Treatment of osteoarthritis of the ankle joint is similar to that of any other large joint and includes conservative and surgical treatments. Surgical option in severe osteoarthritis is joint fusion or joint replacement, whereas conservative treatment is limited and includes mainly ankle supports, physical therapy, and oral medication. Hyaluronic acid was discovered in 1934 and now has been widely used in the knee and shoulder joints. We reviewed the articles about an intra-articular hyaluronic acid injection in the treatment of osteoarthritis of the ankle joint.

A knee joint's longevity depends on the proper integration of structural components in an axial alignment. If just one of the components is abnormally off-axis, the biomechanical system fails, resulting in arthritis. The complexity of various failures in the knee joint has led orthopedic surgeons to select total knee replacement as a primary treatment. In many cases, this means sacrificing much of an other-wise normal joint. Here, we review novel computational approaches to describe knee physiotherapy by introducing a new dimension of foot loading to the knee axis alignment producing an improved functional status of the patient. New physiotherapeutic applications are then possible by aligning foot loading with the functional axis of the knee joint during the treatment of patients with osteoarthritis.
Objective: This study aimed to investigate the effect of adding hip abductor strengthening to conventional rehabilitation on muscle strength and physical function following total knee replacement (TKR) for knee osteoarthritis. Design: Randomized controlled trial Methods: Thirty-five participants were randomly allocated to exercise groups I (n=18) and II (n=17). Group I underwent hip abductor training and conventional rehabilitation for 30 min per day, 5 days per week for 4 weeks. Group II underwent conventional rehabilitation for 30 min per day, 5 days per week for 4 weeks. The participants in both groups also received continuous passive motion therapy for 15 min per day, 5 days per week for 4 weeks. To investigate the effect of the intervention, the Biodex dynamometer was used to measure the peak torque of both knee extensors and hip abductors. This study used the Knee Outcome Survey-Activities of Daily Living Scale (KOS-ADLS) to assess physical function, as well as the figure-of-8 walk test (F8W) and the stair climb test (SCT). Results: According to the interventions, exercise groups I and II showed significantly improved muscle strength and KOS-ADLS, F8W, and SCT scores (p<0.001). Compared with that of exercise group II, exercise group I showed significantly improved hip abductor strength (p<0.001) and KOS-ADLS, F8W, and SCT scores (p<0.05). Conclusions: The results of this study indicate that the combination of hip abductor strengthening and conventional rehabilitation is an effective exercise method to increase hip abductor muscle strength and physical function after TKR.
Objectives : This study was to investigate the effects of GCP treatment on the monosodium iodoacetate-induced osteoarthritis in rats. Methods : Arthritis was induced by injection of Monosodium lodoacetate(0.5mg) into knee joints of rats. Arthritic rats were divided into control(n=8) and treated(n=8) group. Control group was taken distilled water for 20 days. Treated group was taken extracts of GCP by oraly for same duration. Normal group(n=8) was injected with normal saline and was taken distilled water for 20 days. Body weight was measured at 0, 5, 10, 15, 20 days after injection. Macroscopic examination and histopathological study on articular cartilage of knee joint were operated at 20 days after injection. Proteoglycan(PG) content of articular cartilages of knee joint was represented by safranine O staining, was measured at 20 days injection. Tumor necrosis $factor-{\alpha}$, $Interleukin-1{\beta}$, Interleukin-6 in synovial fluid were measured with ELISA kit at 20 days after injection. Immunohistochemical staining of COX-2, iNOS in knee joints were observed at 20 days after injection. Results : 1. Body weight of the treated group increased compare with control group at 20 days after injection. 2. Macroscopically, degree of osteoarthritis in the treated group were evaluated compared with the control group. 3. PG content in articular cartilage of the treated group was significantly increased compared with the control group. 4. Histopathologically, osteoarthritic scores of the treated group was significantly decreased compared with the control group. 5. $TNF-{\alpha}$ content in synovial fluid of the treated group was decreased compared with the control group. 6. $IL-1{\beta}$ content in synovial fluid of the treated group was significantly decreased compared with the control group. 7. Positive reaction of COX-2 in chondrocytes and synovial membrane of the treated group was faint compared with the control group. Conclusions : On the basis of these results, we concluded that GCP has inhibiting effects on the $IL-1{\beta}$ and COX-2 secretion of chondrocytes and synovial membrane in Monosodium lodoacetate-Induced osteoarthritis model of rats.

