The study was conducted targeting 25 patients who underwent the respiratory gated radiation therapy in the abdominal region at Radiation Oncology of a University Hospital from December 2013 to June 2014 and types of cancer included liver(64%), CBD(8%), gastric(8%), GB(8%), pancreas(8%), SMA(4%). The means of ITV and PTV volume are 471.44 cm3 and 425.48 cm3, showing an increase in volume. Normal tissue volume was also found to have increased due to the increase of the section selected from PTV section to ITV section. Right kidney showed a significant increase in differences between increase in normal tissue volume, increase in target volume and increase in therapy irradiation area and difference between the means of dose applied to normal tissue. There was no significant difference in the mean dose applied to normal tissue according to the respiratory average. Both kidneys showed a significant difference in the difference between mean doses of target moving and normal tissue. In this study, both therapy methods through PTV section and ITV section volume setting were appropriate for protection doses of normal tissue and distributed over 95% of the prescribed dose and therefore, it is considered to be okay to be optionally used depending on the patient's therapeutic purpose. But in order to minimize the unexpected side effect, the plan of PTV section and ITV section should be established and used by evaluating normal tissue protection dose.

In this paper, we evaluated the performance of 3D CRT, IMRT and three kind of RA plannings to investigate the clinical effect of RA with liver cancer case. The patient undergoing liver cancer of small volume and somewhat constant motion were selected. We performed 3D CRT, IMRT and RA plannings such as 2RA, limited triple arcs (3RA) and 3MRA with Eclipse version 8.6.15. The same dose volume objectives were defined for only CTV, PTV and body except heart, liver and partial body in IMRT and RA plannings. The steepness of dose gradient around tumor was determined by the Normal Tissue Objective function with the same parameters in place of respective definitions of dose volume objectives for the normal organs. The approach between the defined dose constraints and the practical DVH of CTV, PTV and Body was the best in 3MRA and the worst in IMRT. The DVHs were almost the same among RAs. Plans were evaluated using Conformity Index (CI), Homogeneity Index (HI) and Quality of coverage (QoC) by RTOG after prescription with dose level surrounding 98% of PTV in the respective plans. As a result, 3MRA planning showed the better favorable indices than that of the others and achieved the lowest MUs. In this study, RA planning is a technique that is possible to obtain the faster and better dose distribution than 3D CRT or IMRT techniques. Our result suggest that 3MRA planning is able to reduce the MUs further, keeping a similar or better targer dose homogeneity, conformity and sparing normal tissue than 2RA or 3RA.

Objectives The purpose of this study is to report the possibility of treatment of liver cirrhosis with traditional Korean medical treatment. Methods A 57-year-old man diagnosed with hepatocellular carcinoma and liver cirrhosis was admitted to Wonkwang University Oriental Medical Hospital in August 2014. He underwent liver lobectomy and later experienced abdominal pain, anorexia, and insomnia. To control cirrhotic ascites and prevent relapses, he opted to receive HangAmDan-B (HAD-B) and cultivated wild ginseng herbal acupuncture therapy. Results After treatment, the volume of ascites decreased and the patient&’s symptoms such as abdominal pain and anorexia improved. Conclusions This report suggests that HAD-B and cultivated wild ginseng herbal acupuncture therapy may play a therapeutic role in liver cirrhosis occurring after liver lobectomy.

