item was 4.0, and for demonstration-level, 3.5. This indicates that military hospital head nurses demonstrate a higher level of managerial competencies than the moderate level on all items. But items which were related to resource/ cost/ information managament, staff development management and professionalism management got relatively low ratings in the need-level. 2) The mean score for need-level of each category was 4.14, and for demonstration-level, 3.53. Categories on the individual dimension got a higher rating than categories on the group or organization dimension in both need-level and demonstration level. 3) The gap between need-level and demonstration-level appeared in all items(p<.05) and categories(p<.001). Although the gap was relatively low, it indicates that it is essential to plan a developmemt program for all nursing management competencies for military hospital head nurses. 4) There were significant differences in the gap between need-level and demonstration-level according to specific characteristics of the subjects. The gap did not appear in many categories on the individual dimension where the number of nursing staff was more than 10, a major grade, ICU head nurse or for head nurses having a long career. 5) Need-level and demonstration-level showed a difference according to specific characteristics of the subjects, because need-level and demonstration-level were higher where the number of nursing staff was more than 10, a major grade, and for ICU or Medical ward head nurses. The categories which showed need-level difference and demonstration-level differences according to specific characteristics of the subjects existed almostly completely in the group and organization dimension. Gap-level differences according to the number of hospital bed existed in only two categories. 6) The general causes of the gap were indicated to be 'Knowledge/ skill/ experience deficit', 'Limitation of rules and systems/ Inappropriate organizational environment' for most items, categories, and dimensions. The results of this study indicate that extensive competency developing strategies must be developed, because a gap was found in all items and categories. Specially, there is a need to concentrate attention on competencies in the group and organizational dimension which had a wider gap level. And it is important to take an individual approach according to the cause for each gap. In order to produce effective competency developing strategies, top managers must have sinsights into the importance of nursing staff development and nursing officer's efforts to develop themselves must be achieved. Further multi-dimensional(education, personnel-policy, nursing/ organizational environment) solutions to the gap must be developed and utilized.

간호학 교수들의 간호전문직에 대한 태도 조사연구 (Attitudes of Nursing Educators Toward Percussional Nursing)

  • This study of nurse educator's attitudes which found aspects of professional nursing was carried out from September 10 to 30, 1977. Subjects were 205 respondents from a total population of 314 nurse educators from the level of instructors to professors in all the diploma (post high school 3 year), professional junior college (3 year level) and university (4 year) nursing schools in Korea. Specific objectives were to determine their attitudes about the 1. social position of nursing as a profession, 2. nurses attitudes towards their word, 3. factors influencing the development of nursing, 4. the future of nursing, 5. nursing educational problems, 6. their own educational position's intrinsic job satisfaction and 7. their salary and benefits. The instrument used was a questionnaire developed from consultations with nursing educators and sociologists, and based on earlier. worts by Burke (1976), Mason (1974) and Hong 1969. Data were analysis by computer using one-way analysis. Statistically significant findings included the following; 1. Responses were positive toward all seven aspects of professional nursing. 2. Compared to diploma nursing school staff whose responses were positive, professional and university school faculty response toward the social position of nursing were neutral. 3. Faculty with clinical experience of 10 to 19 years were most positive in their responses about nurses' attitudes towards their work. In all three types of schools, in comparison to professors, associate professors' responses were more positive about nurses' attitudes toward their work. Faculty with longer clinical experience expressed high agreement with statements about factors which influence the development of nursing. Without any differences between school, all faculty had positive attitudes towards the future of nursing, the younger the staff member, the more positive were the responses. 4. faculty in diploma schools and those with 10 to 19 years clinical experience expressed high agreement with statement about nursing educational problems, 6. their own educational position's intrinsic job satisfaction and 7. their salary and benefits. 요양병원 종사자의 메르스에 대한 지식, 태도 및 감염예방행위 관계 (Knowledge, Attitudes and Compliance Regarding Infection Preventive Behaviors for MERS among Staff in Long-term Care Hospitals)

