자동노출제어장치를 이용한 요추 측면 방사선검사 시 환자 중심 위치 변화가 선량과 화질에 미치는 영향 (Effects of Dose and Image Quality according to Center Location in Lumbar Spine Lateral Radiography Using AEC Mode)

  • The purpose of this study is to consider usefulness of using AEC mode and importance of patient center location in L-spine lateral radiography by comparing dose and image quality according to the change of patient center location with using AEC mode or not. In this study, guide wire is attached to the human body phantom's lumbar spine and the lead ruler is attached to the bottom of the wall detector to find out center location in detector. ESD, mAs, and EI were selected as dose factors, and image quality was compared through SNR. With the lumbar spine located center of the detector, dose factors and image quality were compared according to using AEC mode or not. Afterwards, phantom moved 4 cm and 8 cm back and forth and compared dose factors and image quality. The exposure parameters were 85 kVp, 320 mA, x-ray field size 10×17 inch, and the distance between the center X-ray and the detector was fixed at 100 cm. The center X-ray was perpendicular to the fourth lumbar spine and the only bottom AEC chamber was used. All data were analyzed by independent t-test and ANOVA. As a result of this study, with AEC when the center is matched, ESD was 1.31±0.01 mGy, without AEC was 2.12±0.01 mGy. SNR was shown to be 22.81±1.83, and 23.44±1.87 respectively. When the phantom's center moves 4 cm, 8 cm forward, and 4 cm, 8 cm backward, ESD were 1.09±0.004 mGy, 0.32±0.003 mGy, 1.19±0.017 mGy, 1.11±0.006 mGy respectively, SNR were 18.29±0.60 dB, 11.11±0.22 dB, 18.98±0.80 dB, 17.71±0.82 dB. Using AEC in L-spine lateral radiography reduced ESD by 38%, EI by 35%, and mAs by 38%, without any difference in SNR(p<0.05). When the phantom's center moves 4 cm, 8 cm forward, and 4 cm, 8 cm backward, ESD was decreasing each 16%, 75%, 9%, 15%, EI was decreasing each 14%, 77%, 15%, 20%, mAs was decreasing each 15% 75% 9%, 15%. SNR was decreasing each 19%, 51%, 17%, 22%.

Population attributable fraction of indicators for musculoskeletal diseases: a cross-sectional study of fishers in Korea

  • Background: The musculoskeletal disease (MSD) burden is an important health problem among Korean fishers. We aimed to investigate the indicators of the prevalence of MSD and contributions of significant indicators to MSD in Korean fishers. Methods: This cross-section study included 927 fishers (male, 371; female, 556) aged 40 to 79 years who were enrolled from 3 fishery safety and health centers. The outcome variable was one-year prevalence of MSD in 5 body parts (the neck, shoulder, hand, back, and knee). Independent variables were sex, age, educational attainment, household income, job classification, employment xlink:type, hazardous working environment (cold, heat, and noise), ergonomic risk by the 5 body parts, anxiety disorder, depression, hypertension, diabetes, and hyperlipidemia. The adjusted odds ratio of MSDs by the 5 body parts were calculated using multiple logistic regression analysis. We computed the population attributable fraction (PAF) for each indicators of MSDs using binary regression models. Results: The one-year prevalence of MSD in the neck, shoulder, hand, back, and knee was 7.8%, 17.8%, 7.8%, 27.2%, and 16.2% in males vs. 16.4%, 28.1%, 23.0%, 38.7%, and 30.0% in females, respectively. The ergonomic risk PAF according to the body parts ranged from 22.8%-59.6% in males and 22.8%-50.3% in female. Mental diseases showed a significant PAF for all body parts only among female (PAF 9.1%-21.4%). Cold exposure showed a significant PAF for the neck, shoulder, and hand MSD only among female (25.6%-26.8%). Age was not a significant indicator except for the knee MSD among female. Conclusions: Ergonomic risk contributed majorly as indicators of MSDs in both sexes of fishers. Mental disease and cold exposure were indicators of MSDs only among female fishers. This information may be important for determining priority risk groups for the prevention of work-related MSD among Korean fishers.

