구강 전암병소 및 구강암 예방 (Oral precancerous lesion and oral cancer prevention)

  • Oral precancerous lesion is a morphologically altered tissue in which oral cancer is more likely to occur than is apparently normal counterpart. As dentists always do oral examination and dental treatment, with fundamental knowledge and attention of this lesion, it is relatively easy to find one. If followed by proper treatment and management, it is possible to minimize its oral cancer progression, or at least delay it. Even if it were to progress to oral cancer, very early detection is possible. However, no specific biomarkers are present at the moment that could reveal oral precnacerous lesion that is high risk of oral cancer progression. Since early detection of oral cancer followed by treatment could show good prognosis with just a simple ablative surgery. Dentists should also instruct people to avoid risk factor related oral cancer progression and take natural compound having anticancer effect. Hereby, As a primary care givers, dentists play an important role in prevention of oral cancer.

태음인(太陰人) 청심연자탕(淸心蓮子湯)이 SD rat 의 항산화(抗酸化) 및 면역조절효과(免疫調節效果)에 대한 실험적 연구 (The Anti-oxidative and Immune-regulatory Effect of Chungsimyeonja-tang in Aged Rat)

  • 1. Objectives The purpose of this study is to investigate the anti-oxidative and immune regulatory effect of oral administration of Chungsimyeonja-tang decoction in aged rats. 2. Methods The rats in the normal group were 14 weeks old, and those in the control and Chungsimyeonja-tang groups were 40 weeks old. The rats in Chungsimyeonja-tang group were orally treated with chungsimyeonjatang decoction daily for 2 months. To observe the anti-oxidative effect and immune regulatory effect of Chungsimyeonja-tang, we performed blood chemistry analysis, histological analysis. 3. Results and Conclusions 1) The number of WBC in blood was significantly increased and the number of PLT in blood was significantly decreased by oral administration of Chungsimyeonja-tang decoction. 2) The levels of albumin, LDL cholesterol, ALP and GOT in serum were significantly decreased, and the level of glucose in serum was significantly increased by oral administration of Chungsimyeonja-tang decoction. 3) The concentrations of glutathione and catalase were significantly increased and the concentrations of NO and MDA were significantly decreased in the liver of early aged rats by oral administration of Chungsimyeonja-tang decoction. 4) The concentrations of SOD and catalase were significantly increased and the concentration of MDA was significantly decreased in the lung of aged rats by oral administration of Chungsimyeonja-tang decoction. 5) The levels of IL-4 and IL-6 were significantly decreased while the levels of IFN-${\gamma}$ were significantly increased in the spleen of early aged rats by oral administration of Chungsimyeonja-tang decoction.

노인의 주관적 건강상태와 건강관련 삶의 질의 관련성 : 구강건강수준의 조절 효과 중심으로 (Relationship between perceived health status and health-related quality of life in the elderly: A focus on moderating effects of oral health)

  • Objectives: This study aimed to validate the moderating effect of oral health on the relationship between perceived health status and health-related quality of life in the elderly and to use this information as primary data to suggest oral health policies for the aged society. Methods: This study included 3,707 subjects aged over 65 years who answered all the variables used in the study model and completed the health questionnaire and screening survey based on the sixth Korea National Health and Nutrition Examination Survey. Baron & Kenny's linear regression analysis using SPSS 22.0 and SPSS Macro Version 3.1 programs was performed to confirm the moderating effect of the number of remaining natural teeth, chewing difficulty, and cardiovascular disease on the relationship between perceived health status and healthrelated quality of life in the elderly. Results: The number of remaining natural teeth, chewing difficulties, and cardiovascular diseases affected the perceived health status of the elderly. As the number of remaining natural teeth increased, the effect of perceived health status on the quality of life in the elderly was buffered. The effect of perceived health status on the quality of life increased with chewing difficulties and the number of cardiovascular diseases. In particular, it was confirmed that chewing difficulties, rather than the number of cardiovascular diseases. had a greater effect on the quality of life in the elderly. Conclusions: Oral health policies and projects are required to ensure oral rehabilitation with dentures and implants and restore chewing function to improve the quality of life of the elderly in Korea.

