Aplastic anemia is a hematopoietic disorder characterized by marked reduction or absence of erythoid, granulocytic, and megakariocytic cells in the bone marrow with resultant pancytopenia. To control of infection & bleeding secondary to leukopenia and thrombocytopenia, the inflammatory lesions in oral & maxillofacial area should be removed. Material & Method: The extractions were performed on 21 patients with severe aplastic anemia. The initial, pre-operative and postoperative CBCs were checked up. Amount and kind of transfused platelet in each patient and increment of platelet level were recorded. The complications were documented. Result : A mean of 2.9 teeth were extracted from each patient(ranging between 1 and 13). Furthermore, surgical extractions including ostectomy and odontectomy of the third molar were performed on 11 patients. The preoperative WBC levels presented between $600/{\mu}L$ and $5000/{\mu}L$(mean $2376/{\mu}L$). The WBC values decreased by an average of $145/{\mu}L$ per patient after extractions had been performed. The teeth of 16 patients were extracted under 10.0g/dL, and the mean change in postoperative hemoglobin levels in comparison with preoperative hemoglobin levels was -0.06 per patient. The initial platelet levels were between 1000/(L and $81,000/{\mu}L$(mean $20,174/{\mu}L$). In five patients, extractions were performed with platelet levels less than $50,000/{\mu}L$. Conclusion : The results suggest that more active and preventive treatments in the oral and maxillofacial area are possible and are necessary to remove the infectious foci on the patients with severe aplastic anemia. We report the results of our experiences and literature reviews in treatment of the patients with severe aplastic anemia in our department.
Background and Objectives: Necrotizing lymphadenitis or Kikuchi's disease is characterized by cervical lymphadenopathy of unknown etiology with unique histologic findings in young female patients. The importance of this disease lies in the fact that it can be easily misdiagnosed as malignant lymphoma, hence, clinicians need to aware of this disease entity. The purpose of this study is to report the clinicopathologic findings, radiographic findings, and many laboratory tests in order to contribute to the diagnosis and treatment of necrotizing lymphadenitis. Materials and Methods: We evaluated 31 patients, who were diagnosed as necrotizing lymphadenitis by excisional biopsy or fine needle aspiration cytology or ultrasound guided 18G cutting needle biopsy, retrospectively. Result : The median age was 24.8 years (range 12 to 43 years) and the male to female ratio was 1 : 2.4(9:22), with 14 females (45.1%) under 30 years. The common chief complaints were neck mass, easy fatigue and fever. Lymph node enlargement was limited to the cervical area in most cases (28cases : 90.3%). The involved lymph nodes were usually multiple (20cases : 64.5%), unilateral (26cases 83.9%) and small sized. Leukopenia (19cases : 61.3%) and elevation of ESR (18cases : 58.1%) appeared most frequently in the abnormal laboratory data. These symptoms will be gone spontaneoulsy without any specific treatment in several weeks or months. Conclusion : We should consider open biopsy or fine needle aspiration cytology or ultrasound guided cutting needle biopsy with lymph node in patients who have cervical lymphadenopathy with easy fatigue and fever, especially young women to exclude other conditions such as malignant lymphoma and tuberculosis, etc.
An 11-year-old, female, Maltese dog was presented with abdominal distension and intermittent slight lameness on left hindlimb. Palpable abdominal pain and purulent vaginal discharge were observed on physical examination. Severe leukopenia with toxic change, and azotemia, hyperphosphatemia, and hyponatremia were identified on blood profile. On radiographs and abdominal sonograms, remarkable soft tissue mass containing echogenic material deviating intestines craniodorsally compatible with pyometra. On stifle radiographs, 4-5 fragmented sesamoid bone (fabella) was identified on both stifle joint with medial patella luxation. Immediate ovariohysterectomy was performed and the patient was recovered well with normal blood work. No specific treatment was attempted to the left hindlimb showing minimal and intermittent lameness. No remarkable abnormal gait was reported for 3-month follow-up period. The fragmented lateral fabella is considered congenital multipartite of lateral fabella not related to the lameness.
