A 9-month-old, female Miniature Pinscher(MP) dog weighing 1.97kg was presented because of periodic syncopal episode for 5 months. This case was diagnosed as primary dilated cardiomyopathy based on respiratory distress history, weak femoral pulse, generalized cardiomegaly, pulmonary edema, marked dilation of left atrium(LA) and left ventricle(LV), decreased wall thickness of LV and interventricular septum(IVS), increased EPSS in echocardiography, and young age of onset in the absence of other cardiovascular disorders. The patient was stabilized by application of diuretics(Furosemide, 2 mg/kg, SC, q 1 hr) and venodilator(Nitroglycerine patch, 0.5 mg/kg, q 12 hrs). Clinical signs were improved with medical management of positive inotropic vasodilator(Pimobendan, 0.2 mg/kg, PO, q 12 hrs) and angiotensin-converting enzyme(ACE) inhibitor(benazepril, 0.5 mg/kg, PO, q 12 hrs), potassium gluconate gel(2 mEq/dog, PO, q 12 hrs) and, L-carnitine(50 mg/kg, PO, q 12 hrs). The dog still maintains stable clinical status 10 months after the first visit. We report the rare case of DCM in small breed dog, which corresponds to the diagnosis and treatment of typical DCM in large breed dog.
Background: Despite the recent promising efforts to improve survival in patients with esophageal carcinoma, the long term survival results of patients with esophageal cancer have seldom been reported in Korea. To establish standard control for future studies, we re trospectively analyzed the surgical treatment results of the esophageal cancer patients managed in our department at Seoul National University Hospital. Material and Method: From January 1984 to December 1996, 734 patients were diagnosed with esophageal cancer. Among them, 253 patients underwent surgery in our department. We retrospectively analyzed the operative results and long term survival rates of these patients. Result: The majority of patients(237) had squamous cell histology and only 6 patients had adenocarcinoma. The final TNM stage grouping for these patients was based on the 1988 revised American Joint Commitee on Cancer classification. Twenty one patients were surgically classified as stage I, 109 as stage II, and 107 as stage III. C respiratory failure in 8, sepsis in 1, hepatic failure in 1, bleeding in 1 and unknown etiology in the remaining 3. The actuarial survival of 222 patients in whom the curative resection was accomplished at 1-, 2-, 3- and 5 years was 74.7${\pm}$3.1%, 46.5${\pm}$3.7%, 32.3${\pm}$3.7%, and 19.9${\pm}$3.3%, respectively. CONCLUSION: The poor long term survival rates suggest that an alternative treatment method such as intensive combined modality therapy should be developed for the management of esophageal cancer.
Objectives: The objectives of this study are to estimate the inhalation exposure level of benzene for workers using Tier 1 exposure models ECETOC TRA (European Center for Ecotoxicology and Toxicology of Chemicals Target Risk Assessment) and Stoffenmanager, and to investigate their reliability for exposure assessment in K-REACH. Methods: Two exposure scenarios, 'manufacture of benzene' and 'use as solvents,' were developed for assessment of workers' exposure to benzene. The Process Category (PROC) for ECETOC TRA was collected from the European Chemical Agency (ECHA) registration dossier, and the Activity for Stoffenmanager was converted from PROC using translation of exposure models (TREXMO). The information related to exposure, such as working duration, Respiratory Protective Equipment (RPE), Local Exhaust Ventilation (LEV), and Risk Management Measure (RMM) were classified into high, medium, and low exposure conditions. The risk was determined by the ratio of the estimated exposure and occupational exposure limits of benzene. Results: Under high exposure conditions, the worker exposure level calculated from all PROCs and Activities exceeded the risk level, with the exception of PROC 1 and Activity 1. In the medium exposure condition, PROC 8a, 8b, and 9 and Activity 3, 7, and 8 all exceeded the risk, whereas in the low condition, all PROCs and Activities were determined to be safe. As a result, action corresponding with the low exposure condition is required to reduce the risk of exposure among workers in workplaces where benzene is manufactured or used as a solvent. In addition, the predicted exposure levels derived from the exposure models were lower than measured levels. The exposure levels estimated from Stoffenmanager were more conservative than those from ECETOC TRA. Conclusions: This study demonstrates the feasibility of exposure models for exposure assessment through the example of occupational inhalation exposure assessment for benzene. For more active utilization of exposure models in K-REACH, the exact application of collected information and accurate interpretation of obtained results are necessary.