피톤치드 처리 후 구강 내 잔존 S. thermophilus의 P. gingivalis에 대한 효과 (The Effect of S. thermophilus Isolated from Saliva Treated with Phytoncide on P. gingivalis)

  • 치주질환과 구취를 유발시키는 중요한 원인균인 P. gingivalis에 대한 피톤치드의 항균효과와 항균작용은 이미 연구되어 있으나, 정상인의 구강상주균에 대한 연구는 아직 희귀한 편이다. 이에 본 연구에서는 건강한 정상인의 타액에 편백 피톤치드를 첨가하였을 때 사멸되지 않고 생존하는 타액세균을 분리하여 구강 유해균과 함께 배양한 후 구강 유해균에 대한 생존 타액세균의 억제효과를 파악함으로써 향후 프로바이오틱으로 작용할 수 있는 구강상주균의 균종을 동정하여 다음과 같은 결론을 얻었다. 1. 정상인의 전타액에 1% 피톤치드를 적용하였을 때 잔존 생균수는 감소하는 경향을 보였다. 2. 피톤치드 적용 후 생존한 주 세균종은 S. thermophilus (53%)로 나타났다. 3. 피톤치드 적용 후 생존한 균을 P. gingivalis A7A1-28과 P. gingivalis W83에 교차배양한 결과 생존균의 대부분(72.5%) 이 P. gingivalis A7A1-28과 P. gingivalis W83의 성장을 억제하였다. 4. 생존 S. thermophilus의 85.8%, S. sanguinis는 75.8%가 P. gingivalis 를 억제하는 것으로 나타났다. 이상의 결과로 미루어, P. gingivalis 등 구강 내 유해균을 직접 억제할 수 있는 것으로 알려진 피톤치드로 처리할 경우 피톤치드에 생존하는 구강상주균이 P. gingivalis에 대해 부가적으로 억제작용을 할 수 있기 때문에 피톤치드의 사용은 치주질환을 예방하고, 그 결과 치주질환 및 구취환자의 구강 환경을 크게 개선할 수 있을 것으로 생각된다.

Retinoic acid가 사람 정상 구강각화세포의 방사선감수성에 미치는 영향에 관한 연구 (Effect of retinoic acid on the radiosensitivity of normal human oral keratinocyte)

  • Purpose : To evaluate the effect of all-trans-retinoic acid (ATRA) on the radiosensitivity of normal human oral keratinocyte (NHOK). Materials and methods: Relative cell survival fraction including SF2 (survival fraction at 2 Gy) was calculated on the basis of colony formation assay. Data were fitted to the linear-quadratic model to establish the survival curve and calculate α and β values. Using flow cytometry at 1, 2, 3, 4, and 5 days after exposure to 2 and 10 Gy irradiation, cell cycle arrest and apoptosis were analysed. To understand the molecular mechanism of the radiosensitization of ATRA on NHOK, proteins related with apoptosis and cell cycle arrest were investigated by Western blot analysis. Results: Treatment with ATRA resulted in a significant decrease of SF2 value for NHOK from 0.63 to 0.27, and increased α and β value, indicating that ATRA increased radiosensitivity of NHOK. ATRA increased LDH significantly, but increasing irradiation dose decreased LDH, suggesting that the radiosensitizing effect of ATRA is not directly related with increasing cell necrosis by ATRA. ATRA did not induce appotosis but increased G2 arrest after 10 Gy irradiation, implying that the increased radiosensitivity of NHOK may be due to a decrease in mitosis casued by increasing G2 arrest. ATRA inhibited the reduction of p53 at 3 days after l0Gy irradiation and increased p21 at 1 day after 10 Gy irradiation. Further study is required to determine the precise relationship between this effect and the radiosensitizing effect of A TRA. Conclusion: These results suggested that ATRA increase radiosensitivity by inhibiting mitosis caused by increasing G2 arrest.

울산지역 사업장 근로자의 구강보건지식과 행태에 관한 연구 (The study on the Oral Health Knowledge and Behavior of Industrial Workers at Ulsan Province)