In this proposed study, observing and analyzing contrast enhanced abdominal CT images, we segmented the liver automatically. We computed the ratio of each gray value from the estimated gray value range. With the average value of mesh image, we distinguished the liver from the noise parts. We divided the region based on immersion simulation. The threshold value is determined from the mesh image which is generated from each gray value portion of the liver and is used in dividing the liver to the noise region. To get the outline of the liver, we generated template image which represents the lump of the liver, and subtracted it from the binary image. With the results we use the proposed algorithm using 8-connectivity instead of the present opening algorithm, to reduce the processing time. Purpose: To evaluate the outcomes of stereotactic body radiation therapy (SBRT) for patients with liver oligo-recurrence and oligo-progression from various primary tumors. Materials and Methods: Between 2002 and 2013, 72 patients with liver oligo-recurrence (oligo-metastasis with a controlled primary tumor) and oligo-progression (contradictory progression of a few sites of disease despite an overall tumor burden response to therapy) underwent SBRT. Of these, 9 and 8 patients with uncontrollable distant metastases and patients immediate loss to follow-up, respectively, were excluded. The total planning target volume was used to select the SBRT dose (median, 48 Gy; range, 30 to 60 Gy, 3-4 fractions). Toxicity was evaluated using the Common Toxicity Criteria for Adverse Events v4.0. Results: We evaluated 55 patients (77 lesions) treated with SBRT for liver metastases. All patients had controlled primary lesions, and 28 patients had stable lesions at another site (oligo-progression). The most common primary site was the colon (36 patients), followed by the stomach (6 patients) and other sites (13 patients). The 2-year local control and progression-free survival rates were 68% and 22%, respectively. The 2- and 5-year overall survival rates were 56% and 20%, respectively. The most common adverse events were grade 1-2 fatigue, nausea, and vomiting; no grade ${\geq}3$ toxicities were observed. Univariate analysis revealed that oligo-progression associated with poor survival. Conclusion: SBRT for liver oligo-recurrence and oligo-progression appears safe, with similar local control rates. For liver oligo-progression, criteria are needed to select patients in whom improved overall survival can be expected through SBRT.

Success rate of transplantation of body organs improved due to development of medical equipment and diagnostic technology. In particular, a liver transplant due to liver dysfunction has increased. With the development of image processing and analysis to obtain the volume for liver transplantation have increased the accuracy and efficiency. In this thesis, we try to reconstruct the regions of the liver within three dimensional images using the mevislab tool, which is effective in quick comparison and analysis of various algorithms, and in expedient development of prototypes. Liver is divided by applying threshold values and region growing method to the original image, and by removing noise and unnecessary entities through morphology and region filling, and setting of areas of interest. It is deemed that high temporal efficiency, and presentation of diverse range of comparison and analysis module application methods through usage of MeVisLab would make contribution towards expanding of baseline of medical image processing researches.

The purpose of this study is to evaluate the variation of radiation dose distribution for liver tumor located in liver dome and for the interest organs(normal liver, kidney, stomach) with the pencil beam convolution (PBC) algorithm versus anisotropic Analyticalal algorithm (AAA) of the Varian Eclipse treatment planning system, The target volumes from 20 liver cancer patients were used to create treatment plans. Treatment plans for 10 patients were performed in Stereotactic Body Radiation Therapy (SBRT) plan and others were performed in 3 Dimensional Conformal Radiation Therapy (3DCRT) plan. dose calculation was recalculated by AAA algorithm after dose calculation was performed by PBC algorithm for 20 patients. Plans were optimized to 100% of the PTV by the Prescription Isodose in Dose Calculation with the PBC algorithm. Plans were recalculated with the AAA, retaining identical beam arrangements, monitor units, field weighting and collimator condition. In this study, Total PTV was to be statistically significant (SRS: p=0.018, 3DCRT: p=0.006) between PBC and AAA algorithm. and in the case of PTV, ITV in liver dome, plans for 3DCRT were to be statistically significant respectively (p=0.013, p=0.024). normal liver and kidney were to be statistically significant (p=0.009, p=0.037). For the predictive index of dose variation, CVF ratio was to be statistically significant for PTV in the liver dome versus PTV (SRS r=0.684, 3DCRT r=0.732, p<0.01) and CVF ratio for Tumor size was to be statistically significant (SRS r=-0.193, p=0.017, 3DCRT r=0.237, p=0.023).
To investigate the effect of low magnetic field on dose distribution in SABR plans for liver cancer, we calculated and evaluated the dose distribution to each organ with and without magnetic fields. Ten patients received a 50 Gy dose in five fractions using the $ViewRay^{(R)}$ treatment planning system. For planning target volume (PTV), the results were analyzed in the point minimum ($D_{min}$), maximum ($D_{max}$), mean dose ($D_{mean}$) and volume receiving at least 90% ($V_{90%}$), 95% ($V_{95%}$), and 100% ($V_{100%}$) of the prescription dose, respectively. For organs at risk (OARs), the duodenum and stomach were analyzed with $D_{0.5cc}$ and $D_{2cc}$, and the remained liver except for PTV was analyzed with $D_{mean}$, $D_{max}$, and $D_{min}$. Both inner and outer shells were analyzed with the point $D_{min}$, $D_{max}$, and $D_{mean}$, respectively. For PTV, the maximum change in volume due to the presence or absence of the low magnetic field showed a percentage difference of up to $0.67{\pm}0.60%$. In OAR analysis, there is no significant difference for the magnetic field. In both shell structure analyses, although there are no major changes in dose distribution, the largest value of deviation for $D_{max}$ in the outer shell is $2.12{\pm}2.67Gy$. The effect of low magnetic field on dose distribution by a Co-60 beam was not significantly observed within the body, but the dose deposition was only appreciable outside the body.