  • 본 연구는 메르스 대유행시 요양병원 종사자의 메르스에 대한 지식, 태도 및 감염예방행위 실천도와의 관계를 파악하기 위하여 시도되었다. 연구대상자는 J도 소재 요양병원 종사자 211명으로 구조화된 설문지를 이용하였으며, 자료수집기간은 2015년 6월 15일부터 7월 15일까지였다. 자료 분석은 SPSS/WIN 24.0을 이용하여 independent t-test, ANOVA, Welch test로 분석하였고, 사후 검정은 scheff test, Grames-hawell test 방법으로 시행하였다. 요양병원 종사자의 메르스에 대한 지식점수는 간호사와 간호조무사, 감염관리 교육경험이 있는 경우, 지난 해 인플루엔자 예방접종을 한 경우, 손소독제 사용이 용이한 경우에 높았으며, 메르스에 대한 태도는 간호사가 간호조무사와 간병인보다 긍정적이었다. 메르스 감염예방행위 실천도는 일상생활에서는 지난 해 인플루엔자 예방접종을 한 경우, 환자 간호 시에는 지난 해 인플루엔자 예방접종을 한 경우, 손소독제 사용이 용이한 경우에 높았다. 요양병원 종사자의 메르스에 관한 지식, 태도 및 감염예방행위 실천도와의 관계는 지식이 높고, 태도가 긍정적일수록 감염예방행위 실천도가 높게 나타났다. 이에 메르스와 같은 신종감염병이 유행할 때, 요양병원에서는 메르스 질병 특성 및 전파, 예방법이 포함된 감염관리 교육을 실시해 메르스에 대한 지식수준을 높이고, 태도를 긍정적으로 유도하여, 감염예방행위 실천도를 향상시킬 필요가 있을 것이다.

적정 간호인력 등급별 입원료 추정 모델을 이용한 간호관리료 차등제 정책개선 재정부담 추계 (Financial Projection of the Nursing Fee Differentiation Policy Improvement Proposal in the National Health Insurance: Using a Break-even Analysis Model for the Optimal Nursing Fee)

  • Purpose: This study was done to propose an improvement in the Nursing Fee Differentiation Policy to alleviate polarization of nursing staffing level among hospitals and to rectify the confusion of legally mandated standards between the Korean Medical Law and National Health Insurance Act. Methods: The policy regulation was reconstructed related to nurse staffing standards and nurse-to-patients ratios. Data on nurse staffing grades were obtained from database of the Health Insurance Review & Assessment Service (HIRA) for the third quarter of 2010 for 44 tertiary hospitals, 274 general hospitals, and 1,262 hospitals. A break-even analysis was used to estimate financial burden of the revised policy improvement proposal. An industrial engineering method was used to calculate Nurse-to-Patients ratios per shift. Results: Twelve tertiary hospitals were downgraded. 74 general hospitals and 102 hospitals were upgraded after application of the regulation. Finances for total hospitalization expenditures changed from -3.55% to +3.14%. Conclusion: The results indicate that the proposed policy would decrease polarization between tertiary hospitals and small hospitals, and would not put a major strain on the finances of the Korean National Health Insurance. Therefore, it is suggested that government stake-holders and many interest groups consider this policy proposal and build a consensus.

의료기관 특성에 따른 간호등급 변화 추이: 2008~2010년 (Changing Trend in Grade of Nursing Management Fee by Hospital Characteristics: 2008-2010)

  • Purpose: This study was aimed to examine changes of hospital nurse staffing by hospital characteristics during 2008-2010. Methods: The study sample included 44 tertiary hospitals, 226 general hospitals, and 532 non-general hospitals that were operating during 2008-2010. Grade of nursing management fee was categorized from Grade 1(highest) to 6 (lowest) in tertiary hospitals, l or Grade 7 in general hospitals and non-general hospitals based on the nurse-to-bed ratio. For data analysis, ${\chi}^2$ and GEE were conducted. Results: For three years, the number of tertiary hospitals below Grade 2 were increased from 8 to 12, the number of those above Grade 4 were decreased from 15 to 6. The number of general hospitals above Grade 6 decreased from 123 to 86. Tertiary hospitals and general hospitals had more possibilities to improve nurse staffing grade than general hospital (OR 79.69, 95% CI 50.77~125.09, OR 11.25, 95% CI 8.15~15.53, respectively). Greater likelihood of improvement in grade of nursing management fee was found in university hospital or hospitals with 300 or more beds than other types of hospitals. Conclusion: Differentiating nurse staffing system by hospital type and increasing financial incentives according to the grades are needed to improve hospital nurse staffing.