A 3-year-old, 38 kg, male Golden Retriever dog was referred to Veterinary Teaching Hospital of Konkuk University because of chronic formation of crust on nasal bridge and planum nasale. Abnormalities of physical examination included hyperkeratosis on the footpad, symmetrical alopecia and erythma around olecranon, and crust on nasal bridge and planum nasale. Results of the hematological examination showed a mild leukopenia with neutropenia, monocytosis, and mild lymphocytosis. In addition, the result of serum chemistry and thyroid gland profile were normal. Results of fungal and bacterial culture was negative. Acanthocytes in cytological evaluation of nasal crust were observed in direct microscopic examination. Examination of skin biopsy exhibited vacuolation of basal cell layer, degeneration and necrosis of basal cell with defluxion, mild monocytes filtrations between epidermis and dermis, and mild acanthosis with hyperkeratinization. Based on results of examination described above, Discoid Lupus Erythematosus (DLE) was diagnosed.
Isolation of the toxic substance which was produced by Streptentyces sp. and it's biorhemical characterestics and toxicity to fishes were reported in the previous paper. The present report includes antibiotic activity of the substance, inhibitory activity of the substance on the succinic dehydrogenase of fishes, and its effect on the blood corpuscles of a rabbit. An evident antibiotic activity of this substance was observed on Candida yeasts, but n$ot^1$on molds or bacteria. The substance inhibited the growth of Candida japonica and C. utilis to the 50% at the concentrations of 7.5 and 10.2ug per ml, respectively. The activity of succinic dehydrogenase obtained from various organs of Cyprinous carpio L. was also found to be inhibited by this substance. Original activities of the enzymes from tht brain, kidney, and liver, were inhibited by 75.4%, 38.2%, and 26.2%, respectively, but not the enzymes from the heart and spleen. Neither leukopenia nor leukocytosis was detected after the intravenous administration of the substince to the rabbit at the level of 6 mg per 2 kg body weight.
In the present case, the effect and toxicity of carboplatin and cyclophosphamide chemotherapy combined with surgery of mammary tumors in a dog were examined. An 8-year old spayed female Beagle presented with a mammary tumor. Physical examination, radiography, ultrasonography, computed tomography (CT), and laboratory examination were performed. Metastasis of the mammary tumor was confirmed by CT scan. Chemotherapy using a combination of carboplatin and cyclophosphamide was initiated following surgery. The first cycle of chemotherapy was planned to last for 6 weeks; it was planned that carboplatin would be intravenously administered for the first week (1 day) and cyclophosphamide would be intravenously administered for the next 3 weeks (22 days). Between the end of cycle 1 and the beginning of cycle 2, based on CT, it was confirmed that the number and size of tumors were unchanged and the tumors had not spread to other organs. However, at the end of cycle 2 and the beginning of cycle 3, CT revealed an increase in the number and size of mass in the lung. Chemotherapy was associated with adverse effects such as lethargy, anorexia, leukopenia, and hair loss. In conclusion, this case showed that a combination of carboplatin and cyclophosphamide suppressed the development of new neoplasms as well as metastasis for a certain period of time but did not improve the survival time. Although more cases are required, this chemotherapeutic procedure remains challenging.
Background: As the coronavirus disease 2019 (COVID-19) outbreak continues to evolve, it is crucially important for pediatricians to be aware of the differences in demographic and clinical features between COVID-19 and influenza A and B infections. Purpose: This study analyzed and compared the clinical features and laboratory findings of COVID-19 and influenza A and B infections in children. Methods: This retrospective study evaluated the medical data of 206 pediatric COVID-19 and 411 pediatric seasonal influenza A or B patients. Results: COVID-19 patients were older than seasonal influenza patients (median [interquartile range], 7.75 [2-14] years vs. 4 [2-6] years). The frequency of fever and cough in COVID-19 patients was lower than that of seasonal influenza patients (80.6% vs. 94.4%, P<0.001 and 22.8 % vs. 71.5%, P<0.001, respectively). Ageusia (4.9%) and anosmia (3.4%) were present in only COVID-19 patients. Leukopenia, lymphopenia, and thrombocytopenia were encountered more frequently in influenza patients than in COVID-19 patients (22.1% vs. 8.5%, P=0.029; 17.6% vs. 5.6%, P=0.013; and 13.2% vs. 5.6%, P=0.048, respectively). Both groups showed significantly elevated monocyte levels in the complete blood count (70.4% vs. 69.9%, P=0.511). Major chest x-ray findings in COVID-19 patients included mild diffuse ground-glass opacity and right lower lobe infiltrates. There were no statistically significant intergroup differences in hospitalization or mortality rates; however, the intensive care unit admission rate was higher among COVID-19 patients (2.4% vs. 0.5%, P=0.045). Conclusion: In this study, pediatric COVID-19 patients showed a wide range of clinical presentations ranging from asymptomatic/mild to severe illness. We found no intergroup differences in hospitalization rates, oxygen requirements, or hospital length of stay; however, the intensive care unit admission rate was higher among COVID-19 patients.

