응급구조학과 대학생들의 죽음의식과 심폐소생술 금지(DNR)에 대한 인식 및 태도 (Paramedic students' awareness and attitude toward a do-not-resuscitate (DNR) order and death)

  • Purpose: This study aimed to investigate paramedic students' awareness and attitude toward a do not resuscitate (DNR) order and death. Methods: This research was conducted among 421 students from the Department of Emergency Medical Technology in a 4-year college located in the Chungcheong and Daejeon districts, from May 14 to 22, 2014. Data were analyzed by using IBM SPSS 21.0. Results: The mean level of attitude toward death was 2.17. The paramedic students with clinical experience showed a positive attitude toward death, of whom 72.0% answered that a DNR order is necessary. The mean level of attitude toward DNR was 2.88. The paramedic students with clinical experience showed a positive attitude toward a DNR order. They indicated that sanctity of life should be respected rather than extending ineffective treatment and that patients' decisions on DNR should be respected. The students who had more knowledge about DNR and felt the necessity of DNR had a positive attitude about death and DNR. Conclusion: Paramedic students need systematic education for proper recognition and values establishment about death and DNR.

노인요양 시설 간호 인력의 의료적 처치 및 관리 경험 (Nursing staffs' Experience in Treatment and Managing Medical Problem in Long Term Care Settings)

  • Purpose: The purpose of this study is to describe the experience of medical treatment and management of nursing staffs in long term care settings. Methods: This study is qualitative research. The participants were seven nursing staff (five nurses and two nursing assistants) who had experience over six months in long term care facilities. Data were collected through in-depth interviews using semi-structured research questions. The collected data were analysed using qualitative thematic analysis method. Results: Four main themes were emerged as 'various medical treatments', 'difficulties about managing medical problems', 'lack of regulatory support' and 'strategies for solving medical problems'. Conclusion: The findings of this study provide the nursing staffs' difficulties and opinion about medical treatment and management in long term care settings. To provide more effective health service for the aged, the regulatory complement about medical treatment in long term care facilities should be considered.

구급대원의 리더십에 대한 인식 유형 (Leadership perception types among 119 emergency medical technicians)

  • Purpose: The purpose of this study was to provide basic data for developing strategic programs based on 119 emergency medical technicians leadership perception types. Methods: The subjects were thirty six 119 emergency medical technicians working for out-of hospital in Jeollanam-Do, Korea. Data were collected from July 20, 2017 to October 30, 2017. Q sample of 40 statements and P sample of 36 persons using PC QUANL software. Results: Three types of leadership styles were identified from the explanation (71.5% of the variables). The relationship oriented progressive type (the most common), the performance and value oriented type (the second most common), and the experience oriented filed participation type (the third most common). Conclusion: The fire department will need to develop and implemented leadership improvement programs based on the leadership types in order to meet the organization's goals.

의료서비스 실패유형 재조명: 복구 가능과 복구 불가능 서비스 (Reexamination of Failure Type in Medical Service: Recoverable and Irrecoverable Service)

  • 지금까지 의료서비스에 대한 다양한 연구가 진행되었지만 기존 연구들은 원인변수와 결과변수와의 관계를 규명하는 데만 초점을 두고 있다. 하지만 의료서비스 문제는 다른 서비스 산업과 상대적으로 비교해볼 때 문제 발생 시 고객이 느끼는 감정은 다를 것이라 예측되며, 최근 의료관광과 더불어 의료서비스의 중요성이 부각되는 시점이다. 이에 본 연구는 의료서비스 문제에 관한 정성적 자료를 실증 분석하였으며, 단어구름기법도 이용하였다. 연구의 주요결과를 살펴보면, 의료서비스 문제는 항목별로 의료과실, 간호사 업무미숙, 무심한 진료, 과잉검사 및 진료, 치료강요 및 거부, 응급대기, 불친절, 예약문제, 프로세스문제, 불편함 등 총 10개의 요인으로 나타났다. 그 중 복구 불가능한 서비스 실패에서 가장 많이 산출된 주요 단어는 의료과실, 무심한 진료, 간호사의 업무미숙 순이며, 복구 가능한 서비스 실패에서는 불친절한 태도와 예약시스템에 관한 부정적 경험의 주요 단어가 가장 많이 도출되었다. 의료서비스 문제 후 고객행동은 대부분 강력한 항의를 하며, 아주 심각한 문제에 대해서는 공개적 항의를 하거나 법적대응을 하는 것으로 나타났다. 본 연구의 결론에서는 연구결과 요약과 시사점, 그리고 향후 연구에 대한 제언을 하였다.