3차원 입체정위 유방생검술의 정확도 및 정밀도 평가 (Evaluation of the Accuracy and Precision Three-Dimensional Stereotactic Breast Biopsy)

  • 본 연구는 3차원 입체정위 유방생검술의 정확도를 알아보고, 심부침생검을 이용하여 Stereotactic biopsy과 Sonoguided biopsy의 정확도와 정밀도를 평가하고자 한다. Stereotactic QC phantom을 이용하여 실제 5개의 target 위치로 3D sterotactic machine의 정확도를 측정하고, CT장비로 Scan하여 실측을 구해 X, Y, Z의 길이의 정밀도를 비교한다. 유방조직과 유사하게 제작한 Agar power phantom을 이용하여 5개의 각기 다른 needle tip Target을 통해 3D sterotactic machine과 2D ultrasound machine의 정확도를 비교하고, Z축을 장비별로 실측하여 정밀도와 신뢰도를 비교하며, 6개의 모조병소 Target을 심어놓은 Medical application phantom으로 표적하여 육안검사와 Specimen검사를 통해 정확도를 확인하였다. Stereotactic QC phantom으로 측정한 3D sterotactic machine의 정확도는 100%였으며, CT와 비교한 정밀도는 X, Y, Z축이 모두 p>0.05로 나타났다. Agar powder phantom으로 측정한 두 장비의 정확도는 100%의 정확도를 보였으며, CT와 두 장비 사이에는 p > 0.05로 차이가 없었다. 그러나 2명의 방사선사가 측정한 신뢰도분석에서 3D sterotactic machine은 ICC가 0.954였고, 2D ultrasound machine은 0.785로 2D ultrasound machine이 술자에 따라 차이가 있었다. Medical application phantom의 실험에서 3D sterotactic machine은 Sliced boneless ham을, 2D ultrasound machine은 small chalk powder group를 찾을 수 없었다. Phantom을 이용한 3차원 입체정위 유방생검술의 정확성은 우수하게 나타났고, 인체조직과 비슷한 Agar powder phantom과 유방 조직과 비슷한 Medical application phantom을 이용하여 Stereotactic biopsy과 Sonoguided biopsy의 정확도와 정밀도 모두 우수하게 나타났다. 또한 Medical application phantom의 심부침생검의 정확성 평가에서 각 검사에 따라 생검 표본이 병소의 형태에 따라 상이하게 채취되었고, 3차원 입체정위 유방생검술의 재현성이 유방 초음파검사보다 술자의 영향없이 우수하였다.

이동형 디지털 X선 촬영장치의 구축 및 성능평가 (Construction and Performance Evaluation of Digital Radiographic System)

  • 최근 국내에서는 원거리에 있는 응급환자에 대한 원격의료서비스를 제공하기 위한 이동형 응급 의료 시스템에 대한 연구가 활발히 진행되고 있다. 본 연구의 목적은 응급구조 차에 탑재 할 수 있는 이동형 디지털 X선 촬영 장치를 구축하고 시스템으로부터 획득한 영상에 대하여 영상 성능평가의 주요인자인 MTF (modulation transfer function), NPS (noise power spectrum) 및 DQE (detective quantum efficiency)를 정량화함으로써 응급상황 시 임상적으로 효용성 있는 영상의 획득 가능성을 평가하는 것이었다. 이동형 X선 장치(Mobix-1000; LISTEM, Wonju, Korea)와 디지털 X선 detector 시스템 (Alpha-R4000; Teleoptic PRA, Kyiv, Ukraine)으로 구축된 영상시스템에 대하여 성능평가를 수행하였다. 측정결과로 10% MTF는 2.4 cycles/mm를 나타내었고, DQE (0)는 조건선량 0.19, 0.5 그리고 1.3 mR에 따라 각각 54%, 55%, 그리고 76%로 측정되었다. 본 연구에서 획득한 영상평가 결과는 연구 중인 이동형 디지털 X선 촬영 장치의 응급 원격 의료에의 사용가치를 확인 할 수 있는 기초자료로서 이용될 수 있을 것으로 판단된다.

