Purpose: Chloral hydrate is a common drug frequently used for procedural sedation. But data on chloral hydrate use in the newborns are limited. This study examined the frequency of adverse effects of chloral hydrate and factors related to the adverse effects. We also examined if there were additional adverse effects when an additional sedative was used. Methods: The medical records of 104 patients admitted to neonatal intensive care unit of Seoul St. Mary's Hospital from March 2010 to February 2011 who used chloral hydrate for procedural sedation were retrospectively reviewed. Results: Adverse effects after administration of chloral hydrate were noted in 41.3% of the 104 patients. The adverse events included oxygen desaturation (18.8%), increase in apneic episodes (17.5%), increase in bradycardia (10%), and feeding intolerance (3.8%). Using oxygen at the time of chloral hydrate administration was independently associated with adverse effects (odds ratio [OR], 10.911: 95% confidence interval [CI], 2.082-57.178) and with the necessity for an additional sedative after administration of chloral hydrate (OR, 4.151: 95% CI, 1.455-11.840). Using one additional sedative agent after chloral hydrate showed no difference in adverse effects except feeding intolerance. Conclusion: Patients dependent on oxygen at the time of chloral hydrate administration may were found to be at higher risk for adverse effect of chloral hydrate and for an additional sedative. When an additional sedative is needed, it could be used with monitoring feeding intolerance after chloral hydrate administration.
Recently, the number of diabetic patients have been increased with westernized living way and meal habitation. The regulation of blood sugar concentration is very important for diabetic patients to keep homeostasis and, to prevent acute or chronic complications. Epinephrine combined with lidocaine is used in dental clinic extensively. And epinephrine combined with lidocaine also effects on decreasing the blood leakage volume by constricting micro vessle and arteriole. But, So far there are few researches about the effect of epinephrine contained in dental local anesthetic agent on the blood sugar of diabetes during minor oral surgery. The purpose of this study was intended to investigate whether epinephrine which combined with dental local anesthetics influence body glucose level in diabetes patients by glucose monitoring. The subject of this study were 38 diabetic patients and 38 normal adults, each patient was checked body glucose after a meal 2 hours later with resting state, and injection 1:100000 epinephrine with dental lidocaine 54 ml or lidocaine 54 ml only. And then the body glucose level was checked 5 minutes and 30 minutes after injection. The results were analyzed by two way ANOVA test (p<0.05). The results were as follows: In the experimental group 1, the mean of the blood sugar level was 180.3 mg/dl before an anesthetic injection, 182.8 mg/dl after 5minutes of the anesthetic injection and 182.2 mg/dl after 30minutes of the anesthetic injection. The change of the blood sugar level was not statistically by two-way ANOVA tests (p<0.05). In the experimental group 2, the mean of the blood sugar level was 237 mg/dl before an anesthetic injection, 234.5 mg/dl after 5minutes of the anesthetic injection and 231.8 mg/dl after 30minutes of the anesthetic injection. The change of the blood sugar level was not statistically by two-way ANOVA tests (p<0.05). In the controlled group 1, the mean of the blood sugar level was 117.6 mg/dl before an anesthetic injection, 119.1 mg/dl after 5minutes of the anesthetic injection and 129.3 mg/dl after 30minutes of the anesthetic injection. The change of the blood sugar level was not statistically by two-way ANOVA tests (p<0.05). In the controlled group 2, the mean of the blood sugar level was 104.2 mg/dl before an anesthetic injection, 102 mg/dl after 5minutes of the anesthetic injection and 105.3 mg/dl after 30minutes of the anesthetic injection. The change of the blood sugar level was not statistically by two-way ANOVA tests (p<0.05).
The Intelligent Web Service is proposed for the purpose of automatic discovery, invocation, composition, inter-operation, execution monitoring and recovery web service through the Semantic Web and the Agent technology. To accomplish this Intelligent Web Service, the Ontology is a necessity for reasoning and processing the knowledge by the computer. However, creating service ontology, for the intelligent web service, has two problems not only consuming a lot of time and cost depended on heuristic of service developer, but also being hard to be mapping completely between service and service ontology. Moreover, the markup language to describe the service ontology is currently hard to be learned by the service developer In a short time. This paper proposes the efficient way of designing and creating the service ontology using MDA methodology. This proposed solution reuses the creating model in terms of desiEninE and constructing Web Service Model using UML based on MDA. After converting the Platform-Independent Web Service Model to the dependent model of OWL-S which is a Service Ontology description language, it converts to OWL-S Service Ontology using XMI. This proposed solution has profits, oneis able to be easily constructed the Service Ontology by Service Developers, the other is enable to be created the both service and Service Ontology from one model. Moreover, it can be effective to reduce the time and cost as creating Service Ontology automatically from a model, and calmly dealt with a change of outer environment like as the platform change. This paper cites an instance for the validity of designing Web Service model and creating the Service Ontology, and validates whether the created Service Ontology is valid or not.
