계절리듬이 겐타마이신의 약물동태에 미치는 영향 (The Effect of Seasonal Rhythm on the Gentamicin Pharmacokinetics in Healthy Volunteers)

  • Seasonal rhythmic changes in gentamicin pharmacokinetics was evaluated in 10 healthy male volunteers after single intravenous 80 mg administration of gentamicin at 9:00 a.m. during summer and winter. The mean terminal half-life and AUC of gentamicin were $3.56\pm0.14\;hr\;and\;25.03\pm2.84\;{\mu}g/ml{\cdot}hr$ in winter and $3.08\pm0.41\;hr\;and\;21.84\pm2.51\;{\mu}g/ml{\cdot}hr$ in summer. The mean total body clearance $(CL_t)$ and elimination rate constant $(k_{10})$ of gentamicin was $3.17\pm0.43\;L/hr,\;0.458\pm0.06\;hr^{-1}\;in\;winter\;and\;3.66\pm0.45\;L/hr,\;0.561\pm0.07\;hr^{-1}$ in summer, The mean volumn of distribution $(V_{dss})$ of gentamicin at steady state was $12.65\pm1.09$L in winter and $12.39\pm1.25$ L in summer. Serum concentrations of gentamicin in winter were increased significantly during 4-8 hr (p<0.05) compared to those of gentamicin in summer. The elimination rate constant $(k_{10})$ of gentamicin in winter was decreased significantly $(p<0.05)$ compared to that of gentamicin in summer. The mean volume of distribution at steady state $(V_{dss})$, AUC, mean total body clearance ($CL_t$) and terminal half-life of gentamicin in the winter were increased but were not significant. The mean intrasubject fluctuations in terminal half-life, AUC and $CL_t$ between winter and summer were 8.2, 11.0 and $6.0\%$ respectively.

Effects of Sexual Rehabilitation Using the PLISSIT Model on Quality of Sexual Life and Sexual Functioning in Post-Mastectomy Breast Cancer Survivors

  • Background and Objectives: As one of the most common treatments for breast cancer, mastectomy has adverse effects on the quality of sexual life and sexual functioning in the impacted women. Various strategies have therefore been proposed to resolve their sexual problems. The present study was conducted to determine the effect of sexual rehabilitation using the PLISSIT model in post-mastectomy breast cancer survivors. Materials and Methods: The present quasi-experimental study was conducted on a population of post-mastectomy breast cancer survivors and their husbands. Sample size was calculated as 50 each for intervention and non-intervention groups. The former received sexual counseling based on the PLISSIT model consisting of four levels of intervention: permission, limited information, specific suggestion and intensive therapy, presented in four 90-minute sessions. Data were collected using the Sexual Quality of Life-Female (SQOL-F) questionnaire and the Female Sexual Function Index (FSFI). Results: No significant differences were observed in the mean quality of sexual life scores between the intervention and control groups (P>0.05) before the intervention; however, a significant difference emerged between the groups after the intervention (P<0.01). Thus the mean score for sexual functioning in the intervention group was $26.3{\pm}3.76$ before and $30.0{\pm}4.38$ after the intervention (P<0.0001). In the control group, however, the difference between the pre- and post-intervention mean scores was not statistically significant (P=0.713). Conclusion: The present study showed that nurses can use the PLISSIT model in conjunction with chemotherapy and radiotherapy to teach coping and problem-solving skills to women with breast cancer and their husbands and to encourage their participation in group programs for expressing their feelings and attitudes about their current sex life and thus help enhance quality of sexual life and sexual functioning in this group.

일부지역 성인의 생의 의미 (Meaning in Life of a Resident Community of Normal Adults)

  • Purpose: The purpose of this study was to measure meaning in life for a normal adult and to offer basic data for developing nursing strategies for improving the meaning in life of adults. Method: There were 351 subjects, from 20 to 60 years old, staying in Busan and KyungNam Province. The data was collected from August to October 2000. The instrument was designed by researchers. The data were analyzed with frequency, mean(${\pm}SD$), t test & ANOVA. Result: 1. The mean MIL for adult was 187.7(${\pm}18.1$). 2. The mean MIL of basic premise phase for discovery of meaning was 30.5, method phase for discovery of meaning was 111.1, and result phase for discovery of meaning was 46.4. 3. The levels of MIL, 56.4% of the subjects were in pursuit of meaning in life, 23.3% felt a loss of meaning in life. 4. There were significant differences in MIL according to the level of education(P=0.00), marital status(P=0.01) and monthly income (P=0.00). Conclusion: On the viewpoint of the above results, 23.3% of the subjects were in a level of existential vaccum which required keens Logotherapy. Therefore, it shows that developing nursing-logotherapy to improve the meaning in life for adults is demanded.