Purpose: The goal was to use electronic health records to identify factors and outcomes associated with falls among patients admitted to hematology units. Methods: This retrospective case-control study included data from a tertiary university hospital. Analysis was done of records from 117 patients with a history of falls and 201 patients with no history of falls who were admitted to the hematology unit from January 1, 2013 to December 31, 2014. Risk factors were analyzed using hierarchical logistic regression; patient outcomes were analyzed using multiple logistic regression, Cox proportional hazards regression, and multiple linear regression. Results: Clinical factors such as self-care nursing (OR=4.47, CI=1.64~12.11), leukopenia (OR=6.03; CI=2.51~14.50), and hypoalbuminemia (OR=2.79, CI=1.31~5.96); treatment factors such as use of narcotics (OR=2.06, CI=1.01~4.19), antipsychotics (OR=3.05, CI=1.20~7.75), and steroids (OR=4.51, CI=1.92~10.58); and patient factors such as low education (OR=3.16, CI=1.44~6.94) were significant risk factors. Falls were also associated with increased length of hospital stay to 21.58 days (p<.001), and healthcare costs of 17,052,784 Won (p<.001). Conclusion: These findings can be a resource for fall prevention education and to help develop fall risk assessment tools for adults admitted to hematology units.

The hG-CSF (human Granulocyte Colony-Stimulating Factor) is a growth and stimulation factor capable of inducing the proliferation of bone marrow cells, several types of leukocytes, among other hematopoietic tissue cells. hG-CSF is used in used to treat anomalies that reder a small number of circulating white blood cells, which may compromise the immune defenses of the affected person. For these reasons, the production of hG-CSF in a bioreactor system using the mammary gland of genetic modified animals is a possibility of adding value to the bovine genetic material and reducing the costs of hG-CSF production in pharmaceutical industry. In this study, we aimed the production of transgenic hG-CSF bovine through the lipofection of bovine primary fibroblasts with an hG-CSF expression cassette and cloning these fibroblasts by the somatic cell nuclear transfer (SCNT) technique. The bovine fibroblasts transfected with the hG-CSF cassette presented a stable insertion of this construct into their genome and were efficiently synchronized to G0/G1 cell cycle stage. The transgenic fibroblasts were cloned by SCNT and produced 103 transferred embryos and 2 pregnancies, one of which reached 7 months of gestation.
Feline panleukopenia virus (FPV) causes leukopenia and severe hemorrhagic diarrhea, killing 50% of naturally infected cats. Although intact FPV can serve as an antigen in the hemagglutination inhibition (HI) test, an accidental laboratory-mediated infection is concern. A non-infectious diagnostic reagent is required for the HI test. Here, we expressed the viral protein 2 (VP2) gene of the FPV strain currently prevalent in South Korea in a baculovirus expression system; VP2 protein was identified by an indirect immunofluorescence assay, electron microscopy (EM), Western blotting (WB), and a hemagglutination assay (HA). EM showed that the recombinant VP2 protein self-assembled to form virus-like particles. WB revealed that the recombinant VP2 was 65 kDa in size. The HA activity of the recombinant VP2 protein was very high at 1:215. A total of 143 cat serum samples were tested using FPV (HI-FPV test) and the recombinant VP2 protein (HI-VP2 test) as HI antigens. The sensitivity, specificity, and accuracy of the HI-VP2 test were 99.3%, 88.9%, and 99.3%, respectively, compared to the HI-FPV test. The HI-VP2 and HI-FPV results correlated significantly (r = 0.978). Thus, recombinant VP2 can substitute for intact FPV as the serological diagnostic reagent of the HI test for FPV.