This study proposes a new polling algorithm allowing dynamic change in polling period based on line utilization prediction. Polling is the most important function in network monitoring, but excessive polling data causes rather serious congestion conditions of network when network is In congestion. Therefore, existing multiple polling algorithms decided network congestion or load of agent with previously performed polling Round Trip Time or line utilization, chanced polling period, and controlled polling traffic. But, this algorithm is to change the polling period based on the previous polling and does not reflect network conditions in the current time to be polled. A algorithm proposed in this study is to predict whether polling traffic exceeds threshold of line utilization on polling path based on the past data and to change the polling period with the prediction. In this study, utilization of each line configuring network was predicted with AR model and violation of threshold was presented in probability. In addition, suitability was evaluated by applying the proposed dynamic polling algorithm based on line utilization prediction to the actual network, reasonable level of threshold for line utilization and the violation probability of threshold were decided by experiment. Performance of this algorithm was maximized with these processes.

Diagnostic approaches such as angiography, ultrasound, computed tomography and nuclear magnetic resonance have limitation for contributing to the early clinical diagnosis of atherosclerosis. Recently, $^{99m}Tc-labelled$ low density lipoprotein was developed to detect early atherosclerotic lesion by external imaging with gamma camera. To determine whether $^{99m}Tc-LDL$ scintigraphy can visualize the active atherosclerotic lesion, rabbits were injected with $^{99m}Tc-LDL$, 3 months after feeding dietary fat (lanolin) and we obtained following results. 1) Labelling efficiency of $^{99m}Tc-LDL$ was $79\sim88%$. 2) Biodistribution study of normal rabbits with $^{99m}Tc-LDL$ revealed the high activities in spleen, adrenal gland, liver, kidney which are major organs of high metabolic rate of LDL. 3) Three months after feeding lanolin, serum cholesterol was markedly increased from $74{\pm}17mg/dl$ to $979{\pm}153mg/dl$ and histologic study of aorta after sacrificing the rabbit demonstrated marked atherosclerotic changes. 4) Atherosclerotic lesion of abdominal aorta which was confirmed with histologic study could be demonstrated in $^{99m}Tc-LDL$ scintigraphy after feeding lanolin for 3 months. In conclusion, the results of this preliminary investigation suggest that it may be possible to image active atheromatous lesion with $^{99m}Tc-LDL$. It is anticipated that this promising agent may allow the in vivo monitoring of preclinical atherosclerotic lesions and may be useful to evaluate the metabolic pathway of LDL in humans.
This paper is to construct a healthcare database using information obtained from healthcare home environments, and use this one for healthcare home services, Especially, our researching focus in this paper is how to design healthcare database scheme and how to use this constructed database on the Framework for Supporting Healthcare Integrated Service(FSHIS) we developed previously. Healthcare information is designed to database schemes in accordance to the specific save types of the data collected from various typed-sensors. The healthcare database constructed by using this information for the purpose of healthcare home services is divided into the base information with real schemes and the context based information with view schemes. Firstly, the base information includes low data obtained from physical sensors relevant to locations, healths, environments, and the personnel healthy profiles. The other is the context based information that is produced and fused by using the based information. This context based information might be got via various view schemes according to healthcare application services. Finally, for verifying the practical use of healthcare database constructed in this paper, Via interconnecting this database to our FSHIS, we show an example of healthcare home monitoring service using information (basic and context based information), emergency call, home appliance control, and so on needed from living activity area for elderly living alone.
Endothelial protein C receptor (EPCR) plays a pivotal role in augmenting Protein C activation through the thrombin-thrombomodulin complex. EPCR activity is markedly changed by ectodomain cleavage and released as the soluble protein (sEPCR). EPCR shedding is mediated by tumor necrosis factor-${\alpha}$ converting enzyme (TACE). Lycopene found in tomatoes and tomato products has anti-oxidant, anti- cancer and anti-inflammatory effects. However, little is known about the effects of lycopene on EPCR shedding. We investigated this issue by monitoring the effects of lycopene on the phorbol-12-myristate 13-acetate (PMA), tumor necrosis factor (TNF)-${\alpha}$, interleukin (IL)-$1{\beta}$ and on the cecal ligation and puncture (CLP)-mediated EPCR shedding. Data showed that lycopene potently inhibited the PMA, TNF-${\alpha}$, IL-$1{\beta}$ and CLP-induced EPCR shedding by suppressing TACE expression. Furthermore, lycopene reduced PMA-stimulated phosphorylation of p38, extracellular regulated kinases (ERK) 1/2 and c-Jun N-terminal kinase (JNK). Given these results, lycopene should be viewed as a candidate therapeutic agent for the treatment of various severe vascular inflammatory diseases via inhibition of the EPCR shedding.

