결핵 입원환자의 치료결과에 영향을 미치는 환자 특성과 의료기관 특성 (The Effects of Patient and Hospital characteristics on Hospital Care Outcome of the Patients with Tuberculosis)

  • In spite of effective curative therapy, morbidity and mortality remain high for hospitalized patients with tuberculosis(TB) in Korea. The purpose of this study was to identify patient and hospital characteristics associated with hospital care outcome. Using annual patient survey data produced by Korea Institute for Health and Social Affair, we identified 8,562 hospital discharge with primary diagnosis of TB. Logistic regression analyses were performed on a model that included age, gender, residence area, insurance status, hospital admission source, length of stay, hospital ownership and class of hospital as the explanatory variables and outcome of treatments as the dependent variable. The results show that negative outcome was associated with the patients older than 65 years, medical aid beneficiary, admission through emergency department, and the patients admitted to public owned hospitals. On the other hand, the patients who were admitted to teaching hospitals were associated with positive outcome. To improve hospital treatment outcome of TB patients, more vigorous strategies should be implemented targeting the older and poor population in regard to social support as well as the clinical management and prevention.

야뇨의 치료 평가 기준에 관한 연구 (Outcome criteria in nocturnal enuresis treatment)

  • Objectives : Nocturnal enuresis is one of common disorders in children. However, there are not a lot of researches going on about this disease, and also standardized criteria for analyzing were insufficient in Korea. Therefore, clinical researches were not in the confidence level. Methods : Korean journals which were published in 1990 to 2006, and online journals about nocturnal enuresis were used for analyzing based on outcome criteria. Results : The International Children's Continence Society and the World Health Organization have published outcome criteria about nocturnal enuresis, but different, often idiosyncratic, outcome criteria and/or definitions have been adopted in published research on treatment for nocturnal enuresis in recent years. But a new set of criteria suggested by Butler, Robinson, and et. al. referred to as a "dryness scale", which focuses on the percentage of dry nights accomplished at a point in time, will be an alternative proposal. These criteria will be of help to make standardized and proper outcome criteria in oriental medical studies. Conclusions : Agreed standardized outcome criteria in nocturnal enuresis treatment is needed.

보건의료체계 재원조달 유형별 건강결과 결정요인 -OECD 국가를 중심으로- (The Determinants of Health Outcome between Two Health Care Financing Systems)

  • The purpose of a national health care system is to improve health care outcome among population. The objective of the study was to explore the determinants of health outcome in the 24 OECD countries between two health care financing systems. The study employed the pooled time series and cross-sectional analysis with tax-funded and social insurance-funded countries over the period of 1980 to 1999 using OECD Health Data 2002. The study revealed that health expenditure per capita, physicians per 1,000 of the population and calorie intake were positively significantly associated, smoking rate was negatively associated with health outcome while controlling all variables in the tax-funded countries. But in the insurance-funded countries, health expenditure per capita and the number of physicians were not statistically significant factors explaining health outcome. Only the calorie intake was positively associated with, and smoking rate, alcohol consumption per capita, and total nitrogen oxide emission per capita were negatively significantly associated with health outcome. In conclusion, healthy life style factors were much more important to improve health outcome in the both systems.

간호교육 프로그램학습성과 평가를 위한 Course Embedded Assessment(CEA) 적용방안 기초연구 (A Study on application Course Embedded Assessment(CEA) for Program Outcome Assessment in Nursing Education)

  • 본 연구는 간호교육 인증평가의 핵심인 프로그램학습성과 평가에 Course embedded Assessment(CEA)를 적용하기 위한 기초연구로 시행되었다. 선행연구과 미국대학 평가 사무국의 자료를 통해 CEA의 개념, 장 단점, 단계, 적용 시 주의사항 등을 고찰하였으며, 이를 근거로 간호교육 프로그램학습성과 평가체계에 적합한 CEA모형과 세부지침을 제시하였다. CEA 과정은 프로그램학습성과 설정, 학습성과의 가중치 산정, 교육과정에 연계, 루브릭 개발, 평가, 결과 및 성과달성도 분석, 교육과정개선을 위한 피드백으로 구성하였으며 선행연구에서 제시된 단점과 주의사항을 보완하여 시행 지침을 작성하였다. 본 연구를 토대로 간호교육 프로그램학습성과평가 체계를 구축한다면 학생의 성장과 성취에 대한 일관성 있는 평가와 지속적인 교육과정 개선으로 간호교육의 질적 향상에 기여할 것으로 사료된다.

성과중심교육과정 개발사례: 가톨릭대학교 의과대학 (Development of an Outcome-Based Medical Curriculum: A Case Report from The Catholic University of Korea School of Medicine)

  • The recent medical education paradigm shift from teacher-centered to student-centered education, has led to a concentration on students' performance and competency. This means that a physician should be able to provide adequate health care in any real medical treatment situation. In order to reflect such a paradigm shift, The Catholic University of Korea School of Medicine launched a new curriculum in 2009 that emphasizes students' performance and competency-based education, known as "outcome-based education." In outcome-based education, the educational process is determined by the desired outcome, signifying the detailed competency that a graduating student should have. Thus, in outcome-based education, we should first determine the competency that results from adequate training and education, followed by specific teaching and learning strategies, methods, and assessment. This paper reviews how The Catholic University School of Medicine developed its new curriculum according to the development steps of outcome-based education.

