Effectiveness of an extraoral cold and vibrating device in reducing pain perception during deposition of local anesthesia in pediatric patients aged 3-12 years: a split-mouth crossover study

  • Background: Local anesthetic injections may induce pain in children, leading to fear and anxiety during subsequent visits. Among the various approaches recommended to reduce pain, one is the use of a Buzzy BeeTM device that operates on the concept of gate control theory and distraction. The literature regarding its effectiveness during the deposition of local anesthesia remains limited; hence, the aim of the present study was to determine the efficacy of extraoral cold and vibrating devices in reducing pain perception during the deposition of local anesthesia. Methods: A split-mouth crossover study in which 40 children aged 3-12 years requiring maxillary infiltration or inferior alveolar nerve block for extractions or pulp therapy in the maxillary or mandibular posterior teeth were included. The control intervention involved the application of topical anesthetic gel for one minute (5% lignocaine gel), followed by the administration of local anesthetic (2% lignocaine with 1:80,000 adrenaline) at a rate of 1 ml/ minute. Along with the control protocol, the test intervention involved using the Buzzy BeeTM device for 2 minutes before and during the deposition of the local anesthetic injection. The heart rate and face, legs, arms, cry, and consolability revised (FLACC-R) scale scores were recorded by the dentist to assess the child's pain perception. Results: The mean age of the participants in Group A and Group B was 7.050 ± 3.12 years and 7.9 ± 2.65 years respectively. A reduction in the mean heart rate and FLACC-R score was observed during the deposition of local anesthetic solution in the tissues when the Buzzy BeeTM was used in both groups at different visits in the same subjects (P < 0.05) The Buzzy BeeTM device was effective in reducing the heart rate and FLACC-R scores when used during maxillary infiltration and inferior alveolar nerve block local anesthesia techniques (P < 0.05). Conclusion: The use of extraoral cold and vibrating devices significantly reduces pain perception during local anesthetic deposition in pediatric patients. Considering the results of this study, the device may be incorporated as an adjunct in routine dental practice while administering local anesthesia in children.

A Study on Estimations and Long-term Forecasts of SO$_2$ Pollution in Each City & County of Korea (시군별 이산화황(SO$_2$) 오염도의 현황 진단과 장기 예측에 관한 연구)

  • It is more likely that the degree of air quality degradation that we have faced would be much lessen, if integrated management of air pollution control and assessment had been fully enforced by the local administrations, not by the centralized environmental agency in the first place. As the selfgoverning local administrations have been established since 1995, the need for air quality control by the local administration or local agency is getting the ground. However, in practice, air quality control by the local administration rarely put into effect due to lack of basic data which cover the present trends of air quality in each local city or county and are necessary for decision making. The emissions of SO$_2$ in each city and county of Korea in 1993 were calculated in this study, based on energy consumptions and emission factors. The ambient concentrations of SO$_2$ also were estimated by applying modified Miller-Holtzworth model. Observed and estimated concentrations of SO$_2$ showed that about 17.5 percents of cities and counties in the country were more polluted than the target value, 20ppb/year. The emissions and ambient concentrations of SO$_2$ in each city and county in 2000, 2005, and 2010 were also forecasted, assumed business as usual senario. It was shown that, in 2010, the emissions of SO$_2$ will be 2.8 times more than those of 1993 and much of them are from industrial sector. Also shown that 38.3 percents of cities and counties will be more polluted than the target value and most of them are polluted areas in 1993. The methods and results of this study could be used in developing the efficient reduction strategies in each city and county.

Effect of Shading on Shoot Growth and Quality of Sedum Sarmentosum in Korea (차광재배가 돌나물의 수량 및 품질에 미치는 영향)

  • To product the edible fresh shoots of Sedum sarmentosum in summer season, 4 local strains were cultivated with cutting propagation under 0, 30, 50, 70, 90% shading. Effect of shading on shoot growth, leaf chroma value, SPAD value, and bitterness were investigated at 35 days after cutting. Plant height, the number of node and leaf stem diameter in $30{\sim}90%$ shading were increased than in the control, and Pohang local strain had little-overgrowth shoot, thicker stem, a few brenches per shoot, and larger leaf compared to the other local strains. Fresh and dry weight under shading were increased than in the control, dry weight of pohang local strain was highest in 50% shading. Hunter L and b values were decreased out of proportion to higher shading rate. SPAD value in $50{\sim}90%$ shading was significantly lower than in the control. The bitterness of fresh shoot was decreased out of proportion to higher shading rate. In Summer season, producing the fresh shoot of S. sarmentosum was excellent in 50% shading, and Pohang local strain among 4 local strains was more stable and good in yield and quality under shading.

Physiological Responses of Wearing Industrial Gaiters in a Hot Environment (더운 환경에서 산업용 각반 착용 시 인체생리반응)

  • The gaiter is one of the personal protective equipments worn in various industrial sites. This study was performed on humans to investigate the physiological strain of wearing gaiters and to compare control gaiters that are currently on the market and new gaiters that are developed for alleviating heat stress. Experiments were conducted in a climatic chamber of WBGT $30.0\pm0.7^{\circ}C$ under five differed experimental conditions: None, Control A, Control B, New A, New B. The results were as follows. The temperature inside gaiters was significantly lower in both New A and New B than in both Control A and Control B and the difference between news and controls was 1$^{\circ}C$ (p<.01). The humidity inside gaiters in both New A and New B were higher than that in Control A, and lower than that in Control B (p<.01). The outermost surface temperature of the gaiter was the lowest in None and it increased in the following order: New B < New A < Control A < Control B. Mean skin temperature was higher by 0.14$^{\circ}C$ in wearing gaiters than in no gaiters. Skin temperatures in lower body were lower in Control than in New and skin temperature in upper body were higher in Control than in New (p<.01). Local sweat rate, total weight loss and subjective sensations did not show a significant difference according to the gaiters. It was concluded that wearing gaiters affected distribution of skin temperature and local sweat rate.