본인부담상한제 적용 요양병원 환자의 의료이용가수요 예측요인 분석 (An Aanalysis of Predictive Factors of Medical Service Overuse for Inpatients Applied Out-of-Pocket Maximum in Long-Term Care Hospitals in South Korea)

  • Background: The out-of-pocket maximum is one of the distinctive healthcare systems which sets a ceiling on co-payment in order to reduce the burden of households from the unpredictable medical expenditure. However, this leads to an increase in the demand for healthcare services especially in long-term care hospitals (LTCHs) in Korea. Methods: This study analyzed the influence factor of medical service overuse of 165,592 inpatients in LTCHs which out-of-pocket maximum is applied, by utilizing data from the National Health Insurance Service (2016). Based on Anderson Model, the medical service overuse, as a dependent variable, was defined as long-stay admission more than 180 days at the LTCHs. Independent variable was comprised of predisposing factors (gender, age), enabling factors (income level, types of out-of-pocket maximum) and need factors (illness level, patient use of tertiary hospital). Results: The most powerful factor of medical service overuse in LTCHs was availability of pre-payment for the out-of-maximum (odds ratio [OR], 191.66; p<0.001). This tendency was found in high income level status (p<0.001). Furthermore, mild inpatients (OR, 1.50; p<0.001) which had no experience with the tertiary hospitals (OR, 2.06; p<0.001) were more relevant to the medical service overuse in LTCHs, compared to the severe inpatients. Conclusion: It is suggested that a separate standard of out-of pocket maximum with regards to LTCHs is required to secure the beneficial functions of long-term hospitals and prevent unnecessary financial leakage to achieve sustainable and financially sound National Health Insurance.

치과 내원환자들의 스켈링과 정기검진 및 전화서비스와의 관련성 (Relationship between scaling, regular check-up and dental telephone services in dental clinic patients)

  • Objectives : The purpose of the study is to investigate satisfaction, awareness and utilization of dental telephone services in dental clinic patients. Methods : A self-reported questionnaire was filled out by 220 dental clinic patients in G metropolitan city from March 11 to September 27, 2013. The data were analyzed by a descriptive analyses, ${\chi}^2$-test and relevant factors were determined using logistic regression analysis by SPSS 12.0. Results : The experience rate of scaling was 68.0% and the average number was $1.04{\pm}1.13$. The experience rate of regular check-up was 41.0% and the average number was $1.01{\pm}1.29$. Satisfaction and utilization rate of scaling and regular checkup was higher than that in those who did not receive the services. The major variables influencing the experience of scaling were woman, utilization of dental telephone service and regular check-up were awareness, utilization of dental telephone service and reason of utilization of dental telephone service(preventive). Conclusions : The dental telephone service in dental patients improved oral health. Accordingly, it is necessary to develop the professional dental hygiene program for oral health in dental patients.