This study was performed to examine theoretical foundations and the function of strategic purchasing. And we examined the functions of strategic purchasing in Korea based on the framework proposed by Preker(2005). For this purpose, we reviewed literature related to purchasing. we defined the strategic purchasing as strategic activities to provide health care services people need within a given budget, which is carried out by certain organizations, purchasing organizations. These activities include selecting appropriate providers, designing and operating the payment system, setting the price, and determining the target populations and their needs etc. The relationships among government, purchasers and healthcare providers can be explained by the principal-agent theory. In addition to Preker's framework, we emphasized the importance of the infrastructure such as decision making support systems, information systems, health care resource management systems, or expenditure monitoring systems. The National Health Insurance Service and the Health Insurance Review & Assessment Service play major roles in performing strategic purchasing.

Objectives: This on-site study was performed to evaluate the reduction efficiency of an air cleaner on particulate matters and biological agents in a swine facility. Materials and Methods: Particulate matter was measured using a real-time monitoring recorder and biological agents were sampled with a one-stage impactor and then analyzed based on the microbial culture method. An experimental process for the reduction effect on airborne pollutants through air cleaner operation consisted of three conditions: no treatment, wet scrapper by water spray and wet scrapper by disinfectant spray. Results: Geometric mean levels of particulate matter(TSP, $PM_{10}$, $PM_{2.5}$ and $PM_1$) were presented at $1,608{\mu}g/m^3$, $1,373.8{\mu}g/m^3$, $401.8{\mu}g/m^3$ and $144.5{\mu}g/m^3$ for no treatment; $1,503{\mu}g/m^3$, $1,017{\mu}g/m^3$, $159.4{\mu}g/m^3$ and $69.8{\mu}g/m^3$ for wet scrapper by water spray; and $1,222.17{\mu}g/m^3$, $477.17{\mu}g/m^3$, $33.2{\mu}g/m^3$ and $11.1{\mu}g/m^3$ for wet scrapper by disinfectant spray, respectively. In the case of biological agents, the geometric averaged concentrations of total airborne bacteria and fungi were as follows: $45,371cfu/m^3$ and $13,474cfu/m^3$ for no treatment, $43,286cfu/m^3$ and $8,610cfu/m^3$ for wet scrapper by water spray, and $2,440cfu/m^3$ and 1,867 cfu/ for wet scrapper by disinfectant spray, respectively. Regardless of particulate matter and biological agent, the highest concentrations were found for no treatment, while the lowest concentrations were found with wet scrapper by disinfectant spray. Conclusions: Based on the results obtained from this on-site evaluation, there was a significant reduction effect on particulate matter and biological agents through the application of an air cleaner in this study.
Background: Biogenic fabrication of silver nanoparticles from naturally occurring biomaterials provides an alternative, eco-friendly and cost-effective means of obtaining nanoparticles. It is a favourite pursuit of all scientists and has gained popularity because it prevents the environment from pollution. Our main objective to take up this project is to fabricate silver nanoparticles from lichen, Usnea longissima and explore their properties. In the present study, we report a benign method of biosynthesis of silver nanoparticles from aqueous-ethanolic extract of Usnea longissima and their characterization by ultraviolet-visible (UV-vis), Fourier transform infrared (FTIR) spectroscopy, transmission electron microscopy (TEM) and scanning electron microscopy (SEM) analyses. Silver nanoparticles thus obtained were tested for antimicrobial activity against gram positive bacteria and gram negative bacteria. Results: Formation of silver nanoparticles was confirmed by the appearance of an absorption band at 400 nm in the UV-vis spectrum of the colloidal solution containing both the nanoparticles and U. longissima extract. Poly(ethylene glycol) coated silver nanoparticles showed additional absorption peaks at 424 and 450 nm. FTIR spectrum showed the involvement of amines, usnic acids, phenols, aldehydes and ketones in the reduction of silver ions to silver nanoparticles. Morphological studies showed three types of nanoparticles with an abundance of spherical shaped silver nanoparticles of 9.40-11.23 nm. Their average hydrodynamic diameter is 437.1 nm. Results of in vitro antibacterial activity of silver nanoparticles against Staphylococcus aureus, Streptococcus mutans, Streptococcus pyrogenes, Streptococcus viridans, Corynebacterium xerosis, Corynebacterium diphtheriae (gram positive bacteria) and Escherichia coli, Klebsiella pneuomoniae and Pseudomonas aeruginosa (gram negative bacteria) showed that it was effective against tested bacterial strains. However, S. mutans, C. diphtheriae and P. aeruginosa were resistant to silver nanoparticles. Conclusion: Lichens are rarely exploited for the fabrication of silver nanoparticles. In the present work the lichen acts as reducing as well as capping agent. They can therefore, be used to synthesize metal nanoparticles and their size may be controlled by monitoring the concentration of extract and metal ions. Since they are antibacterial they may be used for the treatment of bacterial infections in man and animal. They can also be used in purification of water, in soaps and medicine. Their sustained release may be achieved by coating them with a suitable polymer. Silver nanoparticles fabricated from edible U. longissima are free from toxic chemicals and therefore they can be safely used in medicine and medical devices. These silver nanoparticles were stable for weeks therefore they can be stored for longer duration of time without decomposition.