Objectives : 3-Acetyl-11-keto-𝛽-boswellic acid (AKBA) is a major active compound in Boswellia serrata. We investigated the arthritic changes following AKBA administration in monosodium iodoacetate (MIA)-induced osteoarthritis rats. Methods : All rats were randomly divided into five groups: Normal, Control, INDO (indomethacin 2 mg/kg treated), AKBA30 (AKBA 30 mg/kg treated), and AKBA60 (AKBA 60 mg/kg treated); drugs were given 2 weeks before MIA injection. For all groups except the normal group, 50 µL of sterile saline with MIA (80 mg/mL) was injected into the right knee joint 2 weeks after drug administration. The drug administration was continued for 4 weeks from 1 week after osteoarthritis induction. The histomorphological changes of knee joint cartilage were observed by H&E staining. Also, the levels of glycosaminoglycan (GAG), cartilage oligomeric matrix protein (COMP), 5-lipoxygenase (5-LOX), 5-LOX-activating protein (FLAP), and leukotriene B4 (LTB4) in the knee joint were determined by the ELISA kits. The expressions of mitogen-activated protein kinases (MAPKs), inflammatory cytokines, and matrix metalloproteinases (MMPs) in knee joint were detected by Western blot. Results : Data show that levels of 5-LOX, FLAP, LTB4, and COMP were downregulated significantly in the AKBA treated groups when compared to those in the Control group. On the other hand, GAG levels were significantly elevated. As a result of Western blot, the AKBA-treated groups significantly inhibited phosphorylation of MAPKs. In addition, significant downregulation of the expression of inflammatory cytokines and MMPs was found in the AKBA-treated groups. Conclusion : Our findings suggest that administration of AKBA could exert better chondroprotective and anti-inflammatory effects for MIA-induced osteoarthritis rats.
Background: Due to the aging population in Korea, knee osteoarthritis (KOA) has become an increasingly common condition. Many patients with KOA prefer analgesics, herbal medicines, acupuncture, or exercise, rather than arthroscopic surgery or a knee replacement. Gyebutang (GB) granules are a herbal extract widely used to treat KOA in traditional Korean medicine, but there is insufficient evidence of its efficacy and safety. Methods: A multicenter, randomized, assessor-blinded, 2-armed parallel, controlled clinical trial has been designed to investigate the efficacy and safety of GB combined with acupuncture for the treatment of KOA. There will be 100 patients with KOA enrolled in the study from 3 traditional Korean medicine hospitals. The participants will be randomly allocated to an experimental group (GB and acupuncture) or a control group (celecoxib and acupuncture) in a 1:1 ratio. Both groups will receive acupuncture treatment once a week for 6 weeks; one group will receive GB and the other will receive celecoxib for the same duration. Results: The primary outcome will be the change of knee osteoarthritic pain, based on scores on a 100 mm visual analog scale. The secondary outcomes will be scores on a numeric rating scale, the Western Ontario and McMaster Universities osteoarthritis index, patient global assessment, European quality of life 5-dimension 5-level scale, and adverse events. Conclusion: The results of this study will provide evidence of efficacy and safety of GB as a treatment for patients with KOA.

Purpose : This study investigated the effects of osteoarthritis therapy using images and histological diagnosis after microcurrents were applied to osteoarthritic rats. Methods : Osteoarthritis was induced with a mixture of 2% carrageenan and 2% kaolin. The mixture (0.1 $m{\ell}$) was injected into the knee joints of rats. Osteoarthritis with articulation received microcurrent stimulation for four weeks (15 min/day, 5 days/week): treated with pulsation frequency of 5 pps and a stimulation intensity of 25 ${\mu}A$. Results : Osteoarthritis of the control group constantly changed. The group with a stimulation intensity of 25 ${\mu}A$ applied to the surface of the articular cartilage experienced near normal recovery according to image diagnosis. The result of histological and immunohistochemistry inspection confirms that microcurrent stimulation had a positive effect on the treatment of osteoarthritis. Conclusion : The differences among images and histological diagnoses show that steoarthritis will experience constant progression from stimulation.