사무직 근로자와 육체 노동자의 요통특성에 관한 비교 고찰 (Comparative Study of Low Back Pain between White Collar Workers and Blue Collar Workers)

  • This study has been attempted to be helpful for the back rehabilization of Korean workers by analyzing the general, occupational, social aspects of low back pain and to identify possible risk factors for back pain in White and Blue collar workers. The primary data were collected from 380 workers in Seoul city by means of a Questionnaire with random which was distributed from March 10 to 31, 1990. For the test of statistical significance, chi-square analysis was used to compare the back pain characteristics between above two groups. The results were as follows : 1. The incidence of low back pain in all walkers studied was $79.7\%$. The incidence of Blue collar with low back pain $(87.2\%)$ was higher than that of White collars $(75.0\%)$. 2. With regard to the relationship of back pain to the occupational characteristics, statistically significant differences were observed between workers with and without back pain concerning the job factors on work-time, job satisfaction, mental stress, chair fittness, work posture, work rotation, weight lifting, monotonous repetitive movements, vibration, and heavy noise (p<0.05). 3. With regard to the relationship of back pain to the social characteristics, there were no differences with respect to having car, personality types, drinking habits, and leisure-time activities. However, significant differences were showed between no pain and pain groups for the using bed, sleeping posture, traffic time amount, walking health state, smoking habits, and physical exercise (p<0.05). 4. The comparative analysis of back pain related to work factors showed highly significant differences with respect to mental stress, chair fittness, work posture, trunk rotation, weight lifting, monotonous repetitive work in White collar group (p<0.01) ; and job satisfaction, mental stress, trunk rotation, weight lifting, monotonus repetitive work, exposure to vibration and heavy noise in Blue collar group(p<0.01). 5. The comparative analysis of social factors in two groups showed differences with respect to the using bed, sleeping posture, walking amount, health state, physical exercise, smoking habits in White cellar group (p<0.05) ; and walking amount, traffic time amount, health state, smoking habits, physical exercise in Blue cellar group (p<0.05). 6. In regard to the general aspects of back pain between two groups, there were differences concerning etiolgy of back pain, counselling partners, treatment types, and sick-leaves(p<0.05), except pain duration, and awareness of back pain.

소아 흉부 CT 검사에서 관전압 80 kVp 조건으로 스캔 모드별 방사선량 감소와 화질 평가 (The Evaluation of Dose Reduction and Quality of Images According to 80 kVp of Scan Mode Change in Pediatric Chest CT)

  • 소아 흉부 CT 검사 시 방사선 피폭을 최소화 하면서 진단적으로 가치가 높은 영상을 얻기 위해서 Helical mode, High-pitch mode, Volume Axial mode를 적용하여서 선량, 검사시간, 화질을 비교하여 유용성을 평가하고자 한다. Revolution(GE Healthcare, Wisconsin USA)을 이용하여 PBU-70팬텀을 Helical mode, High-pitch mode, Volume Axial mode로 각각의 그룹으로 나누어서 관전압 80kVp, 조건으로 30회 검사를 실시하였다. 영상을 획득 한 후에 각각의 영상에 심장(Heart), 뼈(Bone), 폐(Lung), Back-ground air에 ROI를 설정하고 CT number(HU)와 noise(SD)값을 측정하여 평균값을 구하고 SNR과 CNR을 측정하였고, 장비에서 직접 제공하는 DLP값 비교하였다. 통계적인 유의성을 확인해보기 통계 분석은 SPSS 21.0을 사용하여 ONE-WAY-ANAOVA를 시행하였다. 본 실험을 통해 검사 시 volume axial mode 사용 시 가장 적은 선량으로 영상의 화질 저하 없이 빠른 시간에 검사가 가능하였다. 16cm의 detector coverage 가 모든 소아 흉부 CT검사에 적용하기에 제한점이 있으나 가능한 소아환자에 있어 적극적인 활용을 추천하며 volume axial mode의 다른 검사부위 적용에 대한 지속적인 연구가 필요하리라 사료된다.