  • To improve the efficiency and safety of railway systems, the train control system has been considerably evolved from the ground-equipment-based control system (e.g. track circuit, interlocking system, etc.) into the on-board-equipment-based control system. In addition, this train control system enables the rolling stock to intelligently control the trackside facilities by introducing the information and communication technologies (ICT). Accordingly, since the ICT-based train control system makes the railway system be simplified (i.e. the heavy ground-equipment can be removed), the efficient and cost-effective railway system can be realized. In this paper, we perform the feasibility test of the ICT-based train control system via a simulation. To this end, we have developed the prototypes of the on-board controller and wayside object control units which control the point and crossing gate and performed the integrated operation simulation in a testbed. In this paper, before the field test of the on-board-controller-based train control system, we perform the Consecutive operation test for prototypes of the on-board controller, wayside object control units and local control computer.

  • In this paper, to alleviate the effect of approximation error and discontinuous variation of the controller parameters, the variable structure control scheme using neural networks is presented. In the proposed method, the variable structure control rules for each local linear models are designed to reject the effect of linearization error caused by linearization of the nonlinear system. And neural network infer approximate controller gains from combination of local linear control gains. The proposed control methods can be used to control nonlinear systems and it has robust characteristic against system parameter variations and external disturbances.

  • The DTL water skid cooling system and Resonant Control Cooling Systems (RCCS) will employ a control system that can be operated by a local, programmable logic controller (PLC), interfaced through a touch screen interface, mobile alarm SMS server system, or it can be operated through the PEFP global control system network. The RCCS is implemented using Experimental Physics and Control System (EPICS) based hardware and software and is integrated with other networked PEFP EPICS systems. This presentation discusses the features of the local control system.

Negative impact of pretreatment anemia on local control after neoadjuvant chemoradiotherapy and surgery for rectal cancer

  • Purpose: Although anemia is considered to be a contributor to intra-tumoral hypoxia and tumor resistance to ionizing radiation in cancer patients, the impact of pretreatment anemia on local control after neoadjuvant concurrent chemoradiotherapy (NACRT) and surgery for rectal cancer remains unclear. Materials and Methods: We reviewed the records of 247 patients with locally advanced rectal cancer who were treated with NACRT followed by curative-intent surgery. Results: The patients with anemia before NACRT (36.0%, 89/247) achieved less pathologic complete response (pCR) than those without anemia (p = 0.012). The patients with pretreatment anemia had worse 3-year local control than those without pretreatment anemia (86.0% vs. 95.7%, p = 0.005). Multivariate analysis showed that pretreatment anemia (p = 0.035), pathologic tumor and nodal stage (p = 0.020 and 0.032, respectively) were independently significant factors for local control. Conclusion: Pretreatment anemia had negative impacts on pCR and local control among patients who underwent NACRT and surgery for rectal cancer. Strategies maintaining hemoglobin level within normal range could potentially be used to improve local control in rectal cancer patients.

Study on Local Path Control Method based on Beam Modeling of Obstacle Avoidance Sonar (장애물회피소나 빔 모델링 기반의 국부경로제어 기법 연구)

  • Recently, as the needs of developing the micro autonomous underwater vehicle (AUV) are increasing, the acquisition of the elementary technology is urgent. While they mostly utilizes information of the forward looking sonar (FLS) in conventional studies of the local path control as an elementary technology, it is desirable to use the obstacle avoidance sonar (OAS) because the size of the FLS is not suitable for the micro AUV. In brief, the local path control system based on the OAS for the micro AUV operates with the following problems: the OAS offers low bearing resolution and local range information, it requires the system that has reduced power consumption to extend the mission execution time, and it requires an easy design procedure in terms of its structures and parameters. To solve these problems, an intelligent local path control algorithm based on the beam modeling of OAS with the evolution strategy (ES) and the fuzzy logic controller (FLC), is proposed. To verify the performance and analyze the characteristic of the proposed algorithm, the course control of the underwater flight vehicle (UFV) is performed in the horizontal plane. Simulation results show that the feasibility of real application and the necessity of additional work in the proposed algorithm.

Comparison of Local Educational Autonomy between Korea and the U.S. (한국과 미국의 지방교육자치 구조의 비교)

  • The purpose of this study was to comparatively analyze local educational autonomy(LEA) between Korea and the U.S. and investigate the new directions that Korea can follow. To do so, a comparative analysis was employed to study the background, development process, principles and structural elements of the respective LEAs. The conclusions drawn from the study were as follows: First, the structure of the Korean LEA should be selected and established. Second, the LEA principles should be discussed again and decided anew to accomplish the goals of the LEA by mobilizing all local potential resources. Third, 'control by people,' one of essential LEA principles, should be expanded to include the participation of the people.
