Ankle osteoarthritis is a debilitating condition that causes severe pain associated with functional impairment and decreased activity. Ankle osteoarthritis, unlike that of the knee or hip joint, is rare in primary arthritis. Most cases are traumatic arthritis that occur after ankle sprain or fractures or chronic ankle instability. Although ankle fusion has been regarded as the standard treatment of ankle osteoarthritis in the past, total ankle arthroplasty (TAA) is increasing due to the development of the implant design and surgical techniques. TAA is biomechanically superior to ankle fusion by preserving the movement of the ankle joint. In particular, it is functionally superior to ankle fusion because it enables normal joint motion during gait. In addition, there is an advantage of preserving the movement of the hindfoot and reducing the abnormal stress applied to the adjacent joints after ankle fusion to prevent the occurrence of long-term adjacent joint arthritis. Although the short-term and mid-term results of TAA have been reported to be excellent, long-term follow-up has a relatively low survival rate and high complication rate compared to total knee or hip arthroplasty. Therefore, continuous and further research is needed.
Sparassis crispa (Wulf.) is an edible/medicinal mushroom and has been reported to biological activities such as antitumor, anti-angiogenesis, antioxidant and wound healing. However, there have not been many researches on osteoarthritis of S. crispa. The aim of this study was to investigate the effects of S. crispa extract on rats with osteoarthritis induced by MIA. Osteoarthritis is a gradually developmental disease that early stage, causes joint stiffness and complains of joint pain. In addition, it gives rise to edema and hypo-function. The results of this study, S. crispa extract effectively inhibited ROS production, increased the production of antioxidant protein SOD and catalase in knee joint cartilage tissue. In addition, S. crispa extract inhibited the expression of pro-inflammatory cytokines and enzymes such as NOX4 and $P47^{phox}$, which are involved in the expression of COX-2, iNOS and the production of ROS. Also, S. crispa extract inhibited the destruction of synovial tissue, cartilage tissue and proteoglycans in articular cartilage in rats.
Purpose: The aim of this study was to explore the effects of dynamic balance training on pain, physical function, and dynamic balance in individuals with knee osteoarthritis. Methods: Fourteen patients with knee osteoarthritis participated in this study. The patients were randomly assigned to two groups: an experimental group (n=7) or a control group (n=7). All the patients took part in a lower extremity strength program for 30 min. In addition, the experimental group participated in a 30-min dynamic balance program. Both groups performed the program five times a week for 3 weeks. Outcomes, including the numeric rating scale (NRS), Western Ontario and MacMaster Universities Arthritis Index (WOMAC), and Community Balance and Mobility Scale (CB&M), were measured at baseline and after 3 weeks. Results: Both groups showed pre-to-post intervention improvements on all outcome measures (p<0.05). The experimental group showed a significant improvement in WOMAC (p = 0.00; Z = -2.82) and CB&M (p = 0.03; Z = -2.20) scores after the intervention as compared with those of the control group. Conclusion: The results revealed that dynamic balance training improved physical function, as well as balance ability, in patients with knee osteoarthritis as compared with that of a control group with no balance training.
Purpose: The purpose of this study was to examine the effects of Tai-Chi self-help program for osteoarthritis patients. Method: This study was performed from 1th August. to 30th Dec. 2006. 27 osteoarthritis patients were participated in the Tai-Chi self-help program. Outcome measures were pain, flexibility and balance, self-efficacy. Data were analysed by SPSS WIN 12.0. Result: There were statistically significant in balance and knee flexibility. but no significant differences were found in pain and self-efficacy. Conclusion: In conclusion, Tai-Chi self-help program was proved to be an effective nursing intervention to increase the balance and flexibility. Also 12 forms of Tai-Chi self-help program has been found safely applicable to the patients with osteoarthritis for 8 weeks. A rigorous research design should be further considered to identify the long-term benefit for this population.

Osteoarthritis is thought to be induced by the ageing-related loss of homeostatic balance between degeneration and repair mechanism around cartilage tissue in which inflammatory mediators such as reactive oxygen species, cytokines and prostaglandins are prone to overproduction under undesirable physiological conditions. Phlorotannins are unique polyphenolic compounds bearing dibenzo-1,4-dioxin skeleton which are not found in terrestrial plants but found only in some brown algal species such as Ecklonia and Eisenia families. Phlorotanninrich extracts of Ecklonia cava including LAD103 showed significant antioxidant activities such as DPPH radical scavenging, ferric ion reduction, peroxynitrite scavenging, and inhibition of LDL oxidation, indicating their possible antioxidative interference both in onset and downstream consequences of osteoarthritis. LAD103 also showed significant down regulation of $PGE_2$ generation in LPS-treated RAW 246.7 cells, and significant inhibition of human recombinant interleukin-$1{\alpha}$-induced proteoglycan degradation, indicating its beneficial involvement in pathophysiological consequences of osteoarthritis, the mechanism of which needs further investigation. Since LAD103 showed strong therapeutic potentials in arthritic treatment through several in vitro experiments, it is highly encouraged to perform further mechanistic and efficacy studies.

