Anti-Inflammatory Effect of Polygoni Avicularis Herba Herbal-Acupuncture at KI10 on LPS-Induced Nephritis in Rats (음곡(KI10) 편축 약침의 LPS 유도 급성신장염 억제 효과)

  • Objectives : This study aimed to evaluate the effects of Polygoni avicularis Herba Herbal-acupuncture( PaH-HA) at KI10(Umgok) on nephritis induced by lipopolysaccharide(LPS) in rats. Methods : Rats were allocated into normal, control, and 3 experimental groups. The rats in the control group were intra-peritoneally injected with LPS for nephritis induction. The rats in the groups of experiment 1, experiment 2, and experiment 3 were treated with single needle prick, saline injection, and PaH-HA, respectively at KI10 three times for a week and then injected with LPS. To evaluate the effects of PaH-HA at HI10, WBC count in blood, serum CINC-1, renal TNF-${\alpha}$ and renal MPO were measured. Results : Needle prick at KI10 suppressed the increase of WBC in blood and CINC-1 in serum of LPS-stimulated rats. Saline injection at KI10 suppressed the increase of WBC in blood. PaH-HA at KI10 suppressed the increase of WBC in blood, CINC-1 in serum, and MPO in kidney of LPS-stimulated rats. Conclusions : PaH-HA at KI10 has an anti-inflammatory effect on LPS-induced nephritis in rats and there may be a synergism between KI10(Umgok) stimulation and PaH-HA Solution.

  • Henoch-$Sch{\ddot{o}}nlein$ purpura (HSP) is the most common vasculitis in children, mainly affecting the small vessels of the skin, joints, gastrointestinal tract, and kidneys. Although most cases of HSP resolve spontaneously without sequelae, serious nephrological and intestinal problems may occur in some cases. We experienced a case of HSP complicated by simultaneous intussusception and nephritis in a 14-year-old boy who developed a sudden abdominal pain and gross hematuria on the 11th day after onset of the disease. Imaging studies revealed intussusception that required emergency laparotomy. Despite treatment with steroid and angiotensin-converting enzyme inhibitors, nephritis and nephrosis progressed for 4 weeks, and renal biopsy was performed to confirm the diagnosis. Cyclosporin A therapy was started, and remission of proteinuria was achieved after 5 months. However, the nephritis recurred and worsened to end-stage renal failure during 15 years of follow-up.

  • Objective : This study aimed to evaluate the effects of Plantaginis Semen herbal-acupuncture (PS-HA) at KI10 (Umgok) on nephritis induced by lipopolysaccharide (LPS) in rat. Methods : The authors performed several experimental items including measurements of urinary volume, WBC in blood, BUN, creatine, TNF-$\alpha$, CINC-1 in serum, creatinine, total protein in urine, TNF-$\alpha$, MPO in kidney and histological analysis of renal tissue. Results : PS-HA at KI10 significantly reduced WBC in blood, BUN, TNF-$\alpha$ in serum of LPS-stimulated rats. PS-HA at KI10 significantly increased urinary volume in LPS-stimulated rats. And PS-HA at KI10 significantly reduced MPO in kidney of LPS-stimulated rats. Conclusion : Taken together, PS-HA at KI10 has a therapeutic effect on nephritis in LPS-stimulated rat. Therefore, it is suggested that PS-HA at KI10 may be an useful therapeutics for nephritis in clinical field.

A case report of tubulointerstitial nephritis and uveitis syndrome in children with an unfavorable outcome

  • Tubulointerstitial nephritis and uveitis (TINU) syndrome is defined as the occurrence of tubulointerstitial nephritis and uveitis in the absence of other systemic diseases. Three pediatric cases have been reported in the Republic of Korea, and we now report a fourth case. A 15-year-old girl presented to the ophthalmology department with a 1-week history of bilateral ocular discomfort that worsened on the day of presentation with redness and pain in both eyes. She was diagnosed with bilateral uveitis, and her baseline examination revealed moderate renal dysfunction and mild proteinuria. A renal biopsy was performed and confirmed the diagnosis of TINU syndrome. She was started on steroid eye drops and a 12-week course of oral steroids at a dose of 40 mg/m2/day, which completely resolved the proteinuria and mild renal function to an estimated glomerular filtration rate of 60 mL/min/1.73 m2. However, the uveitis did not improve, and despite the addition of oral methotrexate as a second-line treatment, the uveitis remains unresponsive to treatment over 21 months. Further evaluation and treatment are ongoing, and active therapeutic intervention is suggested even at a pediatric age, considering the lack of improvement in renal function and uveitis to date.

  • Reversible posterior leukoencephalopathy syndrome (RPLS) is an acute encephalopathy, usually characterized by seizures, visual disorder, mental change and reversible posterior white and gray matter lesions on brain magnetic resonance imaging (MRI). Conditions commonly associated with RPLS include acute renal failure, eclampsia, hypertension, immunosuppresion state, and use of intravenous immunoglobulin or chemotherapy, and additional contributing factors. We report the case of RPLS associated with cyclosporine use in lupus nephritis. In this case, it was not clearly elucidated which was main cause of RPLS. We could think RPLS developed maybe due to combined effect of cyclosporine and lupus nephritis.