임상간호사(臨床看護師)의 윤리적(倫理的) 가치관(價値觀)과 윤리적(倫理的) 갈등(葛藤)에 관한 연구(硏究) (A Study of Ethical Sense of Value and Discord of the Clinical Nurse)

  • Professional nurse shall possess the firm nursing idea and ethical nursing philosophy based on the professional knowledge and technology but due to the rapid social situations has changed the value to man thus the nurse's sense of value in the nursing secenes has been confused and changed bringing the new ethical problems and discord due to the ethically difficult problems. This study is aimed to know about the discord between the ruling ethical sense of value and the ethical discord exeriencing in the clinical scenes of the nurse and to help them establish affirmative ethical sense of value and provide them with the materials which can effectively meet the ethical discord. The study research has been conducted by selecting 515 clinical nurses in 8 general hospitals as the subject from Mar. 13, 1995 to Apr. 3. The tool measuring the ethical sense of value disigned by Lee, Young Sook has been used and supplemented and the tool measuring the ethical discord was the question papers about the ethical dillemma of Han, Sung Sook. The collected materials have been analyzed by the statistical methods of arithmetical everage, t-test, ANOVA, Pearson's Correlation Coefficient and etc. The result of the study is as the below ; 1) The average point of the ethical sense of value of the nurse was 3.62(maximum point : 5) which showed that the ethical level was so high and the highest question item in the ethical level of the whole items of the ethical sense of value was "They conduct as directed by the doctor in case the disposition of the doctor looks mistaken"(4.56 point), "They keep the secret of the patients while serving them."(4.56) and the lowest item was "using placobo for the patients is not allowed" : (1.85 point). 2) Statistical variation which showed the significant difference in the relation with the ethical sense of value according to the general characters of the nurse has shown as scholarly years(F=3.47, p=.016), religion(F=1.66, p=.004), interest degree of ethical education(F=4.18, p=.006),attitude to the job of nursing(F=6.76, p=.006), ethical standard(F=3.28, p=.021), and recognition degree on ethical principles(F=4.53, p=.001). 3) The average point of the ethical discord of the nurse was 0.54(The maximum-1 point) point and the ethical discord in the clinical scenes : "the problems arising from the lack of manpower of the nurse"(0.86), "the discord from the uncooperative relation between them and the medical staff and"(0.75) and indifference of the doctor about the report of the nurse(0.73). 4) The variation which showed the significant difference statistically in the ethical discord according to the general characters of the nurse was that Age(F=19.88, p=.000), schooling(F=5.32, p=.001), Experience(F=15.77, p=.000), position(F=13.58, p=.000) and ethical standard(F=2.63, p=.049). 5) The results of the analysis of correlation between ethical sense of value and discord(r=0.519, p=.239) showed no significant correlation statistically.

  • 수액은 일반적으로 환자에게 특정 주사를 일정 시간동안 주입하기위해 사용되어진다. 환자나 보호자는 수액의 투입종료 시점을 예상하여 의료진을 호출하여야 하는 경우가 많고, 취침시간 등 특정 시간에 수액투여가 종료될 경우 환자나 보호자에게 더 많은 피로를 줄 수 있다. 또한 의료진은 수액의 투입종료시점을 확인하기 위하여 몇 번이고 현장에 찾아가 수액의 양을 모니터링 해야 하는 수고로움으로 업무의 비효율성을 가져올 수 있을 것이다. 본 논문에서는 이러한 문제를 해결하기 위해서 수액의 수위를 감시하는 최소한의 장비와 능동형 RFID를 결합한 형태의 위치기반 서비스 시스템을 제안하고, 환자별 수액의 양과 환자의 위치를 실시간 파악하여 환자나 보호자 또는 의료인(간호사)에게 정확한 정보를 제공하여 수액 투여가 마무리될 시점에 의료진이 환자의 위치로 찾아가는 서비스로 의료서비스의 질을 향상시킬 수 있을 것으로 기대한다.