만성 관절염 환자의 대응양상에 따른 통증, 우울 및 삶의 질 (The Study of Pain, Depression and Quality of Life According to the Coping Strategy in Chronic Arthritis Patients)

  • Purpose: The aim of this study was to investigate the difference of in the level of pain, depression, and quality of life by coping strategy in the chronic arthritis patients. Method: These data were collected by means of a self-reported questionnaire from 90 chronic arthritis patients undergoing treatment at a university hospital in Daegu city from June 10, 2000 to July 15, 2000. The instruments of study were coping strategy scale by Jeon(1985), pain scale by Lee and Song(1987), depression scale by Radloff(1977) and quality of life scale by Cho(1987). Data analysis was done by SPSS Win 10.0 program and Pearson correlation, t-test, Cronbach's-${\alpha}$ were used. Result: The results of this study were as follows: 1. The item mean score of pain for chronic arthritis patient was 6.54. The item mean score of depression for chronic arthritis patient was 2.18. The item mean score of quality of life for chronic arthritis patient was 5.69. 2. The chronic arthritis patients with problem-focused coping experienced pain higher than those with emotion-focused coping(t=o.400, p=o.o45). The chronic arthritis patients with emotion-focused coping experienced depression higher than those with problem-focused coping(t=-2.564, p=0.012). The chronic arthritis patients with problem-focused coping experienced quality of life higher than emotion-focused coping(t=-1.682, p=0.046). Conclusion: In conclusion this study showed that any coping strategy can control the arthritis patient's problem. Therfore, The development of a coping strategy to reduce pain and depression and improve the quality of life in the chronic arthritis patient will be needed.

와파린 복용 환자의 약물지식, 환자역할행위와 삶의 질과의 관계 (A Study on the Relationship between Drug Related Knowledge, Sick Role Behavior and Quality of Life of Patients Taking Warfarin)

  • Purpose: This study was conducted to investigate the relationship between drug-related knowledge, sick role behavior and quality of life of patients on wafarin therapy in an outpatient unit. Methods: The participants were 122 patients. Data were collected using a questionnaire and analyzed using t-test, ANOVA and Pearson correlation. Results: The Duration of wafarin averaged $28.80{\pm}32.99$ months, and 78.7% of participants remembered their blood coagulation value. The mean score for drug-related knowledge was 11.32 points out of a possible 13 points. Sick role behavior of the patients showed a moderate value with a mean of 49.83 points out of 68 points. The mean score for quality of life was rather low at 104.43 out of 175. Drug-related knowledge was significantly correlated to sick role behavior (r=0.39, p<.001), but the relationship between sick role behavior and quality of life was not significant. Also drug-related knowledge was not significantly correlated with quality of life. Conclusion: The resultsindicate that there is a need to enhance the level of drug-related knowledge in order to increase positive behavior as part of the sick role of these patients and thus improve quality of life.

  • "Bioethics" may have various meanings depending on its roles. It may mean professional ethics for scientists and physicians, etc. It may also mean an academic discipline using interdisciplinary approach as well as a philosophical or a legal approach. "Bioethics" as an interdisciplinary study should often deal with public policy on bioethical issues. I call this role bioethics as a study of bioethics policy, which has to be developed as a new discipline. From this perspective, I deal with bioethical issues relevant to a human life before birth. There are various and often conflicting arguments about the moral status of a human life before birth such as the fertilization argument, the argument of genetic identity, so-called the "14 days" argument focusing on the formation of primitive streak, the argument of sentient being, and Michael Sandle's argument of an embryo as a being between a thing and a person. I argue that each of them is reasonable. Thus we are faced with reasonable disagreement on the views over whether a human life before birth has the same right to life as that of a person or whether right to life may be considered to be a matter of degree. If we acknowledge reasonable disagreement, as John Rawls pointed out, we should tolerate the views from ours in a plural society. Therefore, we cannot help making a policy that allows abortion and embryonic research with some limitations. When we say a certain act is morally permissible, "moral permissibility" does not mean that the act is morally right for all. Rather it means that the act cannot help being morally allowed for some persons although the others do not believe its moral rightness because they cannot right now rationally persuade others to accept their view.

  • In this paper we propose smooth nonparametric estimator of Mean Residual Life(MRL) based on a complete sample. This estimator is constructed using estimator of cumulative failure rate which is derived as the maximum likelihood estimate of cumulative failure rate in the class of distributions which have piecewise linear failure rate functions between each pair of observations. We derive the asymptotic properties of the our estimator. The proposed estimator is compared with previously known estimator by Monte Carlo study.

  • The problem of trend change in the mean residual life is great Interest in the reliability and survival analysis. In this paper, a new test statistic for testing whether or not the mean residual life changes its trend Is developed. It is assumed that neither the change point nor the proportion at which the trend change occurs is known. The asymptotic null distribution of test statistic is established and asymptotic critical values of the asymptotic null distribution is obtained. Monte Carlo simulation is used to compare the proposed test with previously known tests.

  • In a resent paper, Na and Kim(2000) develop a family of test statistics for testing whether or not the mean residual life changes its trend based on complete data and show that the new tests perform better than previously known tests. In this paper, we extend their tests to the randomly censored data. The asymptotic normality of the test statistics is established. Monte Carlo simulations are conducted to compare our tests with a previously known test by the power of tests.

  • In this paper, for whole line ($0,\;{\infty}$), the estimation procedure of mean residual life function using product-limit estimator is studied with asymptotic properties. And also, the small sample properties of proposed estimator of MRLF are investigated through Monte Carlo study and compared with Kaplan-Meier type estimator.
