Diffuse Pigmented Villonodular Synovitis of Knee Presenting as a Popliteal Cyst - One case report - (슬와 낭종을 형성한 슬관절의 미만성 색소 융모 결절성 활액막염 - 1례 보고 -)

  • Pigmented villonodular synovitis most commonly occurs in the knee joint and rarely extends into the popliteal space, which not only has mimicked Baker's cyst but also has been misdiagnosed as malignant tumor. We report a case of a diffuse pigmented villonodular synovitis of knee joint which is extended into the popliteal space as like a popliteal cyst. We treated this case by arthroscopic total synovectomy and excision of cyst. There was no recurrence during more than one year after operation.

  • The authors describe one case of a 25-year-old male with nodular tenosynovitis in left knee joint who did not have a history of joint trauma. He had joint pain and restricted flexion of the left knee at 100 degrees. After arthroscopic excision of mass, symptom was relieved completely and the patient had gained full range of motion of the left knee. No recurrence of symptom and loss of motion of the left knee were noticed during follow up period for 12 months.

  • Purpose: The purpose of this study is to report four operative cases of recurrent peroneal tendon dislocation being effectively treated by superior peroneal retinaculum reconstructive operation and rerouting of calcaneofibular ligament. Materials and Methods: Four male adults with recurrent peroneal tendon dislocation including one of patient with redislocation after the treatment of plication of inferior peroneal retinaculum were followed up after being treated with superior peroneal retinaculum reconstructive operation using Achilles tendon and rerouting of calcaneofibular ligament. Results: All patients followed up for over one year revealed no pain and no redislocation. One patient who experienced redislocation after the treatment of plication of inferior peroneal retinaculum had no other complication after the treatment of superior peroneal retinaculum reconstructive operation using Achilles tendon. Conclusion: The reconstructive operation presumed to be effective for recurrent peroneal tendon dislocation.

Treatment for Brachymetatarsia by Callotasis (가골 신연술을 이용한 단중족증의 치료)

  • Purpose: To evaluate the effectiveness of callotasis using the external fixator for the treatment of brachymetatarsia. Materials and Methods: Eleven patients(15 cases) who underwent callotasis were available. Follow-up averaged 23 months(15-38 months). Both the patients' postoperative satisfaction and the postoperative radiographic results were retrospectively evaluated. Results: The duration from applying the external fixator to remove averaged 15 weeks (8-21 weeks). We did osteotomy again in two cases because of early consolidation at the osteotomy site during distraction period. All cases were evaluated mild claw toe deformity and motional pain in metatarsophalangeal joint, but all patients satisfied the postoperative results except one complained motional pain and stiffness in metatarsophalangeal joint. The metatarsal shortening averaged 13mm(9-18mm) before operation. The amount of distraction for shortened metatarsals averaged 15mm(5-22mm). We found the 6 cases of narrowing of the metatarsophalangeal joint, 2 cases of subluxation of the metatarsophalangeal joint. Conclusion: We think that callotasis with external fixator is good operative method for the brachymetatarsia.

  • Little literature exists about the risk of atypical femoral fracture in patients received zoledronic acid for prevention of skeletal metastasis. We report an atypical subtrochanteric femoral fracture in a patient with multiple myeloma received zoledronic acid. The patient was treated by closed reduction and internal fixation with cephalomedullary nailing.

  • Background: In the present study, we examined gender roles, job satisfaction, and the relationship between the two among orthopedic manual physical therapists (OMT). Methods: The subject of this study was 208 physical therapists working in the general hospital and rehabilitation in Daejeon. In order to examine the nature of the subject, we calculated the ratio of second finger length for fourth finger of predominant hand and completed the questionnaire to determine the general characteristics and job satisfaction. Results: The OMT and NOMT (non OMT) digit ratio results were not significantly different (p>.05). Digit ratio based on gender was also not significantly different between groups (p>.05). Among the job satisfaction survey items, OMT was significantly higher than NOMT in terms of Pride, Reflection of opinions, aptitude match, and field satisfaction (p<.05); however, there were no significant differences between groups for the remaining items. Digit ratio was not significantly related to any of the job satisfaction items (p>.05). Conclusions: We determined that gender roles had a limited influence on selecting and continuing with orthopedic physical therapy.

  • Benign soft tissue mesenchymoma is a tumor composed of at least 2 mesenchymal elements, not ordinarily found together in a tumor. Very few cases are reported worldwide and even more, preponderance for cartilage in benign mesenchymoma is extremely rare. Up to our knowledge, total of 11 cases are reported to have cartilaginous rich benign soft tissue mesenchymoma. We are reporting a case of three-year-old male who developed benign soft tissue mesenchymoma rich in cartilaginous materials that occurred on the lower aspect of the back.

  • Pulmonary embolism appears to be a very rare complication of arthroscopic knee surgery. Most cases of pulmonary embolism have been clinically silent in the literature. We describe a case of symptomatic pulmonary embolism after arthroscopic partial menisectomy in 40-year-old male patient.

  • Purpose: The purpose of this study was to evaluate the effectiveness of limb reconstruction and functional recovery using vascularized fibular graft in the treatment of extensive bone defect of long bone caused by various diseases. Materials and Methods: From september 1995 to March 2005, 21 patients with segmental bone defects were managed with vascularized fibular graft: 13 males and 8 females, aged 39 years on average (range, $8{\sim}65\;years$). The reconstructed site was the humerus in 9 patients, the femur in 5, the tibia in 4 and the forearm bone in 3. The length of bone defect ranged from $8{\sim}17\;cm$. Results: Twenty grafts were successful. The mean period to obtain radiographic bone union was 5.7 months on average. Conclusion: Fibular grafts allow the use of a segment of diaphyseal bone and of sufficient length to reconstruct most skeletal defects of the long bone. The vascularized fibular graft is indicated in patients with intractable nonunions where conventional bone grafting has failed or large bone defects.

  • Purpose: To evaluate the usefulness of arthroscopic repair that was related with full thickness rotator cuff tear and assess clinical result. Materials and Methods: Twenty-one cases of arthroscopically repaired full thickness tear of rotator cuffs were studied. Between October 1998 to July 2002 we have analysed 21 repairs of FTRCT the average age 54(42∼74) years old, mean follow-up was 24(12∼41) months We analyzed the results statistically by paired t-test. Results: Postoperative VAS of pain improved average 7.2 to 1.9, UCLA score improved 13.9 to 31.9, ADL improved 11.5 to 25.5 respectively(all, p<0.001). Eighty-seventh % of the patients showed excellent St good results at the final follow-up. The satisfied rate was 90.5%(19cases). Conclusions: Arthroscopic repair in full thickness rotator cuff tear is effective surgical methods.