Cleidocranial dysplasia (CCD) is an autosomal dominant skeletal dysplasia and is caused by mutation in the CBAFA1 gene of 6p21 chromosome band. Patients with CCD express skeletal dysplasia such as hypoplastic/aplastic clavicle, brachycephalic skull, midface hypoplasia and moderate short stature. In addition to skeletal dysplasia, specific symptoms may appear in respiratory organs, auditory area, and the more distinguished, dentition. Dental findings include: delayed eruption of permanent tooth, multiple supernumerary tooth more than five, malocclusion, etc. In Patients presenting excessive SNT, complications of SNT could be prevented and will be managed through pertinently timed treatment such as tooth extraction, using space maintainer, and orthodontic management after early diagnosis. This case is about the treatment of eruption disorders in permanent teeth owing to SNT in CCD patients, who are three family members in the $3^{rd}$ generation inherited from maternal grandfather through atavism. We performed the extraction of numerous SNT and orthodontic treatment on them in this case. On evaluating panoramic and cephalometric views, some classical signs of skeletal dysplasia due to CCD were recognized in a pool of three patients, the clavicle was distinctively displayed in all patients.

Improvements in disaster management has become a global necessity because the magnitude of disasters is intensifying in parallel with the increased disaster damage. The disaster risk in Korea is also increasing due to the emergence of new types of disaster; such as the Middle East respiratory syndrome coronavirus, the increase of complex disasters, and the heightened probability of a catastrophic event due to climate change. This paper aimed to identify the disaster loss-frequency relationship from 1948 to 2014 in Korea by using four types of variables. In addition, this paper found major disasters that resulted in the reformation of disaster response organizations, and inputted the deaths and economic loss attributed to those disasters into the disaster loss-frequency graph. The research result substantiated that the disaster loss-frequency relationship in Korea follows the Power Law and found the coefficients of each Power Function. Additionally, this paper found that most of the reformations of disaster response organizations happened after major disasters that concentrated societies attention and anger due to the high human and economic impact; such events are labelled as "focusing events." These focusing events, with the characteristics of a low probability and high impact, are located in the long tail of the Power Law Distribution. This paper suggests that the effective public policy for disaster response needs to be developed by paying attention to 'low probability and high impact' focusing events that are located in the long tail of the Power Law Distribution.
Purpose : In this study, we listed common diseases in pediatric inpatients and evaluated the distribution of diseases by period and age group, in order to estimate the epidemiologic trend. Methods : Patients who were admitted to the Department of Pediatrics between 1997 and 2008 were included. Demographic characteristics, date of admission, and International Classification of Diseases (ICD) code of patients were indentified. Study period was divided into two; early (1997-2002) and late (2003-2008), and age of patients were grouped into four; infancy, early childhood, late childhood, and adolescence. Results : A total of 33,513 patients were admitted for 12 years. In the list of ICD code, Pneumonia (J12-J18; 21.2%) was the most prevalent, followed by gastroenteritis (A00-A09; 17.8%), bronchiolitis (J21; 11.9%), and so on. Common diseases ranked from 1 to 10 comprised the majority (79.1%) of all the inpatients. There was increase in the number of inpatients with respiratory infectious disease (bronchiolitis, otitis media, and sinusitis), enlarged lymph node, or impetigo/cellulitis, but decrease in the number of inpatients with aseptic meningitis, intussusceptions, measles, or nephritic/nephrotic syndrome. The distribution of diseases also showed age group-specific difference. Conclusion : The distribution of diseases by period and age group was different. The epidemiologic trend should be considered in developing the management of strategy for the Department of Pediatrics.