Objectives We have studied to know effects of Bangkeehwangkee-tang on the monosodium iodoacetate (MIA) induced osteoarthritis in rats. Methods Osteoarthritis was induced by injection of monosodium iodoacetate into knee joint cavity of rats. Rats are devided into 4 groups; Sham, control, Bangkeehwangkee-tang, Shinbaro. Sham group was injected by normal saline into knee joint cavity only. Control group was taken no treatment and Experimental groups were taken extracts of Bangkeehwangkee-tang and Shinbaro by orally once a day for 4 weeks. The content of TNF-$\alpha$, IL- $1{\beta}$ and IL-6 in synovial fluids were analysed. COX-2 in chondrocytes and blood PGE2 concentration were analysed. Histopathological examination was performed by Safranin-O fast green staining and Mankins score checking. Results The content of TNF-$\alpha$, IL-$1{\beta}$ in experimental groups were decreased compared with control group. COX-2 in chondrocytes and blood PGE2 concentration in experimental groups were decreased compared with control group. Histopathologically, osteoarthritic scores of experimental groups were decreased compared with control group. But the content of IL-6 in synovial fluids was not significantly compared with control group. Conclusions According to these results, it can be suggested that Bangkeehwangkee-tang has anti-arthritic effects on the MIA induced osteoarthritis in rats.
The PACE (People nth Arthritis Can Exercise) is an exercise program developed by the Arthritis Foundation to improve muscle strength and joint flexibility for patients with arthritis. The purpose of this study was to explore the effects of PACE program on self-efficacy, pain, and joint function in the Korean immigrant elderly. The PACE program was held twice a week for 6 weeks for Korean immigrant elderly who had osteoarthritis. Twenty four subjects completed the program, who were recruited in two places : 10 elderly in a senior residential apartment, and 14 elderly in a senior center supported by Congregated Meal Program for Korean Elderly. Self-efficacy(Sherer et al., 1982), pain severity(by using Visual Analogue Scale), and number of painful joints were measured before and after the PACE program. To examine the joint flexibility and strengthening, the followings were measured : the extent of the upward arm reach in both sides(flexibility of shoulder), the ability to touch fingertips of the both hands in back pat and rub(flexibility of arm), the degree of range of motion(ROM) of both ankles in their dorsiflexion(flexibility of ankle) and plantarflexion with standing with toe(strengthening of ankle), and the degree of knee extension. Wilcoxon signed rank test was used for data analysis and the significance of the differences in the variables was examined to compare the data obtained before and after the PACE program. After the PACE, followings were found : 1. Self-efficacy was significantly increased. 2. Pain severity and number of painful Joints was significantly decreased. 3. The flexibility of both shoulders and arms were significantly improved, but the flexibility of knee was not changed. 4. The flexibility and strengthening of both ankle was significantly improved. In conclusion, PACE was clearly proved to be an effective exercise program to promote self-efficacy, to reduce pain, and to enhance joint function in the elderly with osteoarthritis. It is suggested that the PACE program should be recommended as one of the useful and appropriate nursing interventions for elderly with osteoarthritis.

This study was to investigate the effects of Gyeonbi-Tang Treatment on the monosodium iodoacetate(MIA)-induced mild osteoarthritis in rats. Arthritis was induced by injection of MIA(0.25 mg) into knee joints of rats. Arthritic rats were divided into control(n=8) and treated(n=8) group. Control group was taken distilled water and treated group was taken extracts of Gyeonbi-Tang by orally for 20days. Body weights were measured at 0, 5, 10, 15, 20 days after MIA injection. At the end of experiment(20day after MIA injection), gross and histopathological examination on the articular structures of knee joints were performed. Proteoglycan(PG) contents in articular cartilages were analysed. And also, tumor necrosis factor-${\alpha}$(TNF-${\alpha}$) and interleukin-1${\beta}$(IL-1${\beta}$) contents in synovial fluids were measured by ELISA method. Body weights of the treated group were significantly increased compared with control group at 15, 20 days after injection. Grossly, the severity of osteoarthritis in the treated group were alleviated compared with control group. PG contents in articular cartilages of the treated group were increased compared with control group. Histopathologically, osteoarthritic scores of the treated group was decreased compared with the control group. TNF-${\alpha}$ contents in synovial fluids of the treated group were significantly decreased compared with control group. On the basis of these results, we concluded that Gyeonbi-Tang Treatment has anti-arthritic effects on the monosodium iodoacetate-induced mild osteoarthritis in rats. And it's effects were related with reduced secretion of TNF-${\alpha}$ from synovial membranes.
