To evaluate radiation effect on the hematopoietic system, we analyzed 44 patients who were treated with conventionally fractionated radiation therapy (RT) at Chonbuk National University Hospital. According to the treatment sites, we classified them into three groups: group I as head and neck, group II as thorax, and group III as pelvis. White blood cell, lymphocyte, platelet and hemoglobin were checked before and during RT The results were as follows; 1. White blood cell (WBC) and lymphocyte count were declined from the first week of RT to the third week, and then slightly recovered after the third or fourth week. There was prominent decrease in lymphocyte counts than WBC. 2. Platelet counts were declined until the second week of the RT, showed slight recovery at fourth week in all groups. Hemoglobin values were slightly decreased in the first week and then recovered the level of pretreatment value, gradually. 3. Lymphocyte count were declined significantly on group III(p<0.05), WBC and platelet counts were decreased on group II but statistically not significant.
HRccine was administered subcutaneously for 4 weeks to rabbits at dose levels of 300, 60 and 12 times the expected clinical dose to evaluate the subacute toxicity. There were no effects in clinical signs, body weight changes, food consumption, water consumption, urinalysis and blood biochemistry in any animals tested. In hematological examinations, decrease of lymphocyte counts and increase of platelet counts were observed in the medium- and high-dose treated groups. Absolute weights of thymus were tending to decrease, but no pathological changes were observed in microscopic examinations. The no-effect dose in subacute toxicity study of rabbits was considered to be 300 times the expected clinical dose.
Purpose: Platelet-rich plasma (PRP) is known to accelerate and/or enhance hard and soft tissue healing and regeneration. As such, PRP has been used in various clinical fields of surgery. Recently there have been several attempts to use PRP in the field of tissue engineering. However, some controversies still exist on exact mechanism and benefits of PRP. Therefore various animal experiments are necessary to reveal the effect of the PRP. However, even if animal experiment is performed, the efficacy of the experiment could not be validated due to absence of an animal PRP model. The purpose of this study is to establish rat PRP model by comparing several PRP fabricating methods, and to assay growth factor concentration in the PRP. Materials and methods: Rat blood samples were collected from nine SD rat (body weight: 600-800g). PRP was prepared using three different PRP fabricating methods according to previously reported literatures. (Method 1: 800 rpm, 15 minute, single centrifuge; Method 2: 1000 rpm, 10 minute, double centrifuge; Method 3: 3000 rpm, 4min and 2500 rpm, 8 min, double centrifuge). Platelet counts were evaluated in an automated machine before and after PRP fabrications. In terms of growth factor assay, prepared PRP were activated with 100 unit thrombin and 10% calcium chloride. Growth factor (PDGF-BB, VEGF) concentrations on incubation time were determined by sandwich-ELISA technique. Results: An average of 3ml (via infraorbital venous plexus) to 15ml (via celiac axis) the rat blood could be collected. By using Method 3 (3000 rpm, 4 min and 2500 rpm, 8 min, double centrifugation), around 1.5ml of PRP could be prepared. This method allowed us to concentrate platelet 3.77-fold on average. PDGF-BB concentration (mean, 1942.10 pg/ml after 1 hour incubation) and VEGF concentration (mean, 952.71 pg/ml after 1 hour incubation) in activated PRP were higher than those in untreated blood. Also PDGF-BB showed constant concentration during 4-hour incubation, while VEGF concentration was decreased after 1 hour. Conclusion: Total 11,000 g minute separation and condensation double centrifuge method can produce efficient platelet-rich plasma. Platelet-rich plasma activated with thrombin has showed higher concentrations of growth factors such as PDGF-BB and VEGF, compared to the control group. Platelet-rich plasma model in a rat model was confirmed in this study.

Background: It has been demonstrated that neutrophil:lymphocyte (NLR) and platelet:lymphocyte (PLR) ratios are associated with prognosis in cancer patients. The aim of this study was to investigate whether pretreatment white blood cell (WBC), neutrophil, lymphocyte, monocyte, platelet, basophil and eosinophil counts, LDH level, NLR and PLR are associated with prognosis in patients with malignant pleural mesothelioma (MPM). Materials and Methods: We retrospectively reviewed files of 50 patients who were managed with a diagnosis of MPM between 2005 and 2010. Demographic and clinical characteristics, treatments, response to treatment and prognostic factors were evaluated, along with relationships between pretreatment blood parameters and prognosis. Results: Overall, 38 men and 12 women were included to the study. Mean age was $61.5{\pm}9.4$ years (range: 39-83 years). There was advanced disease in 86% (n=43) and the histological type was epithelial mesothelioma in the majority (82%). Of the cases, 17 (34%) received radiotherapy, while 42 cases underwent first- and second-line chemotherapy, with cisplatin plus pemetrexed as the most commonly used regimen. In the assessment after therapy, it was found that there was complete response in 4 cases (8%), partial response in 10 cases (20%), stable disease in 17 cases (34%) and progression in 19 cases (38%). Median follow-up was 10 months (range: 10 day-30 months). Median overall survival was found to be 20.7 months while median progression-free survival as 10 months. In univariate and multivariate analyses, it was found that factors significantly affecting overall survival included stage (p=0.030), response to treatment (p=0.026) and monocyte count (p=0.004), while factors affecting disease-free survival included NLR (p=0.018), response to treatment (p=0.001), and PLR score (p=0.003). Conclusions: Overall and disease-free survival was found to be better in cases with a WBC count<8.000, platelet count<300,000, and low NLR and PLR scores in malignant pleural mesothelioma.