The management of malignant pleural effusion is a difficult problem. Tetracycline was probably the most commonly used sclerosing agent, but parenteral tetracycline is no longer available. This prospective study was designed to determine the efficacy of insufflated talc in producing plerodesisin patients with malignant pleural effusions. Fifteen patients managed between March 1995 and March 1997 underwent thoracoscopy and had 4 gm sterile asbestos-free talc insufflation throughout the pleural space. The mean interval between the procedure and last follow-up was 101 days(range: 25-233 days). The results of treatment were clinically and radiologically compared with pre-operative status. The results were as follows: 1. The patients were 6 men and 9 women with a mean age of 55.1 years(range: 31-74 years). The primary tumors were lung cancer in 10 patients, breast cancer in two, hepatoma in one, stomach cancer in one, and adenocarcinoma of unkown origin in one. 2. Cancer cell positive ratio was 36% in sputum cytology study and 100% in thoracoscopic biopsy. 3. Symptoms improved in 94% of patients treated by talc pleurodesis. 4. After 30 days of the procedure, there was radiologically no recurrence of plural fluid in 94% patients. 5. There were wound infection in one, respiratory failure in one. In conclusion, the insufflation of 4 gm sterile asbestos-free talc in the pleural space is an effective method to control malignant pleural effusions and patients selection is important factor for reexpansion the lung.
Recently, the problem of air pollution has become a global concern due to industrialization and overcrowding. Air pollution can cause various adverse effects on human health, among which respiratory diseases such as asthma, which have been of interest in this study, can be directly affected. Previous studies have used clinical data to identify how air pollutant affect diseases such as asthma based on relatively small samples. This is high likely to result in inconsistent results for each collection samples, and has significant limitations in that research is difficult for anyone other than the medical profession. In this study, the main focus was on predicting the actual asthmatic occurrence, based on data on the atmospheric environment data released by the government and the frequency of asthma outbreaks. First of all, this study verified the significant effects of each air pollutant with a time lag on the outbreak of asthma through the time-lag Pearson Correlation Coefficient. Second, train data built on the basis of verification results are utilized in Deep Learning algorithms, and models optimized for predicting the asthmatic occurrence are designed. The average error rate of the model was about 11.86%, indicating superior performance compared to other machine learning-based algorithms. The proposed model can be used for efficiency in the national insurance system and health budget management, and can also provide efficiency in the deployment and supply of medical personnel in hospitals. And it can also contribute to the promotion of national health through early warning of the risk of outbreak by atmospheric environment for chronic asthma patients.
From January 1990 through December 1995, 43 patients underwent diaphragmatic plication for the management of phrenic nerve palsy .complicating various pediatric cardiovascular surgery. Their mean age at plication was 11.1 months and sex ratio was 31 males to 12 females. In order of decreasing incidence, the primary cardiovascular procedures included modified Blalock-Taussig shunt (7), total correction for the Tetralogy of Falloff (7), arterial switch operation (6), unifocalization for the pulmonary atresia with VSD (3), modified Fontan operation (3), VSD patch closure (3) and others. The involved sides of diaphragm were right in 17, left in 2) and bilateral in 3. Extensive pericardial resection with electocauterization of resected margin was thought to be the most common cause of phrenic nerve palsy (20). The interval between primary operation and plication ranged from the day of operation to 98 days (median 11 days). The methods of plication were central pleating technique(plication with phrenic nerve branch preservation) in 41, and other technique In 2. 10 patients died after plication (7: early, 3; late), and the causes of death were thought to be unrelated to plication itself. Among the 36 early survivors, extubation or cessation of positive pressure ventilation could be accomplished between 1 and 24 days postoperatively(mean : 4.5). Cumulative follow-up was 92 patient years without major complications. Postoperative follow-up fluoroscopy was performed in 6 patients, and the location and movement of plicated diaphragms were satisfactory in 5 patients. We concluded that diaphragmatic plication with preservation of phrenic n rve branch could lead to cessation of positive pressure ventilation and complete recovery of diaphragmatic function in the long term, unless the phrenic nerve was irreversibly damaged.

Streptococcus pneumoniae is a common cause of acute otitis media, sinusitis, pneumonia, and the invasive bacterial infections in children. Rarely, S. pneumoniae is an uncommon cause of hemolytic-uremic syndrome (HUS). We report a 33 month-old girl who presented with pneumonia, and subsequently developed HUS. Her pulmonary infection was complicated by necrotizing pneumonia and acute respiratory distress syndrome. Cultures from blood and pleural fluid grew S. pneumoniae, serotype 19A. She was treated with antibiotics, dialysis and mechanical ventilatory support. She was discharged with normal renal function after 2 months of management. She remained healthy without renal complications at the 5 year follow-up visit.