신체억제대에 대한 일반인의 인식, 태도, 지식 및 교육경험과 교육요구도 조사 (Recognition, attitudes, knowledge, education experience, and education demand regarding physical restraint among laypersons)

  • Purpose: This study aimed to investigate recognition, attitudes, knowledge, education experience, and education demand regarding physical restraint among laypersons. Methods: A self-reported questionnaire was completed by 105 randomly recruited laypersons in D city. Excluding 2 incomplete answers, 103 data were analyzed by frequency analysis, t-test, ANOVA, and chi-square test using SPSS 23.0. Results: Laypersons' recognition of physical restraints was rated at 3.37 out of 5 points, and showed a positive perspective. Knowledge on physical restraints was rated at 13.68 out of 18 points and was affected by age and education level (F = 2.845, p = .028)(F = 3.126, p = .029) A majority of the participants had no education on physical restraints. Nevertheless, participants showed intention to receive education in physical restraint to reach further understanding. Conclusion: Education on physical restraints is necessary for lay persons. However, there were education limits for people living in modern times. As an intervention addressing this problem, observing videos on the internet is recommended. Moreover, for advertising/educational purposes, accessing the internet and using smart-phone applications are suggested.

응급구조학과 학생 대상 한국판 시뮬레이션 경험 만족도 도구의 타당도 검증 (Validation of a Korean version of the satisfaction with simulation experience scale for paramedic students)

  • Purpose: This study aims to assess the reliability and validity of a Korean version of the satisfaction with simulation experience (SSE) scale for low-, medium-, and high-fidelity simulations among paramedic students. Methods: Exploratory factor analysis with varimax rotation was used to determine construct validity. Cronbach's alpha was calculated to assess internal consistency reliability. Results: Cronbach's alpha for the overall scale was .841. Exploratory factor analysis yielded a three-factor structure composed of clinical learning, clinical reasoning and debriefing and reflection. Each of the subscales had high internal consistency, with Cronbach's alphas of .852, .790 and .913, respectively. With respect to year, freshmen scored the highest on the SSE scale. With respect to type of simulation, the medium-fidelity group reported the highest satisfaction. Generally, the students were satisfied with debriefing and reflection. Conclusion: The Korean version of the SSE scale was shown to be a reliable and valid instrument for assessing satisfaction with simulation learning in paramedic students.

약물이상반응에 대한 응급구조학과 학생의 지식과 태도 조사 (Investigating attitudes and knowledge of paramedic students regarding adverse drug reactions)

  • Purpose: This study aims to investigate the knowledge and attitudes regarding adverse drug reactions (ADR) of emergency medical services (EMS) students, so that suggestions for further education on ADR can be made. Methods: A survey on knowledge and attitude was created and modified according to Lee's and Kim's test tool. In total, 149 students' data were collected and compared with previous studies. Results: Knowledge of ADR was relatively low (54%) but attitude was relatively high (75%). Clinical experience was important in knowledge and attitude. Knowledge and attitude showed a positive correlation. Conclusion: Increased knowledge of ADR leads to a more active attitude towards it. Hence, modification of the education system to provide a more personalized education is required.

의료취약지 스마트의료에 대한 만족도와 요구도의 결정요인 (Determinants of Satisfaction and Demand for Smart Medical Care in Vulnerable Areas)

  • There are few domestic studies on medical services in medically vulnerable areas where medical use is not met due to a lack of medical resources. The past studies on smart medicine targeting medically vulnerable areas grasp only the overall satisfaction level, or the sub-dimensions of satisfaction are not classified clearly. Also, it lacks consideration of the patient's needs. This study aims to analyze the effect of users' experience of the smart medicine pilot project conducted in medically vulnerable areas on satisfaction and demand. The user's experience was measured by variables in the dimensions of structure, process, and outcome. Among the pilot project participants, 282 subjects responded to the 2019 survey. Using the hierarchical regression method, we tried to find out the determinants of satisfaction and service demands. Experience factors affecting satisfaction were found to be accessibility, certainty, effectiveness, and efficiency. In addition, it was found that the demand in their 60s was high and that accessibility, certainty, effectiveness, and efficiency had a statistically significant effect on the demand. It is expected that the smart medicine pilot project will be effectively operated by well utilizing the factors influencing satisfaction and demand revealed in this study.