Purpose: The purpose of this study was to identify the effects of 12 weeks of Tai Chi exercise on physical function, stress and depression in middle aged women with osteoarthritis. Method: The research design was a nonequivalent control group pretest-posttest design. Forty two participants were assigned to either the experimental group (21) or control group (21). Frequency, percentages, mean, standard deviations, $X^2$-test, Wilcoxon rank sum test and repeated measured ANOVA with SPSS 12.0/PC program were used to analyzed the collected data. Results: For physical function the physical fitness of the experimental group improved significantly more than the control group: Leg strength, balance, flexibility and grip strength. Difficulty of performing activity had a tendency of decrease. Stress showed a tendency of as time passed but the decrease was not significant. Depression decreased and the change was significant. Conclusion: The Tai Chi exercise was effective in improving physical function and reducing the stress and depression of middle aged women with osteoarthritis. Therefore the Tai Chi exercise could be used as a nursing intervention for osteoarthritis in middle aged women.
Objectives : The purpose of this study is to report the effect of acupotomy for patients with Osteoarthritis on Knee using "Five-Finger placed to marking position". Methods : We treated 12 patients who have Osteoarthritis on Knee with acupotomy. Results : It was effective to 10 patients among the 12 patients. Conclusions : This study demonstrate that acupotomy using "Five-Finger placed to marking position" is useful effect on Osteoarthritis on Knee. As though we had not wide experience in this treatment, more research is needed.
The purpose of this study was to describe and compare the temporo-spatial gait characteristics of healthy elderly people with those osteoarthritis patients. 100 patients reported knee osteoarthritis, diagnosed at the hospital or clinic located in Daegu and Kyungbuk province and 100 normal elderly subjects were participated in this study. Temporal and spatial parameters of gait were analysed for using the computerized GAITRite system. The system integrates specific components of locomotions to provide a single, numerical representation of gait, the Functional Ambulation Performance score. Differences in gait characteristics between the two groups were examined using a correlated t-test and Pearson Correlation(p<.05). Significant differences were observed between the groups for temporal parameters(step time, double support time, stance phase, mean velocity) and spatial parameters(step length, step/extremity ratio)(p<.05). Also there was difference in the functional ambulation performance score between normal elderly subjects and knee osteoarthritis patients(p<.05). Consequently, it may help detect the abnormal gait pattern indicated the main problem in degenerative knee osteoarthritis patients as well as provide data analysing the pathokinesiologic components by comparing normal elderly.

Purpose : This study was to explore the effects of knee joint taping exercise on muscle strength, bone mineral density, pain and IGF-1 in blood of elderly women with knee Osteoarthritis. Methods : Thirty elderly women with knee osteoarthritis were divided into three groups: the taping with exercise group (n=10), the regular exercise group (n=10) and control group (n=10). Participants' muscle strength, bone mineral density, pain and IGF-1 in blood were measured three times : before exercise, after 6 weeks, and after 12 weeks. Results : Participants in both exercise (taping & non-taping) groups showed improvement in muscle strength, bone mineral density, pain and IGF-1 in blood after 6 and 12 weeks compared to before exercise. In particular, the taping exercise group had a greater effect on muscle strength than the regular exercise group. Conclusion : Both exercise programs considerably improved muscle strength, bone mineral density, reduced pain and IGF-1 in blood in elderly women with knee Osteoarthritis. The knee joint taping exercise is perhaps a better exercise to improve muscle strength than the regular exercise in treating elderly women with knee Osteoarthritis

Spring-like leg models have been employed to explain various dynamic characteristics in human walking. However, this leg stiffness model has limitations to represent complex motion of actual human gait, especially the behaviors of each lower limb joint. The purpose of this research was to determine changes of total leg stiffness and lower limb joint stiffness with gait speed in knee osteoarthritis. Joint stiffness defined as the ratio of the joint torque change to the angular displacement change. Eight subjects with knee osteoarthritis participated to this study. The subject walked on a 12 m long and 1 m wide walkway with three sets of four different randomly ordered gait speeds, ranging from their self-selected speed to maximum speed. Kinetic and kinematic data were measured using three force plates and an optical marker system, respectively. Joint torques of lower limb joints calculated by a multi-segment inverse dynamics model. Total leg and each lower limb joint had constant stiffness during single support phase. The leg and hip joint stiffness increased with gait speed. The correlation between knee joint angles and torques had significant changed by the degree of severity of knee osteoarthritis.