This study was carried out to isolate and characterize the glycomacropeptide (GMP) prepared from cow's milk and Korean native goat's milk and to examine the effects of their tryptic hydrolysates on inhibition of platelet aggregation in an in vitro experiment. The GMP derived from Holstein, Korean native goat and Hanwoo migrated at 20 KDa. Sialic acid contents in skim milk of Holstein, Korean native goat and Hanwoo were $36.86{\pm}2.36$, $37.98{\pm}1.27$ and $31.19{\pm}1.87{\mu}g/mg$, respectively. Tyrosine was detected in both bovine and caprine GMP. The in vitro inhibition rate of platelet agregation by tryptic hydrolysates of Holstein, Korean native goat and Hanwoo GMP were 4.02, 5.51 and 12.77%, respectively at reaction time 30 seconds. The inhibition of platelet aggregation by tryptic hydrolysates of bovine and caprine GMP are increased with increasing reaction time. The platelets staining revealed higher counts of platelets after the addition of GMP hydrolysates; however addition of ADP reduced the platelet count within 30 seconds and the platelets were not detected after 120 seconds. The results of this study indicate that tryptic hydrolysates of bovine and caprine GMP contain some small peptides with platelet aggregation inhibition properties. Further research on these lines may help prevent platelet aggregation related abnomalities in human.

Objective : The purpose of this study was to analyze the variability of clopidogrel responses according to duration of a clopidogrel drug regimen after stent-assisted coil embolization (SAC), and to determine the correlation between the variability of clopidogrel responses and thromboembolic or hemorrhagic complications. Methods : A total of 47 patients who underwent SAC procedures to treat unruptured intracranial aneurysms were enrolled in the study. Preoperatively, patients received more than seven days of aspirin (100 mg) and clopidogrel (75 mg), daily. P2Y12 reaction unit (PRU) was checked with the VerifyNow test one day before the procedure (pre-PRU) and one month after the procedure (post-PRU). PRU variability was calculated as the difference between the initial response and the follow-up response. Patients were sorted into two groups based on their response to treatment : responsive and hypo-responsive. Results : PRU variability was significantly greater in the hypo-responsive group when compared to the responsive group (p=0.019). Pre-PRU and serum platelets counts were significantly correlated with PRU variation (p=0.005 and p=0.004, respectively). Although thromboembolic complication had no significant correlated factors, hemorrhagic complication was correlated with pre-PRU (p=0.033). Conclusion : In conclusion, variability of clopidogrel responses during clopidogrel medication was correlated to serum platelet counts and the initial clopidogrel response. Thromboembolic and hemorrhagic complications did not show correlation with the variability of clopidogrel response, or the clopidogrel response after one month of medication; however, hemorrhagic complication was associated with initial clopidogrel response. Therefore, it is recommended to test patients for an initial clopidogrel response only, as further tests would be insignificant.
Objectives: Recently, angiogenesis has gained an increasing interest as a prognostic factor in breast cancer. In this study we aimed to assess the anti angiogenic effects of HAD, a botanical anticancer remedy which has been prescribed in Daejeon University Oriental Hospital in Korea, on patients with breast carcinoma by measuring the serum vascular endothelial growth factor (VEGF), basic fibroblast growth factor (b-FGF) and platelets levels. Methods: The study included 26 consecutive breast cancer patients (mean age$\pm$standard deviation: 47.5$\pm$8.7 years) with stage II to IV disease who were treated with HAD (mean duration $\pm$ standard deviation: 264.5$\pm$121.6 days). In addition to routine laboratory and staging procedures, serum VEGF, b-FGF levels and platelet counts were determined as antiangiogenic markers. The antiangiogenic effects of HAD were evaluated by analyzing the differences between the values of the antiangiogenic markers before and after the treatment with HAD. Results: Serum b-FGF concentrations were significantly reduced after the treatment with HAD (P=0.042). Serum VEGF concentrations were found to have a somewhat decreasing change, though the change was not statistically significant (P=0.229). Platelet counts had little changes (P=O.80). Conclusions: It is supposed that HAD has effects on decreasing the serum b-FGF levels related with the clinical outcome of breast cancer patients.