  • Acute interstital nephritis can occur by acetaminophen, but it is rarely presented as acute renal failure with azotemia. We report a case of acute interstitial nephritis induced by acetaminophen in a 14-year-old girl who developed non-oliguric acute renal failure. She has taken acetaminophen to control the persistent throat pain for the last two months. Renal biopsy revealed diffuse infiltration of mononuclear inflammatory cells admixed with eosinophils in the edematous interstitia. After the discontinuation of acetaminophen and the administration of corticosteroid, the serum creatinine level returned to normal.

Management of IgA vasculitis nephritis (Henoch-Schonlein purpura nephritis) in Children

  • Immunoglobulin (Ig)A vasculitis nephritis (IgAVN), also referred to as Henoch-Schönlein purpura nephritis, is a relatively benign disease in children. However, two 24-year European cohort studies have reported high sustained rates of hypertension, severe proteinuria, and renal dysfunction in patients with IgAVN. Notably, the incidence and exacerbation rates of proteinuria, hypertension, and renal dysfunction during pregnancy were high even in women who recovered from IgAVN before pregnancy. Patients with IgAVN need lifelong care. Trials have been performed to investigate early biomarkers and genes associated with poor prognosis to identify high-risk patients in whom IgAVN may progress to severe renal disease. Urinary IgA/cr, IgM/cr levels, and HLAB35 and angiotensinogen gene expression were shown to be predictors of progression of IgAVN to severe renal dysfunction. The 2019 Single Hub and Access point for paediatric Rheumatology in Europe (SHARE) initiative group published guidelines for pediatric IgAVN, following the Kidney Disease: Improving Global Outcomes (KDIGO) guidelines established in 2012. Compared with the KDIGO guidelines, the SHARE guidelines recommend earlier corticosteroid administration in cases of mild proteinuria (>0.5 g/d). Clinical trials of targeted budesonide delivery to the distal ileum, monoclonal antibody targeting C5, eculizumab and anti-CD20 monoclonal antibody administration, among others are currently underway in patients with IgA nephropathy. It is expected that newer therapeutic agents would become available for IgAVN in the near future. This review summarizes IgAVN with emphasis on recently published literature, including possible preventive strategies, predictive biomarkers for progression of IgAVN, and various treatments.

  • Objective : This study aimed to evaluate the effects of Benincasae Semen Herbal-acupuncture (BS-HA) at KI10 (Umgok) on nephritis induced by LPS in rat. Methods : Rats with nephritis induced by LPS, were treated with Benincasae Semen Herbal-acupuncture(BS-HA) injection at KI10. Two control groups, N.P. group and saline group, were treated with 26 gauge needle at KI10, 3 times a week. In Saline group normal saline was injected at KI10. To evaluate the effects of Benincasae. Semen Herbal-acupuncture at KI10 on nephritis in rats, WBC, Neutrophils in blood, BUN, Creatinine TNF-$\alpha$, CINC-1 in serum, urinal volume and creatinine and Total protein in urine, reanl TNF-$\alpha$, renal MPO were measured and reanl tissue was also analyzed. Results: BS-HA injected at KI10 significantly inhibited WBC and neutrophil in blood. creatinine in serum, and MPO in kidney of LPS-stimulated rats. BS-HA injected at KI10 reduced concentration of neutrophil in renal tissue of LPS-stimulated rats. Conclusion : BS-HA at KI10 has a therapeutic effect for nephritis in LPS-stimulated rat There fore, it is suggested that BS-HA at KI10 may be an useful therapeutics in clinical field after further researches.

Multifocal interstitial nephritis of pigs slaughtered in Jeju (제주지역 도축돈의 간질성 신염)

  • Total 160 head of porcine kidneys were examined for gross and histopathological lesions and polymerase chain reaction (PCR) for porcine circovirus type 2 (PCV-2), porcine parvovirus (PPV), Leptospira species and porcine reproductive and respiratory syndrome virus (PRRSV). Grossly, 137 kidneys (85.6%) had lesions characterized by the presence of the scattered white foci. Microscopically, multifocal interstitial nephritis, which classified into 4 grades such as, no lesion (Score 0), mild lesion (Score 1), moderate lesion (Score 2) and severe chronic lesion (Score 3) with fibrosis, was observed in 159 cases (99.4%). The histopathologic mean score for multifocal interstitial nephritis was significantly different (P<0.05) between the cases of PCV-2 single infection and the cases of co-infection with PCV-2 and PPV. According to PCR evaluation, PCV-2 were detected in 73.8% (118 cases), PPV were in 66.9% (107 cases), however Leptospira spp. and PRRSV were negative in all kidneys. Both PCV-2 and PPV were detected in 52.5% (84 cases). In 84 cases co-infected with PCV-2 and PPV, the occurrence of lymphoid follicle and vasculitis were observed as 65.5% (55 cases) and 26.2% (22 cases), respectively. These results revealed that PCV-2 and PPV were major infectious agents for interstitial nephritis in slaughtered pigs, Jeju. And the histopathologic lesions of multifocal interstitial nephritis were more severe in the case co-infected with PCV-2 and PPV.

Effects of Polyporus Herbal-acupuncture at KI10 on LPS-induced nephritis in rats (LPS로 유도된 흰쥐의 신장염에 대한 음곡(KI10) 저령약침의 효과)

PO-HA at KI10 significantly suppressed the increase creatinine, total protein in urine of LPS-stimulated rats. Conclusions: According to these results, it is postulated that PO-HA at KI10 has an anti-inflammatory and renal-protective effects on LPS-induced nephritis in rats. Therefore, it is suggested that PO-HA at KI10 may be an useful therapeutics for nephritis in clinical field after further researches.
