수면장애 노인 환자와 일반 노인 환자의 수면양상, 수면장애요인 및 수면증진행위 (Comparison of Sleep Pattern, Factors of Sleep Disturbance and Sleep Enhancement Behaviors between Sleep Disturbance and Non-Sleep Disturbance Aged Patients)

  • Purpose: This study was done to compare sleep patterns, factors of sleep disturbance and sleep enhancement behaviors between sleep disturbance and non-sleep disturbance elderly patients. Method: The participants were 117 patients over 65 years old who were hospitalized at 5 general hospitals in Seoul. There were 83 patients in the good sleeper group and 34 in the poor sleeper group. The survey questionnaires included questions on general characteristics, sleep patterns, sleep disturbing factors, and sleep enhancement behaviors of the patients. Results: For sleep patterns and environmental factors of sleep disturbance, the poor sleepers was significantly higher than that of the good sleepers. Significant differences between good sleepers and poor sleepers were also found on the following variables: Environmental factors of sleep disturbance -'Sound of other patient's or care-giver's', 'Discomfort of bed, linen, pillow and patient's gown', 'Light in the room', 'Emergency situations', 'Temperature of patients rooms too hot or too cold'. The significant physical factor of sleep disturbance was 'Hard to breath even without moving' and for sleep enhancement behaviors: 'Use of ear plugs or eye covers' Conclusion: Consequently it is necessary to develop positive and differentiated programs for sleep enhancement for the inactive and poor sleeper among the elderly patients.

재가 여성노인의 수면에 영향을 미치는 요인 (Factors Influencing Sleep of Elderly Women)

  • Purpose: The study was done to identify factors influencing the sleep of elderly women. Methods: Data were collected by questionnaires from 203 elderly women in Seoul and Gyeong-gi province. Measures were physical health state, family support, life satisfaction, sleep, depression, and quality of life. Data were analyzed using Pearson correlation coefficients, t-test, ANOVA, and stepwise multiple regression with SPSS/WIN 13.0 version. Results: The mean score for sleep for elderly women was 39.04. Sleep for elderly women according to religion, education level, spouses, and monthly income showed significant differences. Sleep for elderly women showed significantly positive correlations to physical health state, family support, life satisfaction, and quality of life. The significant factors influencing sleep of elderly women were physical health state, family support, depression, and quality of life, which explained about 70.4% of the variance. The strongest factors influencing the sleep was physical health state. Conclusion: The results suggest that in developing nursing interventions and practice for sleep pattern of elderly women spouses and family support should be considered.

중환자실 환자의 수면양상, 수면장애특성 및 수면장애요인 (Sleep Patterns of ICU Patients, Characteristics and Factors Related to Sleep Disturbance)

  • Purpose: This study was done to examine the sleep pattern of ICU patients, as well as characteristics of, and factors related to sleep disturbances, and state anxiety. It was done to provide the basic data fur effective nursing interventions to improve quality of sleep for these patients. Methods: The participants were 104 patients who were admitted to the surgical ICU in a hospital in Seoul. To measure sleep patterns, the Verran and Snyder-Halpern Sleep Scale and the Sleeping Scale developed by Oh et al.(1998) were used. Characteristics of sleep disturbances were measured using the Sleep characteristics scale developed by Park(1999). To measure factors related to sleep disturbances, thirty items developed by Oh(1998) were used. Results: There was no statistically significant difference in the sleep patterns according to the general characteristics of the patients. The characteristics of sleep disturbances showed statistically significant differences in general and specific characteristics of sleep patterns but there was no statistically significant difference according to the general characteristics of the patients. There were significant negative correlations between factors related to sleep disturbances and general and specific characteristics of sleep patterns but there were no statistically significant difference according to the general characteristics of the patients. Conclusion: The results showed that the majority of patients staying in the surgical ICU experienced sleep disturbances and that the physical factor was the major factor of sleep disturbances.