The present study aimed to determine the effect of oral ${\beta}$-glucan on mucositis and leukopenia in 62 consecutive patients with colorectal cancer treated with an adjuvant FOLFOX-4 regimen. The patients were retrospectively evaluated in 2 groups: one group received ${\beta}$-glucan and the other did not (control group). Leucocytes, neutrophils, and platelets were evaluated before and 1 week after chemotherapy and oral mucositis and diarrhea were noted. Leucocyte and neutrophil counts after chemotherapy in the ${\beta}$-glucan group were $7,300/mm^3$ and $3,800/mm^3$, respectively, and the reductions, as compared to baseline, were not significant (p=0.673 and 0.784). The median platelet count was $264,000/mm^3$ after chemotherapy in the ${\beta}$-glucan group and the reduction, as compared to baseline, was borderline significant (p=0.048). In the control group, reduction in leucocyte, neutrophil, and platelet counts was statistically significant. Oral mucositis and diarrhea were less common in the ${\beta}$-glucan group. We conclude that ${\beta}$-glucan can be used to reduce the adverse effects of chemotherapy.
Purpose: This prospective study was conducted to reveal the haematological index change by low level radiation exposure in radiological environment our hospital workers. Materials and Method: We gathered the cumulative dose by Thermoluminenscent Dosimeters (TLD) over 9-yr period and examined hematological index counts change (RBC, Hb, Platelet, WBC, Monocyte, Lymphocyte, Neutrophilic, Basophilic, Eosinophilic) both occupational workers and controls. Of a total 370 occupational workers and 335 controls were compared. Results: This analysis has led to the following general observations 1) The average cumulative dose in male and female were $9.65{\pm}15.2\;mSv$, $4.82{\pm}5.55\;mSv$ respectively. 2) In both male and female, there were very low relationship between occupation period and cumulative dose (r< ${\pm}0.25$). 3) Occupation period was more increased, in male, WBC counts decreased and increased workers, RBC counts decreased workers were more than controls group (p<0.05). In female, WBC counts decreased and increased workers and W-eosino counts decreased workers were more than controls group (p<0.01). 4) Cumulative dose was more increased, in male, W-Lympho counts decreased workers and Platelet counts deceased workers were more than controls group (p<0.05). In female, W-lympho counts decreased workers and RBC counts decreased workers were more than controls group (p<0.05). Conclusions: We can find some kinds of blood index abnormal distribution in occupational radiation workers by comparing with controls. Occupational workers cannot avoid radiation exposure, in spite of the control it. Actually low level radiation adverse effect occurred not dose but probability. So workers must always try to reduce exposure by ourselves, furthermore as long as possible the government should provide rapidly that national system on radiation control for worker's health.
Excessive blood loss secondary to cardiopulmonary bypass(CPB) may be encountered after open heart surgery and platelet dysfunction appears to be especially responsible for this problem. To evaluate the effect of low-dose aprotinin during hypothermic CPB on platelet aggregation, anticoagulation and clinical hemostasis,.40 patients undergoing valve replacement using hypothermic CPB procedures were randomized to give either a low dose aprotinin(2$\times$ 106 KIU in the CPB priming sol- ution, n=20) or a placebo(n=20). During postoperative 24 hours, blood and hemoglobin loss were lower in the aprotinin group (225.5 $\pm$ 121.9ml, and 11.3$\pm$2.4g) than the control group(572.2$\pm$)35.5ml and 26.3$\pm$9.8g)(P<0.01). The total blood and hemoglobin loss were lower in the aprotinin group (622.0$\pm$ 186m1 and 14.7$\pm$6.8g) than the con- trol group (102.1 $\pm$483.5ml and 39.7$\pm$ 16.4g) (P<0.01). The amonut of packed red cell needed decreased in the aprotinin group: 197.7$\pm$56.3ml vers s 651.2: 147.5ml (P<0.01). Hemoglobin concentration, platelet counts and fibrinogen checked at fixed times perioperatively did not differ between the two groups. Platelet aggregation was induced by ADP, collagen, epinephrine and ristocetin before and after CPB. Maximum platelet aggregation was significantly reduced after CPB in control group (ranging from -31 % to -58% relative to prebypass values). Significant prolongation of activated clotting time(ACT) after 5 minute and 30 minute of hypothermic CPB were observed: 955.9 $\pm$35.1 and 967.5$\pm$32.7sec versus 743.8 $\pm$ 52.1 and 731.2: 54.6sec (P<0.01). There was no complication associated with aprotinin infusion. These results demonstrate that low-dose aprotinin significantly reduces blood loss and blood requirment and provides improved postoperative hemostasis which might be related to protection of platelet aggregation capacity.