  • 연구개발의 성과를 체계적으로 가시화하는 데는 많은 어려움이 존재한다. 이러한 적용상의 문제점은 연구개발의 input, output, outcome의 개념을 도입하여 Input-Output-Outcome Roadmap 작성으로 해소될 수 있다. 본 연구에서는 지하수 오염저감기술 연구사업에 대한 적용을 통하여 사업의 경쟁력 및 사업성을 확보를 위한 연구개발 성과의 체계적 지표를 제시하였다.

  • There have many studies seeking for an effective IS planning approach. These studies mainly considered organizational characteristics, external factors, and IS planning process as key factors influencing ISP outcome. ISP output, however, was less focused, which is believed to have a more direct relationship with ISP outcome. In this study we attempt to study empirically how architectural quality of ISP output impacts on ISP outcome.

간호학생의 성과기반교육 달성도에 미치는 영향 (The Influences of Accomplishment of Outcome-based Education in Nursing Students)

  • 본 연구는 간호학생의 성과기반교육 달성도에 영향 미치는 요인을 확인하기 위해 364명의 간호학생을 대상으로 의사소통능력, 비판적 사고성향, 셀프리더십, 연구능력, 성과기반교육 달성도로 구성된 자가보고 질문지로 자료를 수집하였다. 간호학생의 성과기반교육 달성도는 학년, 성과기반교육 이해도 및 만족도, 임상실습 경험여부에 따라 통계적으로 유의한 차이가 있었다. 성과기반교육 달성도는 의사소통능력, 비판적 사고성향, 셀프리더십, 연구능력과 양의 상관관계로 나타났다. 간호학생의 성과기반교육 달성도에 영향을 미치는 요인은 성과기반교육 만족도(${\beta}=0.33$, p<0.001), 셀프리더십(${\beta}=0.21$, p<0.001), 연구능력(${\beta}=0.18$, p<0.001), 비판적 사고성향(${\beta}=0.17$, p=0.003), 의사소통능력(${\beta}=0.14$, p=0.006), 임상실습 경험(${\beta}=0.10$, p=0.023)의 순으로 나타났으며, 성과기반교육 달성도에 대한 이들 변수의 총 설명력은 38.1%였다. 성과기반교육의 달성도를 높이기 위해 셀프리더십, 연구능력, 비판적 사고성향 및 의사소통능력을 향상시킬 수 있는 교과 및 비교과 활동 전략을 개발하고 운영해야 할 것이다.

뇌졸중으로 인한 운동 관련 후유증의 한의 치료 연구에서 뇌졸중 시기 및 평가 척도에 대한 국내 문헌 고찰 (A Review of the Stroke Stage and Outcome Measures in Stroke Motor Sequelae Clinical Studies in Korea)

  • Objectives: The aims of this study were to evaluate the fidelity of stroke stage reporting, the timeliness of the outcome measures, and the use of the core outcome set. Methods: We searched the literature using 6 domestic databases. We selected studies that used Korean medicine interventions and targeted stroke patients with motor sequelae. We examined whether the included studies reported the stroke stage and whether they used the outcome measures in the appropriate period based on the recommendations of the "Stroke Evidence Database to Guide Effectiveness". We also confirmed the use of the essential assessment tools suggested by the core outcome set. Results: Overall, 77 studies were finally selected, with 16 (21%), 55 (71%), and 6 (8%) published on the acute, subacute, and chronic phases, respectively. Only 11 of the studies directly mentioned the stroke stage. The most commonly used assessments were the National Institutes of Health Stroke Scale, Modified Barthel Index, and Manual Muscle Testing. Only 5 studies failed to apply the stage-related outcome measures at the recommended period. The outcome variables used inadequately were the National Institutes of Health Stroke Scale, Functional Ambulation Categories, 36-Item Short Form Health Survey, and Mini-Mental State Examination. Among the core outcome set items, some studies used liver and renal function tests, but no herbal medicine safety reporting was conducted. Conclusions: In future studies, we propose to ensure accurate reporting of the stroke stage with reliable outcome measures to deliver better clinical and research outcomes. Furthermore, in future clinical studies on stroke, a standard protocol that reflects the core outcome set should be developed.

성인학습자의 평생교육의식, 참여동기, 학습성과에 관한 구조적 분석 (A Structural Analysis of Adult Learners' Lifelong Education Consciousness, Participation Motivation, Learning Outcome)

  • 이 연구는 성인학습자의 평생교육의식, 참여동기, 학습성과에 관한 구조적 관계를 알아보기 위한 목적으로 수행되었다. 연구대상은 대전광역시 5개구에서 운영 중인 평생교육기관에서 평생학습 프로그램에 참여중인 성인학습자 319명을 대상으로 자료를 수집하였다 연구결과는 다음과 같다. 첫째, 성인학습자의 평생교육의식은 학습성과에 유의한 영향을 미치는 것으로 나타났다. 둘째, 성인학습자의 평생교육의식은 참여동기에 유의한 영향을 미치는 것으로 나타났다. 셋째, 참여동기는 학습성과에 유의한 영향을 미치는 것으로 나타났다. 넷째, 성인학습자의 평생교육의식과 학습성과는 참여동기를 매개로 유의한 영향을 미치는 것으로 나타났다. 학습성과에 직접적인 영향을 미치는 관련변인으로 평생교육의식, 참여동기가 영향력을 갖는 것으로 검증되었다. 이러한 결과는 평생교육의식 전환이 이루어져, 평생학습에 대한 참여동기가 부여되고, 학습성과 향상으로 이어져, 궁극적으로 평생학습 참여기회가 확대될 수 있음을 시사해준다.