An Assessment of the Usefulness of Time of Flight in Magnetic Resonance Angiography Covering the Aortic Arch

  • Carotid angiography covering the aortic arch includes contrast-enhanced magnetic resonance angiography (CEA), which is applied to a large region and usually employs contrast media. However, the use of contrast media can be dangerous in infants, pregnant women, and patients with chronic renal failure (CRF). Follow-up patients informed of a lesion may also want to avoid constant exposure to contrast media. We aimed to apply time-of-flight (TOF) angiography to a large region and compare its usefulness with that of CEA. Ten patients (mean age, 58 years; range, 45~75 years) who visited our hospital for magnetic resonance angiography (MRA) participated in this study. A 3.0 Tesla Achieva magnetic resonance imaging (MRI) system (Philips, Netherland) and the SENSE NeuroVascular 16-channel coil were employed for both methods. Both methods were applied simultaneously to the same patient. Three TOF stacks were connected to cover the aortic arch through the circle of Willis, and CEA was applied in the same manner. For the quantitative assessment, the acquired images were used to set the regions of interest (ROIs) in the common carotid artery (CCA) bifurcation, internal carotid artery, external carotid artery, middle cerebral artery, and vertebral artery, and to obtain the signal-to-noise ratio (SNR) and the contrast-to-noise ratio (CNR) for the soft tissues. Three radiologists and one radiological resident performed the qualitative assessment on a 5-point scale - 1 point, "very bad"; 2 points, "bad"; 3 points, "average"; 4 points, "good"; and 5 points, "very good" - with regard to 4 items: (1) sharpness, (2) distortion, (3) vein contamination, and (4) expression of peripheral vessels. For the quantitative assessment, we estimated the mean SNR and CNR in each of the 5 ROIs. In general, the mean SNR was higher in TOF angiography (166.1, 205.2, 154.39, 172.23, and 161.95) than in CEA(92.05, 95.43, 84.76, 73.69, and 88.3). Both methods had a similar mean CNR: 67.62, 106.71, 55.9, 73.74, and 63.46 for TOF angiography, and 67.82, 71.19, 60.52, 49.45, and 64.07 for CEA. In all ROIs, the mean SNR was statistically significant (p<0.05), whereas the mean CNR was insignificant (p>0.05). The mean values of TOF angiography and CEA for each item in the qualitative assessment were 4.2 and 4.28, respectively for item 1; 2.93 and 4.55, respectively, for item 2; 4.6 and 3.13, respectively, for item 3; and 2.88 and 4.65, respectively, for item 4. Therefore, TOF angiography had a higher mean for item 3, and CEA had a higher mean for items 2 and 4; there was no significant difference between the two methods for item 1. The results for item 1 were statistically insignificant (p>0.05), whereas the results for items 2~4 were statistically significant (p<0.05). Both methods have advantages and disadvantages and they complement each other. However, CEA is usually applied to a large region covering the aortic arch. Time-of-flight angiography may be useful for people such as infants, pregnant women, CRF patients, and followup patients for whom the use of contrast media can be dangerous or unnecessary, depending on the circumstance.

MDCT에서 선량 변화에 따른 딥러닝 재구성 기법의 유용성 연구 (A Study on the Usefulness of Deep Learning Image Reconstruction with Radiation Dose Variation in MDCT)

