SBAR를 이용한 의사소통이 간호사의 의사소통 인식과 환자안전에 대한 태도에 미치는 효과 (The Effect of SBAR Communication on Nurse's Perception about Communication and Attitudes toward Patient Safety)

  • Purpose: The purpose of this study was to investigate the effect of SBAR communication program on nurse's perception about communication and attitudes toward patient safety. Methods: A single-group pre-post experimental study was conducted. A SBAR education program was provided to 167 nurses working in 9 general wards of a hospital in Seoul. A total of 153 questionnaires were included for the final analysis. Statistical analysis included analysis of variance, paired sample t-test, and Cochran-Mantel-Haenzel test. Results: After applying SBAR communication education, nurses perceived significant improvement in three of the five categories of communication between nurses and doctors; satisfaction (p=.001), accuracy (p=.001), and understanding (p=.002). The indicators of communication between nurses were also improved significantly in the order of accuracy (p=.001), satisfaction (p=.001), shift communication (p=.001), and openness (p=.016). The scores of nurse's attitudes toward patient safety demonstrated a significant increase in the five categories out of the six; perception of management (p=.001), working condition (p=.001), safety climate (p=.001), teamwork climate (p=.001), job satisfaction (p=.012). Conclusion: It is recommended that nurses and doctors use SBAR communication in their practice. Developing education programs and utilization methods is required for the effective establishment of SBAR communication.

제 2형 당뇨병 환자의 흡연여부에 따른 당화혈색소와 당뇨병성 만성합병증 (Glycated Hemoglobins and Chronic Complications of Diabetes Mellitus, based on the Smoking Status of Patients with Type 2 Diabetes Mellitus)

  • Purpose: This research seeks to identify differences between general characteristics, disease-related characteristics, glycated hemoglobin (HbAlc) levels, and aspects of chronic complications of diabetes mellitus in type 2 diabetes. Methods: This research was conducted from the 1st to the 15th of February in 2016, on 263 in patients. Patients' electronic medical records were used to identify their general characteristics, disease-related characteristics, HbAlc, and chronic diabetic complications. Chi-square test, ANOVA, ANCOVA, and the Cochran-Mantel-Haenszel test were used for data analysis. Results: Statistical significance was observed for general characteristics, based on smoking status, such as age, and education level; disease-related characteristics differed according to the duration of diabetes. Smoking status did not differ according to HbAlc level. In term of chronic diabetic complications, statistically significance was observed for diabetic nephropathy, based on smoking status. Conclusion: Patients who had a history of smoking, but were not currently smoking, were likely to display higher HbAlc levels and diabetic nephropathy. Therefore, there is need for regular checkups for diabetic complications among patients with a history of smoking and it is important to emphasize smoking cessation.

황벽나무 자연집단의 유전다양성 및 유전구조 분석 (Genetic Diversity and Genetic Structure of Phellodendron amurense Populations in South Korea)

  • 본 연구는 ISSR 표지자를 이용하여 국내 분포하는 황벽나무 7개 집단의 유전다양성과 유전구조를 분석하였다. 6개의 ISSR primer를 이용하여 분석한 결과 primer 당 평균 4.5개의 다형성 band를 확인하였고, 각 집단의 다형성 유전자좌의 비율은 평균 78.8%로 나타났다. Shannon의 유전다양성 지수(I)는 0.421로 나타났고, 이형접합체 기대치($H_e$)는 평균 0.285로 베이즈 방법을 이용한 평균 이형접합체 기대치(hs=0.287)와 유사하였다. AMOVA에서 전체 유전변이의 92.4%가 집단내 개체간 차이에 기인하며, 7.6%는 집단간 차이에 기인하였다. 베이즈 방법을 이용한 유전분화(${\theta}^{II}$)는 0.066으로 추정되었으며, 전체 집단의 근친교배율(f)은 0.479로 계산되었다. 유연관계 분석과 베이즈 군집분석결과 우리나라 황벽나무 집단은 가평, 화천, 봉평, 용평이 하나의 군집을 형성하였고, 산청 지역의 2개 집단(삼장 및 시천)이 다른 하나의 군집을 형성하였으며, 무주 집단이 산청지역의 집단과 지리적으로 근접함에도 불구하고 독립적인 군집을 나타내었다. Mantel's test 결과 집단간 유전적 유연관계와 지리적 분포의 상관성은 나타나지 않았다. 황벽나무의 유전자원보존을 위한 대상 집단 선정 시 생태적 및 생활사적 특징과 함께 본 연구결과에서 나타난 유전다양성과 군집구조 분석결과를 고려하는 것이 효과적일 것으로 사료된다.