한국 일반인의 한증과 수면의 질과의 상관성 연구 (Study on the Relationship between Cold type and Sleep Quality in Koreans)

  • The purpose of this study was to investigate the relationship between sleep quality and sleep duration according to individual characteristics, and the relationship between sleep quality and cold type. A total of 1998 participants aged 30-55 who participated in a community cohort study at KIOM's Oriental Medicine Data Center (KDC) was analyzed. Cold type was diagnosed with a common the Questionnaire of Cold Pattern Identification (8 items). Sleep quality was assessed by the Pittsburgh Sleep Quality Index. The participants in this study were classified into 1193 with cold type and 805 with non-cold type. The poor sleep quality was found in more women than men, and the Body Mass Index (BMI) was higher in the non-cold type. In the cold type group, average sleep quality was poor and sleep duration was short. There was a statistically significant correlation between cold score, sleep quality, and sleep duration. Cold type was related to the sleep quality. The risk of poor sleep quality increased 1.46 with increased cold type (95% CI = 1.16-1.84, p <.001). In particular, it was found that various programs to improve the quality of sleep, focusing on the cold type among personal characteristics, are needed to maintain adequate sleep to improve the sleep quality. In the future, when analyzing the relationship between cold type and sleep quality, it is thought that an attempt to derive other objective quantitative indicators is needed.

피톤치드 아로마 흡입과 운동의 복합요법이 대학생의 기분상태, 대학생활 스트레스 및 수면에 미치는 영향 (Phytoncide Aroma Inhalation and Exercise Combination Therapy Mood state, college life stress and sleep of College Students)

  • 본 연구는 대학생들의 피톤치드 아로마 흡입과 운동의 복합 처치가 그들의 대학생활스트레스, 기분상태 및 수면에 미치는 영향을 12주 장기간 실험을 통하여 규명하고자 하였다. 피톤치드 아로마 흡입과 운동의 복합 처치의 효과를 탐색하고자 피톤치드 아로마 흡입군(18명), 운동군(17명), 피톤치드 아로마 흡입군+운동군(17명), 대조군(20명)의 4집단 총 72명을 대상으로 하였으며, 사전측정(0week), 중간측정(6week), 사후측정(12week)으로 구성되는 $4{\times}3$ 반복측정에 의한 요인설계($4{\times}3$ factorial design with repeated measure)로 설계하여 대학생의 대학생활 스트레스 지수, 기분상태의 변화를 살펴보았다. 그 결과 통제집단을 제외한 모든 집단에서 기분상태, 수면, 대학생활스트레스의 점수가 향상된 것으로 나타났다. 특히 피톤치드 아로마 흡입과 운동만을 실시한 집단보다 피톤치드 아로마 흡입과 더불어 운동을 병행한 집단이 더 유익한 효과를 나타냈다는 점에서 매우 의미 있는 연구라고 생각된다.

정상인과 비만인의 수면 일주기 리듬과 수면 질의 차이 (Difference in Sleep Circadian Rhythm and Sleep Quality between Normal-weight and Obese Group)

  • Purpose: The purpose of this study was to understand sleep circadian rhythm and sleep quality between normal-weight and obese group according to Body Mass Index to develop education and nursing intervention programs for the obese. Methods: This study involved 186 subjects who visited at S hospital obesity clinic, K province. They were divided into 2 groups: normal-weight group 91 and obese group 95. Data were collected from October 18th to November 12th in 2013. Data were analyzed with frequency, percentage, ${\chi}^2$-test, Mann-Whitney U test, ANCOVA, t-test and ANOVA with using SPSS version 20.0. Results: The results showed that morning type 1.1%, middle type 91.2% and evening type 7.7% of sleep circadian rhythm in normal-weight group and middle type 92.6% and evening type 7.4% of sleep circadian rhythm in obese group. There were statistically significant results on sleep quality with covariance sex and stress, sleep duration, habitual sleep efficiency and sleep disturbance between normal-weight and obese group. There were statistically significant results on sleep quality, sleep duration, habitual sleep efficiency and sleep disturbance in middle type of sleep circadian rhythm between normal-weight and obese group. Conclusion: Therefore, it is necessary to consider subject's sleep pattern to develop education and nursing intervention programs for the obese.