  • MDCT의 딥러닝 재구성 기법(TrueFidelity, TF)의 유용성을 평가하고자 기존의 필터보정역투영법(Filtered back projection, FBP)과 적응형 통계적 재구성 기법(Adaptive Statistical Iterative Reconstruction-Veo, ASIR-V)의 화질을 비교 평가하였다. FBP, ASIR-V 50%, TF-H의 재구성 기법에서 선량을 17.29 mGy로 고정한 것과 10.37 mGy, 12.10 mGy, 13.83 mGy, 15.56 mGy로 변화시킨 영상을 획득하여 노이즈, CNR, SSIM을 측정하였다. 17.29 mGy에서 재구성 기법 변화를 주었을 때 TF-H가 FBP, ASIR-V에 비해 화질이 우수하다. 선량에 변화를 주었을 때 10.37 mGy TF-H와 FBP 비교 시 노이즈, CNR, SSIM은 유의한 차이가 있고(p<0.05), 10.37 mGy TF-H와 ASIR-V 50% 비교 시 유의한 차이가 없다(p>0.05). 선량이 가장 높은 15.56 mGy ASIR-V 50%와 선량이 가장 낮은 10.37 mGy TF-H 화질이 동일하므로 TF-H는 30%의 선량 감소 효과가 있다. 따라서 딥러닝 재구성 기법(TF)은 반복적 재구성 기법(ASIR-V)과 필터보정역투영법(FBP)보다 선량을 감소시킬 수 있었다. 이로 인해 환자의 피폭선량을 감소시킬 것으로 사료된다.

Outbreak of Sudden Cardiac Deaths in a Tire Manufacturing Facility: Can It Be Caused by Nanoparticles?

  • Objectives: The purpose of this study was to review clinical characteristics and working environments of sudden cardiac death (SCD) cases associated with a tire manufacturer in Korea, and review possible occupational risk factors for cardiovascular disease including nanoparticles (ultrafine particles, UFPs). Methods: We reviewed (i) the clinical course of SCD cases and (ii) occupational and non-occupational risk factors including chemicals, the physical work environment, and job characteristics. Results: Possible occupational factors were chemicals, UFPs of rubber fume, a hot environment, shift work, overworking, and noise exposure. The mean diameter of rubber fume (63-73 nm) was (larger than diesel exhaust [12 nm] and outdoor dust [50 nm]). The concentration of carbon disulfide, carbon monoxide and styrene were lower than the limit of detection. Five SCD cases were exposed to shift work and overworking. Most of the cases had several non-occupational factors such as hypertension, overweight and smoking. Conclusion: The diameter of rubber fume was larger than outdoor and the diesel exhaust, the most well known particulate having a causal relationship with cardiovascular disease. The possibility of a causal relation between UFPs of rubber fume and SCD was not supported in this study. However, it is necessary to continue studying the relationship between large sized UFPs and SCD.

디지털 신호처리 칩(GCD4101)을 사용한 컬러 CCD 카메라 구현 (Implementation of color CCD Camera using DSP(GCB4101))

  • 본 연구에서는 CCD 카메라 전용의 디지털 신호 처리기(DSP)를 이용하여 고해상도의 CCTV 카메라를 구현하였다. 기존의 아날로그 신호 처리기를 사용한 CCTV 카메라는 카메라의 기본 기능인 자동노출(AE), 자동백색조정(AWB), 역광보정(BLC) 등의 처리에 한계점을 갖고 있으며, 영상의 열화 현상이 심하고, 카메라를 생산하는 경우에 조정 매개변수를 수동으로 조정하여야 하는 등 여러 가지 문제점을 내포하고 있다. 따라서 본 연구에서는 이상의 문제점을 해결하기 위하여 국산의 CCD 카메라 전용의 디지털 신호 처리기를 사용하여, 이를 마이컴으로 제어함으로써 자동노출, 자동백색조정, 역광보정 둥의 처리를 화상의 질에 따라 자동으로 조절할 수 있도록 하였다. 또한 마이컴의 채택으로 화면에 문자를 표시하는 기능을 구현함으로써 사용자와의 대화적 형식의 카메라 제어가 가능하도록 구현하였으며, 생산 과정에서 수동으로 카메라 동작 매개 변수를 조정하여야 하는 문제점을 해결하기 위하여 전자적인 가변저항(EVR)을 채택하였다. 그 결과 카메라 조정 변수를 프로그램에 의해 조정하는 것이 가능해졌으며 파라미터 값의 재현성 및 조정 용이성 부분에서의 신뢰성과 조정시간의 감소로 인한 생산단가 절감의 효과를 가져올 수 있었다.