수박 시판 품종의 식별을 위한 Genomic과 Expressed Sequence Tag (EST)에서 유래된 Microsatellite Marker의 이용 (Use of Microsatellite Markers Derived from Genomic and Expressed Sequence Tag (EST) Data to Identify Commercial Watermelon Cultivars)

  • 국내에서 시판되고 있는 수박 102 품종에 대한 DNA 프로 파일 데이터베이스를 구축하기 위하여 genomic microsatellite(gMS)와 expressed sequence tag(EST) microsatellite(eMS) 마커의 다형성 정도의 비교와 유전적 연관성 분석을 통한 품종식별력 검정 등에 대한 일련의 연구를 수행하였다. 수박 gMS 마커를 이용하여 국내에서 시판되고 있는 수박 102 품종을 검정하였을 때 마커당 3.63개의 평균 대립유전자가 검출되었으며, 평균 PIC 값은 0.479로 나타났다. 이에 반해 eMS 마커는 평균 대립유전자의 수가 2.50개, PIC 값이 0.425로 나타나 gMS 마커보다 다형성 정도가 낮게 나타났다. gMS와 eMS 및 이들 두 종류의 마커를 병합하여 작성된 계통도는 microsatellite 마커의 다형성에 따라 수박 102개 품종을 6-8개의 그룹으로 구분하였고 대부분의 품종의 식별이 가능하였다. 3가지 마커 유형에 따라 작성된 계통도를 Mantel test에 의해 상관 정도를 분석하였을 때 높은 상관($r{\geq}0.80$)을 나타내었다. 따라서 본 연구에 활용된 microsatellite 마커는 수박 유전자원의 특성평가, 순도검정 및 품종의 지문화 작업의 수단으로 유용하게 활용될 수 있을 것이다.

ESTP 표지를 이용한 국내 소나무 집단의 유전변이 (Genetic Variation of Pinus densiflora Populations in South Korea Based on ESTP Markers)

  • 소나무의 유전다양성과 유전구조를 추정하기 위해 9개의 ESTP 표지를 13개 소나무 집단에 적용하였다. 소나무 집단의 유전다양성은 관찰된 대립유전자 수(A)가 2.2개, 유효 대립유전자 수(Ae)가 1.8개, 다형적 유전자좌 비율(P)이 98.8%, 이형접합도 관찰치(Ho)가 0.391, 이형접합도 기대치(He)가 0.402로 나타났다. 안강과 강릉 집단을 제외한 11개 집단이 하디-바인베르그 평형을 만족하였다. 집단간 유전분화도(FST)는 0.057으로, 동위효소나 nSSR 표지분석 결과보다 강하게 나타났다. 군집분석에서 집단의 유전적 거리와 지리적 분포간에 뚜렷한 연관성은 확인할 수 없었으며, 집단의 유전분화와 지리적 인접성도 상관이 없는 것으로 나타났다(Mantel 검증, r = 0.017, P = 0.344). 유전자좌에 대한 FST-outlier 분석을 실시한 결과, 빈도주의 방법에서는 FST 값이 신뢰하한 이하인 3개 유전자좌와 신뢰상한 이상인 3개 유전자좌가 특이값으로 추정되었고, 베이즈 방법에서는 3개 유전자좌들만 특이값으로 확인되었다. 두 방법에서 공히 특이값으로 판정된 3개 유전자좌(sams2+AluⅠ, sams2+RsaⅠ, PtNCS_p14A9+HaeⅢ)중 sams2 표지에서 유래된 2개 유전자좌는 balancing selection의 영향을 받는 것으로 추정되었다.

Geostatistical Analysis of Soil Enzyme Activities in Mud Flat of Korea

  • Spatial variations of physicochemical and microbiological variables were examined to understand spatial heterogeneity of those variables in intertidal flat. Variograms were constructed for understanding spatial autocorrelations of variables by a geostatistical analysis and spatial correlations between two variables were evaluated by applications of a Cross-Mantel test with a Monte Carlo procedure (with 999 permutations). Water content, organic matter content, pH, nitrate, sulfate, chloride, dissolved organic carbon (DOC), four extracellular enzyme activities (${\beta}-glucosidase$, N-acetyl-glucosaminidase, phosphatase, arylsulfatase), and bacterial diversity in soil were measured along a transect perpendicular to shore line. Most variables showed strong spatial autocorrelation or no spatial structure except for DOC. It was suggested that complex interactions between physicochemical and microbiological properties in sediment might controls DOC. Intertidal flat sediment appeared to be spatially heterogeneous. Bacterial diversity was found to be spatially correlated with enzyme activities. Chloride and sulfate were spatially correlated with microbial properties indicating that salinity in coastal environment would influence spatial distributions of decomposition capacities mediated by microorganisms. Overall, it was suggested that considerations on the spatial distributions of physicochemical and microbiological properties in intertidal flat sediment should be included when sampling scheme is designed for decomposition processes in intertidal flat sediment.