  • 본 연구는 울산 현대 중공업 근로자 244명을 대상으로 2008년 5월 20일부터 6월 30일까지 구강건강 수준 및 인식도와 행태를 분석하는 설문조사를 실시하였으며, SPSS 12.0 프로그램을 이용하여 분석하였다. 1. 근로자의 예방목적 치과방문은 20대 보다 50대가 높았고(p < .001), 구강위생용품 사용은 중학교 졸업군보다 대학교 졸업 군의 경우가 통계적으로 유의하게 높게 나타났다(p < .05). 2. 구강보건지식은 중학교 졸업 군보다 고등학교 졸업군의 경우가 통계적으로 유의하게 높았다(p < .05). 1일 잇솔질 횟수는 3-5회가 높았고, 성별로는 여자가 남자보다 높게 나타났고(p < .01), 교육정도별로는 중학교 졸업보다 대학교 졸업이 높게 나타났다(p < .001). 3. 구강건강상태에서 연령별은 50대 군보다 20대 30대군의 경우가 통계적으로 유의하게 높게 나타났다(p < .001). 교육정도별은 중학교 졸업 군보다 고등학교, 전문대학, 대학 졸업 군의 경우가 통계적으로 유의하게 높았다(p < .001). 구강건강 감수성에 대한 자기평가는 중학교 졸업 군보다 대학교 졸업 군의 경우가 통계적으로 유의하게 높게 나타났다(p < .05). 4. 치과방문, 발치 수, 잇솔질 횟수 간의 상관관계는 치과방문과 구강보건지식 간에 가장 큰 유의한 양의 관계를 보였다(r = .233, p = .001). 구강보건지식과 잇솔질 횟수 간에도 통계적으로 유의한 수준에서 양의 상관관계로 나타났다(r = .161, p = .05). 또한 잇솔질 횟수와 발치 간의 관계는 음의 관계를 보였다(r = -.145, p = .05).

  • We selected 100 patients necessitating the surgical or non-surgical treatment among the patients who visited our hospital. We administered the newly developed non-steroid, PROFENID capsule (25mg per capsule), to the selected patients Via per OS and obtained the following results. 1) Cases of 56% at about 15 minutes after the administration of Profenid capsules, 35% at about 30 minutes after the administration of Profenid capsules, 35% at about 30 minutes and 6% at 60 minutes or more presented the initial effects of that drug respectively. 2) In the patients with the comparatively heavy swelling, the appearances of the effect of that drug were relatively delayed. 3) Sexual difference was not seen in that drug effect. 4) Untoward effects of that drug seemed to be not found in this case study.

Postantibiotic Effects and Postantibiotic Sub-MIC Effects of Antibiotics on Fusobacterium nucleatum and Porphyromonas gingivalis

  • Postantibiotic effects (PAE) refer to suppression of the bacterial growth following limited periods of exposure to an antibiotic and subsequent to the removal of the antibiotic agent. Fusobacterium nucleatum and Porphyromonas gingivalis are Gram-negative anaerobic bacteria associated with several periodontal diseases. In this study, postantibiotic effects (PAE), postantibiotic sub-MIC effect (PA SME) and sub-MIC effect (SME) of antibiotics on F. nucleatum ATCC 25586 and P. gingivalis W50 were investigated. The PAE was induced by 10X the MIC of antibiotic and antibiotic was eliminated by washing. The PA SMEs were studied by addition of 0.1, 0.2 and 0.3X MICs during the postantibiotic phase of the bacteria, and the SMEs were studied by exposition of the bacteria to antibiotic at the sub-MICs only. Amoxicillin, doxycycline and tetracycline induced PAE for F. nucleatum ATCC 25586 and P. gingivalis W50. But metronidazole and penicillin induced PAE for only F. nucleatum ATCC 25586. Metronidazole and doxycycline induced PA SME and SME for both species of anaerobic bacteria used in this study. The PA SME values for both strains were substantially longer than the SME values. The present study showed the existence of PAE, PA SME and SME for various antibiotics against F. nucleatum ATCC 25586 and P. gingivalis W50.

Cytotoxic Effect of Syringic Acid on Human Oral Epithelioid Carcinoma Cells

  • This study was undertaken to clerify the cytotoxic effect of syringic acid by colorimetric assay on human cancer cells. For the evaluation of cytotoxicity of syringic acid, the cell viability and cell adhesion activity of syringic acid on cancer cells, human oral epithelioid carcinoma cells were determined using by colorimetric assays such as MTT (3-[4,5­dimethylthiazol-2-yl]-2,5-diphenyltetrazolium bromide) assay and XTT (2,3-bis-[2-methoxy-4-nitro-5-sulfophenyl]­2H-tetrazolium-5-caboxanilide) assay, respectively after human oral epithelioid carcinoma cells were treated with syringic acid for 48 hours. In this study, the cell viability of syringic acid on human oral epithelioid carcinoma cells showed a significant decrease by MTT assay compared with control, and also, the cell adhesion activity by XTT assay was decreased significantly in these cells after cells were treated with various concentrations of syringic acid for 48 hours. $MTT_{50}\;and\;XTT_{50}\;were\;282.3\;{\mu}M\;and\;418.8{\mu}M$ syringic acid, respectively. These results suggest that syringic acid shows midcytotoxic effect on human oral epithelioid carcinoma cells by the decreasement of the cell viability and the cell adehision activity assessed by colorimetric assay in these cultures.