Purpose : To evaluate influences associated with radiation treatment planning obtained with the patient breathing freely. Materials and Methods : We compared reduction or elimination of planning target volume (PTV) margins with 2-D conventional plan with inclusion of PTV margins associated with breathing with 3-D conformal therapy. The respiratory non gated 3-D conformal treatment plans were compared with respiratory gated conventional 2-D plans in 4 patients with hepatocellular carcinomas. Isodose distribution, dose statistics, and dose volume histogram (DVH) of PTVs were used to evaluate differences between respiratory gated conventional 2-D plans and respiratory non gated 3-D conformal treatment plans. In addition. the risk of radiation exposure of surrounding normal liver and organs are evaluated by means of DVH and normal tissue complication probabilities (NTCPs). Results : The vertical movement of liver ranged 2-3 cm in all patients. We found no difference between respiratory gated 2-D plans and 3-D conformal treatment plans with the patients breathing freely. Treatment planning using DVH analysis of PTV and the normal liver was used for all patients. DVH and calculated NTCP showed no difference in respiratory gated 2-D plans and respiratory non gated 3-D conformal treatment plans. Conclusion : Respiratory gated radiation therapy was very important in hepatic tumors because radiation induced hepatitis was dependent on remaining normal liver volume. Further investigational studies for respiratory gated radiation.
Purpose: To retrospectively analyze dosimetric parameters of volumetric-modulated arc therapy (VMAT) and three-dimensional conformal radiotherapy (3D-CRT) delivered to extranodal marginal zone B-cell lymphomas of mucosa-associated lymphoid tissue in the stomach (gastric MALT lymphoma) to find out advantages of VMAT and conditions for definite benefits of VMAT. Materials and Methods: Fifty patients with stage I-II gastric MALT lymphoma received VMAT (n = 14) or 3D-CRT (n = 36) between December 2005 and April 2018. Twenty-seven patients were categorized according to whether the planning target volume (PTV) overlaps kidney(s). Dosimetric parameters were analyzed by dose-volume histogram. Results: Radiation dose to the liver was definitely lower with VMAT in terms of mean dose (p = 0.026) and V15 (p = 0.008). The V15 of the left kidney was lower with VMAT (p = 0.065). For those with PTV overlapping kidney(s), the left kidney V15 was significantly lower with VMAT. Furthermore, the closer the distance between the PTV and kidneys, the less the left kidney V15 with VMAT (p = 0.037). Delineation of kidney(s) by integrating all respiratory phases had no additional benefit. Conclusions: VMAT significantly increased monitor units, reduced treatment time and radiation dose to the liver and kidneys. The benefit of VMAT was definite in reducing the left kidney V15, especially in geometrically challenging conditions of overlap or close separation between PTV and kidney(s).