업무상 정신질환과 자살 산업재해 신청자 추이 및 특성 분석 (An Analysis of Trends and Characteristics of Work-Related Mental Disorders in Korean Workers)

  • Purpose: This study was conducted to analyze the annual trends and characteristics of work-related mental disorders including suicide among Korean workers. Methods: Employing the Industrial Accident Compensation Insurance (IACI) data collected by the Korea Worker's Compensation & Welfare Service, we analyzed total work-related mental disorders and suicide cases (n=455) claimed to IACI from 2008 to 2012. We paid a special attention to workers' socioeconomic status and occupational characteristics. Results: The number of injured workers and industrial accident rate decreased during the study period, whereas the case of work-related mental disorders showed significant increase. Mental disorders cases were more prevalent in workers who were male, aged the 40s, and engaged in finance, as well as with 1-5 years of working duration than their respective counterparts. Increasing trend of suicide cases was statistically significant by the Cochran-Mantel-Haenszel test (p=.002). Conclusion: This study suggests that work-related mental disorder management strategy and system should be established.

A Meta-Analysis on the Associations of Gly460Trp Polymorphism of the $\alpha$-Adducin Gene with Hypertension

  • Gly460Trp polymorphism of a-adducin gene has been reported to be associated with hypertension in some populations, and we, therefore, attempted to replicate this finding in Korean population. There were no significant differences in allele and genotype frequencies of Gly460Trp polymorphism in Korean normotensives and hypertensives, respectively. The meta-analysis revealed that the Mantel-Haenszel estimate of the odds ratio across the studies was 1.17 (95%CI 1.04-1.31), and that there was significant evidence against homogeneity of the odds ratio among the studies included (Breslow-Day test = 27.34, df = 9, p = 0.001). Although the meta- analysis appeared in favour of association between the Gly4601tf polymorphism of $\alpha$-adducin gene and hypertension, there was the considerable heterogeneity among the studies and the evidence is also rather borderline. Further comprehensive approaches are needed to resolve this debatable issue.

고등학생의 수면 부족, 비만, 스크린 타임 사이의 관련성 연구 (Relationships between Short Sleep, Obesity, and Screen Time in High School Students)

  • Purpose: The purpose of this study was to investigate relationships between short sleep, obesity, and screen time. Methods: Data were obtained from a survey using a sample of 1,166 academic, and 597 vocational high school students. A structured questionnaire was used to assess demographics, hours spent in sleep, TV viewing, internet usage, and game playing. Obesity was determined using the body mass index. Descriptive statistics, Cochran Mantel Haenszel Chi-square test, and multiple logistic regression with dummy variables were performed. Results: Short sleep (<8 hours/day) was significantly related to obesity, not in academic school students, but in vocational school students. Academic high school students who had enough sleep (${\geq}8$ hours/day), had more time spent on TV viewing, internet usage, and game playing. Similar results were found in vocational high school students except for internet use. Interestingly, students with better school performance spent significantly less time in TV viewing, internet usage, and game playing, and therefore had a lower risk of obesity in academic school students. Conclusion: There is a significant relationship between short sleep and obesity that was found only in vocational high school students. The relationship between short sleep and screen time was not significant in Korean high school students.

Comparison of RAPD, AFLP, and EF -1 α Sequences for the Phylogenetic Analysis of Fusarium oxysporum and Its formae speciales in Korea

  • Although Fursarium oxysporum causes diseases in economically important plant hosts, identification of F. oxysporum formae speciales has been difficult due to confusing phenotypic classification systems. To resolve these complexity, we evaluated genetic relationship of nine formae speciales of F. oxysporum with random amplified polymorphic DNA (RAPD), amplified fragment length polymorphism (AFLP), and translation elongation factor-l alpha ($EF-1{\alpha}$) gene. In addition, the correlation between mycotoxin content of fusaric acid and isolates based on molecular marker data was evaluated using the modified Mantel's test. According to these result, these fusaric acid-producing strains could not identify clearly, and independent of geographic locations and host specificities. However, in the identification of F. oxysporum formae speciales, especially, AFLP analysis showed a higher discriminatory power than that of a the RAPD and $EF-1{\alpha}$ analyses, all three techniques were able to detect genetic variability among F. oxysporum formae speciales in this study.