임상실습기간 중 간호대학생의 수면 영향요인: Spielman의 수면장애모델을 근간으로 (Factors Influencing Sleep Patterns during Clinical Practice Weeks among Nursing Students : Based on Spielman's Model)

  • Purpose: The purpose of this study was to identify the sleep patterns of nursing students using self-report questionnaire and physiologic measurement, to examine the factors influencing sleep patterns in based on Spielman's model. Methods: Participants were 119 nursing students who were in the clinical practice period. Self-report questionnaires and actigraphy were used to collect the data. Data were analyzed using descriptive statistics, correlation and regression by the SPSS/WIN 21.0 programs. Results: When sleep was measured by self-report questionnaire, 84 students (70.6%) showed poor sleep quality. The mean sleep efficiency was 82.6%, and 67 students(56.3%) showed low sleep efficiency (less than 85.0%). The factors affecting subjective sleep pattern measured by KMLSEQ were circadian sleep type (${\beta}=.28$, ${\rho}=.003$) and alcohol (${\beta}=.20$, ${\rho}=.031$). The factors affecting total sleep time were sedentary behavior (${\beta}=-.27$, ${\rho}=.003$) and daytime sleepiness (${\beta}=-.33$, ${\rho}$<.001). Conclusion: Many nursing students in their clinical practice period expressed sleep disturbance. Factors affecting the perceived sleep measured by the self-report questionnaire and objective sleep evaluated by physiologic measures were different. The evening type of students perceived poor sleep quality, however, sedentary life style and daytime sleepiness resulted in short sleep time. Therefore, more studies measuring the objective sleep characteristics are needed using subjective and objective characteristics.

사업장 교대근무자와 비교대근무자의 수면문제와 건강상태 비교 (Comparison of Health Status and Sleep Patterns between Shift Workers and Non-shift Workers in Manufacturing Plants)

  • Purpose: This study was conducted to compare and estimate sleep patterns and health status of the shift and non-shift factory workers. Methods: The subjects of this study were 290 male workers in 13 companies, which manufacture automobile parts in Daegu, Korea. Survey data were collected from March 5th, 2007 to April 5th, 2007. PSQI was scored to check the quality of sleep, ESS for excessive daytime sleep disorder, ISI for insomnia, and SF-36 for health status. The data were analyzed by $X^2$ and t-test using SPSS 12.0 for windows. Results: There was a difference in quality of sleep patterns between shift and non-shift workers (p=.033). A difference in the pattern of excessive daytime sleep (P=.005) and in the degree of insomnia (p=.030) were observed between the two groups. However, there were no significant difference in health status between the two groups. Conclusion: Based on these results, shift workers are in need of careful and continuous monitoring regarding sleep disorders and we need to develop health promotion programs including sleep hygiene for shift workers.

  • 본 논문은 수면을 이루는 침실의 수면 환경 데이터를 수집하고, 얻어진 조건 데이터들과 수면간의 관계를 분석하여, 이를 바탕으로 시뮬레이션 모델을 추출하여 개개인에 따른 최적의 수면 환경을 제공하도록 한다. 또한 수면 과정에 따라 피로도, 음주도, 공복도 등의 신체 상황에 따른 수면 상황의 차이점 및 패턴을 정의하여 보다 안정적인 수면 솔루션을 제공할 수 있도록 하였다. 따라서 적절한 실내 환경 변화를 주고 보다 더 쾌적한 생활을 누리도록 도움을 주었다.

수술 후 환자의 수면 개선에 대한 한의치료 임상연구 동향 -한약, 침, 뜸 치료를 중심으로- (Research Trends of Korean Medicine Treatments for Postoperative Sleep Improvement: Focusing on Herbal Medicine, Acupuncture, and Moxibustion)

  • Objectives: To review trends of clinical trials on Korean medicine treatments for postoperative sleep improvement. Methods: We searched randomized controlled trials (RCTs) on Korean medicine treatments for postoperative sleep improvement from ten domestic and foreign databases. Sample sizes, diseases, types of operation, diagnosis tools, pattern identification, interventions, outcome measurements, and main results of included studies were extracted and analyzed. Results: A total of 20 RCTs were selected. Most studies were published in China. The most common target disease was cancer, followed by cardiovascular disease. Most studies lacked detailed description regarding participants such as onset, duration of sleep disturbance, and preoperative sleep issues. Herbal medicine was the most frequently used in 12 studies. The most commonly used prescription was Suanzaoren decoction. The effectiveness of Korean medicine treatment on improving postoperative sleep was found to be significant in most studies. Conclusions: Korean medicine treatments might be effective in postoperative sleep improvement. However, the quality of included studies was low. Therefore, further well-designed research studies are needed to provide high quality clinical evidence on Korean medicine treatments for postoperative sleep improvement.
